妊娠期糖尿病(gestational diabetes mellitus,GDM)是孕妇最常见的代谢性疾病之一,严重影响着妊娠期间母婴健康状况、分娩过程、妊娠结局、分娩后并发症等.因此需要针对GDM作出早期的预测和诊断.脂肪酸结合蛋白4(fatty acid-binding protein,FABP4)被大量科学研究证明与胰岛素抵抗和GDM有关.这种新型的促炎脂肪因子在细胞水平上参与调节脂质代谢.本文主要对FABP4与GDM之间的研究进展和其各种作用机制进行了综述,为临床寻找有效预测、预防、诊断和治疗方法提供参考.
根据中国疾病预防控制中心性病艾滋病预防控制中心的数据统计,截至2020年底,中国现有105.3万报告的人类免疫缺陷病毒(HIV)感染者[1],在全球范围预计有3 690万人感染HIV.在中国,宫颈癌是女性第二常发肿瘤,仅次于乳腺癌,对此持续高危人乳头瘤病毒(HPV)感染与宫颈癌患病有密切的相关性[2].HIV与HPV都是性传播疾病的病原体,二者之间有着复杂的相关性,为进一步探明二者之间的关系、更有效的减少二者感染和恶性事件发生、维护人类卫生健康,国内外学者于多个方面进行了诸多研究.本文旨在从现状、机制等方面总结近些年HIV与HPV感染相关性的研究,为后续深入研究提供参考和方向.
To evaluate the associations between maternal serum concentrations of high-density lipoprotein cholesterol (HDL-c) throughout pregnancy and neonatal birth weight (BW) and small for gestational age (SGA) births. A prospective cohort of 2241 pregnant women was followed from recruitment to delivery in three hospitals in Beijing, China between January 2014 and December 2017. Maternal fasting serum lipids concentrations were measured at gestational week 6–12, 16, 24 and 36. Major outcome was neonatal BW. The associations between maternal HDL-c and BW were estimated by linear regression and linear mixed-effects models. Odds ratios (ORs) and 95% confidence intervals of SGA births in relation to HDL-c were evaluated via logistic regression analysis. There was a tendency that mothers with higher HDL-c concentrations throughout gestation gave birth to infants with lower BW. A negative association was found between maternal HDL-c concentrations and BW at 24th and 36th gestational weeks (B = − 34.044, P = 0.034; B = − 53.528, P = 0.000). The HDL-c trend of change was inversely associated with BW (B = − 442.736, P = 0.000). Mothers with SGA neonates had higher serum HDL-c concentration at the 36th gestational week (P < 0.01). The incidences of SGA in the three groups (HDL-c: 1.84–2.23 mmol/L, 2.24–2.59 mmol/L and ≥ 2.60 mmol/L) were higher than the group with the lowest concentration of HDL-c (< 1.83 mmol/L) (P < 0.01, P < 0.01, P < 0.001) at 36th week. Higher maternal HDL-c concentrations at 36th week (HDL-c: 1.84–2.23 mmol/L, 2.24–2.59 mmol/L and ≥ 2.60 mmol/L) were positively associated with the incidence of SGA (OR = 1.900, P = 0.008; OR = 1.893, P = 0.008; OR = 1.975, P = 0.004). The HDL-c trend of change was positively associated with SGA births (OR = 9.772, P = 0.000). Maternal serum HDL-c concentrations were inversely associated with BW at 24th and 36th gestational weeks. The high concentrations of HDL-c at the 36th gestational week increased the risk of SGA. The maternal HDL-c trend of change across pregnancy was associated with smaller neonatal size.
Objective :To explore influence of posture control combined core muscle group training on motor function of lower limb ,balance function and walking ability in patients with stroke .Methods :A total of 94 patients with hemiplegia after stroke during rehabilitation period ,who were treated in our hospital from Dec 2015 to Dec 2017 , were selected .Patients were randomly and equally divided into routine rehabilitation group and combined training group (received posture control and core muscle group training based on routine rehabilitation group ) ,both groups were treated for four weeks .Scores of Fugal‐Meyer motor assessment (FMA) scale ,Berg balance scale (BBS) and Holden functional ambulation classification (FAC) were observed and compared between two groups before and af‐ter treatment .Results :Compared with before treatment ,there were significant rise in scores of lower limb FMA , BBS and FAC class in two groups after four‐week treatment , P=0. 001 all.Compared with routine rehabilitation group after four‐week treatment ,there were significant rise in lower limb FMA score [ (22.61 ± 1. 84) scores vs. (25.84 ± 1.67) scores] ,BBS score [ (23. 28 ± 1.92) scores vs .(28.82 ± 1. 36 ) scores] and FAC class [ (2. 96 ± 0.71) classes vs.(4.02 ± 0.46 ) classes] in combined training group , P= 0. 001 all.Conclusion : Posture control training combined core muscle group training can significantly promote rehabilitation of lower limb motor function , improve balance and walking ability in patients with hemiplegia after stroke ,which is worth extending .
目的 探讨经腹腔镜预防性宫颈环扎术对孕中晚期并发症及围产结局的影响.方法 选取2012年1月至2017年1月在首都医科大学附属复兴医院宫腔镜中心行经腹腔镜预防性宫颈环扎术的患者36例作为研究组,另选取同期年龄和体重指数匹配、在首都医科大学附属复兴医院妇科行经阴道预防性宫颈环扎术的患者36例作为对照组,所有患者孕期均在首都医科大学附属复兴医院进行系统保健并分娩.比较分析两组患者的孕中晚期并发症及其围产结局.结果 与对照组比较,研究组患者的孕中期流产次数更多[(2.12±1.17)次vs(1.15±1.18)次],手术前宫颈长度更短[(25.34±3.88) mmvs (28.15±5.73) mm],差异有显著性(P<0.05).研究组的孕期腹痛发生率(38.2%)、孕期住院保胎率(41.2%)、黄体酮使用时间[(28.34±2.78)周]均显著高于对照组[分别为14.7%,17.6%, (3.68±2.63)周],差异有显著性(P<0.05),而孕晚期宫颈长度、孕期阴道炎发生率两组比较差异无显著性(P>0.05).两组的早产率、分娩孕周、新生儿出生体重、产后出血量比较差异无显著性(P>0.05),研究组环扎带与周围组织粘连率高于对照组(35.3%vs 0),胎膜早破(11.8%)和病理证实的宫腔感染发生率(6.7%)低于对照组(分别为38.2%和25.9%),差异有显著性(P<0.05).与足月分娩者比较,早产者胎膜早破发生率(71.4% vs 14.0%)、病理证实的宫腔感染发生率(57.1% vs 10.0%)更高,差异有显著性(P<0.01).Logistic回归分析显示,控制环扎前宫颈长度和内口松弛情况后,病理证实的宫腔感染是早产的唯一高危因素.结论 经腹腔镜预防性宫颈环扎术患者的围产结局与经阴道预防性宫颈环扎术相似,但孕中晚期并发症发生率高于后者.预防性宫颈环扎术后患者是否发生早产与感染相关,而与手术前后宫颈长度及宫颈内口无关.
目的 探讨预防性宫颈环扎术后患者不同孕期糖脂代谢的特点.方法 选取2012年6月至2018年6月在首都医科大学附属复兴医院行预防性宫颈环扎并分娩者69例作为环扎组,另选同期的正常孕妇138例作为对照组,比较两组不同孕期糖脂代谢情况.结果 环扎组孕次、妊娠期糖尿病发生率、妊娠期糖尿病需药物控制率、子痫前期发生率均高于对照组,而产次、分娩孕周、新生儿出生体重均小于对照组,差异有显著性(P<0.05).环扎组75g口服葡萄糖耐量试验服糖后1、2h血糖浓度、孕晚期空腹血糖水平均高于对照组,差异有显著性(P<0.05).环扎组孕早、中、晚期甘油三酯水平均高于对照组,而高密度脂蛋白低于对照组,差异有显著性(P<0.05).环扎组孕早期低密度脂蛋白水平高于对照组,差异有显著性(P<0.05).两组孕早、中、晚期的总胆固醇水平差异均无显著性(P>0.05).结论 预防性宫颈环扎术后患者孕中晚期糖脂代谢异常发生率高,需重视加强预防性宫颈环扎术后患者孕期糖脂代谢管理.
目的:探讨PBL教学法在产科住院医师规范化培训中应用效果。方法对比分析我院自2011年9月~2015年9月经产科住院医师规范化培训的60名住院医师,按照带教方法分为两组:一组实施PBL教学法,一组采用传统带教方式。结果3个月培训结束进行理论及实践考核,问卷调查,并对培训结束阶段考核通过率进行比较。PBL教学组对带教方式满意度、出科考核成绩、阶段考核通过率均高于传统教学组。结论 PBL教学法在产科住院医师规范化培训中可激发产科学习和工作兴趣,提高规范化培训效果。
Objective To analyze the glucose and metabolism and perinatal outcome in pregnant women with different extent gestational diabetes mellitus (GDM) of IADPSG diagnostic criteria,and to investigate the necessity to manage mild GDM.Methods Total 368 pregnant women at 24-28 gestational weeks were given 75 g glucose oral glucose tolerance test (OGTT).They were divided into three groups according OGTI result,single abnormal group (GDM1 group,148 cases),combination abnormal group (GDM2 group,120 cases) and normal group (100 cases).The three groups were given determinations of glycosylated hemoglobin,fasting C peptide (FCP),fasting insulin (FINS),total cholesterol (TC),triglyceride (TG),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),apolipoprotein (Apo) A1 and ApoB,calculated homeostasis model of assessment for insulin resistance index (HOMA-IR) and homeostasis beta cell index (HBCI).Results From normal group,GDM1 group to GDM2 group,the fasting plasma glucose,glycosylated hemoglobin,FCP,HOMA-IR,TG were gradually increased [(4.39 ± 0.38),(4.98 ± 0.49),(5.25 ± 0.59) mmol/L; (5.10 ± 0.29)%,(5.27 ± 0.36)%,(5.46 ± 0.46)% ;(1.37 ±0.67),(1.73 ±0.73),(1.99 ±0.96) μ g/L;1.07 ±0.42,1.26 ±0.54,1.37 ±0.43; (2.77 ±0.67),(3.05 ± 0.78),(3.34 ± 0.92) mmol/L,P < 0.01],while HBCI was gradually decreased (5.93 ± 0.48,5.42 ±0.45,5.29 ± 0.44,P < 0.01).ApoA1 in GDM1 group and GDM2 group were significantly lower than that in normal group [(2.21 ±0.39),(2.15 ±0.35) g/L vs.(2.42 ±0.37) g/L,P <0.01],but there was no significant difference between GDM1 group and GDM2 group (P> 0.05).ApoB/ApoA1 in GDM2 group was significantly higher than that in normal group and GDM1 group (0.49 ±0.14 vs.0.42 ±0.14 and 0.45 ±0.12,P < 0.01),but there was no significant difference between normal group and GDM 1 group (P > 0.05).There was no significant difference in FINS,TC,HDL-C,LDL-C,ApoB,neonatal weight and the incidence rate of premature delivery among the three groups (P > 0.05).The incidence rates of eclampsism,large for date infant and cesarean section in GDM1 group and GDM2 group were significantly higher than those in normal group [8.1%(12/148) and 14.2%(17/120) vs.2.0%(2/100),14.9%(22/148) and 16.7%(20/120)vs.6.0%(6/100),52.0%(77/148) and 54.2%(65/120) vs.30.0%(30/100),P< 0.01].Conclusions Different extent GDM of IADPSG diagnostic criteria has different extent abnormal glucose and lipid metabolism and abnormal perinatal outcome,so it is necessary to diagnose and manage GDM pregnant women even mild GDM.

Objective To investigate the glucose metabolism and lipid metabolism and their relationship in patients with different extent Gestational diabetes mellitus.Methods There were totally 384pregnant women at 24~28gestational week received antenatal care in the clinic of our hospital from 2010to 2011.Each woman underwent 75g OGTT and lipid testing.Patients were divided into normal group(NGT,n=100),and GDM group(n= 266)which including mild GDM1(n=148),moderate GDM2(n=86)and severe GDM3(n=32)with respect to the results of OGTT.Results The levels of HOMA-IR[(4.18±1.67)and(3.13±1.22)],HBCI[(5.41 ±0.44),(5.93±0.48)],TG[(3.18±0.85)and(2.77±0.67)mmol/l and ApoB/ApoA1(0.47±0.14)and(0.42±0.14)]were significantly different between GDM and NGT(P0.05).2.The levels of HOAM-IR(5.06±1.79),LnHBCI(5.09±0.48),TG(3.78±1.12)mmol/L,Ln(TG/HDL-C)(0.25±0.14),ApoB/ ApoA1(0.56±0.17)and ApoB(1.13±0.26)g/Lin GDM3were significantly different from GDM1or GDM2(P0.05).3.The TG level was positively correlated with the levels of FPG,FIN,HbA1cand HOMA-IR;the level of HDL-C was negatively correlated with the levels of FPG,HbA1cand HOMA-IR,but positively with the levels of HBCI and ApoA1.Conclusion The patients with different extent gestational diabetes mellitus have abnormal glucosemetabolism and lipidmetabolism.The serum lipid disorder is closely associated with pancreatic islets secretion function and insulin resistance.

Objective To investigate the effect on the menstrual pattern of decline of ovarian function in women during menopausal transition.Methods 177 cases of women in our hospital were analyzed in this study. The subjects were divided into three groups according to the change of the menstrual cycle in a year.Reproductive period group: 39 cases, age 37.2±6.4 years, woman's menstrual cycles remain regular,the menopausal transition group: according to the characteristics of the menstrual cycle was divided into: early menopausal,transition group (EMT group): 55 cases, age 44.5±3.1 years, the cycle length changed by seven days or more (variable cycle length more than 7 days but amenorrhealess than 60 days);late menopausal transition group (LMT group): 59 cases, age 48.4±3.5 years, was characterized by two or more skipped menstrual cycles and at least one intermenstrual interval of 60 days or more .Postmenopausal group: 24 cases, age 54.2±4.7 years.Menopause was the anchor point that defined after12 months of amenorrhea following thefinal menstrual period. Serum was assayed in all cases includingfollicle-stimulating hormone (FSH), luteinizing hormone (LH), estrogen (E2), inhibin B (InhB), anti-Mullerian hormone (AMH).Results Value of AMH, InhB, FSH amongfour groups were the difference,P<0.01.There was statistical significance of LH value among reproductive group ,LMT group and postmenopausal group,P<0.05.Postmenopausal group compared with other groups,E2 value was significant lower,P<0.01.177 cases were divided into 4 groups according to age. The results showed thatthe menstrual cycle to keep regular before the age of 40; 35% women age between 40~44 years whosemenstrual cycle changed ; 52% women age between 45~49 ,the menstrual become irregular, otherwise 17.4% get into menopause period.When over 50 years of age, only 4.7% of women with regular menstrual cycle, 31% of the women appeared irregular menstruation, 63% women enter menopause.By chi-square test, comparison of the percentage change in menstrual pattern is very significant,P=0.0012.Conclusion Change of AMH, 1nhB, FSH value was a sensitive indicator of ovarian function decline; menstrual pattern variation as well as aging can also be used as the reference for ovarian failure.
[目的]探讨医院实行住院总护士值班制的效果。[方法]通过严格人员选拔任用,加强岗位培训,借鉴住院总医师的管理经验,建立住院总护士值班制度。[结果]培养了一支具备扎实的基础护理知识、过硬的专科护理技能的住院总护士队伍,有效提高了护理质量及抢救病人的成功率,减少了不必要的医疗纠纷。[结论]住院总护士岗位的建立有助于培养专业化护理骨干,提升临床护理质量。
[目的]探讨单纯性肥胖患者伴发非酒精性脂肪肝病与外周血脂肪细胞因子的关系.[方法]160例研究对象分为正常对照组30例(A组),单纯性肥胖组40例(B组),非酒精性脂肪肝组50例(C组)、单纯性肥胖合非酒精性脂肪肝组40例(D组);检测各组血脂和血清瘦素(Leptin)、脂联素(APN)的水平变化.[结果]与A组比较,B组、C组、D组的总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、Leptin水平升高,高密度脂蛋白(HDL)、脂联素(APN)水平下降,差异均有统计学意义(P<0.05或P<0.01);上述指标在B组和C组之间比较差异不显著(P>0.05).D组的TC、TG水平均较B组、C组明显升高(P<0.05),且LDL水平也高于B组(P<0.05);D组的Leptin水平较B组上升更明显,APN水平较B组、C组下降更明显,差异均有统计学意义(P<0.05).[结论]脂肪细胞因子与单纯性肥胖及非酒精性脂肪肝相关,其可能参与了单纯性肥胖患者非酒精性脂肪肝病的病理生理过程.

Objective To investigate the effects of scavenger receptor A on the catabolism of lipid emulsions and further to see if it differently affects long-chain triglyceride (LCT) and fish oil (FO) emulsions. Methods A total of 24 C57BL/6J female mice, 10 to 12 weeks old, were randomly divided into 4 groups with 6 mice in each group. Two groups of mice were intravenously injected with dextran sulfate ( DexSO4 ) ( 1 mg/mouse) followed by intravenous injection of [1α, 2α(n)-3H] cholesteryl oleoyl ether [(3H)CEt] labelled LCT or FO emulsions (0.4mg triglycerde/mouse) at 2 minutes respectively, and other two groups were injected by saline as controls before injection of (3H)CEt labelled LCT and FO emulsions. Then, blood was drawn at fixed intervals to measure the radioactivities and the emulsion's fractional catabolic rates (FCR) were calculated. With the same procedures above mentioned, non-radiolabelled LCT and FO emulsions were intravenously injected to mice to determine liver uptake of lipid emulsions under electromicroscopy. Finally, THP1 cell line was used to examine the effects of DexSO4 on cell uptake of LCT and FO emulsions in vitro. Results Pre-injection of DexSO4 to mice decreased the FCR of both LCT and FO emulsions at 72.38% and 47.38% respectively, as compared to controls ( P =0.020 ). Electromicroscopy showed that pre-injection of DexS04 decreased the uptakes of LCT and FO emulsions by Kupffer cells and sinusoidal endothelial cells similarly. In hepatocytes, no lipid droplets existed in mice with LCT emulsion injection, whereas some lipid droplets were still shown in mice with FO emulsions but with less quantities compared to control mice.In vitro, addition of DexSO4 to medium decreased THP1 cell uptakes of LCT and FO emulsions ( P =0.003 and 0.008) by 30.74% and 41.60% respectively. However, no differences were found in the effects of DexS04 on cell uptakes between LCT and FO emulsions ( P =0.080). Conclusion Scavenger receptor A plays important roles in catabolism of lipid emulsions to some extent, and it's effects on FO emulsions may be less than LCT emulsions.
目的探讨性激素、年龄、月经模式变化对卵巢储备功能的预测价值。方法 2005年6月至2008年9月在首都医科大学附属复兴医院就诊的162例妇女作为研究对象,根据年龄及月经模式不同进行分组及性激素检测,对其中的84例进行卵巢卵泡密度计数。结果年龄增加,卵泡密度下降,≤29岁,~39岁,~44岁,~49岁及≥50岁各年龄组间比较,卵泡密度差异有统计学意义(P<0.05,P<0.01),年龄增至30岁后,卵泡密度仅为<30岁的1/2;40岁时,是<30岁组的1/4;45岁后几乎降至0。各组人抑制素B(INHB),卵泡刺激素(FSH)值比较,≤29岁,~39岁,~44岁,~49岁,≥50岁各组间有统计学意义(P<0.01)。按月经模式分组后,正常月经组,绝经过渡早期组、绝经过渡晚期组、绝经组之间血清FSH、抗苗勒管激素(AMH)与INHB水平比较有统计学意义(P<0.01)。四组卵泡密度计数分别为10.19(0.11~35.8),0.34(0~6.89),0(0~0.67),0(0~0.25)个/mm3,具有统计学意义(P<0.01)。女性月经周期由规律转向不规则时,卵泡密度接近0,卵泡储备几近耗竭。结论年龄与月经模式的变化提示卵巢储备功能下降,在各性激素变化中,AMH与INHB下降是女性生殖力减退的敏感指标。
目的:讨论甲型H1N1流感的紧急应对措施。方法:成立应急领导小组和应急救治小分队,按应急预案采取集中授课和实战演练的方式进行培训,成立感染控制组,切实有效的防止医院感染的暴发,并实行规范化管理措施。结果:接诊的100名医学观察病例中,均在24 h内完成了排查、诊断和治疗。结论:应急疫情下,规范化的工作程序、应急预案的培训和演练是工作取得成效的关键。
目的:探讨提高艾滋病晚期患者生存质量的方法.方法:回顾性总结86例艾滋病晚期患者住院期间采取姑息护理的效果.结果:实施姑息护理有利于降低艾滋病晚期患者的负性情绪,改善预后,提高生活质量.结论:通过姑息护理,使艾滋病晚期患者保持良好心态,增强战胜疾病信心,积极参与治疗,减轻痛苦不同程度的提高了生活质量.
Objective To investigate the different effects of dietary fish oil and flaxseed oil n-3 polyunsaturated fatty acids(n-3 PUFAs) on brain accretion of docosahexaenoic acid (DHA) and its distribution in brain phospholipids.Methods Three to 4 weeks old C57BL/6J female mice were fed 2 different types of diets containing n-3 PUFAs-flaxseed oil[rich in α-linolenic acid(LNA)] and fish oil (rich in DHA) diets for 12 weeks,with a n-3 PUFAs deficient diet derived from sunflower oil as controls.In n-3 PUFAs diets,the ratio of n-6/n-3 PUFAs was 1:1,with sunflower oil as source of n-6 PUFAs.Thin-layer chromatography was used to separate brain phospholipids-phosphatidylserine(PS),phosphatidylethanolamine(PE),phosphatidylinositol(PI) and phosphatidylcholine(PC).Fatty acids in plasma and brain phospholipids were analyzed by gas chromatography and expressed with as percent (%) of total fatty acids.Results The concentrations of DHA in mouse plasma and brain were increased after feeding with either fish oil or flaxseed oil n-3 PUFA diets.Compared with the flaxseed oil diet,in the fish oil diet the concentrations of DHA[(15.92±0.26)% vs (16.68±0.20)%] and total n-3 PUFAs (∑n-3 PUFAs) (21.24±0.33)% vs (22.56±0.14)% in the brain were increased and those of arachidonic acid(AA)[(10.28±0.05)% vs (9.02±0.05)%]and total n-6 PUFAs (∑n-6 PUFAs)[(13.33±0.11)% vs (11.05±0.12)%]were decreased,leading to decreased ratios of AA/DHA (0.65±0.01 vs 0.54±0.01) and ∑n-6/n-3 PUFAs (0.63±0.01 vs 0.51±0.01),although plasma concentration of DHA[(4.95±0.15)% vs (2.04±0.43)%]was reduced (Pa0.05).Analysis on fatty acid profiles in the 4 types of phospholipids showed that the fish oil diet reduced the ratios of AA/DHA (0.63±0.05 vs 0.52±0.03) and ∑n-6/n-3 PUFAs (0.66±0.10 vs 0.56±0.05) in PE (Pa0.05),compared with the flaxseed oil diet,although DHA concentrations in PS,PE,PI and PC were not affected by the fish oil diet feeding.Conclusions Pre-formed DHA in diets is better for brain accretion than that converted from LNA inside the body.Therefore,pre-formed DHA in diets more than those converted by LNA inside the body is better for brain accretion.
