
目的 探讨 2 型糖尿病(type 2 diabetes mellitus,T2DM)患者 24 h尿糖与胰岛素抵抗(insulin resis-tance,IR)的关系.方法 采用回顾性分析法,选取2016年6月—2019年1月上海交通大学附属第六人民医院收治的1 143例2型糖尿病患者,包括电子病案中病史、体格检查、血液生化指标检测及24 h尿糖检测等信息,根据24 h尿糖中位数分为低UGlu组572例和高UGlu组571例.比较两组性别、年龄、病程、血脂、血糖、胰岛素水平、糖化血红蛋白(HbA1c)、体质指数(BMI)、腰围(WC)等指标的水平,并计算胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β).结果 两组病程、高血压史、体质指数(BMI)、腰围(WC)、空腹胰岛素(INS)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)比较,差异无统计学意义(P>0.05).两组年龄、性别、吸烟史、饮酒史、糖化血红蛋白(HbA1c)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)、餐后 2 h血糖(2 hPG)、餐后 2 h胰岛素(2 h-INS)、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)、肾小球滤过率(eGFR)比较,差异有统计学意义(P<0.05).Spearman相关分析显示,24 h尿糖与HOMA-IR呈正相关(r=0.236,P<0.001),而与餐后2 h胰岛素水平呈负相关(r=-0.128,P<0.001).结果显示,24 h尿糖与FPG、2 hPG、HbA1c、TC、LDL-C、ECFR呈显著正相关;而与年龄、HOMA-β呈显著负相关.多元线性回归分析显示,校正混杂因素后FPG、2 hPG、HbA1c、2 h-INS、HOMA-IR、eGFR均为24 h尿糖的影响因素.结论 T2DM患者HOMA-IR、2 h-INS水平与24 h尿糖排泄量呈正相关.胰岛素抵抗程度可能是全天尿糖排泄的预测因素.
Objective To investigate the effectiveness of color Doppler ultrasound in the diagnosis of diabetic ne-phropathy.Methods 55 patients with diabetic nephropathy in Feixian People's Hospital from April 2022 to April 2023 were selected and divided into three groups according to urinary protein excretion rate.There were 17 patients in study group 1(urinary protein excretion rate<20 µg/min),18 patients in study group 2(urinary protein excretion rate urinary 20-<200 µg/min),20 patients in study group 3(urinary protein excretion rate≥200 µg/min),and 55 healthy medical examiners were selected as the reference group.All of enrolled patients were provided with color Doppler ul-trasound diagnosis.Analyzed the application effect of color Doppler ultrasound in the diagnosis of diabetic nephropa-thy.Results The end-diastolic minimum blood flow velocity and resistance index of study group 1,study group 2,and study group 3 were higher than those of the reference group,the differences was statistically significant(P<0.05).There was no significant difference in the minimum blood flow velocity and resistance index in study group 1,study group 2 and study group 3(P>0.05).Renal volume in study group 1 was higher than that of reference group,study group 2 and study group 3,and the difference was statistically significant(P<0.05).Conclusion Color Doppler ultra-sound is effective in the diagnosis of diabetic nephropathy and has high application value,which can assess the hemo-dynamic condition of the detector and provide important guidance for the clinical treatment of diabetic nephropathy,and the severity of the condition can be clarified through the observation of changes in renal blood flow,which can help the clinic intervene in a timely manner to avoid deterioration of the condition and improve the prognosis,which is worthy of promotion.
Objective To investigate the effect of insulin aspart combined with insulin detemir in the treatment of ges-tational diabetes mellitus.Methods 70 patients with gestational diabetes mellitus admitted to Beijing Yanhua Hospital from January 2020 to May 2023 were selected as study subjects and divided into two groups according to the coin toss method.There were 35 cases in the reference group and 35 cases in the study group,treated with insulin aspart and in-sulin aspart+insulin detemir,individually.The blood glucose levels(glycosylated hemoglobin level,fasting blood glu-cose level,and 2-hour postprandial blood glucose level),the time to reach the blood glucose standard(2-hour post-prandial blood glucose standard,fasting blood glucose standard,and the time to reach the standard at the same time)were compared between the two groups of gestational diabetes mellitus,and the adverse maternal and infant outcomes of the two groups were also compared statistically.Results Before treatment,compared with the blood glucose levels between the two groups,and the difference was not statistically significant(P>0.05).After treatment,the levels of the blood glucose of patients in the study group were lower than those of the reference group,and the difference was statis-tically significant(P<0.05).The postprandial 2-hour blood glucose standardization time,fasting blood glucose stan-dardization time,and simultaneous standardization time unfolding of the study group were shorter than those of the ref-erence group,and the difference was statistically significant(P<0.05).The total incidence of adverse maternal and in-fant outcomes of the study group were lower than that of reference group,and the difference was statistically signifi-cant(P<0.05).Conclusion Clinical treatment of gestational diabetes mellitus patients with insulin aspart + insulin de-temir can effectively promote the reduction of blood glucose levels,shortening of the time to achieve blood glucose standards and the improvement of maternal and infant adverse outcomes.
Objective To investigate and analyze the influencing factors of diabetic retinopathy(DR)in patients with type 2 diabetes mellitus.Methods The clinical data of a total of 300 patients with type 2 diabetes in Songxian Hospi-tal of Traditional Chinese Medicine of Luoyang City from July 2022 to July 2023 were retrospectively analyzed.114 patients with type 2 diabetes with retinopathy were selected as the study group,and 186 patients without retinopathy were selected as the control group.To analyze the influencing factors of retinopathy in type 2 diabetic patients.Results Among the 300 patients with type 2 diabetes included in this study,114 patients developed retinopathy,with an incidence rate of 38.00%.There were statistically significant differences in age,age of onset of diabetes,duration of diabetes,positive of urinary protein,fasting blood glucose,2-hour postmeal blood glucose,glycated hemoglobin in retinopathy group compared with those in non-retinopathy group(P<0.05).Urinary protein positive,age,duration of diabetes,age of onset of diabetes,fasting blood glucose,growth hormone,insulin-like growth factor-I,C-peptide,retinol-binding protein4,neutrophil/lymphocyte ratio,platelet/lymphocyte ratio were all independent influencing fac-tors for the occurrence of retinopathy in patients with type 2 diabetes(P<0.05).Age,positive urinary protein,duration of diabetes,fasting blood glucose,growth hormone,insulin-like growth factor-I,retinol-binding protein-4,neutrophil/lymphocyte ratio,and platelet/lymphocyte ratio were all risk factors for retinopathy in type 2 diabetes patients.Age of onset of diabetes and C-peptide are protective factors for retinopathy in type 2 diabetes.Conclusion Patients with type 2 diabetes have a high incidence of retinopathy,and clinical attention should be focused on high-risk patients with type 2 diabetes,such as patients with positive urinary protein,young age,long course of diabetes,early onset of diabetes,and elevated fasting blood glucose.In order to ensure the outcome of patients,attention should be paid to the control of blood sugar and blood lipids to improve the prognosis of patients.
Objective To investigate the association between 25 hydroxyvitamin D3 level[(25(OH)D3]and type 2 dia-betes mellitus nephropathy.Methods 120 type 2 diabetes mellitus nephropathy patients hospitalized in the Depart-ment of Endocrinology,Luohu People's Hospital,Shenzhen from June 2019 to June 2022 were selected and divided into the normal group[30 ng/mL≤25(OH)D3≤100 ng/mL](n=31),the insufficient group[20 ng/mL≤25(OH)D3<30 ng/mL](n=44),and the deficient group[25(OH)D3<20 ng/mL](n=45)based on the 25(OH)D3 level.Comparative analy-ses of cystatin C,glycosylated hemoglobin,and urinary albumin creatinine ratio(UACR)were performed in the three groups,respectively.Results Cystatin C levels in three groups were compared,and the difference was statistically sig-nificant(P<0.05).The difference was statistically significant of total cholesterol level among the three groups(P<0.05).UACR in deficiency group and deficiency group was higher than that in normal group,and the difference was statistically significant(P<0.05).Analysis of correlation results showed that 25(OH)D3 was negatively correlated with UACR values(r=-0.640,P<0.05)and 25(OH)D3 was negatively correlated with cystatin C(r=-0.587,P<0.05).Conclusion 25 hydroxyvitamin D3 deficiency may be related to the occurrence and development of diabetes mellitus nephropathy,and cystatin C may reflect the severity of diabetic nephropathy to some extent.
Objective To explore the clinical effect of insulin + metformin after completing the treatment of pediatric type 1 diabetes mellitus patients.Methods 60 children with type 1 diabetes admitted to Linyi Central Hospital from May 2020 to December 2022 were selected as the study objects,and were divided into control group and study group according to random number table method,with 30 cases in each group.The control group was treated with insulin alone,and the study group was treated with metformin in addition to the control group.Blood glucose control,length of hospital stay,daily insulin dosage and incidence of adverse reactions were compared between the two groups.Results Before treatment,there was no statistically significant difference in blood glucose control between the study group and the control group(P>0.05).After treatment,the gglycosylated hemoglobin,fasting plasma glucose and fluctuation range of blood glucose in the study group were reduced more significantly than those in the control group,and the dif-ference was statistically significant(P<0.05).There was no statistically significant difference in hospitalization time be-tween the study group and the control group(P>0.05).The decrease of daily insulin dosage in the study group was sig-nificantly higher than that in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of ketoacidosis between the study group and the control group(P>0.05).The incidence of hypoglycemia events in the study group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion During the treatment of pediatric patients with type 1 diabetes mellitus,the combined application of insulin and metformin can achieve significant results in control-ling blood glucose levels and reducing the incidence of hypoglycemic events and improve the efficacy of treatment.
Objective To explore the effect of blood glucose control in root canal treatment in patients with diabetes mellitus.Methods A total of 100 diabetic patients with periapical periodontitis who underwent root canal treatment in Linyi Geriatric Hospital from January 2022 to January 2023 were selected as the study objects and divided into obser-vation group and control group by lot,with 50 cases in each group.Root canal therapy was performed in both groups.The control group was treated with oral hypoglycemic agents,and the observation group was treated with insulin injec-tion.Blood glucose index,blood glucose fluctuation index and therapeutic effect were compared between the two groups.Results After treatment,the blood glucose levels in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the indexes of blood glucose fluctua-tion decreased in both groups,and the observation group was lower than the control group,the difference was statisti-cally significant(P<0.05).The effective rate of observation group(94.00%)was higher than that of control group(76.00%),and the difference was statistically significant(χ2=6.353,P=0.012).Conclusion Insulin injection can effec-tively reduce blood sugar and improve the effect of root canal treatment for diabetic patients with periapical periodonti-tis.
Objective To investigate and analyze the effects of Chaihu Guizhi Ganjiang decoction on blood lipids in patients with type 2 diabetes mellitus with bile-heat and spleen-cold type identified by traditional Chinese medicine.Methods A total of 60 patients with type 2 diabetes were selected from Shuangyashan Hospital of Traditional Chinese Medicine from January 2021 to December 2022.Random number table method was used to divide Chaihugui Zhigan Jiangtang combined with metformin group(treatment group)and metformin group(control group),with 30 cases in each group.The indexes of blood lipid and blood sugar after treatment were compared between the two groups.Results After treatment,the indexes of blood lipid and blood sugar in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Traditional Chinese medicine compound Chaihu Guizhi Ganjiang decoction can effectively reduce blood lipids and blood sugar in patients with type 2 diabetes,has the advantages of multi-target and high efficiency.
Objective To explore the clinical effects of different insulin use methods in the treatment of diabetic keto-acidosis in children.Methods Retrospectively analysed the clinical data of 60 children with diabetes ketoacidosis in the affiliated hospital of Putian University from February 2020 to February 2023.According to the method of insulin use,they were divided into continuous intravenous drip group and multiple subcutaneous injection group,with 30 cases in each group.compared of glucose metabolism indicators,pH value,serum cytokine levels,incidence of ad-verse events,and clinical indicators between the two groups.Results After medication,the levels of glycated hemoglo-bin,fasting blood glucose,2-hour postprandial blood glucose,serum interleukin-6,and interleukin-12 in the continu-ous intravenous drip group were lower than those in the multiple subcutaneous injection group,and the pH value was higher than that in the multiple subcutaneous injection group,the difference was statistically significant(P<0.05).The incidence of adverse events in the continuous intravenous drip group(3.33%)was lower than that in the multiple sub-cutaneous injections group(26.67%),the difference was statistically significant(χ2=4.706,P<0.05).Conclusion Intra-venous continuous drip insulin treatment of diabetic ketoacidosis in children has a better clinical effect than multiple subcutaneous injections of insulin,and is more capable of lowering glucose metabolism indexes and serum cytokine levels in children,elevating pH,and reducing the occurrence of daverse events.
Objective To investigate the cognitive status of non-insulin-dependent diabetes mellitus patients on hypo-glycemia,and to provide scientific basis for medical personnel to accurately formulate health education plan and implement health education measures for this type of diabetes mellitus patients.Methods A self-designed question-naire on hypoglycemia awareness of non-insulin-dependent diabetes mellitus patients was used to conduct a face-to-face questionnaire survey on 84 non-insulin-dependent diabetes mellitus patients hospitalized in the Department of Endocrinology of the First Affiliated Hospital of the University of Traditional Chinese Medicine from June to August 2020,of whom 80 responded to the questionnaire with valid responses.Results 80 patients had knowledge related to hypoglycemia,and relatively low cognition.Gender,educational level,and medical history of hypoglycemia are the in-fluencing factors of hypoglycemia cognition in patients with type 2 diabetes(P<0.05).Conclusion Clinically,a compre-hensive and personalized health education program should be developed for diabetic patients to educate them about the prevention of hypoglycemia,clinical manifestations,therapeutic care,emergency treatment,and other aspects of health knowledge,in order to avoid or reduce the incidence of hypoglycemia,and to achieve timely recognition and ef-fective measures after the occurrence of hypoglycemia,to reduce the harm caused by hypoglycemia to patients,and to improve the quality of life of patients.
Objective To explore the clinical application effect of Dapagliflozin in the treatment of patients with heart failure(HF)combined with diabetes mellitus type 2(T2DM).Methods A total of 82 HF patients with T2DM treated in Quanzhou First Hospital from July to December 2022 were selected as the study objects,and were divided into study group and control group according to random number table method,with 41 cases in each group.The control group was treated with conventional treatment,and the study group was treated with dagaglizin on the basis of it.The differ-ence in efficacy between the two groups after 6 months of treatment was compared,the changes in blood biochemical indexes,blood glucose indexes,cardiac function indexes and HF symptoms of the two groups before and after 6 months of treatment were analyzed,and the incidence of adverse reactions in all patients during the course of treat-ment was statistically analyzed.Results After treatment,the symptom score of heart failure in the control group was compared with before treatment,and the difference was not statistically significant(P>0.05).The scores of physical symptoms and psychological symptoms in the Chinese version of the Memorial HF Symptom assessment scale in both groups were lower than before treatment,and the scores of heart failure symptoms in the study group were lower than before treatment,the differences were statistically significant(P<0.05).After treatment,fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin and N-terminal B-type brain natriuretic peptide were lower than be-fore treatment,and the differences were statistically significant(P<0.05).The fasting blood glucose,2-hour postmeal blood glucose,glycated hemoglobin,N-terminal B-type brain natriuretic peptide and scores of Chinese version of Me-morial HF Symptom Assessment Scale in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).Insulin,left ventricular ejection fraction and cardiac output in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).During treat-ment,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Dapagliflozin can safely and effectively improve the symptoms of patients with HF combined with T2DM,and has positive significance on their prognosis recovery.
Objective To investigate the efficacy of dapagliflozin and empagliflozin combined with sacubitril valsartan sodium tablets respectively in diabetes mellitus with heart failure(HF).Methods Sixty patients with diabetes mellitus and HF treated in Fuzhou Changle District Hospital from July 2020 to August 2022 were selected and divided into ob-servation group(n=30)and control group(n=30)by random number table method.Both groups were given sacubactril and valsartan sodium tablets,control group combined with dagliprazin,observation group combined with engliprazin.Blood glucose parameters,cardiac function parameters,all-cause mortality,HF readmission rate and drug-related ad-verse reactions after 6 months of follow-up were compared between the two groups before and after treatment.Results There was no statistically significant difference in the level of cardiac function parameters between the two groups af-ter treatment(P>0.05).After treatment,all blood glucose parameters in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant differ-ence in the all-cause mortality and the total incidence of drug-related adverse reactions between the two groups(P>0.05).The HF readmission rate of observation group was lower than that of control group,and the difference was statis-tically significant(P<0.05).Conclusion Compared with dapagliflozin,the application of engeletin combined with sacu-bitril valsartan sodium tablets in patients with diabetes mellitus with HF is clinically effective,which can effectively improve the level of blood glucose control and cardiac function indexes of the patients,and reduce the rate of HF read-mission,and the safety of the combined use of medication is high.
Objective To investigate the expression of serum amyloid A(SAA),25-dihydroxyvitamin D3 and its clini-cal value in patients with type 2 diabetes mellitus.Methods 240 patients with type 2 diabetes diagnosed and treated by Beijing Xuanwu Hospital of Traditional Chinese Medicine from August 2022 to May 2023 were selected as the ob-servation group,and 240 healthy people who came to our hospital for physical examination at the same time were se-lected as the control group.Detected the levels of SAA,25(OH)D3,various blood glucose and lipid indicators in two groups,and used Pearson correlation analysis to analyze the relationship between serum SAA,25(OH)D3 levels and various blood glucose and lipid indicators.Results The levels of serum SAA,total cholesterol,triacylglycerol,low-density lipoprotein cholesterol,fasting blood glucose,and fasting insulin in the observation group were higher than those in the control group,while the levels of serum 25(OH)D3 and high-density lipoprotein cholesterol were lower than those in the control group,the difference was statistically significant(P<0.05).The serum SAA level was posi-tively correlated with total cholesterol,triglycerides,low-density lipoprotein cholesterol,fasting blood glucose,and fasting insulin levels, while negatively correlated with high-density lipoprotein cholesterol levels (P<0.05). Serum 25 (OH) D3 levels was positively correlated with high-density lipoprotein cholesterol levels, but negatively correlated with total cholesterol, triglycerides, low-density lipoprotein cholesterol, fasting blood glucose, and fasting insulin levels (P< 0.05).Conclusion Serum SAA high expression and 25 (OH) D3 low expression in patients with type 2 diabetes mellitus have strong correlation with their lipid metabolism disorders and blood glucose, and can be used as a reference index for monitoring and evaluating patients' conditions.
Objective To study the effect of health education on treatment compliance and blood glucose level in pa-tients with chronic periodontitis combined with diabetes mellitus.Methods 80 patients with chronic periodontitis com-bined with diabetes mellitus admitted to Quanzhou First Hospital from August 2021 to August 2022 were selected as the research object,and they were divided into two groups by the principle of randomized numerical table method,with 40 cases in each of the control group and the observation group.The nursing measures of the two groups were conventional nursing care,conventional nursing care + health education,and the treatment compliance and blood glu-cose level fluctuation of the two groups were compared.Results The adherence scores of patients in the observation group were greater than those of the control group in oral management,dietary management,blood glucose manage-ment and medication compliance,and the difference was statistically significant(P<0.05).The blood glucose indica-tors of patients in both groups after 1 week of care was smaller than that before care,and the fasting plasma glucose,2-hour postprandial blood glucose and glycosylated hemoglobin of the observation group after 1 week of care were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The imple-mentation of health education for patients with chronic periodontitis combined with diabetes mellitus is more condu-cive to improving patients'treatment compliance,and is conducive to lowering blood glucose values.
Objective To investigate the influencing factors of microangiopathy in type 2 diabetes mellitus.Methods From January 2021 to December 2022, 210 patients with type 2 diabetes admitted to Longyan People 's Hospital were randomly selected as the research objects. According to the microangiopathy, they were divided into control group (no microangiopathy, 105 cases) and observation group (microangiopathy group, 105 cases). The gender, age, course of dis-ease, blood pressure, height, body mass index, blood glucose, blood lipid and blood routine of the patients were col-lected, and the influencing factors of microangiopathy were analyzed. Results Age, course of disease, systolic blood pressure, serum creatinine, glycosylated hemoglobin, total cholesterol, triglyceride, low density lipoprotein cholesterol, neutrophil count, lymphocyte count and neutrophil-lymphocyte ratio (NLR) were all risk factors for microvascular dis-ease in patients with type 2 diabetes (P<0.05).Conclusion Risk factors for microangiopathy in patients with type 2 diabetes mellitus include age, disease duration, systolic blood pressure, creatinine, glycosylated hemoglobin, total cho-lesterol, triacylglycerol, low-density lipoprotein cholesterol, neutrophil count, lymphocyte count, and NLR, and there-fore patients need to be effectively observed for changes in the relevant indexes and take preventive measures against microangiopathy.
Objective To analyze the value of using α-lipoic acid injection in the treatment of geriatric diabetic pe-ripheral neuropathy.Methods Sixty-four elderly patients with diabetic peripheral neuropathy diagnosed and treated in Haian Hospital of Traditional Chinese Medicine from April 2021 to March 2023 were selected and divided by lottery method.32 patients in the control group were given conventional treatment,and 32 patients in the treatment group were given α-lipoic acid injection.The therapeutic effect,nerve conduction velocity,islet function and adverse reac-tions of the two groups were compared.Results The effective rate of the control group was lower than that of the treat-ment group,the difference was statistically significant(P<0.05).After treatment,nerve conduction velocity and islet function in the treatment group were better than those in the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in control group was higher than that in treatment group,and the differ-ence was statistically significant(P<0.05).Conclusion The application of α-lipoic acid injection in the treatment of diabetic peripheral neuropathy in the elderly can not only improve the therapeutic effect and safety,but also help to enhance the nerve conduction velocity and improve the pancreatic islet function,which is worthy of wide application.
Objective To explore the application of transverse tibial bone transport combined with negative pressure drainage in diabetic foot.The effects of ankle brachial index(ABI),dorsalis pedis artery blood flow velocity,foot skin temperature,Michigan Neuropathy Screening Scale(MNSI),serum basic fibroblast growth factor(bFGF)and epider-mal growth factor(EGF)on diabetic foot.Methods From February 2021 to December 2022,92 inpatients with dia-betic foot in the Department of Orthopedics,Macheng People's Hospital were selected as the research objects.Ac-cording to different treatment methods,they were divided into control group and observation group,with 46 cases in each group.ABI,dorsalis pedis artery blood flow velocity,foot skin temperature,MNSI score,bFGF and EGF were compared between the two groups.Results ABI(0.96±0.12),dorsalis pedis artery blood flow velocity(33.12±3.61)cm/s,foot skin temperature(30.82±1.55)°,MNSI score,bFGF(173.52±32.61)pg/mL,EGF(714.85±95.65)pg/mL in the ob-servation group were significantly better than those in the control group,and the differences were statistically signifi-cant(P<0.05).Conclusion The use of tibial transverse bone carrying combined with negative pressure drainage can improve the blood flow running speed,skin temperature,promote skin growth,and reduce the MNSI score of the pa-tients.
Objective To explore the diagnosis and treatment strategy of diabetes acute abdominal pain combined with ketoacidosis,and provide ideas for the early diagnosis of diabetes acute abdominal pain combined with diabetes ketoacidosis(DKA).Methods 80 cases of diabetes patients with acute abdominal pain treated in Datian County Gen-eral Hospital from June 2021 to June 2022 were selected.They were divided into DKA group(46 cases)and non DKA group(34 cases)according to whether DKA was combined.At the same time,the two groups of patients were treated with fluid infusion,glucose lowering,ketone lowering,and potassium supplementation.The positive rate of peritonitis and imaging examination,as well as symptom relief and death 1,2,and 3 d after treatment,were observed in the two groups.Results The positive rates of peritonitis and imaging examination in the DKA group were significantly higher than those in the non DKA group,and the difference was statistically significant(P<0.05).The symptom relief of pa-tients in the non DKA group was significantly better than that in the DKA group 1,2 and 3 d after treatment,the dif-ference was statistically significant(P<0.05),while the mortality of diabetes patients in the two groups were comoared,the differnece was not statistically significant(P>0.05).Conclusion Patients with diabetic acute abdominal pain com-bined with ketoacidosis have a high rate of positive peritonitis signs and imaging examinations.Clinicians should strengthen the knowledge of this disease in diagnosis and treatment,be alert to early symptoms,and check the rel-evant indexes early to identify the occurrence of DKA.
Objective To evaluate the clinical value of insulin glargine and acarbose combined therapy in elderly pa-tients with type 2 diabetes mellitus.Methods 86 elderly patients with type 2 diabetes treated in Mengyin County People's Hospital from March 2021 to March 2023 were selected.According to the random number table,the patients were divided into two groups,with 43 patients in each group.The control group was treated with insulin glargine,while the study group was treated with insulin glargine and acarbose.The efficacy,blood glucose and intestinal hor-mone indexes and adverse reactions were compared between the two groups.Results After treatment,the total effective rate of the study group 95.35%was better than that of the control group 79.06%,and the difference was statistically significant(P<0.05).Fasting plasma-glucose,glycated hemoglobin and 2 h postprandial blood glucose in the study group were lower than those in the control group,the difference was statistically significant(P<0.05).The fasting C-peptide and glucagon-like peptide-1 indexes in the study group were higher than those in the control group,but the serum gastric inhibitory peptide indexes were lower than those in the control group,the difference was statistically sig-nificant(P<0.05).There was no statistical significance in adverse reaction rate of 6.97%in the study group and 9.30%in the control group(P>0.05).Conclusion Insulin glargine and acarbose can enhance the clinical efficacy,effectively improve the blood glucose index,and greatly reduce the incidence of adverse reactions in the treatment of type 2 dia-betes mellitus.
Objective To analyze the impact of risk care on risk time and quality of life of diabetic patients treated in-tensively with insulin pump.Methods 70 diabetic patients admitted to Xinyi People's Hospital between January 2020 to December 2022 were collected and divided into two groups of 35 cases each according to equal proportions using the double-blind method.The control group was administered conventional care and the observation group was admin-istered risk care.Comparison of the two groups of insulin pump use risk awareness ability,insulin pump intensive treatment during the risk time and patient quality of life.Results Before nursing,there was no statistically significant difference in the insulin pump risk awareness ability score between the two groups(P>0.05).After care,the risk awareness ability score of the observation group was higher than that of the control group,the difference was statisti-cally significant(P<0.05).The time to occurrence of all adverse events was shorter in the observation group than in the control group,the difference was statistically significant(P<0.05).All quality of life scores in the observation group were(19.78±3.07)points,(20.74±2.17)points,(20.56±2.03)points,(21.42±2.96)points,which were higher than those in the control group(15.54±2.14)points,(16.45±2.30)points,(16.33±1.50)points,(18.55±1.78)points,the dif-ference was statistically significant(t=6.703,8.026,9.915,4.916,P<0.05).Conclusion Insulin pump risk care can re-duce the risk disposal time of diabetic patients,improve the awareness of risk,improve the quality of life,which can be promoted.
