Objective To explore the clinical value of serum procalcitonin (PCT) in the diagnosis of ulcerative colitis (UC).Methods One hundred and eighty cases of patients with active UC were enrolled and categorized as observation group according to the Truelove-Witts classification criteria.The control group consisted of 180 healthy people for physical examinations during the same period.Characteristics of serum PCT levels,C-reactive protein (CRP),erythrocyte sedimentation rate (ESR),white blood cell count in both groups were comparatively analyzed.Results Serum PCT levels,CRP levels,ESR,WBC in patients with active UC were significantly higher than those in healthy volunteers (P<0.01).The four indexes in severe UC patients were significantly higher than those in mild-to-moderate UC patients (P<0.01).Serum PCT level in UC patients was positively correlated with the disease severity (P<0.01).The area under the ROC curve of serum PCT level was significantly superior to that of white blood cell count and inferior to those of CRP level and ESR (P<0.01).Conclusions Serum PCT level may be used as the predictive indicator for the severity of UC and has certain instructive value in the diagnosis of UC.
Objective To analyze the clinical features of Helicobacter pylori (Hp) infection in the patients with gastrointestinal diseases .Methods A total of 8936 biopsy samples of gastrointestinal mucosa was collected from the patients with gastrointestinal diseases and Hp was detected .Results Hp infection rate of the patients with gastrointestinal diseases was 44.85% .Hp infection rate of male patients was higher than that of female(48.14% vs .39.75% )(P<0 .01) .Hp infection rate decreased as age increased(P<0 .01) .Hp infection rate of the patients aged more than 30 years with peptic ulcer was higher than those with superficial gastritis ,atrophic gastritis and gastric carcinoma (P<0 .01) . Conclusion Hp infection rate of the patients with gastrointestinal diseases ,especially male and with peptic ulcer ,is higher .
目的:观察中医药辨证治疗功能性消化不良的临床疗效. 方法:100例功能性消化不良患者随机分为治疗组50例、对照组30例与安慰剂组20例. 治疗组采用中药辨证论治,对照组予莫沙必利等常规西药治疗,安慰剂组予中药配方颗粒安慰剂.3组均服药4周后观察并比较临床疗效及主要症状改善情况.结果:治疗组、对照组和安慰剂组中医证候总有效率分别为94.0%、80.0%、45.0%,治疗组疗效明显优于对照组及安慰剂组(P<0.05). 在主要症状方面,治疗组与对照组治疗后上腹痛、上腹烧灼感、餐后饱胀不适及早饱感评分均明显低于治疗前(P<0.01),治疗组患者治疗后上述症状评分明显低于对照组与安慰剂组(P<0.01),上腹痛、上腹烧灼感评分明显低于对照组(P<0.05,P<0.01). 治疗结束1个月后随访,治疗组各症状评分稳定,对照组餐后饱胀不适及早饱感评分明显高于治疗后(P<0.01).结论:辨证治疗功能性消化不良具有较好的近远期临床疗效.
Objective: Through observing the influence of Astragaloside IV(AIV) and β-elemene on immunologic function of dentritic cells(DCs),we will discuss whether AIV and β-elemene coudle enhance the immunologic function of the organism through enhancing the function of DCs.It will provide the experiment basis of anti-tumor mechanism of AIV and β-elemene.Methods: D2SC cells were divided into five groups: control group,LPS group,AIV group,β-elemene group,AIV+β-elemene group and then treated with AIV and β-elemene in vitro.The surface molecules-MHCⅡ,MHCⅠ,CD40,CD80,CD86 and IL-6 and IL-12 were detected by methods of FCM and ELISA. Results: The level of surface molecules of D2SC cells(MHCⅡ、MHCⅠ、CD40、CD80、CD86) in AIV group or β-elemene group was higher than that in control group or LPS group,and the level of MHCⅡ in AIV group was obviously higher than that in the control group(P0.05),the levels of CD80、CD86 in β-elemene group were obviously higher than those in LPS group(P0.05).The levels of MHCⅡ,CD40 in AIV and β-elemene group were obviously higher than those in LPS group(P0.05) and the levels of MHCⅠ,CD86 were obviously higher than those in control group(P0.05).IL-6 or IL-12 level in the supernatant of D2SC cells of AIV or β-elemene group were higher than those in control group or LPS group.And the level of IL-12 and IL-6 in AIV or β-elemene group were obviously higher than those in the control group(P0.05).The levels of IL-12 and IL-6 in AIV and β-elemene group were obviously higher than those in LPS group(P0.05). Conclusion: AIV,β-elemene or combination of them can enhance the level of MHC cells and co-stimulatory factor in the surface of DCs.It also can promote the secretion of cytokine IL-12 and IL-6.So it can present antigen and induced the answer of T cells.The results supply immunological mechanisms for the method of Yiqihuoxue which combine AIV and β-elemene treating gastrointestina cancer.
Objective Through experiments in vitro,we explored the role of Chinese medicine monomer-Astragaloside Ⅳ with DC vaccine in the body immunologic function.Methods Peripheral blood mononuclear cells (PBMCs)from healthy volunteers were isolated,cultured and generated in vitro,pulsed with tumor antigen from SGC7901 gastric carcinoma cell lysates,produced DC vaccine.Observe T-cell proliferation responses stimulated by DC vaccine with AⅣ group,DC vaccine group and AⅣ group respectively,and the anti-tumor effects on SGC7901 cells in vitro.Results ①The T-cell proliferation rate of DC vaccine with AⅣ group and DC vaccine group were significantly higher than AⅣ group and T-cell group(negative control group)(P=0.000).The stimulating efficacy on T-cell proliferation of DC vaccine with AⅣ group was higher than that of DC vaccine group(S/R 1∶5,1∶10,1∶50,1∶100,P=0.013,0.014,0.017,0.019).Compared with T-cell group,the T-cell proliferation rate of AⅣ group had no statistically significance(P=0.185).②The killing rate of effector cells actived by DC vaccine with AⅣ group and DC vaccine group against SGC7901 gastric carcinoma cells were higher than that of AⅣ group and T-cell group(P=0.000).The Killing power of DC vaccine with AⅣ group was stronger than that of DC vaccine(E/T 5∶1,10∶1,20∶1,50∶1,P=0.023,0.012,0.016,0.011);while the group of AⅣ group and T-cell group cannot killing tumor cells.Both had no statistically significance(P=0.267).Conclusion AⅣ can stimulate T-cell proliferation and enhance the activity of killing tumor cells by DC,which induced specific antitumor response against stomach carcinoma cells effectively.
This study was purposed to detect the expression level of human endothelial cell tissue factor (hECTF) and concentration of IL-2, and to investigate the alterations of hECTF and IL-2 after using immunosuppressive agent (cyclosporin, CsA) and explore the significance of endothelial cell (EC) lesion and abnormal expression of tissue factor (TF) in GVHD. Human endothelial cells and allogeneic lymphocytes were mixed and cultured as well as were cocultured with CsA for 4-6 hours in vitro, then the expression level of hECTF was detected by flow cytometry and RT-PCR, the concentration of IL-2 in supernatant was assayed by ELISA. The experiment was divided into 4 groups: 1st group--non-mixed-cultured group (negative control group), 2nd group - mixed-cultured group (positive control group), 3rd group - mixed-cocultured group with 1 µg/ml CsA and 4th group--mixed-cocultured group with 2 µg/ml CsA. The results showed that as compared with non-mixed-cultured group (negative control group), the expression level of hECTF and concentration of IL-2 in another 3 groups significantly increased (p<0.01), while as compared with positive control group, the expression level of hECTF and concentration of IL-2 in cocultured groups with CsA both decreased (p<0.01). It is concluded that the lesion of EC and abnormal expression of TF play a crucial role in GVHD, among which the high expression of TF after being stimulated by donor's lymphocytes may be the key step for occurrence and progression of GVHD.
