<正>资料与方法一般资料随机选择住院及门诊心电图及24小时动态心电图确诊的各类早搏患者124例,男82例,女42例,年龄22~72岁,平均43±9岁。其中冠心病58例,高心病28例,肺心病15例,二尖瓣脱垂7例,原因不明者16例。临床类型:室性早搏68例(其中多源多发性室早38例,二联率室早14例,RONT10例,偶发室早6例),房性早搏42例,交界性早搏14例。
随着社会上老年人口逐步增加,老年病的诊断和治疗越来越受到全社会的普遍重视,尤其是老年人常患的骨质疏松症更加成为各国学者都不断研究的公共健康问题.我们通过对几种血、尿生化指标的检测,探讨它们与老年性骨质疏松症的关系,找出能够简便、快捷、准确地反映骨代谢状况的生化指标,为临床治疗及预防老年性骨质疏松症提供依据.

目前,偏头痛的发病机理尚不清楚,可能与多种因素有关,许多研究已经证实偏头痛可能与血小板活化功能异常有关[1].人们还发现调节脑血管舒缩状态的神经肽P物质(SP)亦可能参与偏头痛的发病[2].本研究分别应用流式细胞技术及放射免疫方法测定偏头痛患者血小板表面活性蛋白血小板颗粒膜蛋白(CD63)、溶酶体膜蛋白(CD62P)及血浆P物质,旨在探讨三者在偏头痛发病机理中的作用.
Objective:To observe the effects of acute angiotensin Ⅱ type 1 (AT1) receptor antagonism and angiotensin converting enzyme inhibition (ACEI) on plasma fibrinolytic parameters in patients with heart failure.Method:Thirty patients with heart failure received peidolapril 10 mg and losartan 50 mg on 2 separate days in a single-blind randomind crossover design. Plasma tissue plasmingon activator (t-PA), plasmingon activator inhibitor (PAI-1), endothelin-1 (ET-I) and angiotensin Ⅱ (Ang Ⅱ ) were measured at baseline and 6 hours after treatment. Result:Acute administration of losartan and peidolapril increased plasma t-PA, and reduced PAI-1,ET-1 and Ang Ⅱ. These changes were more marked during losartan treatment (P < 0. 05) than during peidolapril treatment ( P >0. 05). Conclusion:Acute AT1 antagonism in patients with heart failure is associated with a significant improvement in plasma fibrinolytic parameters that is greater than during ACEI treatment.These benefical effects of AT1 and ACEI would therefore appear to be mediated principally through suppression of angiotensin Ⅱ.
目的:探讨血小板表面活性蛋白血小板颗粒膜蛋白(CD63)、溶酶体膜蛋白(CD62P)、血浆纤溶酶原激活抑制物(PAI-1)及C-反应蛋白(C-RP)在不稳定心绞痛(UAP)发生及发展中的作用,及葛根素对不稳定心绞痛患者CD63,CD62P,PAI-1及C-RP的影响。方法:59例UAP患者随机分成葛根组32例及氯塞匹定组27例,比照观测治疗4周前后血小板CD63,CD62P及血浆PAI-1,C-RP水平的变化。结果:与健康及稳定性心绞痛(SAP)对照组比较,UAP患者血小板CD63,CD62P及血浆PAI-1,C-RP水平明显升高(P<0.05或P<0.01);随着不稳定心绞痛Braunwald分级的加重,血小板CD63,CD62P及血浆PAI-1,C-RP水平亦增加;治疗6周后,葛根素组及氯塞匹定组CD63,CD62P,PAI-1及C-RP测定值分别显著低于治疗前水平(P<0.05或P<0.01);而治疗后两组间上述各指标水平无统计学差异(P>0.05)。结论:血小板活化及纤溶机制异常及炎症反应物这三者在UAP发生及发展过程中起着重要的作用;葛根素及氯塞匹定抗血小板活性、改善纤溶活性及减轻炎症反应的突出作用对预防急性冠状动脉综合征的发生和发展有极其重要的临床意义。
To explore the action of platelet surface activity protein (platelet granule membrane protein CD63 and lysosome membrane protein CD62P), plasminogen activator inhibitor (PAI-1) and C-reactive protein (C-RP) in occurrence and development of unstable angina pectoris (UAP), and the effect of Puerarin on them.Patients with UAP were randomly divided into the treated group (32 cases, treated with Puerarin) and the control group (27 cases, treated with Ticlid), the therapeutic course was 4 weeks for both groups. Changes of CD63, CD62P, PAI-1 and C-RP before and after treatment were observed.The levels of CD63, CD62P, PAI-1 and C-RP were higher in UAP patients than those in normal subjects and in the patients with stable angina pectoris (P < 0.05 or P < 0.01). These parameters were increased along with the aggravating of patients in Braunwald's degree. After 4-week treatment, the above-mentioned parameters lowered in both groups (P < 0.05 or P < 0.01), and comparison between the two groups showed no significant difference statistically.Platelet activation, plasminogen activator and C-RP play important roles in the occurrence and development of UAP. The obvious effect of Puerarin in anti-platelet activation, improving plasminogen activator and relieving inflammatory reaction was of great importance in preventing the occurrence and development of acute coronary syndrome clinically.
众所周知,高血压导致的心、脑、肾及眼底重要靶器官损害正成为老年人生活质量下降,甚至致残、致死的危险因素,因此,寻求高效安全对靶器官有保护作用的抗高血压药是当务之急.血管紧张素Ⅱ(AngⅡ)受体拮抗剂是继血管紧张素转换酶抑制剂(ACEI)之后新的一类抗高血压药物,对常用降压药物疗效不理想的轻、中度高血压患者有较好效果,且降压作用安全可靠,副作用小[1,2],更适合于老年收缩期高血压(ISH)患者.笔者于1998年10月~1999年10月选用AngⅡ受体拮抗剂氯沙坦钾治疗ISH患者60例,旨在探讨氯沙坦钾对ISH的降压疗效及对重要靶器官(心、肾、眼)的影响.
恶性肿瘤是威胁人类健康和生命的最主要的疾病之一.目前对恶性肿瘤的检测手段多种多样,但都不同程度地存在着缺点,如:CT、核磁共振等大型仪器的检查,虽然比较直观、准确,但肿瘤过小就不易查出,不利于早期发现、早期诊断.
目的:为探讨原发性高血压(EH)患者压力反射敏感性(BRS)与部分内分泌激素之间的相关性.方法:应用无创被动倾斜方法估测43例EH患者及30例正常人在60°被动倾斜前后血压、心率变化,心电图上第30个心动周期与第15个心动周期的R-R间期的比值(30∶15比值)及BRS值;同时测定部分患者(20例)及正常人(10例)静息状态下血浆儿茶酚胺(CA)、心钠素(ANP)、内皮素(ET)及一氧化氮(NO)浓度,并与BRS值进行直线相关分析.结果:EH患者BRS明显低于正常人(P<0.01)且与病期、病情相关;BRS与去甲肾上腺素显著负相关(r=-0.56,P<0.01),而与肾上腺素、ANP、ET及NO无相关.结论:EH患者的BRS明显衰减,并与EH的发生发展相平行;交感神经系统的CA参与高血压的压力反射调节;ANP、ET及NO对压力反射功能的影响有待于进一步探讨.
