Objective:To preliminarily evaluate the clinical efficacy and safety of stereotactic body radiotherapy (SBRT) for pulmonary oligometastatic tumors.Methods:A retrospective analysis was performed on 46 patients treated with SBRT for pulmonary oligometastatic tumors from Jan 2017 to May 2018. There were 36 cases with primary pulmonary oligometastasis, 10 cases with secondary pulmonary oligometastasis. The pathological type included 31 cases of adenocarcinoma, 12 cases of squamous cell carcinoma and 3 cases of others. Dosage regimen was designed as 6 MV X-ray, 48-60 Gy/4-8fx. The Kaplan-Meier method was used to analyze the overall survival (OS) rates of 1 and 2 years. The Spearman method was used to analyze the correlation between the prescription dose pattern, target volume, whole lungs dose and the incidence of acute radiation pneumonitis (ARP), respectively. Results:The median follow-up time was 15.5 months. OS of 1 and 2 years were 65% and 37%, respectively. There were 27 cases with grade 1 ARP (59%), 16 cases with grade 2 (35%), 3 cases with grade 3 (7%), none of grade 4 ARP. The grade of ARP was related to the dose pattern of prescription and the planning target volume ( P<0.05). Grade 2 ARP was related with whole lungs V5 ( P<0.05). Conclusions:SBRT was safe and effective in the treatment of pulmonary oligometastases. The dose pattern of prescription, planning target volume and double lung V5 were closely related to the occurrence of ARP and may become important predictors.
目的 分析三阴性乳腺癌患者辅助化疗中应用紫杉醇与卡铂方案联合治疗的效果.方法 58例三阴性乳腺癌患者,经便利抽样法分为对比组及试验组,每组29例.对比组患者采用紫杉醇、环磷酰胺、表柔比星联合治疗,试验组患者采用紫杉醇联合卡铂方案治疗.比较两组患者临床治疗效果、术后局部复发率以及远处转移率.结果 试验组患者治疗总有效率为82.76%,高于对比组的55.17%,差异有统计学意义(P<0.05).试验组患者术后局部复发率3.45%以及远处转移率10.34%均低于对比组的20.69%、34.48%,差异均有统计学意义(P<0.05).结论 三阴性乳腺癌患者辅助化疗中应用紫杉醇与卡铂方案联合治疗病情控制效果理想.
目的 探讨培美曲塞在晚期非小细胞肺癌姑息性化疗中的效果.方法 66例晚期非小细胞肺癌患者,采用随机数字表法分为对照组和观察组,每组33例.两组患者均采用顺铂进行姑息性化疗治疗,在此基础上,对照组增加吉西他滨进行化疗,观察组增加培美曲塞进行化疗.比较两组患者短期疗效、化疗不良反应发生情况及治疗前后免疫功能指标.结果 观察组患者总有效率60.61%(20/33)明显高于对照组的36.36%(12/33),差异具有统计学意义(P<0.05).治疗后,观察组患者CD3+、CD4+、CD4+/CD8+高于本组治疗前,CD8+低于本组治疗前,且观察组患者CD3+、CD4+、CD4+/CD8+高于对照组,CD8+低于对照组,差异具有统计学意义(P<0.05).观察组化疗不良反应发生率18.18%(6/33)明显低于对照组的42.42%(14/33),差异具有统计学意义(P<0.05).结论 在晚期非小细胞肺癌姑息性化疗中应用培美曲塞效果较好,可以有效提高短期疗效,改善机体免疫力,减少化疗不良反应的发生,具有十分重要的临床应用价值,值得推荐患者选用.
Objective To establish glutaredoxin 3(GLRX3)-knocked down nasopharyngeal carcinoma(NPC) cell lines and explore the biological functions of the GLRX3 gene. Methods Retroviral vectors expressing shRNA against GLRX3 were constructed and transfected into HONE1 cells, and stable cell lines showing knocked-down GLRX3 expression were selected. Reduced GLRX3 transcription in the cell lines was confirmed by real-time quantitative PCR.The cells were examined in colony formation and wound healing assays to explore the influence of GLRX3 on these processes. Results Four retroviral shRNA vectors targeting GLRX3(shRNA-pBINNS2) were constructed and stably transfected into HONE1 cells.GLRX3 transcription was effectively suppressed in the stable cell lines.Knocking down GLRX3 inhibited clonogenicity and migration. Conclusions Knocking down GLRX3 inhibits the malignant behavior of NPC cells, suggesting that GLRX3 is a candidate oncogene in NPC.
Cytochrome b5 reductase 2 (CYB5R2), a member of the flavoprotein pyridine nucleotide cytochrome reductase family, is associated with a number of physiological reactions. However, its role in cancer, especially nasopharyngeal carcinoma (NPC), has not been addressed. Here, we investigate the transcript levels and promoter methylation status of CYB5R2 in NPC derived cell lines and tumor biopsies and experimentally address its role as a tumor suppressor gene. We find that CYB5R2 transcript levels are decreased in NPC cell lines and tumor biopsies. Promoter hypermethylation of CYB5R2 was detected in all six tested NPC cell lines and in 84 % of primary NPC tumor biopsies but not in normal nasopharyngeal epithelium. Clinically, CYB5R2 methylation was associated with lymph node metastasis in NPC patients (P < 0.05). The endogenous expression of CYB5R2 could be restored in vitro by the methyltransferase inhibitor 5-aza-2'-deoxycytidine in NPC cell lines. Ectopic expression of CYB5R2 had an inhibitory effect on proliferation, clonogenicity and migration of NPC cells. Moreover, in vivo tests in nude mice indicated that ectopic expression of CYB5R2 reduces the tumorigenicity of CYB5R2-negative NPC cells. Collectively, these findings suggest that CYB5R2 may be a functional tumor suppressor gene, frequently inactivated by hypermethylation of its promoter in NPC. We report here the first instance of epigenetic downregulation in NPC tumor biopsies of a key enzyme, CYB5R2, which is responsible for the detoxification of environmental carcinogens. We propose the possibility of utilizing CYB5R2 promoter methylation as a diagnostic biomarker of NPC in the future.
Objective To explore the application of individuation skin flap in tongue reconstruction,which may make a patient's swallowing and language function recover soon,increase a patient's compliance to surgery,and reduce a patient's mental trauma to the most degree.Method Sixty-seven cases of patients with tongue squamous cell carcinoma were given radical surgery and adopted by implementing individuation skin flap shift and transplantation for tongue reconstruction.The results of postoperative recovery situation,the occurrence of complications,surgical compliance,and the possible mental trauma of the patients were judged and analyzed.Results Sixty-seven cases were healed up during I period of surface wound.Thrombosis occurred in 1 patient,while he was rescued successfully.The other patients had no complications.Survival rate of skip flap is 100%.Most of the recreated tongue's shapes were good.Swallowing function returned to normal,and language functional recovery was excellent or good.Patient's psychological situation was good.Conclusion The surgery of individuation skin flap in tongue reconstruction showed high success rate with low incidence of complications,satisfactory functional recovery,and high compliance and minimal trauma for patients.
