The development of modern treatment technologies in oncology has reduced the mortality of children and adolescents from malignant neoplasms and hematological diseases. As a result, the issue of organizing rehabilitation care has become urgent. In this article, we describe the experience of organizing and developing pediatric rehabilitation in the oncology/hematology profile in Primorsky Krai for the period from 1995 to 2022. Various medical, psychological, social, and environmental aspects are outlined along with rehabilitation technologies used. The effectiveness of rehabilitation and the peculiarities of its implementation in children with oncological and hematological diseases are shown. The expediency of carrying out stages of rehabilitation in the region of residence of patients is substantiated.
Introduction. Atypical teratoid/rhabdoid tumor (ATRT) of the central nervous system (CNS) belongs to the embryonic group, occurs mainly in children under 3 years of age and is characterized by an extremely aggressive clinical course and unfavorable outcome. However, there is not enough data about the heterogeneity of the clinical course of CNS ATRT and the role of clinical and therapeutic prognostic factors in patients under 1 year of age and 1–3 years of age. The aim of this study was to conduct a comparative evaluation of treatment results in children with CNS ATRT in the indicated age groups. Materials and methods. From 2008 till 2021 years 106 patients were included in this study. Each patient underwent a therapeutic program according to determined protocol (ATRT-2006, MUV-ATRT, EU-RHAB and individual variant). All patients were divided into 2 groups according to age: from 1 to 12 months – 41 patients, from 13 to 36 months – 65. Median of the age was 16 (9–23) months. These cohorts of patients (1–12 months and 13–36 months) were comparable in clinical and therapeutic characteristics depending on gender, localization of the primary tumor site, stage of the disease, extent of surgical resection, therapeutic protocol, number of patients, who underwent radiation therapy (RT), high-dose chemotherapy (HDCT) with autologous hematopoietic stem cell transplantation (auto-HSCT), intrathecal/ intraventricular chemotherapy. Results. At the time of analysis 48 (45.3 %) patients were alive, 58 (54.7 %) patients died, of whom 52 (90 %) from disease progression and 6 (10 %) from therapeutic complications. One-year progression-free survival (PFS) in the group from 1 to 12 months was 32 %, 2-year – 18 %, 5-year – 18 %; one-year overall survival (OS) – 53 %, 2-year – 29 %, 5-year – 25 % with a median follow-up 8 and 13 months respectively. One-year PFS in the group from 13 to 36 months was 61 %, 2-year – 36 %, 5-year – 33 %; one-year OS – 86 %, 2-year – 67 %, 5-year – 49 % with a median follow-up 19 and 38 months respectively. In our study we identified the main predictors of the PFS and OS improvement in patients with ATRT CNS of each age group. In the 1–12 month group, these factors were: absence of metastatic involvement, MUV-ATRT therapeutic protocol, RT and HDCT with auto-HSCT; in the 13–36 months group: gross total/near gross total resection of the primary tumor cite, ATRT-2006 therapeutic protocol and RT. Conclusion. Despite the fact, that the presence of a diagnosis of ATRT CNS in children under 3 years of age is usually associated with a high risk of disease progression and recurrence, the results of our study demonstrate the heterogeneity of the clinical and therapeutic profile in this age group of patients.
Medulloblastomas of the WNT molecular group (MB-WNT) represent the smallest group of MB and account for only 10 % of the total. This molecular group is characterized by a favorable prognosis. Given the aggressive treatment regimens for MB, reducing the intensity of therapy for prognostically favorable tumors seems justified. Purpose of the study – to demonstrate the results of treatment of children with MB-WNT and to determine the impact on survival of various prognostic factors. The study included 85 patients with MB-WNT under the age of 18 who received treatment and were followed up from 1993 to 2022. Median age at diagnosis was 10 years (min – 3, max – 17). All patients had classical MB. Metastatic spread of the tumor at the time of diagnosis was detected in 18 (21.2 %) patients, the presence of a residual tumor according to postoperative magnetic resonance imaging – in 32 (37.7 %). Somatic mutations in the TP53 gene were detected in 10 (7.1 %) patients, in the CTNNB1 gene – in 79 (92.9 %), in the APC gene – in 5 (5.9 %), chromosome 6 monosomy – in 76 (89.4 %) children. At the time of the analysis, 74 (87.1 %) patients were alive, 11 (12.9 %) patients died, a relapse was diagnosed in 6 (7.1 %) patients, of which 5 died from disease progression, 1 patient is alive in the second remission. One patient in long-term remission developed secondary meningioma 20 years after the diagnosis of MB. The 10-year progression-free survival (PFS) was 0.92. 5-year overall survival (OS) was 0.90, 10-year – 0.86. The median OS is 112 months. When analyzing the sample of patients with MB-WNT in our study, PFS and OS were statistically significantly higher in girls without metastatic tumor spread, with total resection of the tumor, stratified into the low-risk group, and in the absence of a somatic mutation in the TP53 gene in the tumor tissue. In multivariate analysis, PFS was influenced by the stage of the disease and the presence of a somatic mutation in the TP53 gene in the tumor tissue; on OS – only the presence of a somatic mutation in the TP53 gene in the tumor tissue.
This paper presents the results for the patients with acute lymphoblastic leukemia (ALL) from the high-risk group (HRG) treated according to the ALL-MB 2002 Protocol. The registration phase of the study was performed from 15.04.2002 to 01.01.2008. The study was approved by the Independent Ethics Committee and the Scientific Council of the Dmitry Rogachev National Medical Research Center of Pediatric Hematology, Oncology and Immunology of Ministry of Healthcare of the Russian Federation. The study involved 36 departments (centers) of pediatric hematology/oncology in Russia and Belarus. One hundred and ten primary patients with ALL aged 1 to 18 years who met the criteria for high risk group, were analyzed: 29 patients with t(9;22), 11 patients with t(4;11), and 70 patients without stratifying genetic abnormalities who did not achieve remission by day 36 of induction therapy (16 patients from the standard risk group and 54 patients from the intermediate risk group, at initial). Median follow-up was 10.75 (8.6–13.8) years. First remission (CR1) was achieved in 80.9% of patients. 37.27% of patients relapsed, 51.22% of relapses were very early. The proportion of isolated bone marrow relapse was 73%, while isolated central nervous system relapses were observed in 4.55% of the cases. None of the patients developed a secondary tumor, 5.45% of patients were lost from follow-up. Only 15.7% of patients with CR1 received allogeneic hematopoietic stem cell transplantation. Only half of the patients with Ph-positive ALL received treatment with tyrosine kinase inhibitors. In total, 26.36% of patients remain in CR1. Overall and event-free survival were 32.9 ± 4.6% and 31.5 ± 4.5%, respectively. The cumulative risks of relapses and treatment-related mortality were 37.6 ± 4.3% and 20.9 ± 3.8%, respectively. There were no significant difference in the initial parameters and responses to therapy between the subgroups of patients. Overall and event-free survival were the highest in patients with ALL with t(4;11): 54.5 ± 15% and 45.5 ± 15%, respectively. The lowest overall and event-free survival were observed in the subgroup of patients without stratifying anomalies who did not achieve remission on day 36: 29.1 ± 5.6% and 27.1 ± 5.3%, respectively. The cumulative risk of relapse was the highest in patients who did not respond to induction therapy (42.9 ± 5.2%). The cumulative risk of treatment-related mortality was the highest in patients with Ph-positive ALL (31.0 ± 8.6%). The 5-year overall survival of patients with ALL relapse after high-risk therapy was extremely low – 7.7% (95% confidence interval 0–16.1), median overall survival after relapse in this group was only 187 days. This indicates that the options for second-line therapy of high-risk patients were severely limited at the time of ALL-MB 2002 study, because they included only chemotherapeutic strategies.
The paper presents the results of a multicenter randomized study of two-dose COLI-ASP regimens in therapy of consolidation of standard risk-group (SRG) children and adolescents with acute lymphoblastic leukemia (ALL) treated by the ALL-MB-2002 protocol. The principal aim of the study was to elucidate whether the dose of COLI-ASP might be reduced without losing the efficiency of therapy. The analysis covered primary ALL patients notified at 36 clinics of Russia and Belarus, enrolled for the study in the period of May 1, 2002, to October 10, 2006. Out of 819 patients to be examined, 414 received COLI-ASP in a dose of 10,000 U/m2 (a control group), and 405 children took it in a dose of 5,000 U/m2 (a study group). No statistically significant differences were found between the patients in gender, age, baseline leukocytosis, and spleen size. The study showed no statistically and clinically significant differences in event-free, overall, and relapse-free survival (EFS, OS, and RFS, respectively) and relapse rates with the use of two-dose COLI-ASP regimens. However, in the control group, the death rate in remission was twice greater than that in the study group (p = 0.02). Detailed analysis of the outcomes of therapy in some subgroups revealed no differences between the control and study groups. The Cox multiple regression analysis has indicated that the factors influencing EFS and RFS are the baseline sizes of the spleen, baseline leukocytosis, age, and a response to treatment in the period of induction, rather than the dose of E. coli asparaginase. The authors have also found that among the SRG patients, there is a subgroup of patients without risk factors, in whom the reduction in the COLI-ASP dose may substantially increase survival rates, by lowering toxicity and mortality in remission.
Проанализирована значимость прогностических факторов эффективности терапии у детей с острым лимфобластным лейкозом (ОЛЛ), получающих терапию по протоколу ALL-MB-2002, в зависимости от группы риска. В исследование были включены 1544 первичных больных ОЛЛ в возрасте от 1 года до 18 лет, получавших лечение в 36 клиниках России и Беларуси в период с 15 апреля 2002 г. до 1 января 2008 г. Из 1544 больных, включенных в анализ, пациенты группы стандартного риска (SRG) составили 69,2 %, промежуточного риска (ImRG) — 24,5 % и 6,3 % пациентов были отнесены к группе высокого риска. Анализ, проведенный у пациентов SRG, продемонстрировал резкие различия в выживаемости между отдельными подгруппами больных. Выявлены достоверные различия в показателях выживаемости пациентов в зависимости от возраста (старше и младше 10 лет), инициального лейкоцитоза (более и менее 30 × 10 9 /л), инициального пальпаторного увеличения селезенки (более и менее 4 см из-под края реберной дуги) и параметров раннего ответа на терапию (8-й и 15-й день). При ретроспективном определении больных, относящихся к SRG, с использованием новых критериев (инициальный лейкоцитоз <30 × 10 9 /л; селезенка < 4 см), прогностическая значимость раннего ответа на терапию нивелировалась. Среди пациентов ImRG не обнаружено никаких достоверных различий в показателях бессобытийной, общей и безрецидивной выживаемости и риске развития изолированных нейрорецидивов в зависимости от инициального иммунофенотипа бластных клеток (Т-ОЛЛ/не-Т-ОЛЛ). Шестилетний кумулятивный риск развития изолированных нейрорецидивов не различался у пациентов с Т- и не-Т-ОЛЛ, однако был выше у больных с инициальным лейкоцитозом ≥ 100 × 10 9 /л независимо от иммунофенотипа. Наихудшие показатели выживаемости отмечены у пациентов с не-Т-ОЛЛ и количеством лейкоцитов ≥ 100 × 10 9 /л — бессобытийная выживаемость в этой подгруппе составила всего 58 ± 7 %. При проведении многофакторного анализа выявлено, что для пациентов с Т-ОЛЛ независимое прогностическое влияние на выживаемость оказывает только величина инициального лейкоцитоза (р = 0,0333), тогда как для больных не-Т-ОЛЛ независимое значение имеют возраст (р = 0,0063), инициальный лейкоцитоз (р = 0,0066) и наличие поражения центральной нервной системы (р = 0,0112).Перенести в английский вариант Prognostic significance of different risk factors in children with acute lymphoblastic leukemia (ALL) treating according to ALL-MB 2002 protocol depending on risk group is analyzed. 1544 primary patients aged from 1 to 18 years, treating in 36 clinics of Russia and Belarus from April, 15 th , 2002 to January, 1th, 2008, was included in the study. From 1544 patients included, 69.2% were standard risk group (SRG), 24.5 % — intermediate risk group (ImRG) and 6.3% — high risk group. The expressed differences in survival rate between certain patient’s subgroups were revealed. Significant survival differences according to age (older and younger 10 years), initial leukocyte count (more and less than 30 × 10 9 /l), spleen size (more and less 4 cm below a costal arch at a palpation) and early therapy response (8th and 15th days of induction) are revealed. At retrospective definition of SRG patients with use of new criteria (initial leukocytosis < 30 × 10 9 /l and spleen size <4 cm), prognostic significance of therapy response was leveled in this subgroup of patients. Among ImRG patients any significant differences in EFS, OS, RFS and cumulative risk of isolated CNS (CIR IN ) relapse according to blast cells immunophenotype (T-ALL/non-T-ALL) were not shown. 6-years CIRIN was not different between patients with T-ALL and non-T-ALL, however was high in patients with initial leukocyte count ≥ 100 × 10 9 /l irrespective to immunophenotype. The worst survival appeared in patients with non-T-ALL and leukocyte count ≥100 × 10 9 /l — EFS in this subgroup was only 58±7%. Only initial leukocytosis had prognostic significance for patients with T-ALL (р=0.0333), whereas age (р = 0.0063), initial leukocytosis (р = 0.0066) and CNS involvement (р = 0.0112) as independent prognostic factors for patients with non-T-ALL in multifactorial analysis was revealed.
One of the main risk-factors of acute lymphoblastic leukemia (ALL) is age. The most favorable age group is children from 2 to 9 years old, and the adolescents and young adults have the worst prognosis. Treatment of ALL at adolescents and young adults is a unique problem of modern hematology now. This work presents the analysis of 368 cases of patients with primary ALL 15–18 years old, which were registered in the database of Russian-Belarussian group from April 2002 to November 2014. The aim of the analysis was the estimation of effectiveness of different generations of protocol for adolescents and prognostic significance of different factors to reveal the ways of future therapy optimizing in this age group. Event-free survival of adolescents 15–18 years old according our study was 56 ± 5 % in ALL-MB-2002 study and 57 ± 6 % in ALL-MB-2008 study. We did not received any differences in survival based on immunophenotype of blast cells. But the risk group analysis showed the worst results at patients with ALL from pre-B-precursor cells of high risk. No difference was revealed according the gender. One of the general problems remains high toxicity of therapy at patients of this age group. Treatment related mortality (TRM) was 13.2 and 12 % at ALL-MB-2002 and ALL-MB-2008 accordingly. Main cause of deaths remains infection.
T-cell acute lymphoblastic leukemia (T-ALL) is a tumor developing as a result of malignant transformation of precursor T-cells. Despite the fact that T-ALL is responsible for about 10-15% of all ALL cases in children, it is essential to separate T-ALL in a special subgroup, as a nosological entity by the tumor biology, poor clinical outcome and resistance to therapy. We analyze the results of treatment of 644 patients with T-ALL, registered in the data base of the Russian-Byelorussian Research Group over 24 years of its work. The initial characteristics of the patients, responses to therapy, prognostic factors, and efficacy of various therapeutic options are analyzed.
AIMTo evaluate the efficiency of the original ALL-MB-2002 protocol within the multicenter study of treatment of acute lymphoblastic leukemia (ALL) in children.SUBJECTS AND METHODSA total of 1873 primary patients with ALL aged 1 to 18 years, of whom 1544 patients were enrolled in this study, were notified at 36 clinics of Russia and Belarus from April 15, 2002, to January 1, 2008.RESULTSWith the median observation of 4.12 years, 7-year event-free survival (EFS) was 73 +/- 13%; overall survival (OS) 78 +/- 2%; relapse-free survival 82 +/- 1%. The rates of EFS and OS were equal and amounted to 76 +/- 2 and 80 +/- 2% in the standard-risk group (SRG) and intermediate-risk group (ImRG), respectively. In the high-risk group (HRG) patients, EFS and OS were as high as 30 +/- 6 and 37 +/- 6%, respectively. The frequency of relapses with central nervous system lesion was as much as 4.7% in all the patients, 6-year cumulative risk for isolated neurorecurrences being 2.5% in the SRG patients. Adolescents, patients with the baseline leukocytosis (more than 100 x 10(9)/l), and those with a splenic size of over 4 cm or more from the costal arch margin had substantially worse survival rates. A poor early response to therapy (on induction days 8 and 15) was also associated with its lower efficiency.CONCLUSIONDespite a considerable rise in the number of centers and a slight increase in the intensity of therapy, the results of the new ALL-MB-2002 protocol are as minimum equivalents obtained in the use of the previous ALL-MB-91 protocol. A significant improvement in the overall results of therapy and a reduction in the cumulative risk for isolated neurorecurrences were noted in the ImRG patients.
71 children with malignant neoplasms (hemoblastosis and considerable tumors) were examined. Children were divided in to. 4 groups according to the terms of clinicolaboratory remission of oncologic disease. The 1st group patients receiving supporting chemotherapy (7 children); the 2nd children being in remission of basic disease up to 1 year (8 children); the 3rd in the terms of remission of basic disease from 1 till 5 years (36 children) and the 4th in a complete stable remission more than 5 years (20 children). Control group was composed of 42 healthy children comparable in sex and age with the children from basic groups. All children underwent a coarse of sanatorium rehabilitation in the local health-resort «Builder». As a result of the conducted work peculiarities in the heart activity regulation in children with oncologic diseases were revealed. They displayed in the prevalence of sympatheticoand hypersympatheticotonic influence in the heart rhythm regulation. We have developed rehabilitation complex with the usage of balneowatertreatment witch were administered to the children in dependence of the heart regulation level. According to the investigation conducted before and after health-resort treatment the including of the above-mentioned complexes into rehabilitation measures of oncologic patients may be recommended as healthimproving measures on the teams of the local sanatorium.
