苯所致白血病的归因诊断较为困难,在诊断、鉴定过程中存在争议.通过对一起职业病诊断机构诊断结论为"职业性肿瘤(苯所致白血病)",而市级、省级鉴定机构均诊断为"无职业性肿瘤"的病例资料进行分析,探寻诊断、鉴定工作中存在的不足.认为有无职业性苯接触史的认定是该案例诊断结论与鉴定结论不一致的根本原因.通过该案例,旨在为诊断、鉴定机构工作人员提供经验借鉴.
目的 探讨职业接触电焊烟尘与慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)的关系.方法 检索从建库到2022年有关电焊烟尘和COPD的文献,数据库包括PubMed、Springer、中国知网和万方等.采用美国医疗保健研究和质量局(AHRQ)量表对符合标准的文章进行质量评估.根据I2和P值对文章进行异质性判断,并选用相应的效应模型.通过敏感性分析检验研究结果的稳定性;通过Begg's和Egger's检验评估发表偏倚.结果 共检索到相关文献108篇,最终有2项队列研究和6项横断面研究共11项研究纳入本次meta分析,未接触电焊烟尘的有3 425例COPD患者,接触电焊烟尘的有193.例COPD患者.文献质量评分均在5分以上.各研究间异质性较低(I2=11.8%),采用固定效应模型对合并优势比(OR)进行分析.合并OR值表明接触电焊烟尘会增加COPD的发病风险(OR=1.39,95%CI=1.15~1.69),在控制了吸烟因素后依旧显示接触电焊烟尘会增加COPD的发病风险(OR=1.34,95%CI:1.10~1.63).敏感性分析证实本次研究结果稳定.结论 劳动过程中接触电焊烟尘可增加职业人群患COPD的风险,应重视电焊烟尘工作人员的健康监护,及早控制可能的病情,在脱离岗位后要保持定期随访.
目的 探究职业接触石棉与患肾癌风险的关系.方法 检索有关职业接触石棉与患肾癌的电子文献数据库(中国知网、维普、万方、PubMed、EBSCO、Elsevier Science direct、Medical、Cochrane Library),根据 Newcastle-Ottawa scale(NOS)量表评价纳入文献的质量,对评价低于6分的文献不予纳入.用STATA12.0软件对文献的OR值及其95%置信区间进行合并.通过I2及P值对合并后的数据进行异质性检验.使用敏感性分析判定结果的稳定性,并做亚组分析.结果 共检索到文献248篇,最终纳入8篇文章、10个病例-对照研究,病例组3 725例,对照组6 970例.10项研究之间存在较低的异质性(I2<0.1%,P=0.585).合并的效应值表明工作场所接触石棉会增加患肾癌的风险(OR=1.30,95%CI:1.17~1.45,P<0.01).敏感性分析和亚组分析均证实了本次结果较稳定,并且未见明显的发表偏倚.结论 工作场所接触石棉会增加患肾癌的风险.
目的 探讨长期低浓度苯暴露对职工各职业健康检查指标的影响以及接触指标与损伤指标的关系.方法 采用横断面调查,选择2018-2020年某汽车生产企业进行职业健康检查的135名低浓度苯暴露工人为接触组,149名不接苯工人为对照组,收集其健康检查资料及外周血和尿液样本.以质谱仪检测尿中以尿肌酐校正后的苯巯基尿酸(SPMA)含量作为内暴露指标,采用比色法检测血清中脂质过氧化物丙二醛(MDA)、尿中8-羟基脱氧鸟苷(8-OHdG)水平,比较两组人员检查指标的差异,分析接触组S-PMA与8-OHdG的相关性.结果 该企业苯主要存在于搅拌调漆、底漆和面漆喷涂、烘干、补漆等工序,CTWA为<0.3~0.25 mg/m3,CSTEL为<0.3~0.40 mg/m3.接触组血压偏高检出率为18.12%~22.46%,对照组为11.41%~18.12%;接触组红细胞、血红蛋白量均值均低于对照组(t=-4.135,-2.386,P<0.05),且红细胞偏低检出率高于对照组(x2=7.620,P<0.01);白细胞计数两组间比较,差异无统计学意义(P>0.05);接触组尿液中S-PMA、8-OHdG、MDA中位数均高于对照组,差异均有统计学意义(x2 =0.527,P<0.05);接触组S-PMA与8-OHdG呈正相关(r=0.519,P<0.01).结论 长期低浓度苯暴露会引起职工血压、S-PMA、8-OHdG、MDA的升高,红细胞计数的减少,低浓度苯暴露仍有较高的血液毒性和氧化损伤的健康风险.
BackgroundOccupational stress and depressive symptoms of disease prevention and control personnel are serious.ObjectiveTo investigate the relationship between occupational stress, psychological capital, and depressive symptoms of disease prevention and control personnel, and analyze the potential mediating effect of psychological capital on the relationship between occupational stress and depressive symptoms.MethodsFrom July to September 2020, a cluster random sampling method was used to select 2201 employees from 21 centers for disease control and prevention as study subjects covering all levels of administrative divisions in Jiangsu Province. A total of 2036 valid questionnaires were collected with a recovery rate of 92.5%. The Core Occupational Stress Scale, Patient Health Questionnaire, and Psychological Capital Questionnaire were used to investigate their occupational stress, depressive symptoms, and psychological capital. Stratified regression analysis was used to explore the effects of occupational stress and psychological capital on depressive symptoms. A mediating effect model was used to analyze and verify the potential mediating effect of psychological capital on the relationship between occupational stress and depressive symptoms.ResultsThe total scores in M (P25, P75) of occupational stress, depressive symptoms, and psychological capital in the target population were 42.0 (37.0, 48.0), 8.0 (4.0, 9.0), and 4.6 (4.0, 5.0) respectively. The positive rate of occupational stress was 31.0% (631/2036), and the positive rate of depressive symptoms was 22.0% (448/2036). The dimensional scores of organization and reward, and demand and effort of occupational stress were positively correlated with the total score of depressive symptoms [Spearman correlation coefficients (rs) were 0.371 and 0.269, P<0.05]. The dimensional scores of social support and autonomy of occupational stress and the score of psychological capital were negatively correlated with the total score of depressive symptoms (rs=−0.373, −0.112, −0.494, P<0.05). The organization and reward, and demand and effort had positive effects on depressive symptoms (b=0.188, 0.177, P<0.05), while social support and autonomy had negative effects on depressive symptoms (b=−0.290, −0.078, P<0.05), and associated with a 22.5% increase of explanatory variance. Psychological capital had a negative effect on depressive symptoms (b=−0.368, P<0.05), and associated with an 11.0% increase of explanatory variance. Psychological capital had mediating effects on the associations of social support, organization and reward, and autonomy with depressive symptoms, and the mediating effect values were −0.210 (95%CI: −0.253-−0.171), 0.096 (95%CI: 0.071-0.122), and −0.164 (95%CI: −0.229-−0.103), respectively. The corresponding mediating effect percentages were 40.23%, 26.97%, and 45.56%, respectively.ConclusionOccupational stress of disease prevention and control personnel can directly affect depressive symptoms, but also indirectly through psychological capital. Psychological capital plays a partial mediating role in the associations of social support, organization and reward, and autonomy of occupational stress with depressive symptoms. The occurrence of depressive symptoms can be reduced by decreasing occupational stress and increasing psychological capital.
目的 了解扬州市放射工作人员外周血淋巴细胞微核细胞率的总体水平,为放射工作人员职业健康监护提供依据.方法 对2018年扬州市2 124名放射工作人员的外周血淋巴细胞微核细胞率进行统计分析和评价.结果 2 124名放射工作人员外周血淋巴细胞微核细胞率均≤6‰,在正常范围内.上岗前放射工作人员微核自发率与年龄呈正相关关系(rs=0.513,P<0.01),20~29岁和≥50岁年龄段上岗前人员的微核细胞率均低于在岗及离岗人员的微核细胞率(P<0.05).微核细胞率随着工龄增加有逐渐升高的趋势(rs=0.239,P< 0.01);不同工种放射工作人员的微核细胞率差异有统计学意义(P<0.01),其中从事工业应用和核医学的放射工作人员较高.不同级别放射工作单位之间微核细胞率差异也有统计学意义(P<0.01),其中一级医院及牙科门诊的放射工作人员略高.结论 扬州市放射工作人员职业健康总体状况较好,放射工作人员外周血淋巴细胞微核细胞率较低,且与年龄、工龄和工种有关.
某高科技塑业企业工作场所环境监测数据显示,工作场所空气中苯、甲苯CTWA的中位数M(P0~P100)分别为0.30 (0.30~0.53) mg/m3、0.60 (0.60~10.70) mg/m3,CSTEL的中位数M(P0~P100)分别为0.30(0.30~1.00) mg/m3、0.60 (0.60~11.40) mg/m3,二甲苯的CTWA、CSTEL检测值均小于最低检出限(3.3 mg/m3),三苯检测合格率为100.0%.31名三苯接触工人WBC计数、中性粒细胞偏低检出率分别为12.90%、6.45%.工人接触苯、甲苯和二甲苯所致的非致癌危害商数(HQ)值M (P0~P100)分别为2 849.33 (712.33,15 671.33)、34.19 (8.55, 188.06)和5 157.00 (1 282.20,28 208.40),接触苯致癌风险值为9.81×10-6~3.48×10-5.提示接触三苯的工人处于较高的非致癌和低致癌健康风险水平.
[目的]分析扬州市职业性苯暴露工人的健康状况,为苯所致职业病的预防与控制提供依据.[方法]对2015-2017年扬州市职业性苯接触工人职业病监测数据进行描述与对比分析.[结果]截至2017年11月底,扬州市共申报涉苯企业510家,苯接触工人5 207人.苯作业场所和岗位超标率分别为4.44%、1.58%,2015-2017年涉苯工作场所苯超标率逐年降低(P<0.05),超标场所主要位于江都区.3年间,收集到248家企业的11 444份职业健康检查资料.248家企业以小型(67.74%)为主,主要来自宝应县和邗江区(均为20.97%),高邮市最少,占3.63%;行业分布以化学原料和化学制品制造业(12.10%),皮革、毛皮、羽毛及其制品和制鞋业(1 1.69%)为主.3年间共检出疑似苯中毒病人490例,职业禁忌证83例,其中2015年分别为89例、2例,2016年分别为183例、37例,2017年分别为218例、44例.3年问,疑似苯中毒、职业禁忌证检出率均有逐年升高的趋势(P<0.01).白细胞和中性粒细胞偏低检出率逐年降低(P<0.05).2015-2017年扬州市仅3例职业性苯中毒报告病例,且于诊断后隔年落实工伤保险待遇.[结论]扬州市职业性苯暴露工人职业健康状况不容乐观,建议继续加强职业性苯暴露所致职业病的监测工作.
对某LED生产企业2014-2016年职业病危害因素检测与健康检查资料进行汇总分析.结果显示,该企业3年累计粉尘、毒物、噪声等206个检测点,总合格率为97.09%,硫酸监测点合格率仅45.45%,3年间毒物总合格率差异无统计学意义(P=0.26).企业职业健康检查未检出疑似职业病人和职业禁忌证.不同年度间高仟伏胸片和心电图异常检出率比较,差异有统计学意义(P<0.05).测试/目检岗位与其他岗位工人血压偏高、心电图异常率差异有统计学意义(P<0.05).该企业应将硫酸作为重点防控的职业病危害因素,同时需加强对职工心血管疾病等慢性病的防治.
为了解某发光二极管(LED)外延片及芯片生产企业的职业病危害因素分布及危害情况,采用职业卫生学调查的方法,对其职业病危害因素进行监测并结合作业人员职业健康检查结果进行分析.结果显示,该企业主要职业性危害因素为其他粉尘、氨、氯、硫酸、氟化氢、盐酸、氢氧化钠、硫化氢、丙酮、异丙醇、噪声等.粉尘、噪声合格率均为100%,毒物合格率为91.9%,超标点均为接触硫酸的岗位.2014-2016年在岗期间职业健康检查异常检出率分别为45.8%、42.2%和52.9%,不同年度间以及各项目异常检出率比较,差异均无统计学意义(P>0.05).提示,硫酸为该企业重点防控的职业病危害因素,应加强相关职业病防护措施.
目的 了解某LED生产企业空调作业环境空气污染状况及空气质量状况,为预防急、慢性职业危害提供参考.方法 按照有关工作规范进行布点、现场数据记录、空气采集丙酮、异丙醇、氨气和硫酸等化学毒物和可吸入颗粒物PM10、二氧化碳等空气质量指标的样本.结果 车间空气中氨气、氯气等9种有毒物质的监测点总合格率为92.2%,仅有硫酸存在超标,硫酸监测点的合格率为33.3%;空气质量指标中一氧化碳、甲醛和总挥发性有机物TVOC的监测点超标率均为75.0%,二氧化碳的监测点超标率为58.3%;微小气候中温度、湿度合格率高,为100.0%,但空气流速的合格率仅有42.9%.结论 某LED生产企业空调作业环境空气中有毒物质污染控制较好、但空气质量欠佳,微小气候不适,空调作业环境空气质量应予以改善.
[目的]了解蓝绿光LED产业链生产过程中的职业病危害程度,探讨职业病危害的关键控制点.[方法]采用职业卫生现场调查和职业病危害因素检测相结合的方法,结合危害分析与关键控制点(hazard analysis and critical control point,HACCP)理论,对LED生产的整个工艺流程进行职业病危害关键控制点分析.[结果]蓝绿光LED产业链生产过程中主要职业病危害因素为氨、氯、硫酸、氟化氢、盐酸、氢氧化钠、丙酮、异丙醇、丙烯酸、丁酮、噪声、X射线等.金属有机化合物化学气相沉淀(metal-organic chemical vapor deposition,MOCVD)区、干法刻蚀区、特气供应室、废水处理室等车间存在氨、氯气、氟化氢等高毒物质.LED芯片生产车间化学品存储点、机台自动操作点2个检测点硫酸浓度超过国家限值标准,LED封装企业空压机房(一楼)的噪声声级超标.[结论]蓝绿光LED产业链上游生产外延片的MOCVD区、中游生产芯片的干法刻蚀区、化学区、特气供应室、废水处理室以及下游封装的公用工程空压机房为职业病危害的关键控制点.
采用回顾性队列研究的方法,对不同铅负荷职业接触人员的心血管功能指标进行分析.结果显示,高铅组高血压患病率和脉压差明显高于低铅组,高铅组乳酸脱氢酶浓度、α-羟丁酸脱氢酶、甘油三酯的浓度明显大于低铅组(P<0.05);高铅组红细胞浓度、红细胞压积和血小板压积明显小于低铅组(P<0.05);两组之间其它心血管指标之间差异无统计学意义(P>0.05).超声心动图参数中,与低铅组比较,高铅组左心室舒张末期内径、容量和每搏输出量增大,差异均有统计学意义(P<0.05),其它指标差异均无统计学意义(P>0.05).提示铅可能对接触人员的心血管有损伤作用.
objective To investigate the effect of glyphosate on health of exposed populations.Methods 27 male workers only exposed to glyphosate were selected as exposure group,and 27 people with non-occupational exposure as a control group.Questionnaire investigation was applied and occupational health examinations were taken for all subjects.Results Compared to the control group,mean hemoglobin concentration of the exposure group of workers' was statistically significant lower(Z=2.77,P<0.01);RDW CV(Z=1.99,P=0.04),and mean platelet volume(Z=2.33 ,P=0.02),platelet distribution width(Z=2.03,P=0.04),large platelet ratio(Z=2.28,P=0.02)were statistically significant higher.Compared to the control group,direct bilirubin(Z=2.91,P<0.01),white ball ratio(Z=2.06,P=0.04),glutamic oxalocetie transaminase(Z=2.35,P=0.02),glucose(Z=2.31,P=0.02)in the exposure group were statistically significant increased.Furthermore,creatinine in the exposure group was statistically significant lower(Z=2.36,)P=0.02)compared to the control group.The other indexes were not statistically significant(P> 0.05).Conclusions Glyphosate may affect to the people's hematopoietic function,long-term exposure may lead to anemia and damage to liver cells.
Objective To understand the blood Results of routine examination of benzene-exposed workers and explore influence of blood test Results of the age, industry, working age and gender for the benzene-exposed workers.Methods Analysis was made on the data collected from the occupational physical examination of the 3003 benzene-exposed workers in Yangzhou City of 2014.Results Among the 3003 benzene-exposed workers,the abnormal rate of blood routine detection was 6.9%,the detection rates of reduction of WBC and neutrophils was 4.3% and 5.2%,respectively.The difference of abnormal rate of blood routine was not statistically significant in different age groups and industries (P>0.05).The detection rate of WBC reduction of workers working for ≥10a was higher than that with working for <10a,the difference was statistically significant(P<0.05).The difference of detection rate of WBC reduction between male and female was statistically significant(P<0.01).The difference of detection rate of reduction of neutrophils and PLT was not statistically significant in different working yeas and gender(P>0.05).In chemical industry,the detection rates of WBC reduction in male and female group was 5.2% and 2.0%,respectively.The difference was statistically significant (P<0.01 ).Conclusions The blood routine abnormalities of benzene-exposed workers were reduction of the WBC and neutrophils.Working years nd gender have a certain effect on the blood test Results of benzene-exposed workers.It is suggested that management and worker health protection of benzene-exposed workers should be strengthened,especially the male employees of the chemical industry.
铅是用途十分广泛的一种重金属,在使用过程中会造成环境的污染及人体健康损害.我国铅中毒的机制研究已达到了分子和细胞水平,许多铅导致的疾病已经逐渐被认知.但是目前中国尚缺乏铅导致的肌萎缩侧索硬化症患病率的流行病学数据调查,为此,针对近年来国内外铅暴露的流行病学证据,对居民健康危害及铅暴露的生物标志物研究进展进行综述.
目的:评价顺酐生产过程中存在的职业病危害因素及其控制效果,提出针对性的防护措施。方法采用现场职业卫生调查的方法,对某化工企业顺酐生产项目进行职业病危害因素检测,调查该项目作业场所职业病防护措施,并结合职业健康检查的结果,对该项目的职业病危害控制效果进行评价。结果顺酐项目生产过程中存在的职业病危害因素主要有正丁烷、顺酐、邻苯二甲酸二丁酯、氨、一氧化碳、噪声和工频电场。经检测,各作业点化学有害因素和工频电场的浓(强)度均低于国家职业接触限值,6个工种噪声作业人员8 h等效声级符合相关限值要求。结论顺酐生产项目主要职业病危害因素为顺酐、邻苯二甲酸二丁酯、氨,采用国内先进安全的工艺技术,各项职业病防护措施有效可行,基本符合职业卫生要求。
目的 识别、评价、控制某古筝制品厂产生的职业病危害因素,保护劳动者的身体健康.方法 收集相关资料,开展现场职业卫生学调查、职业病危害因素检测及职业健康体检,结合职业病防护设施、个人职业防护水平对用人单位职业病危害现状进行定性与定量评价.结果 该企业生产过程中可能产生的职业病危害因素主要有苯、甲苯、二甲苯、木粉尘和噪声,压模点、磨灰点木粉尘浓度为5.08 mg/m3和4.25 mg/m3,下料点噪声强度87.7 dB(A),超过国家职业接触限值,其他岗位职业病危害因素均符合标准.职业健康检查发现1名工人存在职业禁忌证.结论 该企业职业卫生基本符合要求.
选取59名单一接触草甘膦工人为调查对象,随机选取同地域非职业接触工作人员42人作为对照组.分别测定并比较两组人员血清生化指标的差异.结果显示,接触组血清中的碱性磷酸酶、肌酸激酶、肌酸激酶同工酶、总蛋白、白蛋白、尿素氮和总胆固醇与对照组比较,差异有统计学意义(P<0.05);两组血清中的碱性磷酸酶、肌酸激酶、肌酸激酶同工酶、总蛋白、白蛋白、尿素氮和总胆固醇的异常率比较,差异无统计学意义(P>0.05).其余生化指标均无统计学意义(P>0.05).提示草甘膦作业环境可能对工人血清生化指标含量有一定影响.
目的 评价某涤纶短纤维生产企业通风系统防护效果,提出合理的改进措施.方法 对涤纶短纤维企业进行工程分析和职业卫生调查,识别其主要职业危害因素;通过职业病危害因素检测、健康监护分析其危害程度,结合卫生工程检测,分析通风系统控制效果.结果 涤纶短纤维生产企业存在的职业病危害主要为氢化三联苯、联苯-联苯醚、NaOH、其他粉尘、高温、噪声等;检测结果显示化学毒物、其他粉尘的合格率为100%,高温除卷绕岗位超过国家控制标准外,其余岗位均合格,合格率为85.7%;车间大多数生产工段的送风量未达到设计要求.结论 通风系统能有效控制涤纶短纤维生产企业有机溶剂、粉尘和高温的危害,但应加强其维护保养.
