目的 总结快速、有效解决自身抗体干扰ABO/RhD血型鉴定的方法,并评价其处理效果.方法 收集血型鉴定初检异常的血液标本,筛选出自身抗体影响血型鉴定案例,患者红细胞予以洗涤、放散或巯基试剂处理,血浆经异体或自体吸收后,再次检测患者血型,评价上述处理方法的有效性.结果 39例疑难血型中有9例受自身抗体影响,经合适的方法处理后,均可以正确鉴定出患者ABO/RhD血型.结论 自身抗体会影响ABO/RhD血型的鉴定,通过分析试验结果和选择合适的处理方法可以提高此类血型鉴定的效率和准确性.
冷凝集素可以导致红细胞自身凝集,健康人体内冷凝集素效价一般<16[1],对ABO血型检测影响不大.而一些疾病如肺炎支原体感染、自身免疫性溶血性贫血等,因为大量冷凝集素的产生,会严重影响ABO血型鉴定.我们遇到1例冷凝集素效价很高的患者,常规方法检测不出其ABO血型,通过一系列辅助试验,最终得出了正确的血型结果,在此做一总结分析如下.
Objective To seek the rational low value platelet detection mode by analyzing and comparing the two platelet detec‐tion channels of the Sysmex XN2000 Hematology Analyzer .Methods Seventy‐two clinical blood routine specimens of the platelet≤50 × 109/L at the first time detection were selected and detected by impedance channel (PLT‐I)and fluorescence staining channel (PLT‐F)offered by the Sysmex XN2000 Hematology Analyzer .According to the blood smear ,the specimens were divided into the non‐aggregation group and aggregation group .Then the results of the non‐aggregation group adopted the paired t test ,linear regres‐sion and Bland‐Altman bias analysis for conducting the statistical analysis ;the detection results were compared between the aggre‐gation group and the manual platelet count .Results The results of the two channels in the non‐aggregation group had large correla‐tion and less bias ,the difference of the paired t test had no statistical significance ;but the results of aggregation group had larger difference ,moreover the PLT‐F results were more close to the results of the manual platelet count .Conclusion When no platelet aggregation ,the detection can select the PLT‐I channel ,but when the instrument has the alarm of aggregation ,selecting the PLT‐F channel will be more accurate .
目的 为提高肺结核诊断的准确度 ,对两种结核分枝杆菌免疫学诊断方法进行比较研究.方法 选择2015年3月1~31日在该院肺科住院治疗的疑似肺结核患者83例 ,进行结核感染 T细胞斑点试验(T-S PO T . TB)和血清学结核抗体(TB-Ab)检测 ,比较两种方法的检测阳性率 ,并对两种方法检测结果的敏感性、特异性、准确性、阳性预测值、阴性预测值、阳性似然比、阴性似然比进行对比.结果 T-SPOT .TB检出阳性为48例 ,检出率为57 .83% ,TB-Ab检出阳性为36例 ,检出率为43 .37% ,T-SPOT .TB的阳性检出率高于 TB-Ab ,差异有统计学意义(P<0 .05);T-SPOT .TB的综合评价指标要优于TB-Ab检测 ;联合使用两种方法诊断肺结核可以提高检测的敏感性.结论 T-SPOT .TB的各项综合评价指标均优于TB-Ab检测 ,选择免疫学方法诊断肺结核 ,在不考虑成本的情况下优先选择T-SPOT .TB.
目的:通过分析和比较检测梅毒的4种血清学方法,寻找合理的梅毒血清学检测模式。方法应用甲苯胺红不加热血清反应素试验(TRUST)、梅毒螺旋体酶联免疫吸附试验(TP-ELISA)、胶体金试验(SYP)和梅毒螺旋体明胶颗粒凝集试验(TPPA)对该院2014年7月的58例临床怀疑梅毒感染患者的血清标本进行分析,并比较TRUST、TP-ELISA、SYP 3种方法所测结果与TPPA结果是否存在差异。结果 TRUST结果与TPPA结果比较,差异有统计学意义(P<0.05),而ELISA、SYP结果与TPPA结果比较,差异均无统计学意义(P>0.05)。结论 ELISA和SYP对梅毒抗体检测均有较好的灵敏性和特异性。而TRUST与TPPA差异较大,需合理利用TRUST的检测结果。结合各方法的优缺点建立一种便捷、准确和合理的检测模式,为临床诊断梅毒提供依据是很有必要的。
Objective Compare Siemens 1650 to Arkray HA-8160 glycated hemoglobin results whether there are differences, to provide accurate and reliable experimental data for the laboratory. Methods Uses ion exchange high performance liquid chromatography (HPLC) method Arkray to HA-8160 automatic glycated hemoglobin analyzer with the Siemens 1650 automatic biochemical analyzer latex enhanced immune method were measured glycosylated hemoglobin, and the results were analyzed and compared. Results of the different methods of the two instruments glycosylated hemoglobin results are very significant differences (P<0.01) results as determined by HPLC reference the latex agglutination immune low turbidimetry results,the relative deviation of 4.21%, the data from two experimental methods regression statistics,r2=0.939.Conclusion HPLC method with latex enhanced turbidimetric immunoassay method of determination of glycated hemoglobin differences between the reference interval should be set up to carry out the comparison.
Objective To compare the difference of results of HBV in clinical samples detected by ELISA and chemiluminescence enzyme immunoassay based on magnetic micro-beads separately.Methods We tested HBV of the 44 clinical serum samples by the two methods at the same time,then used χ2 test for fourfold table data to explore the comparison of the results of HBsAg and HBsAb detected by the two different methods respectively.Results The both probability0.05 in χ2 tests,it means that there was no difference between the two methods in testing HBV of clinical serum samples.Conclusion Although the results tested by the two methods have no statistical difference,there are some discrepancies in some individual samples specifically construed via considering actual situation.
Objective To investigate the correlation between the outer membrane protein(OMP) and multidrug-resistant acinetobacter baumannii(MDR-Ab),and to understand the importance of OMP in acinetobacter baumannii multidrug resistance better. Methods 50 stains of acinetobacter baumannii separated from sputum samples of patients in intensive care unit(ICU)were selected, drug resistance situation of 50 strains of acinetobacter baumannii to ciprofloxacin(CIP) and imipenem(IMP) was analyzed statistically by Whonet5.4 software. The expression situation of MDR-Ab OMP in 8 strains of acinetobacter baumannii was analyzed by modified polyacrylamide gel electrophoresis to study the importance of OMP expression in multidrug resistance of acinetobacter baumannii. Results Drug resistance rate of acinetobacter baumannii strains separated from patients in ICU to CIP reached 88%,and the drug resistance rate to IMP reached 16% with serious drug resistance situation. MDR-Ab had one or several kinds of decline or loss of OMP expression. Conclusion OMP plays an important role in multidrug resistance of acinetobacter baumannii.
