目的:探讨术前应用生长抑素和吲哚美辛栓对于预防内镜下逆行性胰胆管造影术(endoscopic retrograde cholangiopancreatography,ERCP)术后并发急性胰腺炎和高淀粉酶血症的临床应用价值.方法:回顾性分析2016-08~2018-09在我科接受进行ERCP治疗的158例患者的临床病例资料,根据术前是否应用生长抑素、吲哚美辛栓剂,将其分为生长抑素(A)组56例、吲哚美辛栓剂(B)组32例和双药联合(C)组70例.比较3组患者术后12h及24h血清淀粉酶水平,评估3组患者术后急性胰腺炎及高淀粉酶血症的发病率;探讨生长抑素及吲哚美辛栓预防术后急性胰腺炎及高淀粉酶血症并发症的治疗效果及应用价值.结果:ERCP术后12h血清淀粉酶水平:A组(352.32±127.41) U/L,B组(425.93±135.62) U/L,C组(304.27±130.34) U/L.ERCP术后24h血清淀粉酶水平:A组(300.18±85.35)U/L,B组(378.24±131.67) U/L,C组(260.62±60.31)U/L.术后胰腺炎的发生率:A组4例(7.14%),B组3例(9.37%),C组3例(4.28%).B组术后胰腺炎发生率明显高于A组,两组比较差异有显著(P<0.05).C组术后胰腺炎发生率明显高于A组及B组,两组比较差异有显著(P<0.05).术后高淀粉酶血症发生率:A组4例(7.14%),B组3例(9.37%),C组4例(5.71%).B组术后高淀粉酶血症发生率明显高于A组,两组比较差异有显著(P<0.05).C组术后高淀粉酶血症发生率明显高于A组及B组,两组比较差异有显著(P<0.05).结论:①术前应用生长抑素预防ERCP后急性胰腺炎的发生优于吲哚美辛栓.②吲哚美辛栓联合生长抑素可以达到比使用单一药物更佳的术后急性胰腺炎防治效果.
目的:探讨益生菌联合美沙拉嗪对溃疡性结肠炎(Ulcerative Colitis,UC)小鼠结肠组织IFN-γ、IL-4的表达的影响.方法:将40只的健康清洁级KM小鼠,随机分为正常对照组(A1组,10只),其余30只随机分为3组,每组10只:模型组(A2组)、美沙拉嗪组(A3组)、联合治疗组(A4组).正常对照组正常喂食普通饲料,同时自由饮用灭菌水,其他3组自由饮用2.5%葡聚糖硫酸钠溶液(DSS)用以制成UC模型.A3组及A4组均于造模第3天时给予相应药物干预.每天观察一般状态:大便性状及颜色、毛色、弓背、体重等.计算统计疾病活动指数(DAI)及结肠黏膜病理损伤评分(CMDI)以评估监测小鼠结肠黏膜损伤程度.于第8天处死四组小鼠,剖腹取肠组织,采用免疫组化方法检测小鼠结肠组织黏膜中的IFN-γ、IL-4的表达情况.结果:正常对照组、美沙拉嗪组及联合治疗组的DAI评分、结肠黏膜损伤评分、IFN-γ的水平的表达较模型组明显降低,IL-4的表达程度均显著高于模型组,差异具有统计学意义(P<0.05);联合治疗组的DAI评分、CMDI评分、IFN-γ的水平的表达较美沙拉嗪组下降,IL-4的表达程度高于美沙拉嗪组,差异具有统计学意义(P<0.05).结论:益生菌联合美沙拉嗪可能抑制IFN-γ的表达、增强IL-4表达的作用,且联合用药治疗小鼠溃疡性结肠炎优于单独应用美沙拉嗪治疗.
目的:探究雷公藤多苷对溃疡性结肠炎(UC)小鼠肠组织中IL-6、IL-13表达的影响,并探讨雷公藤多苷治疗UC可能存在的机制.方法:30只KM小鼠分成正常组、模型组、雷公藤组(TL组),每组10只,正常组自由饮用清水,模型组和雷公藤组自由饮用2.5%右旋葡聚糖硫酸钠(DSS),观察模型组小鼠活动、排便、体重变化情况,完成UC模型,TL组给予雷公藤多苷灌胃7d,其余组给予等量盐水灌胃7d,使用ELISA酶联法对各组结肠组织IL-6、IL-13的含量进行检测.结果:模型组的肠组织损伤程度、镜下炎症表现均重于正常组和TL组,模型组肠组织中IL-6较正常组和TL组高,IL-13较正常组和TL组低,上述差异均具有显著性(P<0.05).结论:抗炎因子与抑炎因子之间平衡的打破对UC的发生起重要作用,雷公藤多苷可能通过调节细胞因子从而达到抗UC的作用.
Objective: To research the effects and mechanism of feeding Eucommia ulmoides Oliver orthodontic on periodontium remodeling in tooth movement in rats.Methods: 32 rats were divided into two groups randomly.Orthodontic;coil spring was fixed between the maxillary first molar and the two central incisors.The experimental group were fed with 1ml eucommia ulmoides Oliver orthodontic while the control group were fed with normal Nacl 1ml every day.The two groups of rats were executed in batches on the 7th,14th,21th,and the 28th days.The mandible of rats were separated,and the distance of tooth movement was measured.At the same time the maxillary first molar area periodontal tissue biopsies were produced.Light microscopy was used for observation.The immunohistochemical was used to detect RANKL expression.And PASW statistics l8 software was used for data processing,t test and variance analysis.Results: The teeth mobile distance in Eucommia ulmoides Oliver group was significantly greater than the control group,and the difference has statistical significance(P<0.05).Light microscopy showed that the number of salt group of osteoclast in periodontal tissue of the bark of encomia was obviously greater than the control group and the differences were statistically significance(P<0.05).Conclusion: Eucommia ulmoides Oliver can accelerate the orthodontic tooth movement in rats proliferation and differentiation of osteoclasts,alveolar bone absorption and alveolar bone reconstruction and,which is good for orthodontic tooth movement.
Objective: To use p-toluene sulfonic acid as the catalyst for the esterification reaction of cinnamic acid with isoamyl alcohol in liquid phase. Methods: The effects of the ratio of reactants, the amount of the catalyst used, the reaction time and the amount of an water-taking agent on synthesizing isoamyl cinnamate were investigated to obtain a suitable condition for synthesizing the ester. Results: The optimum conditions were found to be as follows: the molar ratio of isoamyl alcohol to cinnamic acid was 5:1,the quantity of catalyst was equal to 20% of the quantity of cinnamic acid, the reaction time was 2h, and the reaction temperature was 110~118℃.The yield reached 76.6%.Conclusion: p-toluene sulfonic acid is an excellent catalyst for synthesizing isoamyl cinnamate, and the technical process is simple, and the yields are higher, which has applied value.
