To investigate the effect of mild moxibustion on transient receptor potential vanilloid type 1 (TRPV1) channel expression in primary dysmenorrhea (PD) rats and explore its mechanism in alleviating central pain sensitization. Thirty-two female non-pregnant Wistar rats were randomized into a blank group, a model group, a mild moxibustion group, and a capsazepine group, with 8 rats in each group. Except for the blank group, the other three groups used estradiol benzoate, ice-water bath, and oxytocin to establish the rat PD model of cold-dampness stagnation pattern. The interventions began on day 1 of modeling, once a day, and lasted 10 d. The mild moxibustion group received mild moxibustion at Shenque (CV8) and Guanyuan (CV4), 20 min/time; in the capsazepine group, capsazepine was injected at a dose of 2 mg/(kg·bw). The abdominal pain threshold was measured 10–30 min after oxytocin injection on day 11; enzyme-linked immunosorbent assay was used to detect serum prostaglandin F2α (PGF2α) level; the expression of TRPV1, cluster of differentiation 11B (CD11B), and proto-oncogene c-Fos in the spinal dorsal horn and hypothalamus was detected by immunofluorescence and Western blotting. Compared to the blank group, the model group showed a decreased pain threshold (P<0.05) and an increased serum PGF2α level with elevated TRPV1, CD11B, and c-Fos protein expression in the spinal dorsal horn and hypothalamus (P<0.05). Compared to the model group, both the mild moxibustion group and capsazepine group showed significantly increased pain thresholds (P<0.05), along with decreased serum PGF2α levels and reduced protein expression levels of TRPV1, CD11B, and c-Fos in the spinal dorsal horn and hypothalamus (P<0.05). Rat pain threshold in the capsazepine group was higher than that in the mild moxibustion group (P<0.05). Serum PGF2α level, the expression levels of CD11B and c-Fos proteins in the spinal dorsal horn, as well as TRPV1, CD11B, and c-Fos proteins in the hypothalamus of the capsazepine group were lower than those in the mild moxibustion group (P<0.05). Mild moxibustion at Shenque (CV8) and Guanyuan (CV4) may alleviate the central pain sensitization in PD rats by down-regulating TRPV1 channel expression in the spinal dorsal horn and hypothalamus, thus playing an analgesic effect.
PurposeTo critically evaluate systematic reviews (SRs) of the Tai Chi (TC) exercise on bone health and provide more recently available evidence.MethodsSRs with or without meta-analysis (MA) of TC on bone health were comprehensively searched in eight electronic databases (PubMed, EMBASE, Cochrane Library, Web of Science, China National Knowledge Infrastructure, Wanfang Database, Chinese Biomedical Literature Database, and Chinese Scientific Journals Database) and in the international prospective register of systematic reviews of (PROSPERO) from initiation to March 2023. Descriptive analyses of SRs were performed, and reporting and methodological quality of the included SRs were evaluated using the updated version of Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) checklist and A Measurement Tool to Assess Systematic Reviews 2 (AMSTAR-2). The certainty of the synthesized evidence was assessed with the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) approach.ResultsEighteen SRs, 15 with MAs, were included. Forty-nine RCTs and 16 NRSIs with 3956 and 1157 participants, respectively, were included in these SRs. The reporting quality of the included SRs ranged from high to low, but most received critically low AMSTAR-2 scores. Efficacy of TC on nine bone health biomarkers has been explored, covering bone mineral density (BMD) and serum biomarkers. The results showed that compare to non-intervention, perimenopausal and postmenopausal participants who practiced TC may benefit in BMD of the lumbar spine [MD = 0.04, 95% CI (0.02, 0.07)], and femoral neck [MD = 0.04, 95% CI (0.02, 0.06)], but not BMD of the femoral proximal trochanter [MD = 0.02, 95% CI (0.00, 0.03)], ward's triangle [MD = 0.02, 95% CI (-0.01, 0.04)], and femoral shaft [SMD = 0.16, 95% CI (-0.11, 0.44)]. Elders practicing TC may benefit in BMD of the femoral neck [SMD = 0.28, 95% CI (0.10, 0.45)], femoral proximal trochanter [SMD = 0.39, 95% CI (0.05, 0.73)], and ward's triangle [SMD = 0.21, 95% CI (0.05,0.37)], but may not in BMD of lumbar spine [SMD = 0.03, 95% CI (-0.22, 0.27)].ConclusionWe have low certainty that for perimenopausal and postmenopausal women, compare to those with no exercise, TC could improve BMD of the lumbar spine, femoral neck. We also have low certainty that in elder population, TC practitioners may benefit in BMD of femoral neck, and Ward's triangle.RegistrationPROSPERO (CRD42020173543).
Objective To observe the effects of sparrow-pecking moxibustion at Shenque (CV8) and Guanyuan (CV4) on the writhing reaction and score, the temperature and blood flow perfusion of moxibustion point area and uterus, the serum levels of arginine vasopressin (AVP), prostaglandin (PG) F 2α , and thromboxane (TX) B 2 in rats with primary dysmenorrhea (PD) due to cold-dampness stagnation, and to explore the possible mechanism of sparrow-pecking moxibustion in treating PD. Methods Thirty-two healthy non-pregnant female Wistar rats were randomly divided into a normal group, a model group, an ibuprofen group, and a sparrow-pecking moxibustion group, with 8 rats in each group. Except for the normal group, the other three groups were subjected to modeling with cold water bath combined with estradiol benzoate and oxytocin injection. Rats in the sparrow-pecking moxibustion group were treated with sparrow-pecking moxibustion at Shenque (CV8) and Guanyuan (CV4) on the 8th day of modeling, 30 min/time, once a day for 3 d; those in the ibuprofen group were treated with 0.8 mL ibuprofen solution (a specification of 125 mg in 10 mL) on the 8th day of modeling, once a day for 3 d; those in the normal group and the model group were given 0.8 mL normal saline, once a day for 3 d. On the 11th day, rats in each group were intraperitoneally injected with oxytocin (2 U/rat), and the writhing incubation period and writhing score in 20 min were observed; the temperature and the blood perfusion of Shenque (CV8), Guanyuan (CV4), and uterus in vivo were detected; the serum levels of AVP, PGF 2α , and TXB 2 were determined. Results The writhing incubation period was significantly longer ( P <0.01) and the writhing score was significantly lower ( P <0.01) in the sparrow-pecking moxibustion group and the ibuprofen group than in the model group; compared with the ibuprofen group, the writhing incubation period was prolonged ( P <0.01) and the writhing score was decreased ( P <0.01) in the sparrow-pecking moxibustion group; compared with the normal group, the temperature and the blood perfusion of Shenque (CV8), Guanyuan (CV4), and uterus were significantly decreased, while the serum PGF 2α , AVP, and TXB 2 levels were significantly increased ( P <0.01) in the model group; compared with the model group, the temperature and the blood perfusion of Shenque (CV8), Guanyuan (CV4), and uterus were significantly increased, and the serum levels of PGF 2α , AVP, and TXB 2 were significantly decreased in the ibuprofen group and the sparrow-pecking moxibustion group ( P <0.05 or P <0.01); compared with the ibuprofen group, the temperature and the blood perfusion of Shenque (CV8), Guanyuan (CV4), and uterus were significantly increased ( P <0.05), the serum AVP and TXB 2 levels were significantly decreased ( P <0.05), while the serum PGF 2α level had no statistical difference in the sparrow-pecking moxibustion group ( P >0.05). Conclusion Sparrow-pecking moxibustion had a remarkable analgesic effect on the rats with PD due to cold-dampness stagnation, and the mechanism may be related to the increased temperature and blood perfusion of the moxibustion point area and uterus, as well as the decreased serum PGF 2α , AVP, and TXB 2 levels.
OBJECTIVE:To observe the effect of acupuncture of "Tianshu"(ST25) at different depths on colonic transportation function, expressions of colonic substance P (SP) and interstitial cells of Cajal (ICC) in rats with slow transit constipation (STC), so as to explore its mechanisms underlying improvement of STC.. METHODS:Fifty male Wistar rats were selected and randomly divided into control,STC model,conventional acupuncture,deep needling group 1 and deep needling group 2 groups,with 10 rats in each group.The STC model was established by gavage of 1 mg/mL compound diphenoxylate suspension (10 mg/kg), once every other day for 21 days, and rats of the control group were given the same dose of distilled water by gavage.EA (2 Hz, 2 mA) was applied to "Tianshu"(ST25), with the acupuncture needle inserted to a depth of 3 mm for rats of the conventional acupuncture group, 4.5 mm for those of deep needling group 1, and 10 mm for those of the deep needling group 2. The acupuncture needle was twirled for 1 min, then retained for 15 min each time, once a day for 15 consecutive days.Following modeling, rats of the 4 groups and the control group received gavage of active carbon 2 mL (100 g/L) for observing the excretion time of the first black stool grain to assess the intestinal transit function. The colonic myoelectric activities (frequency and amplitude) were recorded by using BIOPAC multichannel physiograph. The immunoactivity of SP and c-kit (a transmembrane protein kinase for identification of ICC) of colonic musculature was detected by immunohistochemistry. RESULTS:Compared with the control group,the time of excretion of the first black stool grain, and the amplitude of colonic electromyogram (EMG) were significantly increased (P<0.01), while the frequency of EMG, expressions of SP and c-kit (ICC) were significantly decreased in the model group (P<0.01). In contrast to the model group, both deep needling group 1 and 2 had a decrease in the time of excretion of the first black stool grain, and amplitude of intestinal EMG, and an increase of frequency of intestinal EMG, and immunoactivity of SP and c-kit (P<0.01). The effect of deep needling 2 is superior to that of deep needling 1 in reducing the time of excretion of the first black stool grain (P<0.05), lowering the amplitude of EMG of the gut smooth muscle (P<0.05) and in increasing the frequency of EMG (P<0.05) and the expressions of SP and c-kit (P<0.05). No significant changes were found in the levels of frequency and amplitude of EMG, and expressions of SP and c-kit after routine needling in comparison with the model group (P>0.05), except the excretion time of the first black stool grain (P<0.05). CONCLUSION:Deep needling at ST25 at depth of 4.5 mm and 10 mm,especially at depth of 10 mm,has a significant effect in promoting gut motility to ameliorate constipation in rats with STC, which may be related to its function in up-regulating the expressions of SP and ICC activity.
To deepen the understanding of the acupoint indications, clarify the targeting of acupoints, and provide a basis for the composition of acupuncture prescriptions, it is suggested to add acupoint identification into the textbook Meridians and Acupoints, and a preliminary assumption that relevant acupoints can be identified by taking the indications, locations, and meridians as the key points is proposed. In this paper, acupoints for treating stomach disease, acupoints of eye region, and five-shu points of lung meridian are taken as examples, combined with ancient literature and modern scientific research achievements, the main indications of acupoint is identified, which is of great significance to the discipline's development, talent training, and achievement transformation.
OBJECTIVE:To analyze the application characteristics of acupuncture and moxibustion in clinical treatment of Meniere's disease by using complex network technology, so as to provide evidence for selecting acupoints, needling and moxibustion methods and treatment ideas.METHODS:Articles both in English and Chinese published from the inception of databases of CNKI, Wanfang VIP, Chinese biomedical literature database (SinoMed), PubMed, Embase, EBSCO (Academic Search Pre-mier), Web of Science and Ovid to April of 2021 were retrieved by using key words "acupuncture" or "moxibustion" or "acupuncture and moxibustion" and "Meniere disease" or "Meniere's syndrome" or "Ménières vertigo" or "otogenic vertigo" or "auditory vertigo", followed by screening the literature according to the inclusion and exclusion criteria and establishing a database of clinical li-terature about acupuncture treatment of Meniere's Disease with software Epidata 3.1. Then, the descriptive analysis was conducted first, followed by association rule analysis using SPSS Modeler 18.0, and complex network analysis using Gephi 0.9.2 software.RESULTS:A total of 232 articles were included, containing 152 acupoints [97 body acupoints as Baihui (GV20), Fengchi (GB20), Neiguan (PC6), etc., 28 otopoints as Ershenmen (MA-TF1), Shen (MA-SC), etc., 20 scalp points as Yunting Area, 7 extra-points as Sishencong (EX-HN1), Taiyang (EX-HN5), etc.] which were used to be a total frequency of 1 569. Descriptive analysis showed that the main meridians were the Governor Vessel, Stomach Meridian of Foot Yangming, Trienergizer Meridian of Hand Shaoyang, and Gallbladder Meridian of Foot Shaoyang. Acupuncture was the most commonly used therapy for Meniere's disease. The association analysis showed that the most relevant combination of acupoints was GV20 and GB20, GV20 and PC6, reflecting the principles of local acupoint selection and combination of local and distant acupoints. Finally, "K-core Analytic Hierarchy Process" and "Community Analysis" revealed that 3 core acupoint groups were most frequently used in clinical treatment of Meniere's disease, including 1) auricular acupoints, as MA-TF1, MA-SC, Neier(MA-L), Zhen(MA-AT) and Pizhixia(MA-AT1), 2) acupoints of the 14 meridians and extra-points, as Tinggong(SI19), Yifeng(TE17), GB20, 3) acupoints of the Shaoyang meri-dians of hand and foot, as Shuaigu (GB8), Tinghui (GB2), Zhongzhu (TE3), Ermen (TE21), etc.CONCLUSION:The principle of acupoint selection is mainly based on the combination of acupoints along the meridians and local areas, while paying attention to the coordination among the auricular points, scalp acupoints and extra-points, which may provide a reference for the clinical treatment and scientific research on acupuncture treatment of Meniere's disease.
To observe the effects of ginger-partitioned moxibustion at Shenque (CV8) and Guanyuan (CV4) on the expression levels of endocrine-related molecules and their receptors in rats with primary dysmenorrhea (PD) due to cold-dampness stagnation, thus to explore their analgesic mechanisms. Thirty-two female Wistar rats were divided into a normal group, a model group, a ginger-partitioned moxibustion group, and a Western medicine group according to the random number table method, with 8 rats in each group. Except for rats in the normal group, all other rats were treated with oxytocin combined with ice-water bath to establish the rat models of PD due to cold-dampness stagnation. After successful modeling, rats in the normal group and the model group did not receive treatment; rats in the ginger-partitioned moxibustion group received treatments with ginger-partitioned moxibustion at Shenque (CV8) and Guanyuan (CV4); rats in the Western medicine group received ibuprofen by intragastric administration. The writhing response of rats was compared among groups, and the serum levels of prostaglandin F2α (PGF2α), estrogen (estradiol, E2), progesterone (P), and the mRNA expression of PGF2α and E2 receptors in the uterine tissues were detected. No writhing behavior was observed in the normal group; compared with the normal group, the serum PGF2α and E2 levels in the model group were increased (P<0.01), while the P level was decreased (P<0.01), and the mRNA expression levels of the uterine PGF2α and E2 receptors were increased (P<0.01, P<0.05). Compared with the model group, the writhing behavior latency was prolonged, and the writhing response score was decreased in the ginger-partitioned moxibustion group and the Western medicine group (P<0.01); the serum PGF2α and E2 levels in the ginger-partitioned moxibustion group and the Western medicine group were decreased, while the P level was increased (P<0.05 or P<0.01); the mRNA expression levels of the uterine PGF2α and E2 receptors in the ginger-partitioned moxibustion group and the Western medicine group were decreased (P<0.05). Compared with the Western medicine group, the ginger-partitioned moxibustion group showed a prolonged writhing behavior latency, reduced writhing response score (P<0.05), and decreased serum E2 level (P<0.05), while no statistical differences in the serum PGF2α and P levels, or the mRNA expression levels of uterine PGF2α and E2 receptors (P>0.05). The analgesic effect of ginger-partitioned moxibustion on PD due to cold-dampness stagnation may be related to regulating the mRNA expression levels of PGF2α and E2 receptors in the uterine tissues.
Using the complex network technology, the characteristics of the core acupoint prescriptions and the application of acupuncture-moxibustion techniques were analyzed in treatment of rheumatoid arthritis (RA) so as to provide the evidences for acupoint selection and therapeutic methods for RA treated with acupuncture and moxibustion. The articles of acupuncture and moxibustion for treatment of RA in recent 20 years were collated and imported, and the database of the acupoint prescriptions was developed. Using Cytoscape 3.9.2 software, the acupoints in the prescriptions were visualized for the common occurrence network analysis. The association rule analysis was performed with IBM SPSS Modeler 18.0 software and the complex network analysis was by Gephi 0.9.2 software. A total of 798 articles were screened, in which, 3 258 prescriptions were extracted with 253 acupoints involved. The analysis of acupoint selection was conducted in terms of syndrome/pattern differentiation, acupoint locations and main acupoints, and therapeutic methods. The results showed that the most common TCM syndromes of RA included painful bi syndrome, wandering bi syndrome, fixed bi syndrome and bi syndrome due to wind, damp and heat. Regarding the core combination of acupoints, painful bi syndrome: Guanyuan (CV 4) and Shenshu (BL 23); fixed bi syndrome: Yinlingquan (SP 9), Sanyinjiao (SP 6), Zusanli (ST 36), Pishu (BL 20) and Fenglong (ST 40); wandering bi syndrome: Fengchi (GB 20), Geshu (BL 17), Fengmen (BL 12), Xuehai (SP 10) and Waiguan (TE 5); bi syndrome due to wind, damp and heat: Dazhui (GV 14), Quchi (LI 11) and Hegu (LI 4). Regarding the acupoint locations, the acupoints located in the upper limbs, lower limbs and spinal region were generally selected. Quchi (LI 11) was one of the main acupoints in prescriptions with the highest use frequency. Zusanli (ST 36) and Shenshu (BL 23) presented the highest co-occurrence intensity, while Zusanli (ST 36) and Quchi (LI 11) indicated the highest correlation. The treatment of acupuncture-moxibustion for RA is generally complied with the principle as "the synthesis of main acupoints, supplementary acupoints in local affected area and those based on syndrome differentiation". Warm needling and the combination of acupuncture and herbal medication are the most common therapeutic methods for RA.

OBJECTIVE:To observe the effect of electroacupuncture (EA) of bilateral "Tianshu"(ST25) at different frequencies on the first black stool discharge time, colonic electromyography (EMG) and immunoactivity of vasoactive intestinal peptide (VIP) and substance P (SP) in the colon tissue in rats with slow transit constipation (STC), so as to choose a better stimulating frequency in the treatment of STC. METHODS:A total of 50 male Wistar rats were randomized into control, model, 2 Hz-EA, 100 Hz- EA, and 2 Hz/100 Hz-EA groups, with 10 rats in each group. The STC model was established by intraperitoneal injection of compound diphenoxylate suspension fluid (10 mg/kg). EA was applied to bilateral ST25 for 30 min, once a day for 14 consecutive days. The discharge time for the first black stool was recorded after gavage of mixed suspension fluid of active carbon (2 mL) for assessing the gastrointestinal motility. The colonic EMG was recorded by using a pair of silver electrodes and bioelectric amplifier. The expression of SP and VIP in the colonic tissue was detected by immunohistochemistry. RESULTS:Following mode-ling, the colonic EMG amplitude, the discharge time for the first black stool and VIP immunoactivity were significantly increased (P<0.01), and the EMG frequency and SP immunoactivity were significantly decreased (P<0.01) in the model group compared with the control group. Compared with the model group, the increase of the discharge time for the first black stool, EMG amplitude and VIP immunoactivity, the decrease of EMG frequency and SP immunoactivity were reversed in the 3 EA groups (P<0.01, P<0.05). The therapeutic effect of 100 Hz-EA was notably weaker than that of both 2 Hz-EA and 2 Hz/100 Hz-EA in up-regulating EMG frequency and SP immunoactivity and in down-regulating the discharge time for the first black stool, EMG amplitude and VIP immunoactivity (P<0.05, P<0.01). There was no significant difference between the 2 Hz/100 Hz-EA and 2 Hz-EA groups (P>0.05). CONCLUSION:EA can accelerate colonic EMG activities, which may be associated with its functions in down-regulating VIP immunoactivity and up-regulating SP immunoactivity in the colonic tissues. The therapeutic effects of 2 Hz/100 Hz-EA and 2 Hz-EA are better than that of 100 Hz-EA.
The scalp acupuncture therapy, an important component of the microneedle system of the acupuncturology, is effective in the treatment of multiple types of diseases. In the present paper, we made a comparative analysis about the theoretical basis, acupoint location and main clinical indications of the ten academic schools of scalp-acupuncture, including FANG Yun-peng's Scalp Acupuncture, JIAO Shun-fa's Scalp Acupuncture, ZHU Ming-qing's Scalp Acupuncture, etc. which are widely used in clinic at present after collecting the related articles published in recent 60 years from databases of CNKI, Wanfang and VIP, and relevant books. These nine academic schools are similar in clinical indications (such as neurological disorders, brain-derived di-seases) and in needle inserting angle and depth, and different in the needling manipulations, needlingmethods, needle retaining time, theoretical basis, attending diseases of the same one stimulated region, and the position and yin or yang of holograms. The main problems facing the scalp acupuncture are non-uniform positioning method, non-uniform needling method, and non-uniform indications of the same scalp acupoint. Up to now, it remains unclear that which academic school is better in the therapeutic efficacy, thus, we should strengthen clinical research to find out its inherent law in diagnosis and treatment, constantly optimize the needle scheme, determine the best scalp-acupoint position and best needling manipulations, quantize and standardize the related issues, and accelerate the integration of the different academic schools, so as to improve clinical curative effect and to further promote the application and popularization, serving the patients in a better way.
Objective To observe the effects of electroacupuncture (EA) of different frequencies on transmission function, electromyography, nitric oxide synthase (NOS) content and interstitial cells of Cajal (ICC) expression of colon in rat models with slow transit constipation (STC). Methods Of the 50 healthy male Wistar rats, 10 were randomly selected as a normal group and fed with ordinary diet, and the remaining 40 rats were fed with the diet added with the compound diphenoxylate at a dose of 8 mg/(kg·bw) each day for continuous 120 d. The 40 successfully established STC rat models were randomly divided into a model group, a low-frequency EA group (2 Hz), a high-frequency EA group (100 Hz), and a variable-frequency EA group (2 Hz/100 Hz), with 10 rats in each group. Rats in the normal and the model groups were not given any treatment; the low-frequency EA and the high-frequency EA groups were given EA at Tianshu (ST 25), Zusanli (ST 36) and Zhigou (TE 6) with continuous wave at the designated frequency, and the variable-frequency EA group received sparse-dense wave (2 Hz/100 Hz) EA at the same acupoints, once a day for a total of 15 d. After treatment, the colonic transmission function, electromyography, NOS content and ICC expression (calculated by the difference in the area of the C-kit positive cells) of the rats in each group were measured. Results For the colonic transmission function, compared with the normal group, the first black stool excretion durations of rats in the other groups were significantly prolonged (all P <0.05); compared with the model group, the first black stool excretion durations of rats in the three EA groups were significantly shortened (all P <0.05), which was significantly shorter in the variable-frequency EA group than in the low-frequency EA and high-frequency EA groups (both P <0.05). For the colonic electromyography, compared with the normal group, the amplitude was significantly increased and the frequency was accelerated in rats of the other groups (all P <0.05); compared with the model group, the amplitude was significantly reduced and the frequency was slowed down in the three EA groups (both P <0.05); compared with the low-frequency EA and the high-frequency EA groups, the amplitude was reduced and the frequency was significantly reduced in rats of the variable-frequency EA group (both P <0.05). Compared with the normal group, the colonic NOS contents were significantly increased in the other groups (all P <0.05); compared with the model group, the NOS contents were significantly reduced in the three EA groups (all P <0.05); compared with the low-frequency EA and the high-frequency EA groups, the content was significantly reduced in the variable-frequency EA group (all P <0.05). For the area of rat colonic C-kit-positive cells, compared with the normal group, the areas were significantly reduced in rats of other groups (all P <0.05); compared with the model group, the areas were increased significantly in the three EA groups (all P <0.05); compared with the low-frequency EA group, the area was increased significantly in the variable-frequency EA group ( P <0.05). Conclusion EA, especially EA at the 2 Hz/100 Hz variable-frequency, has a positive treatment effect on the STC model rats. It may improve rats’ colonic function by regulating the electromyography, NOS content and ICC expression of colon.
OBJECTIVE:To investigate the disease spectrum and predominant diseases treated by abdominal acupuncture by data mining and analysis of journal articles on abdominal acupuncture, and to provide a reference for clinical diagnosis and treatment. METHODS:Based on the database of abdominal acupuncture established by the research group, the data mining technique was used for the analysis and extraction of the articles on abdominal acupuncture included in this study. RESULTS:A total of 788 original journal articles were included. Six departments and 96 disease categories, among which there were 45 internal diseases (46.84%) and 18 surgical diseases (18.75%) were involved. As for the diseases involved, cervical spondylosis had the highest frequency of 84, followed by low back and leg pain with a frequency of 77 and stroke sequela with a frequency of 67. Of all 788 studies, 519 (65.86%) used abdominal acupuncture combined with other therapies with a total frequency of 552, among which acupuncture had the highest frequency of 135 (24.46%), followed by oral administration of traditional Chinese medicine with a frequency of 81 (14.67%) and moxibustion with a frequency of 80 (14.49%). Abdominal acupuncture had a marked clinical effect in the treatment of various diseases, with the highest effective rate of 95.10% in surgical diseases. CONCLUSION:Abdominal acupuncture has a wide disease spectrum and is most frequently used for the treatment of cervical spondylosis, with a marked clinical effect. Abdominal acupuncture has unique therapeutic characteristics and advantages, but it can achieve a better clinical effect when combined with other therapies.
OBJECTIVE To investigate the features, dominant indications and application regularities of eye acupuncture therapy in clinical practice. METHODS We first collected original articles, academic dissertations, conference articles, and case reports from databases of CNKI, Wanfang, and Chinese biomedical literature (CBM) from Jan. 1 of 1980 to Dec. 31 of 2017, and medical records from medical books of Zhonghua Yan Zhen (Chinese Eye Acupuncture Therapy), Yanzhen Shijian Lu (Records of Application of Eye Acupuncture Therapy), and Peng Jing-shan's Yanzhen Liaofa Yanjiu (Peng Jing-shan's Eye Acupuncture Therapy) by using keyword of "eye acupuncture". Then, we established an "Eye-acupuncture Database" after sorting, screening, typing-in, checking, and extraction of the collected data by SQL server 2005 and 2005 Analysis Services (SASS), followed by analysis of the dominant indications, needling methods, needle manipulations, principles for acupoint selection, and clinical effect. RESULTS Among the 850 articles retrieved, 178 were eligible to be subjected to analysis, including 42 disorders/diseases in 7 categories. The eye acupuncture therapy has the highest application frequency for 19 types of disorders/diseases of the internal medicine, such as sequela of apoplexy, hiccup, insomnia, vertigo, hypertension, etc., followed by head and body pain (7 types: headache, lumbago, sciatica, shoulder pain, etc.), disorders of the ophthalmology and otorhinolaryngology (deafness, squinting eyes, dry eye syndrome, amblyopia, etc.), traumatic injury, gynecopathy, acute pain (biliary colic, renal colic). The effect rate was 95.87% for disorders of the gynecopathy, 95.54% for head and body pain, 94.53% for traumatic skin injury, 92.16% for emergency, 91.98% for disorders of the five sense organs, 91.43% for problems of the internal medicine, etc. Regarding the principle of acupoint selection, acupoints were chosen mainly according to the meridian running (146 articles), local region of the eye (14 articles) and diseased location (153 articles). A total of 157 articles have a marked description about the needling methods, including horizontal needling (81 articles), intra-orbital straight needling (22 articles), extra-orbital oblique needling (13 articles), etc. The duration of needle retaining was mostly from 15 to 30 minutes, and the handle-scraping was frequently adopted (34 articles), and the overall treatment frequency ranged from 11 to 20 times. In regard to the medical records, a total of 53 types of disorders were involved, including 37 types of the internal medicine such as stroke sequelae (34 articles), facial paralysis (11 articles), etc. The acupoints were selected according to the local region of eye (129 articles), the needle insertion involved horizontal needling (102 articles), and cured rate reported was higher than 90%. CONCLUSION Eye acupuncture therapy is widely used for a variety of disorders and has a positive effect, particularly for stroke sequelae and various acute and chronic pain problems.
OBJECTIVE:To observe the therapeutic effect of acupuncture therapy in the treatment of dysphagia in apoplexy patients. METHODS:A total of 104 patients with post-stroke dysphagia were randomized into a control (conventional swallowing rehabilitation training) group and an acupuncture group (n= 52 cases in each one). In the control group, the conventional swallowing rehabilitation training was conducted, twice daily, 6 times a week for 4 weeks. In the acupuncture group, deep needling of main acupoints Lianquan (CV23) and Yifeng (TE17), and conventional acupuncture of adjuvant acupoints as Fengchi (GB20) and Fengfu (GV16), Wangu (GB12), Lieque (LU7), Jinjin (EX-HN12), Yuye (EX-HN13), etc. as well as electroacupuncture stimulation (15-20 Hz, 5 mA, and duration of 30 min) of ipsilateral CV23-GV16, TE17-GB20, and bilateral Neidaying acupoints were conducted. The acupuncture treatment was given once daily, 6 times a week for consecutive 4 weeks. In addition, patients of the two groups also received routine symptomatic treatment with drugs for anti-platelet aggregation, nourishing cranial nerve, lowering blood pressure, controlling blood glucose, improving cerebral circulation, etc. Before and after the treatment, the standard swallowing assessment (SSA, 18-46 points) and Kubota water swallowing test (WST, 1-5 grades) were conducted to evaluate the patient's swallowing function. The comprehensive therapeutic effect was assessed in accordance with the SSA and Kubota WST, and adverse reactions were recorded. RESULTS:After the treatment, the SSA score was considerably reduced in the two groups relevant to their own pre-treatment (both P<0.05), and was significantly lower in the acupuncture group than in the control group (P<0.05). The Kubota WST grade was evidently increased in the number of patients with grade Ⅰ (P<0.05) and reduced in the numbers of patients with grade Ⅲ-Ⅴ in both groups relevant to their own pre-treatment, and was more patients with grade Ⅰ in the acupuncture group than in the control group (P<0.05). Outcomes of the comprehensive therapeutic effect indicated that of the two 52 cases in the control and acupuncture groups, 8 (15.38%) and 19 (36.53%) were cured, 7 (13.46%) and 15 (28.45%) had marked improvement, 17 (32.69%) and 12 (23.08%) were effective, and 20 (38.46%) and 6 (11.54%) failed in the treatment, with the effective rate being 61.54% and 88.46%, respectively. The total effective rate of the acupuncture group was significantly higher than that of the control group (P<0.05). The adverse reactions such as regional hematoma (in 3 cases) and pain (in 2 cases) in the acupuncture group, and choking-coughing in the control group were seen, being 9.62% and 11.54% in the incidence rate, respectively. CONCLUSION:Deep needling of main acupoints Lianquan (CV23) and Yifeng (TE 17) in combination with conventional acupuncture of other acupoints is effective in improving local glossopharyngeal function in apoplexy patients with dysphagia, which is obviously superior to conventional swallowing rehabilitation training in the therapeutic effect and is applicable in clinical practice.
OBJECTIVE To explore the regularities of clinical application of hand acupuncture therapy to the treatment of diseases of different departments by using data mining technique, so as to provide a reference for clinical practice. METHODS We searched papers, academic dissertations and case reports (published in journals from Jan 1, 1953 to December 31, 2017) from databases of China National Knowledge Internet (CNKI), VIP, and Wanfang, and also searched papers from medical books about hand acupuncture treatment of different diseases/illness by using keywords of "hand acupuncture" and "hand acupoints", followed by constructing a database after sorting, screening, recording, extracting, and statistical analysis by using a computer. Then, the data mining was conducted for summarizing the characteristics and rules of the hand acupuncture therapy in clinical application. RESULTS The hand acupuncture therapy in Chinese medical journals was found to be applied to various clinical departments, with the frequency from high to low successively being 16 types of diseases (37.21%) in the surgery, 15 diseases (34.88%) in the internal medicine, 2 diseases (4.65%) in the pediatrics, 8 diseases (18.60%) in the ophthalmology-otorhinolaryngology, 1 disease (2.33%) in the genecology and 1 disease (2.33%) in the dermatology. In the treatment of diseases, the cure plus markedly effective rate of hand acupuncture was found to be highest for diseases of the pediatrics (90.65%), and was 88.45% surgery, 78.06% internal medicine, 89.96% ophthalmology-otorhinolaryngology, 60.61% gynecology, and 67.73% dermatology, respectively. When hand acupuncture used, the acupoints of the hand region on both sides, healthy side and affected side were often selected, and movement exercise was often used in combination, particularly for problems of the surgery. In the collected medical records and 6 acupuncture books, the hand acupuncture was employed for treatment of 34 and 123 kinds of diseases or problems, respectively, with the tissue injury, lumbocrural pain being most frequently seen in the records, and abdominal pain, lumbocrural, chest obstruction, etc. most frequently seen in the acupuncture books. CONCLUSION Hand acupuncture therapy is widely used in the treatment of many clinical problems of different departments, such as tissue injury, leg pain, etc. of the surgery; abdominal pain, asthma, etc. of the internal medicine; infantile enuresis, diarrhea, etc. of the pediatrics. Acupoints of the hand on the unilateral side are frequently employed and may have a better therapeutic effect.
OBJECTIVE:To analyze the literature regarding wrist-ankle acupuncture therapy by data mining technology, and the dominant diseases of wrist-ankle acupuncture therapy were summarized to provide reference for evaluation of specificity effect of wrist-ankle acupuncture therapy.METHODS:The journal articles regarding wrist-ankle acupuncture therapy from the CNKI, WANFANG, and VIP since January 1, 1975 to December 31, 2017, and the medical cases regarding wrist-ankle acupuncture therapy in Practical Wrist-Ankle Acupuncture Therapy and Microacupuncture System Diagnosis and Treatment were retrieved. Based on the disease types of wrist-ankle acupuncture therapy in the journal articles and medical cases, the association rules method of data mining technology was applied to calculate frequent itemsets. The self-developed database platform for wrist-ankle acupuncture therapy was used to extract and summarize the information to explore the rules of clinical application.RESULTS:The scope of disease involved a total of 7 disease entities in the literature regarding wrist-ankle acupuncture therapy. In the journal articles, there were 83 disease types and the most common surgical diseases were soft tissue injury and periarthritis of shoulder, the frequency were 34 and 24 times respectively. The total effective rates of wrist-ankle acupuncture therapy were 92.74% in internal medicine, 91.39% in surgery, 91.51% in gynecology, 90.88% in dermatology, 96.20% in pediatric, 89.05% in ophthalmology and otorhinolaryngology and 88.78% in psychiatry. In the medical cases of wrist-ankle acupuncture therapy, there were 118 disease types and the most common diseases were pain and Bi syndrome, herpes zoster was also a common disease. Psychiatric diseases used more wrist-ankle acupuncture therapy, and the common diseases were anxiety and mania.CONCLUSION:Wrist-ankle acupuncture therapy is widely used in treatment of clinical diseases and has significant therapeutic effects. It is mostly used in the treatment of surgery, internal medicine (neurology particularly), dermatology (herpes zoster particularly). Besides, the wrist-ankle acupuncture therapy is also applicable in the treatment of diseases of ophthalmology and otorhinolaryngology, gynecology, pediatric and psychiatry.
OBJECTIVE:To explore the regularity and characteristics of clinical application of scalp acupuncture therapy for different types of clinical conditions so as to provide a reference for clinical practice.METHODS:In the present study, "head acupuncture" and"scalp acupuncture" were used as the keywords to search papers and academic dissertations having definite standards for diagnosis and therapeutic effect assessment and published in journals and academic conferences collected in database China National Knowledge Internet(CNKI) from January 1 of 1959 to December 31 of 2016, followed by constructing a database after sorting, screening, recording, extracting, and statistical analysis by using a computer. Then, the data mining was conducted for summarizing the indications of disease categories, involved medical departments, needle-insertion methods, needle manipulation techniques, academic schools, and clinical efficacy of scalp acupuncture therapy.RESULTS:As a result, a total of 587 papers met our including criteria were analyzed. The scalp acupuncture therapy was widely employed in the treatment of various clinical conditions of different departments, with the application frequency being the internal medicine (438 times), surgery (75 times), pediatrics (44 times), etc. Of the indicated 102 types of clinical problems, 55 belong to the internal medicine, constituting of 53.92%, particularly the cerebral apoplexy and its sequelae with the top application frequency being 102 and 115 times, respectively. In terms of needle inserting methods mentioned in partial papers (most papers do not mention), fingernail-pressing-aided needle insertion, needle-twirling insertion, fingers-squeezed-needle insertion, particularly the swiftly rotating needle insertion and rapid needle-propelling insertion were most commonly used.Regarding the needle manipulation method, rapid needle twirling technique was frequently employed, usually at a frequency of approximately 200 times per min. In regard to the academic schools, JIAO Shun-fa's scalp acupuncture system was most frequently used, followed by the international standardized scalp acupuncture. The therapeutic effect of scalp acupuncture is effective in the treatment of different conditions of various departments, especially those of the dermatology and gynecology.CONCLUSION:Scalp acupuncture has superiority in the treatment disorders of the internal medicine and has been demonstrated to have positive effects for many types of problems, particularly for apoplexy and its sequelae. Rapid needle-propelling insertion and rapid needle-twirling technique are often employed.
Objective: To explore the rules of the second metacarpal bone lateral needling in clinical application and its characteristics so as to provide the guidance to clinical practice. Methods: Taking "the second metacarpal holographic acupuncture therapy"" the second metacarpal bone lateral needling" and "the second metacarpal bone" as the search terms, the papers on the second metacarpal bone lateral needling were retrieved from China National Knowledge Infrastructure (CNKI) and Wanfang database from January 1, 1974 to December 31, 2016 and were screened. Using the Excel 2003, the database of the diseases treated by the second metacarpal bone lateral needling was set up. The indications, the acupoints selection rules and needling manipulation of this therapy were analyzed. Results: A total of 1103 papers were retrieved, of which, 446 papers were from CNKI and 657 from Wanfang database. According to the inclusion and exclusion criteria, 54 papers were included for analysis finally. It was found that the indications of the second metacarpal bone lateral needling were predominated by the soft tissue injuries, especially acute lumbar sprain, ankle sprain and knee joint injury. Regarding the acupoints selection rules of the second metacarpal bone lateral needling, the corresponding points and tender points were commonly selected. Regarding the needling manipulation, the strong stimulation was required after needle insertion and the patient was required to move the affected areas while keeping the strong needling sensation. The needles were inserted 20 mm in depth along the skin mainly. The even-needling technique was exerted and the needles were retained for 30 min mainly. Conclusion: The second metacarpal bone lateral needling is applied to the treatment of soft tissue injuries. It is applicable for some internal diseases. The strong needling sensation and the motion of the local affected area are required especially in treatment with this therapy. (c) 2018 Published by Elsevier B. To observe the effects of electroacupuncture (EA) of different frequencies on the expression levels of substance P (SP) and vasoactive intestinal peptide (VIP) in the colon of rats with slow transit constipation (STC).
