Objective:To understand the risk factors of the prognosis of children with hand, foot and mouth disease(HFMD), and the threshold of related indexes.Methods:By retrospective analysis, 80 HFMD children admitted to Zhongshan County People's Hospital from September 2018 to November 2019 were selected. According to the prognosis, the children were divided into good prognosis group ( n=55) and poor prognosis group ( n=25). The general data, occurrence of related symptoms, laboratory indicators were compared between the two groups. ROC curve was used to determine the value of biochemical indicators in predicting the prognosis of HFMD children. Differential information was included in the Logistic regression model to identify the risk factors of poor prognosis of HFMD children. Results:The areas under the curve of lactate clearance, cardiac output and PaO 2/FiO 2 were 0.879, 0.767 and 0.675, respectively. Logistic regression analysis showed that lactate clearance rate < 25.655% ( OR=2.689, 95% CI: 1.467-4.929), cardiac output<2.465 L/min( OR=1.487, 95% CI: 1.078-2.051), PaO 2/FiO 2<271.645 mmHg (1 mmHg=0.133kPa) ( OR=3.689, 95% CI: 1.251-10.878), hyperglycemia >8.3 mmol/L ( OR=5.423, 95% CI: 1.389-21.173) and convulsion( OR=11.258, 95% CI: 1.201-105.531) were the risk factors for poor prognosis of HFMD. Conclusions:Early symptoms such as hyperglycemia and convulsion, lactate clearance rate < 25.655%, cardiac output < 2.465 L/min, PaO 2/FiO 2 <271.645 mmHg may cause poor prognosis in children with HFMD, and it requires timely attention of clinicians and intervention.
目的:通过对呼吸科患者进行痰涂片镜检测,探究痰涂片镜检对痰液细菌培养作用价值.方法:从来我院诊治的2016年1月—2018年12月的呼吸科门诊患者中随机抽取1200份,并进行痰涂片染色和细菌培养,对痰涂片标本培养过程进行跟踪观察并进行对比分析.结果:1200份标本中,含有500份革兰氏染色涂片,占总标本的41.67%,其中,含有230份检出细菌,占500分革兰氏染色涂片的46%,白细胞内有60份标本检出细菌,占230分检出细菌标本的26.09%;在1200分标本中有420份抗酸染色涂片标本,占35%,其中有10份抗酸杆菌,占420份抗酸染色涂片的2.4%.结论:痰标本涂片镜检验痰培养的结果具有可靠性,对于临床诊断具有重要意义,是一项值得推广使用的检验方法.
