Signal enhancement on T2-weighted images of the thalamus on one or both sides by magnetic resonance imaging (MRI) is among the most frequently observed neuroradiological findings in patients with acute tick-borne encephalitis. The article presents a clinical case of a patient who had an encephalo-polyoencephalomyelitic form of tick-borne encephalitis with MRI picture of a typical bilateral thalamic lesion. Such changes according to neuroimaging studies may have a clinical differential-diagnostic value, and the knowledge of these features by neuroradiologists and neurologists will contribute to the correct diagnosis of acute tick-borne encephalitis, especially most severe forms.
Injections of botulinum toxin are widely used in different medical fields, namely neurology, urology, stomatology, cosmetology, gastroenterology etc. Preparations of botulinum toxin type A (BTA) prevent the release of acetylcholine at the endings of motor nerves leading to the long-term muscle relaxation. It has been acknowledged that treatment with BTA has very good safety profile and tolerability. Extremely rare but severe complication of botulinotherapy (BT) is a condition, which is associated with generalized muscle Weakness, swallowing difficulty, respiratory arrest, and may lead to the lethal outcomes in the solitary cases. Such disorders, which present like botulism, are known as botulism-like syndrome and iatrogenic botulism. We report a clinical case of such complication in the paper. The probability of the development of such rare but severe complications necessitates certain awareness and vigilance among clinicians performing BT.

Progressive supranuclear palsy (PSP) is a severe old-age neurodegenerativedisease that is characterized by pronouncedclinical polymorphism and has different phenotypes. Practicaldifferential diagnosis of PSP and Parkinsons disease(PD) presents significant challenge, especially in onset of thedisease. We report an atypical case of PSP characterized bysymmetric levodopa-resistant parkinsonian syndrome, earlypostural instability, supranuclear oculomotor disorders (verticalgaze palsy), MR signs of midbrain atrophy, and a numberof overlapping clinical symptoms similar with the manifestationsof PD. A detailed algorithm of differential diagnosis ofPSP and PD as well as a more adequate diagnostic formulationare proposed for this and similar cases of neurodegenerativepathology.
