Objective: The aim of the study is to describe the season of RSV prevalence in China during the COVID-19 pandemic. Methods: This multicenter retrospective study analyzed the epidemiology of pediatric RSV infections and the possible factors contributing to its variations in China from January 1, 2019, to October 31, 2022. Results: A total of 872,565 children were included. During the pandemic, RSV detection rate increased across various regions, including South China, East China, Central China, and Northeast China. From 2019 to 2021, the detection rates of RSV showed an increasing trend among children aged <1 year, 1-2 years, and 3-5 years, but decreased in 2022. Among those tested positive for RSV, the proportion of children under 1 year old significantly decreased during the pandemic. The spring season of RSV in China in 2020 was shortened, and most regions experienced a summer season of RSV in 2021. This shift led to a year-round RSV outbreak throughout 2021. After April 2022, RSV positive rate significantly decreased, and no clear seasonal pattern was observed. Conclusion: Our study found that the COVID-19 pandemic has disrupted the seasonal pattern of RSV outbreaks in China, leading to increased RSV positive rate and off-season outbreaks.
目的:总结分析儿童SAPHO综合征临床特征和诊疗经过,提高临床医生对该疾病的认识。方法:回顾分析内蒙古医科大学附属医院儿内科病房收治的1例SAPHO综合征患儿的临床特征、实验室检查、影像学资料及诊疗经过。结果:患儿,女,4岁,主要表现为双手足脓疱伴右侧腰骶部疼痛及活动受限。实验室检查结果示:白细胞计数较正常值升高,C-反应蛋白轻度升高,红细胞沉降率升高。影像学检查结果示:骶髂关节MRI:右侧骶髂关节水平髂骨骨髓水肿,右侧坐骨小片状骨髓水肿;骶髂关节CT:右侧骶髂关节髂骨面局限性骨质破坏。掌跖脓疱局部皮肤活检病理:掌跖脓疱病。给予醋酸泼尼松、甲氨蝶呤及阿达木单抗治疗,患儿病情缓解,无新发手足脓疱,无关节肿痛、活动受限,炎症指标恢复正常。结论:儿童SAPHO综合征临床表现和成人有所不同,早发现、早治疗,一般预后较好。
目的:回顾性分析13例儿童布鲁菌病病例的流行病学调查和临床资料,总结其临床特征和治疗经验。方法:回顾性分析2017年3月至2021年7月内蒙古医科大学附属医院儿科收治的13例儿童布鲁菌病患儿的病例资料,对患儿的流行病学特点、临床表现、实验室检查结果、影像学检查结果、临床诊断、治疗方案及预后等情况进行统计与分析。结果:13例患儿平均年龄7.5岁;男11例,女2例。其中3例有羊接触史及家庭成员患布鲁菌病病史,其余均为有牛羊鲜奶或生肉接触史。发病时间全年中以3~8月发病率最高,占发病总数的92.3%(12/13)。临床表现以发热为主,占92.3%(12/13),其次是出汗、疲劳、皮疹、关节炎等。实验室检查显示血清凝集试验阳性率100%(13/13),血培养阳性9例(69.2%)。13例患儿均给予口服多西环素、利福平、复方新诺明联合治疗,疗程结束后均随访1年或至今(每3个月1次),痊愈12例(92.3%),后遗症1例(7.7%)。结论:儿童布鲁菌病临床表现可涉及多器官、多系统,发热为最常见的临床表现;早期治疗预后良好。
本文回顾性分析了2例难治性肺炎支原体肺炎合并心脏血栓的患者,2例患者均以发热、咳嗽起病,病程中通过行超声心动图检查发现心脏血栓,但患者未出现与栓塞有关的临床表现.肺炎支原体肺炎合并心脏血栓在临床上较为罕见,一般无特异性临床表现,临床医生应提高警惕,出现血栓形成的危险因素时,应进行相关检查,以免造成漏诊.
目的:通过对患有肺炎支原体肺炎的患儿血清维生素A的水平、体液免疫蛋白的水平的比较,分析两者之间的相关性,探讨维生素A治疗肺炎支原体肺炎的临床意义.方法:筛选符合研究条件的2017-08~2020-09期间在内蒙古医科大学附属医院儿科病房接受治疗的240例肺炎支原体肺炎患儿作为试验组,筛选同一时间段在本院儿科门诊就诊的240例非肺炎支原体感染的儿童作为对照组,通过比较两组之间血清维生素A的水平及体液免疫蛋白的水平,并分析两者之间有无相关性.结果:对照组测定的血清维生素A平均水平明显高于试验组(P<0.05),对照组测定的血清IgA平均水平明显高于试验组(P<0.05),试验组血清维生素A水平与血清IgA水平呈正相关(P<0.05)结论:血清维生素A水平与儿童肺炎支原体肺炎的发生具有相关性,与体液免疫蛋白水平具有相关性.
Purpose: To employ meta-analysis to assess the effectiveness and adverse events of Xiyanping and ribavirin in children with rotavirus enteritis. Methods: Searches included PubMed, Embase, Medline, Cochrane Library, China National Knowledge Infrastructure (CNKI), China Biology Medicine Database (CBM), China Science and Technology Journal Database (VIP database), and Wanfang database from inception to March 2017. Clinical indicators, with respect to the total effectiveness rate, fever time, duration of diarrhea, creatinine kinase MB isoenzyme (CK-MB), rotavirus (RV) negative conversion rate, and adverse reaction, were compared between Xiyanping and ribavirin groups. Results: Five-hundred and twenty-six records were obtained after searching the databases, and 18 studies (3557 participants) that met the inclusion criteria were included. All of them assessed total effectiveness rate, and two records evaluated RV negative conversion rate. There were significantly higher rates in the Xiyanping group than in the ribavirin group {OR = 3.76, 95 % CI (3.03 - 4.66), p < 0.00001; OR = 2.68, 95 % CI (1.56 - 4.60), p = 0.0004}. Compared with ribavirin group, fever time, duration of diarrhea, and creatine kinase isoenzyme MB (CK-MB) were significantly lower in xiyanping group [MD = -1.23, 95 % CI (-1.64 to -0.81), p < 0.00001; MD= -2.15, 95 % CI (-2.68 to -1.62), p < 0.00001; MD = -16.90, 95 % CI (-17.23 to -16.57), p < 0.00001}. Six studies reported adverse reactions, but there was no significant difference between xiyanping and ribavirin groups {OR = 1.44, 95 % CI (0.40 -5.17), p = 0.58}. Conclusion: This review suggests that xiyanping is more effective than ribavirin in children suffering from rotavirus enteritis.
