Objective To explore the influencing factors of postoperative urinary retention(POUR)in patients with gynecological benign diseases after laparoscopic surgery.Methods A total of 195 patients who underwent laparoscopic surgery for gynecological benign diseases at the Gynecological Mini-Invasive Center of Beijing Obstetrics and Gynecology Hospital affiliated to Capital Medical University from July 2018 to June 2022 were selected,and were divided into POUR group(n=65 cases)and non-POUR group(n=130 cases)based on whether POUR occurred after surgery.The general information,postoperative management time,selection of main puncture hole,preoperative generalized anxiexy disorder-7(GAD-7)score and postoperative visual analogue score(VAS)were collected and analyzed,and the influencing factors of POUR were analyzed by multivariate logistic regression equation.Results Among the 195 patients,the age ranged from 18 to 65 years,with an average age of(38.3±8.7)years.POUR occurred in 65 patients,and the preoperative anxiety and VAS in POUR group were higher than those in non-POUR group(both P<0.05).Multivariate analysis showed that patients with preoperative mild anxiety(OR=4.226,95%CI:1.950-9.158,P=0.000),moderate to severe anxiety(OR=5.698,95%CI:2.180-14.892,P=0.000)and moderate to severe pain(OR=6.941,95%CI:3.195-15.082,P=0.000)were more likey to have POUR.Conclusions Preoperative anxiety and postoperative pain scores are the influencing factors for POUR in gynecological benign diseases.Patients should be fully evaluated for emotional counseling before operation and timely pain relief after operation to reduce the risk of POUR.
Objective To investigate the clinical effect of laser wedge resection for labia minora hypertrophy.Methods A total of 13 cases of labia minora hypertrophy were treated between October 2019 and January 2022. The CO 2 laser wedge resection was used for labia minora reduction. The incision was designed according to the principle of bilateral symmetry for bilateral hypertrophy and the reference to the healthy side for unilateral hypertrophy. Results The operation time was 38-96 min(mean, 55.4±16.1 min). All the 13 cases had one-stage of incision healing without surgical complications. After the operation, the symptoms such as friction pain disappeared, abnormal urine flow direction in 4 cases returned to normal, and 6 patients with sexual life had no sexual discomfort. All the 13 patients were postoperatively followed up for 1-6 months, with an average of 3.5 months. The appearance of the affected labia minora was normal, the color change was natural. The appearance of unilateral hypertrophy was basically the same as that of the healthy side, while bilateral hypertrophy was basically the same on both sides. The width of the labia minora on the affected side was normal(14.13±1.18 mm). Conclusion The CO 2 wedge resection is a safe, effective and ideal method for labia minora hypertrophy, which has less wound damage, good hemostatic effect, and normal and natural shape of the labia minora after surgery.
