Aim. To study the association of high-sensitivity C-reactive protein (hsCRP) level with socio-demographic, behavioral and traditional risk factors in different regions of the Russian Federation (RF). Material and methods. Data of the multicenter epidemiological study of cardiovascular disease in different regions of the Russian Federation (ESSE-RF) were used. Representative sample of the unorganized male and female population aged 25-64 from 6 regions of the Russian Federation was drown. 3407 men and 6354 women (n=9761) were included into the study. Standard questionnaire was applied in all subjects. Gender, age, level of education, place of residence and region of residence, traditional risk factors and diseases were analyzed. HsCRP level (level ≥3.0 mg/l was defined as elevated one) was evaluated to detect of indolent inflammation. Results. HsCRP levels in women was significantly higher compared to males (p<0.005). The average prevalence of elevated hsCRP in the regions was 24.2%, for men - 21.4%, and for women - 25.7% (p<0.005). Significant age-related dynamics of hsCRP (p<0.0001) was found. The average level of hsCRP was significantly lower (p<0.0001) in a cohort of persons with higher education against these with lower level of education. Level of hsCRP as well as prevalence of elevated hsCRP level were higher in villagers than this in the urban population (p<0.05). Prevalence of elevated hsCRP in patients with traditional risk factors after adjustment for sex, age, and region of residence showed that the elevated hsCRP was mostly associated with metabolic factors and diseases that were characterized by systemic inflammation. Conclusion. Elevated level of hsCRP (≥3.0 mg/l) was significantly associated with female gender, with an older, less educated, and smoking population of Russians. After the multivariate adjustment there remained significant associations (p<0.0001) of elevated hsCRP level with obesity, including abdominal obesity, hyperglycemia and hypertriglyceridemia, low levels of low density cholesterol, rheumatoid arthritis, chronic bronchitis and Parkinson's disease.
The article presents the results of pharmacoepidemiological study, conducted with the aim of studying the peculiarities of antihypertensive therapy in patients with arterial hypertension (AH), and chronic obstructive pulmonary isease (COPD) in the Republic North Ossetia-Alania. Retrospective descriptive study according to the medical records of patients (n=568) of all outpatient clinics of the republic was performed. According to the obtained data, recommended by the doctors, the therapy does not correspond with the modern clinical recommendations on the majority of cases. The main lacks of the AH and COPD patients’ treatment are inadequate choice of the antihypertensive preparations for the start therapy, unfoundedly often use of monotherapy, non-rational combination of the medicinal drags, that leads to the absence of the carried out therapy effect in majority of patients.
An increase of antibiotic resistance of bacteria, in particular, Streptococcus pneumoniae, has been registered recently in most developed countries. This necessitates the study of regional characteristics of etiological structure of causative bacterial agents of community-acquired pneumonia and their sensitivity to antibiotics. The aim of the study was to study the spectrum of bacterial pathogens of community-acquired pneumonia and the sensitivity of Streptococcus pneumoniae to antimicrobial agents in hospitalized patients in the Republic of North Ossetia - Alania. Bacteriological examination of sputum with definition of sensitivity of isolated strains of bacteria to antibiotics was performed in 270 patients with community-acquired pneumonia. According to our data, the main causative agent of community-acquired pneumonia in hospitalized patients in the Republic of North Ossetia-Alania is Streptococcus pneumoniae. III generation cephalosporins, respiratory fluoroquinolones, macrolides, showed high activity against Streptococcus pneumoniae isolated from hospitalized patients. Low activity of natural and semi-synthetic penicillins was shown. The obtained results allow optimizing of antimicrobial therapy of community-acquired pneumonia caused by Streptococcus pneumoniae.
The work presents results of a clinical survey of 440 patients with symptoms of allergic rhinitis in the Republic of North Ossetia-Alania. The survey oncluded 272 urban residents and 168 rural residents aged 18-65. The prevalence of allergic rhinitis among adults living in environmentally unfavorable urban areas turned out to be 1.3 times higher in comparison with the conventionally clean rural areas (p <0,05). Predominantly seasonal forms of the disease were revealed among both groups of patients, the prevalence in urban and rural areas amounted to 8.2% and 6.4%, respectively. The main regional etiological factors of pollinosis in the adult population are weed pollen (61.4%), meadow grass (20.9%), fungal allergens – Alternaria, Cladosporium (22.5%). Low appeal ability a hypodiagnosis of seasonal allergic rhinitis has been detected as a result of the survey ,the diagnostics methods of the pathology should be improved.
