Weakened handgrip strength is now a major public health concern. The relationship between body mass index (BMI) and handgrip strength (HGS) is controversial and inconclusive. We conducted cross-sectional and longitudinal analyses to investigate the association between BMI and HGS in middle-aged and older Chinese population. We conducted a population-based cohort study using nationally representative data from the China Health and Retirement Longitudinal Study (CHARLS). Chronic gastrointestinal diseases (CGIDs) significantly affect global health, impairing both physical and mental well-being in the aging population. However, the relationship between CGIDs and the incidence of frailty and loneliness has not been fully explored. This study utilized data from the China Health and Retirement Longitudinal Study (CHARLS) collected from 2011 to 2015, including 13,139 middle-aged and older participants (aged 45 years and above). IntroductionPlacental syndromes, which include pregnancy loss, preterm birth, gestational diabetes mellitus (GDM), and hypertensive disorders in pregnancy (HDP), have a strong association with disorder inflammatory reactions. Nonetheless, the exact causal relationship has not been established. This study aims to investigate the causal relationship between placental syndromes and inflammatory cytokines utilizing Mendelian randomization (MR). Additionally, we examined the interaction between small molecular compounds derived from traditional Chinese medicine and inflammatory cytokines using molecular docking method.MethodsAfter obtaining the data of inflammatory cytokines and placental syndromes, as well as establishing single nucleotide polymorphisms (SNPs), we employed the inverse variance weighted (IVW) method to assess the causal relationship. We also accessed the heterogeneity and the horizontal pleiotropy of these data. The “ClusterProfiler” R package was utilized for Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway and Gene Ontology (GO) term analyses. The protein-protein interaction (PPI) network was constructed using STRING database. AutoDock Vina software was used for molecular docking, and Discovery Studio 2019 was used for visualization purposes.ResultsWe found that the growth regulated oncogene A (GROA) and interleukin-9 (IL-9) were associated with the development of pregnancy hypertension, whereas interleukin-10 (IL-10) and hepatocyte growth factor (HGF) were linked to the occurrence of preeclampsia. Moreover, there were correlations observed between interleukin-18 (IL-18), IL-10, macrophage colony-stimulating factor (MCSF), and platelet-derived growth factor BB (PDGFbb) in cases of chronic hypertension combined with pregnancy (CHP). Additionally, macrophage migration inhibitory factor (MIF) exhibited a connection with GDM, and TNF related apoptosis inducing ligand (TRAIL) demonstrated a causal relationship with preterm birth. It is plausible to suggest that interleukin-1β (IL-1β) might contribute to the promotion of pregnancy loss. All of the binding free energy values of small molecular compounds with inflammatory cytokines were below −5.0 kcal/mol. Furthermore, all of the RMSD values were less than 2.ConclusionsGROA, IL-1β, IL-9, IL-10, IL-18, MIF, MCSF, HGF, PDGFbb and TRAIL were found to be causally associated with placental syndromes. Molecular docking analysis revealed that small molecular compounds, such as puerarin, magnolol, atractylenolide I, paeoniflorin, tumulosic acid and wogonin, are closely bound to these inflammatory cytokines.
IntroductionAccelerated senescence of trophoblast may cause several diverse pregnancy outcomes; however, the cause of accelerated trophoblast senescence remains unclear. The renin-angiotensin system (RAS) is closely related to organ senescence. Therefore, in the present study, we hypothesized that angiotensin (Ang)II, one of the most important RAS family members, accelerates trophoblast senescence through the transforming growth factor β-1 (TGF-β1) pathway.MethodsAngII and Ang1-7 were used to stimulate pregnant rats. AngII and its inhibitor olmesartan were used to stimulate trophoblast. Thereafter, senescence levels were measured. Furthermore, we used AngII to stimulate trophoblast and utilized RNA-sequencing (RNAseq) to analyze the expression of differentially expressed genes (DEGs). After identifying the overlapping genes by comparing the DEGs and senescence-related genes, we employed CytoHubba software to calculate the top five hub genes and selected TGF-β1 as the target gene. We transfected the AngII-stimulated trophoblast with TGF-β1 small interfering RNA (siRNA) and measured the senescence levels.ResultsSenescence markers were upregulated in the AngII group compared with that in the control group. Furthermore, following AngII stimulation and RNAseq measurement, we identified 607 DEGs and 13 overlapping genes. The top five hub genes were as follows: PLAU, PTGS2, PDGFβ, TGF-β1, and FOXO3. Upon knockdown of TGF-β1 expression in AngII-stimulated trophoblast using TGF-β1 siRNA, we observed a downregulation of p53 and p62 mRNA expression.DiscussionAngII accelerates trophoblast senescence through the TGF-β1 pathway.
BackgroundsFrailty is a significant problem for older persons since it is linked to a number of unfavorable consequences. According to observational researches, air pollution may raise the risk of frailty. We investigated the causal association between frailty and air pollution (including PM2.5, PM2.5–10, PM10, nitrogen dioxide, and nitrogen oxides) using Mendelian randomization approach.MethodsWe conducted MR analysis using extensive publically accessible GWAS (genome-wide association studies) summary data. The inverse variance weighted (IVW) method was employed as the primary analysis method. The weighted median model, MR-Egger, simple model, and weighted model approaches were chosen for quality control. The Cochran’s Q test was utilized to evaluate heterogeneity. Pleiotropy is found using the MR-Egger regression test. The MR-PRESSO method was used to recognize outliers. The leave-one-out strategy was used to conduct the sensitivity analysis.ResultsMR results suggested that PM2.5 was statistically significantly associated with frailty [odds ratio (OR) = 1.33; 95%confidence interval (CI) = 1.12–1.58, p = 0.001] in IVW method. We observed no statistical association between PM2.5–10(OR = 1.00, 95% CI = 0.79–1.28, p = 0.979), PM10(OR = 0.91, 95% CI = 0.75–1.11, p = 0.364), nitrogen dioxide (OR = 0.98, 95% CI = 0.85–1.12, p = 0.730), nitrogen oxides (OR = 1.15, 95% CI = 0.98–1.36, p = 0.086) and frailty. There was no pleiotropy in the results. The sensitivity analysis based on the leave-one-out method showed that the individual single nucleotide polymorphisms (SNPs) did not affect the robustness of the results.ConclusionThe current MR investigation shows a causal association between PM2.5 and frailty. Frailty’s detrimental progression may be slowed down with the help of air pollution prevention and control.
BACKGROUND:Pre-eclampsia (PE) is a common condition in pregnancy; however, methods for early diagnosis and effective treatment options are lacking. Ferroptosis is a newly identified iron-dependent cell death pathway. The aim of this study was to investigate the role of ferroptosis-related genes in PE, the underlying mechanism, and their potential diagnostic value using a bioinformatics approach.METHODS:We downloaded the GSE48424 and GSE98224 datasets from the Gene Expression Omnibus database. Differentially expressed genes (DEGs) between PE and healthy pregnancy samples were identified in the GSE48424 dataset and subjected to weighted gene co-expression network analysis; the most relevant modules were intersected with known ferroptosis-related genes to distinctly identify the role of ferroptosis in PE. We further searched transcription factors and microRNAs that are predicted to regulate these ferroptosis-related genes, and patients in the GSE48424 dataset were divided into two groups according to high or low expression of the key ferroptosis-related genes associated with PE. To obtain robust key ferroptosis-related genes in PE, we validated their expression levels in the external dataset GSE98224. Finally, the reverse transcription-quantitative polymerase chain reaction (RT-qPCR) assay was utilized to access the expression of these genes in the PE and normal blood samples.RESULTS:Six ferroptosis-related genes involved in PE were obtained by overlapping 3661 genes most associated with PE, 565 DEGs between PE and normal samples, and 259 known ferroptosis-related genes. Among these genes, patients with PE displaying lower expression levels of NOS2 and higher expression levels of PTGS2 had a higher ferroptosis potential index. The expression pattern of NOS2 was consistent in the GSE48424 and GSE98224 datasets. RT-qPCR data confirmed that NOS2 expression was more significantly elevated in patients with PE than in those with a normal pregnancy.CONCLUSIONS:Our study explored the diagnostic value of ferroptosis-related genes in PE, and identified NOS2 as the key gene linking ferroptosis and PE, suggesting a new candidate biomarker for early PE diagnosis.
ObjectiveSince 2022, Omicron has been circulating in China as a major variant of the novel coronavirus, but the effects of infection with Omicron variants on pregnant women and newborns are unknown. The purpose of this study was to determine the clinical characteristics of Omicron infection during pregnancy and its effect on pregnancy outcomes.MethodsThis study retrospectively analyzed the data of 93 confirmed cases of novel coronavirus infection and 109 non-infected patients admitted to the isolation ward of Guangdong Maternal and Child Health Hospital from December 1, 2022 to January 31, 2023, and statistically analyzed the clinical features of Omicron variant infection during pregnancy and its impact on pregnancy outcomes. Further effects of underlying diseases on Omicron infection in pregnant women were analyzed.ResultsThe incubation period of COVID-19 infection was 0.99±0.86 days, 94.38% of patients had fever or other respiratory symptoms, the lymphocyte count in the infected group was lower than that in the uninfected group, and the lymphocyte count was further reduced in the patients with pregnancy complications or complications. Compared with the uninfected group, APTT and PT were prolonged, platelet count and fibrinogen were decreased in the infected group, all of which had statistical significance. COVID-19 infection during pregnancy increased the rate of cesarean section compared to uninfected pregnant patients, and COVID-19 infection in gestational diabetes resulted in a 4.19-fold increase in cesarean section rate. There was no statistically significant difference in gestational age between the two groups. The incidence of intrauterine distress, turbidity of amniotic fluid and neonatal respiratory distress were higher in the infection group. No positive cases of neonatal COVID-19 infection have been found.ConclusionThe patients infected with omicron during pregnancy often have febrile respiratory symptoms with lymphocyopenia, but the incidence of severe disease is low. Both Omicron infection and gestational diabetes further increase the incidence of cesarean section, and this study found no evidence of vertical transmission of Omicron.
Objective Diode -pumped alkali lasers are a new type of gas lasers that utilize the high energy levels of alkali metal atoms to achieve particle number inversion. However, conventional fused -silica laser windows are easily subjected to corrosion in the alkali metal vapor environment. Therefore, in this study, we select sapphire as a window material, considering its high hardness and thermal conductivity, low expansion coefficient, and high temperature and erosion resistance. Owing to its large refractive index, the sapphire surface presents relevant anti -reflection properties, including high air and interface reflectivity. Anti -reflection surfaces are usually obtained by preparing optical films or microstructures. Anti -reflection films often require coating materials with specific refractive indices that are difficult to source. Moreover, they are susceptible to gaseous corrosion. However, their fabrication technology is relatively mature. Conversely, the production of anti -reflection microstructures requires complex processing. However, using the same substrate material to build the microstructure results in high chemical stability, mechanical properties, and resistance to laser damage. In this study, we implement a diode -pumped alkali metal laser with a 795 nm center wavelength and a working environment temperature of 200 C-degrees by combining two different anti -reflection technologies to obtain a double -sided anti -reflection sapphire window. This is characterized by a microstructure on the side of the vapor chamber and a coating film on the side exposed to air, which enhances the temperature and corrosion resistance of the resulting laser window.Methods Based on vector diffraction theory, the effects of depth, bottom angle, period, and duty cycle on the transmittance of a one-dimensional trapezoidal structure are analyzed using COMSOL software to obtain the process tolerance of the microstructure. We use laser interference exposure to prepare a photoresist mask on a sapphire surface, and then we transfer it using reactive ion beam etching to form microstructures. The ion -beam sputtering is used to prepare an anti -reflection film on the sapphire surface. To ensure that the microstructure satisfies the requirements of the application environment, we test the temperature increase in the sample under the action of a high -power laser. Moreover, we test the changes in the beam quality of the probe light passing through the sample at different temperatures.Results and Discussions Our simulation results show that the transmittance can reach above 99.90% with a period of 400 nm, bottom angle of 78(degrees)+/- 2(degrees), depth of (190 +/- 5)nm, duty cycle of 0.25 +/- 0.05, and large margin of error (Figs. 2-6). Morphology test results show that the duty cycle of the experimentally prepared microstructure is in the range of 0.22-0.31, consistently with the designed parameters, whereas the height is in the range of 155 -175 nm, thus failing to reach the designed value (Table 1). Transmittance test results show that the transmittances of the single -sided anti -reflection microstructure and film reach 99.23% and 99.91%, respectively. The transmittances of the double -sided anti -reflection sapphire window are 98.01% and 98.90% for the one- sided microstructure and anti -reflection film, respectively. The transmittances of these two samples increase by 12.13% and 13.02% compared to that of a bare sapphire substrate (Fig. 10). The temperature rise test results show that when the laser power is increased from 35 W to 99.6 W, the temperatures of substrates 1 and 2 increase by 4.3 degree celsius and 5.9 degree celsius, respectively, whereas the temperature of the double -sided anti -reflection sample increases only by 3.8 degree celsius. Near the target wavelength, the temperature increase in the anti -reflection window is smaller than that in the bare substrate, and the temperature increase in the double -sided anti -reflection sample can be appropriately reduced (Fig. 12). The beam quality test results show that the temperature has a greater effect on the beam quality factor in the longitudinal direction of the sapphire samples compared to that in the transverse direction. Moreover, the sapphire batch significantly influences the associated beam quality factor (Fig. 14). When the temperature of the window is kept at 200 degree celsius under the action of a high -power laser, the beam quality factor of the samples with the double -sided anti -reflection microstructure varies less than 0.05 and 0.06 in the transverse and longitudinal directions, respectively. Therefore, the anti -reflection window has a limited effect on the beam quality of the incident light (Table 2).Conclusions Based on our theoretical simulation, in this study, we develop anti -reflection microstructures on the surface of a sapphire substrate by interference exposure and reactive ion beam etching, which can reach a single -sided transmittance of 99.23% at a 795 nm light wavelength. Using this technique, we prepare two sapphire windows: one characterized by a double -sided microstructure, and one presenting a microstructure on one side and a coating film on the opposite side. At a wavelength of 795 nm, the transmittance of these samples improves by 12.13% and 13.02%, respectively, compared with that of the bare sapphire substrate. The temperature rise test under high -power laser irradiation shows that when the laser power increases from 35.0 W to 99.6 W, the temperature of the bare substrate rises by 5.9 degree celsius, whereas that of both samples obtained using the double -sided antireflective treatment increases only by 3.8 degree celsius . These results indicate that our treatment can effectively reduce thermal effects by exploiting the higher transmittance rates of the double -sided samples. Moreover, beam quality test results indicate that when the microstructure window temperature is kept below 200 degree celsius under high -power laser irradiation, the variation of the beam quality factor for double -sided anti -reflection samples remains below 0.05 and 0.06 along the transverse and longitudinal directions, respectively, indicating that the anti -reflection window has a negligible effect on the beam quality of the incident light. In this study, we successfully fabricate antireflective windows on a sapphire substrate characterized by either a double -sided antireflective microstructure or an antireflective microstructure and antireflective film on the opposite sides, achieving in both cases high temperature and corrosion resistance as well as high transmission performance. Our antireflective fabrication process solves the traditional performance issues of fused -quartz laser windows, which are prone to corrosion in alkali metal vapor environments, and provides a reference framework for fabricating antireflective windows that can be used effectively under harsh conditions.
Abstract Background Pregnant women with pulmonary hypertension (PH) have higher mortality rates and poor foetal/neonatal outcomes. Tools to assess these risk factors are not well established. Methods Predictive and prognostic nomograms were constructed using data from a “Development” cohort of 420 pregnant patients with PH, recorded between January 2009 and December 2018. Logistic regression analysis established models to predict the probability of adverse maternal and foetal/neonatal events and overall survival by Cox analysis. An independent “Validation” cohort comprised data of 273 consecutive patients assessed from January 2019 until May 2022. Nomogram performance was evaluated internally and implemented with online software to increase the ease of use. Results Type I respiratory failure, New York Heart Association functional class, N-terminal pro-brain natriuretic peptide $$\ge$$ ≥ 1400 ng/L, arrhythmia, and eclampsia with pre-existing hypertension were independent risk factors for maternal mortality or heart failure. Type I respiratory failure, arrhythmia, general anaesthesia for caesarean section, New York Heart Association functional class, and N-terminal pro-brain natriuretic peptide $$\ge$$ ≥ 1400 ng/L were independent predictors of pulmonary hypertension survival during pregnancy. For foetal/neonatal adverse clinical events, type I respiratory failure, arrhythmia, general anaesthesia for caesarean section, parity, platelet count, fibrinogen, and left ventricular systolic diameter were important predictors. Nomogram application for the Development and Validation cohorts showed good discrimination and calibration; decision curve analysis demonstrated their clinical utility. Conclusions The nomogram and its online software can be used to analyse individual mortality, heart failure risk, overall survival prediction, and adverse foetal/neonatal clinical events, which may be useful to facilitate early intervention and better survival rates.
Maternal sepsis results in poor outcomes such as fetal or maternal death. The incidence and mortality rates of maternal sepsis vary in different places because of differences in economic development, race and medical conditions. Identifying the clinical features and determining possible mechanisms for avoiding morbidity and preventing poor outcomes would benefit committed patients. Therefore, this was an epidemiological study at a maternity transfer center in Southeast China that aimed to identify local disease features of maternal sepsis. To investigate the incidence and risk factors associated with maternal sepsis and its progression to severe sepsis in a large population-based birth cohort. This local epidemiological study was conducted in at a tertiary care center in Guangzhou, China, from 2015 to 2019. A total of 74,969 pregnant women experiencing childbirth were included in this study; Of these, 74 patients with maternal sepsis were diagnosed according to the sepsis criterion, and 118 patients without sepsis in the same period were selected randomly as the control group to study possible reasons for postpartum sepsis. This retrospective analysis covered the entire period from the first trimester to puerperium. Clinical data were collected using the hospital's electronic medical record system. Multivariate logistic regression was used to analyze risk factors for maternal sepsis. The incidences of maternal sepsis, the maternal mortality, and the fetal mortality were 0.099%, 0.004%, and 0.007%, respectively. Septic shock was associated with a higher severity of illness. All poor outcomes (maternal or fetal death) occurred during pregnancy. Postpartum sepsis had the longest onset period, and was associated with premature rupture of fetal membranes and preeclampsia. Sepsis is an important cause of both maternal and fetal mortality. Herein, we describe an epidemiological study that evaluated the incidence, development, and prognosis of local maternal sepsis. Furthermore, the characteristics of maternal sepsis are likely due to unknown pathological mechanisms, and patients would benefit from identifying more effective treatments for maternal sepsis.
Abstract Background Gastrointestinal cancers are a critical global cancer burden, and tracking their trends would inform the health policies. Methods Trends of years of life lost (YLLs) and years lived with disability (YLDs) caused by three common gastrointestinal cancers were estimated using annual percentage change (EAPC) and age-standardized rate (ASR). Data was extracted from the Global Burden of Disease study 2019. Results The ASR per 100,000 population-year of YLLs caused by esophageal cancer, stomach cancer, and colorectal cancer were 137.98, 264.15, and 282.51 in 2019, respectively. Their overall trends of YLLs declined during 1990–2019, with the respective EAPCs being − 1.42 (95% Confidence Interval [CI]: − 1.71 to − 1.13), − 2.13 (95%CI: − 2.29 to − 1.96), and − 0.25 (95%CI: − 0.30 to − 0.19). Meanwhile, decreasing trends of YLDs caused by esophageal cancer and stomach cancer were observed, in which the EAPCs were − 0.67 (95%: − 0.94 to − 0.40) and − 0.85 (95%CI: − 0.97 to − 0.73), respectively. However, an increasing trend was seen in that of colorectal cancer (EAPC = 0.83, 95%CI: 0.77 to 0.89). Among countries, the largest decrease in trend of YLLs was that of stomacher cancer in the Republic of Korea (EAPC = − 5.88, 95%CI: − 6.07 to − 5.69). However, pronounced increasing trend of YLDs caused by colorectal cancer occurred in China (EAPC = 4.40, 95%CI: 4.07 to 4.72). Conclusions Decreasing trends in YLLs and YLDs caused by esophageal cancer, stomach cancer, and colorectal cancer were observed in most countries and regions, indicating that the great progress had been achieved over the past decades. However, the cancer burden was geographical heterogeneity, and cost-effective measures were still required to decline the burden caused by gastrointestinal cancers.
目的 探讨自发性早产患者绒毛组织中Nod样受体蛋白3(NLRP3)、Toll样受体(TLR)4炎症小体表达及其临床意义.方法 选取2019年6月—2020年4月广东省妇幼保健院收治的自发性早产患者112例为研究组,并以同期就诊正常足月妊娠分娩女性100例为对照组.采用蛋白免疫印迹法(Western blotting)检测两组绒毛组织中NLRP3、TLR4蛋白表达水平.测定相应组织中丙二醛(MDA)、超氧化物歧化酶(SOD)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)浓度.统计研究组早产儿窒息、感染及肺透明膜病等早产儿不良结局发生率,并分析其绒毛组织中NLRP3、TLR4蛋白表达水平与绒毛组织中MDA、SOD、TNF-α、IL-6浓度及早产儿不良结局的关系.结果 与对照组比较,研究组绒毛组织中NLRP3、TLR4蛋白表达水平以及MDA、TNF-α、IL-6浓度均升高,而同期绒毛组织中SOD浓度则降低(P<0.05).研究组早产儿窒息、感染及肺透明膜病等早产儿不良结局发生率为33.04%(37/112).与研究组早产儿结局良好者比较,早产儿结局不良者的绒毛组织中NLRP3、TLR4蛋白表达水平以及MDA、TNF-α、IL-6浓度均升高,而同期绒毛组织中SOD浓度则降低(P<0.05).相关分析结果显示,自发性早产患者绒毛组织中NLRP3、TLR4蛋白表达水平与其绒毛组织中MDA、TNF-α、IL-6浓度以及早产儿不良结局发生率均呈正相关(P<0.05),与其绒毛组织中SOD浓度则呈负相关(P<0.05).结论 自发性早产患者绒毛组织中NLRP3、TLR4炎症小体表达增加且与氧化应激、炎症均相关,可能为自发性早产发病机制之一,且可影响早产儿结局.
Background Preeclampsia (PE), which has a high incidence rate worldwide, is a potentially dangerous syndrome to pregnant women and newborns. However, the exact mechanism of its pathogenesis is still unclear. In this study, we used bioinformatics analysis to identify hub genes, establish a logistic model, and study immune cell infiltration to clarify the physiopathogenesis of PE. Methods We downloaded the GSE75010 and GSE10588 datasets from the GEO database and performed weighted gene coexpression network analysis (WGCNA) as well as Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses. The online search tool for the retrieval of interacting genes and Cytoscape software were used to identify hub genes, which were then used to establish a logistic model. We also analyzed immune cell infiltration. Finally, we verified the expression of the genes included in the predictive model via RT-PCR. Results A total of 100 and 212 differently expressed genes were identified in the GSE75010 and GSE10588 datasets, respectively, and after overlapping with WGCNA results, 17 genes were identified. KEGG and GO analyses further indicated the involvement of these genes in bioprocesses, such as gonadotropin secretion, immune cell infiltration, and the SMAD and MAPK pathways. Additionally, protein-protein interaction network analysis identified 10 hub genes, six (FLT1, FLNB, FSTL3, INHA, TREM1, and SLCO4A1) of which were used to establish a logistic model for PE. RT-PCR analysis also confirmed that, except FSTL3, these genes were upregulated in PE. Our results also indicated that macrophages played the most important role in immune cell infiltration in PE. Conclusion This study identified 10 hub genes in PE and used 6 of them to establish a logistic model and also analyzed immune cell infiltration. These findings may enhance the understanding of PE and enable the identification of potential therapeutic targets for PE.
目的 探究评价拉贝洛尔联合硝苯地平治疗重度子痫前期的有效性和临床疗效.方法 选取2017年8月到2019年9月我院收治的80例重度子痫前期患者作为本次观察对象,利用随机分组的方法将其分为对照组(n=40)和实验组(n=40),对照组患者给予单硝苯地平进行治疗,实验组在此基础上增加拉贝洛尔的联合用药,对比两组患者的临床疗效和血压水平.结果 实验组患者利用拉贝洛尔联合硝苯地平进行治疗,治疗效果明显比对照组好,血压水平得到明显改善,差异具有统计学意义(p<0.05).结论 拉贝洛尔联合硝苯地平治疗重度子痫前期具有较好的临床疗效,值得临床推广应用.
目的 探讨利伐沙班在预防ICU患者静脉血栓栓塞症(VTE)的应用价值.方法 选取2015年06月至2016年12月我院收治的高危VTE风险的患者120例,采用电脑产生随机数法将患者分为治疗组n=60例和对照组n=60例.治疗组采用利伐沙班预防血栓,对照组采用低相对分子质量肝素钙预防血栓,观察和记录治疗组和对照组患者VTE症的发生情况、aPTT、PT、PLT及药物的不良反应情况.结果 治疗组患者中发生VTE共计6例,发生率为10.0%低于对照组的26.7%(P<0.05),治疗组不良反应发生率为10.0%低于观察组11.7%,但差异无统计学意义(P>0.05),未发现治疗前后观察组和对照组aPTT、PT、PLT存在统计学差异(P>0.05).结论 利伐沙班在预防ICU静脉血栓栓塞症效果较好,能减少ICU患者VTE的发生率.
目的 随着生育年龄推迟,高龄孕产妇发生率不断增加,高龄孕产妇术后肠道功能恢复较普通孕产妇减慢,带来不良临床后果.方法 选取2016年1月~12月我院收治的孕产妇180例作为研究对象,将其随机分为正常组、高龄孕产妇组,高龄孕产妇治疗组,各60例.其中,正常组平均年龄(25±3.25)岁、高龄孕产妇组平均年龄(38±3.25)岁,高龄孕产妇治疗组平均年龄(39±2.78)岁.高龄孕产妇治疗组术后耳穴治疗,取穴:三焦、胃、大肠、小肠.结果 提示高龄孕产妇组术后肠鸣音恢复时间、首次排气时间等指标均明显较正常组延长,耳穴治疗后上述时间明显缩短.高龄孕产妇组中腹胀人数较正常组增多,耳穴治疗后可降低腹胀发生率.结论 耳穴治疗可促进高龄孕产妇剖宫产术后肠功能恢复.
目的 通过对产后甲状腺炎(PPT)患者外周血树突状细胞(DCs)免疫功能及状态的观察,探讨DCs与PPT发生发展的关系.方法 用流式细胞术(FACS)分别检测经培养后来源于PPT患者及正常健康人外周血DCs上CD83、CD80、CD86的表达水平.结果 来源于PPT患者外周血DCs上CD83、CD80、CD86分子的表达水平显著高于正常组(P<0.05),同种混合淋巴细胞反应显示PPT患者组DCs免疫刺激能力显著高于正常组(P<0.01).结论 DCs免疫功能及成熟分化的增加与PPT的发生密切相关.
无心畸形(acardius)是一种以无心脏为特征的胎儿畸形, 临床上较为罕见, 文献报道发生率为妊娠总数的1/35000, 占单卵双胎的1%, 再发风险约为1/10000[1].多为双胎之一无心畸形, 而另一胎正常或合并其他部位的畸形.无心畸形胎儿常合并胸部以上部位发育异常, 最常见的是合并无头畸形, 称无心无头畸形.本院2015年共发现2例双胎妊娠中一胎无头无心畸形.
Objective To explore the diagnosis and treatments of cesarean scars pregnancy.Method Analysis was conducted on 42 cases of cesarean scars pregnancy in Guangdong Women and Children Hospital from January 2012 to October 2013.Results 33.3% of all patients were misdiagnosed at first time.1 case was cured by drugs of combination of MTX and mifepristone.6 cases were cured by curettage under ultrasound.29 cases underwent curettage under hysterscope and ultrasound after pretreatment of drugs,including 26 cases cured,1 case refusing further treatment without ideal reduction of HCG level,1 case turning to lesion resection,and 1 case turning to hysterectomy.1 case underwent curettage under ultrasound after pretreatment of drugs and uterine infarction.3 cases underwent lesion resection directly.2 cases underwent hysterectomy directly because of serious bleeding and shock.Conclusion Early diagnosis and proper treatment help to keep uterus and improve prognosis.
目的:探讨针对中期妊娠胎盘异常导致出血患者,观察选择介入治疗的方法进行干预后获得的临床效果.方法:选择2012年10月-2015年9笔者所在医院中期妊娠胎盘异常导致出血患者90例,通过抽签法完成所有中期妊娠胎盘异常导致出血患者的随机分组,分别为B1组(观察组45例)和B2组(对照组45例).B1组临床选择介入治疗的方法,B2组临床选择非介入治疗的方法,比较两组患者止血效果、手术时间等方面的差异.结果:所有中期妊娠胎盘异常导致出血患者完成临床治疗后,在止血效果和出院时间两方面,B1组与B2组比较,差异均有统计学意义(P<0.05);剖宫取胎和子宫切除两方面,B1组与B2组比较,差异均有统计学意义(P<0.05).结论:针对中期妊娠胎盘异常导致出血患者,临床选择介入治疗的方法,能够获得显著疗效,将患者出现不良反应的概率有效降低,将患者的生育功能有效保留,针对引产发挥显著的促进作用.
