To explore the effects of early electroacupuncture (EA) intervention on the high mobility group box 1 (HMGB1)/Toll-like receptor 4 (TLR4) signaling pathway-related protein expression and oligodendrocytes in mice with amyotrophic lateral sclerosis (ALS), and uncover the potential molecular mechanisms underlying the improvement of motor function in ALS mice by early EA intervention. ALS mice carrying the SOD1G93A gene were randomly divided into a model group and an EA group, with 10 mice in each group; 10 littermate mice with a negative SOD1G93A genotype served as the control group. In the EA group, Baihui (GV20), Tianzhu (BL10), and Tianshu (ST25) were selected with needles retained for 10 min, 5 consecutive days per week, with 2 d of rest. One week constituted a course of treatment, and a total of 3 consecutive courses were performed. The other groups were grasped and fixed similarly but without intervention. Motor function was assessed using the open field test (OFT) and Morris water maze (MWM). Subsequently, hematoxylin-eosin staining was used to observe neuron morphology in the M1 region of the cerebral cortex. Immunofluorescence was performed to detect the positive cell rate of TAR DNA-binding protein 43 (TDP-43), and double immunofluorescence staining was used to observe the positive cell rate and cell states of ionized calcium-binding adaptor molecule 1 (Iba-1) and myelin basic protein (MBP) in the M1 region of the cerebral cortex. Western blotting was used to detect the relative expression levels of TDP-43, tumor necrosis factor (TNF)-α, HMGB1, and TLR4 proteins. Compared to the control group, the model group exhibited a reduced total movement distance in the OFT, and an increased escape latency, and fewer platform crossings in the MWM, with statistically significant differences (P<0.01). In the model group, the number of degenerated and necrotic neurons in the M1 region of the ALS mouse cerebral cortex increased, with significant nuclear shrinkage and cytoplasmic vacuolization; the percentage of TDP-43 immunofluorescence positive cells in the M1 region of the cerebral cortex increased (P<0.01), and the relative expression level of TDP-43 protein in the cerebral cortex showed a significant increase (P<0.01); the Iba-1 positive cell percentage increased, while the MBP positive cell percentage decreased (P<0.01); the relative expression levels of TNF-α, HMGB1, and TLR4 proteins increased (P<0.05). Compared to the model group, the EA group showed an increased total movement distance (P<0.01), and a reduced escape latency, and more platform crossings in the MWM, with statistically significant differences (P<0.05). In the EA group, neurons showed improvement, with reduced degeneration and necrosis, and larger, clearer nuclei; the percentage of TDP-43 immunofluorescence positive cells in the M1 region of the cerebral cortex decreased (P<0.05), and the relative expression level of TDP-43 protein also decreased (P<0.05); the percentage of Iba-1 positive cells in the M1 region of the cerebral cortex decreased, while the percentage of MBP positive cells increased (P<0.01); the relative expression levels of TNF-α, HMGB1, and TLR4 proteins decreased (P<0.05). EA intervention can suppress microglial activation, improve the state of oligodendrocytes, and reduce abnormal TDP-43 aggregation in the M1 region of the cerebral cortex in ALS model mice; its mechanism of action may be related to the HMGB1/TLR4 signaling pathway.
This paper introduces Professor LIU Zhibin 's clinical experience in the treatment of subjective tinnitus with acupuncture based on the "kidney-bone-brain" axis. Professor LIU proposes that the disease is most closely related to the kidney and brain. The lesion is located in the brain, and the pathogenesis is kidney essence deficiency, marrow sea loss, and ear orifice dystrophy. The "kidney-bone-brain" shows close correlation in physiological function, pathological changes and treatment. According to the "kidney-bone-brain" axis, Professor LIU proposes that the treatment of subjective tinnitus should be tonifying kidney qi, tonifying essence and filling marrow, and the principle of local acupoint selection, touching bone acupuncture, matching distal acupoints and proximal acupoints, tonifying kidney and benefiting brain should be adopted. The acupoints of Tinggong (SI19) and Yifeng (TE17) are selected to be treated with touching bone acupuncture, combined with Taixi (KI3), Shenshu (BL23), Baihui (GV20) and Shenting (GV24), so as to achieve common benefit of kidney, bone and brain, and multi-angle treatment.
The paper introduces Professor LIU Zhibin's clinical experience in treatment of Parkinson's disease with constipation by the combined therapy of acupuncture and tuina. Professor LIU believes that constipation in Parkinson's disease involves pathological changes in sanjiao (triple energizers) system, i.e. upper, middle, and lower jiao, and proposes Tiaoshen Tongfu (regulating the mind and fu organs) acupuncture and Chang'an Liu's tuina manipulation are used to treat Parkinson's disease with constipation based on the "sanjiao theory". In acupuncture, the scalp points of the anterior hairline micro-acupuncture system (Touxue, Shangjiao, Zhongjiao and Xiajiao) are selected, complemented by Baihui (GV20), Hegu (LI4), Taichong (LR3), Quchi (LI11), Tianshu (ST25), Dachangshu (BL25), Shangjuxu (ST37) and Zhigou (TE6). In tuina manipulation, one finger pushing, rolling and pressing are adopted.
The paper introduces the diagnostic and therapeutic ideas and clinical experience of Professor LIU Zhibin in treatment with acupuncture for Parkinson's disease with mild cognitive impairment. Professor LIU believes that the basic pathogenesis of Parkinson's disease with mild cognitive impairment refers to the deficiency of liver and kidney, and the loss of mind control. Therefore, the treatment focuses on nourishing the liver and kidney, regulating the governor vessel, opening the brain orifice, and regulating the mind. The point prescription is composed of Tongdu Tiaoshen zhen (the points for promoting the circulating of the governor vessel and regulating the mind, i.e. four-mind needles [Extra], Shenting [GV24], bilateral Benshen [GB13] and Fengfu [GV16]), xiusanzhen (three-olfaction needles, including bilateral Yingxiang [LI20] and Yintang [GV24+]) and zhichan bazhen (eight anti-tremble needles, i.e. Baihui [GV20], Lianquan [CV23], and Hegu [LI4], Waiguan [TE5], Taichong [LR3], Zusanli [ST36], Sanyinjiao [SP6] and Taixi [KI3] on the affected side). Besides, the prescription is modified according to the syndrome, and the special technique of bone-touching needling is combined to enhance the therapeutic effect.
Objective:To observe the effects of "olfactory three needles" on cognition, learning and memory abilities, as well as hippocampal microglia (MG) phagocytic activity in vascular dementia (VD) rats, and explore the mechanisms of acupuncture in regulating MG activation and improving remyelination, so as to ameliorate VD. Methods:Among 38 SD rats meeting experimental requirements, 9 rats were randomly assigned to a sham-operation group, and the remaining rats underwent permanent bilateral common carotid artery ligation to establish VD model. Eighteen successfully modeled rats were randomly divided into a model group and an electroacupuncture (EA) group, with 9 rats in each one. In the EA group, EA was performed at "olfactory three needles" ("Yintang" [GV24+] and bilateral "Yingxiang" [LI20]), at disperse-dense wave, the frequency of 2 Hz/15 Hz and the current intensity of 1 mA, for 15 min per intervention, once daily. One course was composed of 7 days, and 2 courses were required, with the interval of 2 days. The novel object recognition test was employed to assess the cognition of rats, and the Morris water maze was adopted to observe learning and memory abilities. Luxol fast blue (LFB) staining was performed to evaluate myelin sheath loss in the hippocampus, the Western blot was used to detect the protein expression of triggering receptor expressed on myeloid cells-2 (TREM2) and proteolipid protein (PLP) in the hippocampus; and the immunofluorescence staining was used to detect the positive expression of PLP, sex determining region Y-box 10 (SOX10), ionized calcium binding adaptor molecule 1 (Iba1)+ TREM2+ and Iba1+ lysosome-associated membrane protein 1 (LAMP1)+ in the hippocampus. Results:Compared with the sham-operation group, the rats in the model group exhibited the prolonged escape latency on day 3 and 4 (P<0.05, P<0.01), the increase of the total distance traveling (P<0.01) and the decrease of the recognition index (RI) and platform crossing frequency (P<0.01). Compared with the model group, the rats in the EA group showed the shortened escape latency on day 3 and 4 (P<0.05), the decrease of total distance traveling (P<0.01) and the increase of RI and platform crossing frequency (P<0.05, P<0.01). When compared with the sham-operation group, the rats of the model group presented uneven staining, sparse arrangement of myelin sheath fibers, unclear contours, and prominent vacuole-like changes in the hippocampal CA1 region. When compared with the model group, the EA group showed more dense staining, the increase of myelin sheath fibers with more orderly alignment, and fewer vacuolar changes in the hippocampal CA1 region. Compared with the sham-operation group, the model group exhibited the increase of TREM2 protein expression and the decrease of PLP protein expression in the hippocampus (P<0.01), whereas the EA group showed the up-regulation of TREM2 and PLP protein expression when compared with the model group (P<0.01, P<0.05). The positive expression of the hippocampal PLP, SOX10, and Iba1+LAMP1+ in the model group was reduced in comparison with the sham-operation group (P<0.05, P<0.01), and the positive expression of Iba1+ TREM2+ was elevated (P<0.05). In the EA group, the positive expression of PLP, SOX10, Iba1+TREM2+, and Iba1+ LAMP1+ was higher compared with that in the model group (P<0.05, P<0.01). Conclusion:"Olfactory three needles" can improve the learning and memory, and cognitive functions of VD rats, and its mechanism may be associated with the up-regulation of TREM2 and LAMP1 to adjust MG phagocytic activity and intracellular degradation, and promote remyelination.

In recent years, the incidence of Parkinson's disease has increased year by year, because its motor symptoms have a clear diagnostic value has been valued, with the deepening of research, non-motor symptoms gradually into the field of vision of researchers. The onset of non-motor symptoms usually precedes motor symptoms, runs through the entire process of Parkinson's disease, and even promotes the progression of Parkinson's disease. In the treatment of non-motor symptoms of Parkinson's disease, the improvement effect of Western medicine treatment is not obvious. A number of studies have shown that acupuncture has a positive effect on slowing down the progression of Parkinson's disease, and the literature research at home and abroad in recent years has been sorted out as follows.
目的 观察艾灸风池穴对偏头痛大鼠血清5-羟色胺(5-HT)、内皮素-1(ET-1)水平及脑干组织中癌基因fos(c-fos)、癌基因jun(c-jun)表达的影响,探讨艾灸治疗偏头痛的可能机制.方法 将40只雄性SD大鼠随机分为空白组、模型组、艾灸治疗组和艾灸预防+治疗组,每组10只.除空白组外,其余组大鼠均采用颈背部皮下注射硝酸甘油方法制备偏头痛模型.艾灸预防+治疗组于造模前7 d(连续7 d)及造模后30 min艾灸双侧风池穴,每日1次,每次30 min;艾灸治疗组于造模后30 min艾灸双侧风池穴1次,持续30 min.记录大鼠造模前及造模后0~30 min、60~90 min、90~120 min的挠头次数;测量造模前、造模结束后、造模后60 min大鼠头面部机械痛阈值;造模结束4 h后,采用酶联免疫吸附法测定血清5-HT和ET-1水平,Western blot法检测脑干组织中c-fos和c-jun蛋白表达情况.结果 模型组大鼠造模后0~30 min、60~90 min、90~120 min的挠头次数均明显高于空白组(P均<0.05),艾灸治疗组、艾灸预防+治疗组大鼠造模后60~90 min、90~120 min的挠头次数均明显少于模型组(P均<0.05).造模结束后,模型组、艾灸治疗组、艾灸预防+治疗组大鼠头面部机械痛阈值均明显低于空白组(P均<0.05);造模后60 min,艾灸治疗组、艾灸预防+治疗组大鼠头面部机械痛阈值均明显高于模型组(P均<0.05),艾灸治疗组与艾灸预防+治疗组比较差异无统计学意义(P>0.05).模型组大鼠血清5-HT水平明显低于空白组(P<0.05),血清ET-1水平及脑干组织中c-fos、c-jun蛋白相对表达量均明显高于空白组(P均<0.05);艾灸治疗组、艾灸预防+治疗组大鼠血清5-HT水平均明显高于模型组(P均<0.05),血清ET-1水平及脑干组织中c-fos、c-jun蛋白相对表达量均明显低于模型组(P均<0.05);艾灸预防+治疗组大鼠血清ET-1水平及脑干组织中c-fos、c-jun蛋白相对表达量均明显低于艾灸治疗组(P均<0.05).结论 艾灸风池穴可明显缓解大鼠偏头痛,且早期介入效果更显著,其机制可能与调节血管舒缩相关的血管活性物质表达和下调c-fos、c-jun蛋白表达而抑制三叉神经血管系统激活有关.
吞咽是人体最重要的功能之一,人体所需要的营养物质大部分是通过吞咽摄入,吞咽功能受损会严重影响人们的生活,而吞咽困难是脑卒中常见的后遗症.构建一个可复制性强、便于操作的脑卒中后吞咽障碍动物模型,对研究该疾病具有非常重要的意义.目前有关脑卒中吞咽障碍的动物模型主要有大脑中动脉短暂性闭塞导致的吞咽障碍模型、结扎双侧颈总动脉制备的吞咽障碍模型,右侧脑出血+右侧舌下神经损伤诱发的吞咽障碍动物模型.评估动物模型吞咽功能的方法有视频透视吞咽检查、肌电图检查等.本文就目前有关脑卒中后吞咽障碍动物模型的造模方法和吞咽功能的评估方法进行综述,为脑卒中后吞咽障碍动物模型的研究提供依据.

OBJECTIVE:To observe the effects of moxibustion at "Baihui" (GV 20) and "Dazhui" (GV 14) at different time points on the serum level of β-endorphin (β-EP), substance P (SP) and expression of interleukin-1β (IL-1β) and cyclooxygenase-2 (COX-2) protein in brainstem in rats with migraine, and to explore the effect and mechanism of moxibustion in preventing and treating migraine.METHODS:Forty male SD rats were randomly divided into a blank group, a model group, a prevention+treatment (PT) group and a treatment group, 10 rats in each group. Except the blank group, the rats in the remaining groups were injected with nitroglycerin subcutaneously to prepare migraine model. The rats in the PT group were treated with moxibustion 7 days before modeling (once a day) and 30 min after modeling, while the rats in the treatment group were treated with moxibustion 30 min after modeling. The "Baihui" (GV 20) and "Dazhui" (GV 14) were taken for 30 minutes each time. The behavioral scores in each group were observed before and after modeling. After intervention, ELISA method was used to detect the serum level of β-EP and SP; the immunohistochemistry method was used to detect the number of positive cells of IL-1β in brainstem; the Western blot method was used to detect the expression of COX-2 protein in brainstem.RESULTS:Compared with the blank group, the behavioral scores in the model group were increased 0-30 min, 60-90 min and 90-120 min after modeling (P<0.01); compared with the model group, in the treatment group and the PT group, the behavioral scores were decreased 60-90 min and 90-120 min after modeling (P<0.01). Compared with the blank group, in the model group, the serum level of β-EP was decreased (P<0.01), while the serum level of SP, the number of positive cells of IL-1β in brainstem and the expression of COX-2 protein were increased (P<0.01). Compared with the model group, in the PT group and and the treatment group, the serum level of β-EP was increased (P<0.01), while the serum level of SP, the number of positive cells of IL-1β and the expression of COX-2 protein in brainstem were decreased (P<0.01, P<0.05). Compared with the treatment group, in the PT group, the serum level of β-EP was increased and COX-2 protein expression was decreased (P<0.05).CONCLUSION:Moxibustion could effectively relieve migraine. The mechanism may be related to reduce the serum level of SP, IL-1β and COX-2 protein expression in brainstem, and increase the serum level of β-EP, and the optimal effect is observed in the PT group.
OBJECTIVE:To observe the effects of acupuncture on swallowing function and quality of life for patients with dysphagia in Parkinson's disease (PD).METHODS:A total of 60 patients of PD with dysphagia were randomly divided into an observation group (30 cases, 2 cases dropped off) and a control group (30 cases, 3 cases dropped off). The control group was given conventional medication therapy and rehabilitation training. On the basis of the treatment as the control group, the observation group was given acupuncture at Fengfu (GV 16), Baihui (GV 20), Shenting (GV 24), Yintang (GV 24+), Yansanzhen and bilateral Fengchi (GB 20), 30 min each time, once a day, 6 times a week for 4 weeks. Before and after treatment, the Kubota water swallowing test, standardized swallowing assessment (SSA) and swallowing quality of life (SWAL-QOL) were used to evaluate the swallowing function and quality of life of the two groups.RESULTS:After treatment, the Kubota water swallowing test grade, SSA scores in the two groups were decreased compared with those before treatment (P<0.05, P<0.001),the SWAL-QOL scores were increased compared with those before treatment (P<0.001); in the observation group,the Kubota water swallowing test grade and SSA score were lower than those in the control group (P<0.05),the SWAL-QOL score was higher than that in the control group (P<0.001).CONCLUSION:On the basis of conventional medication therapy and rehabilitation training,acupuncture could improve the swallowing function and quality of life for patients of PD with dysphagia.
OBJECTIVE:To compare the clinical effect between heat-sensitive moxibustion and mild moxibustion for migraine without aura.METHODS:A total of 54 patients with migraine without aura were randomized into an observation group (27 cases, 2 cases dropped off) and a control group (27 cases, 2 cases dropped off). The basic western medication treatment was adopted in the two groups. In the control group, mild moxibustion was applied at Shuaigu (GB 8), Fengchi (GB 20) and Yanglingquan (GB 34) on the affected side. In the observation group, the frequent acupoint areas of the affected side i.e. Shuaigu (GB 8), Fengchi (GB 20), Taiyang (EX-HN 5), Taichong (LR 3), Yanglingquan (GB 34) were determined, 3 acupoints with strong heat-sensitive sensation were selected each time and mild moxibustion was adopted. The treatment was given once a day, 5 times of treatment was as one course and 2 courses were required in the two groups. Before and after treatment, the scores of migraine symptom, visual analogue scale (VAS), migraine specific quality of life questionnaire (MSQ) were observed, and the clinical efficacy was evaluated after treatment in the two groups.RESULTS:After treatment, the scores of migraine symptom and VAS were decreased compared with those before treatment (P<0.01), while the MSQ scores were increased compared with those before treatment (P<0.01) in the two groups. After treatment, the scores of migraine symptom and VAS in the observation group were lower than those in the control group (P<0.05), while the MSQ score in the observation group was higher than that in the control group (P<0.01). The total effective rate was 92.0% (23/25) in the observation group, which was superior to 72.0% (18/25) in the control group (P<0.05).CONCLUSION:Both heat-sensitive moxibustion and mild moxibustion can effectively alleviate the clinical symptoms, improve the headache degree and life quality in patients with migraine without aura, the clinical efficacy of heat-sensitive moxibustion is superior to that of mild moxibustion.
Dysphagia are common complications of stroke and have a significant impact on the health of patients. With the development of traditional Chinese medicine, moxibustion therapy has shown notable efficacy in the treatment of swallowing disorders in clinical practice. This article provides a concise overview of the research progress on the application of moxibustion therapy for swallowing disorders, aiming to offer new insights and ideas for the clinical treatment of patients with swallowing disorders.
抑郁症是临床中常见的情绪障碍性疾病,其病理机制复杂,涉及脑内炎症反应、下丘脑—垂体—肾上腺轴功能紊乱、神经可塑性改变、肠道菌群微生态失衡等多种学说.目前的抗抑郁药物主要通过提高突触间隙间的单胺类神经递质浓度发挥抗抑郁作用,但存在起效慢、应答率低的局限性.小胶质细胞(MG)作为中枢神经系统的固有免疫细胞,与抑郁症的病理生理机制密切相关,其可通过影响炎症反应、HPA轴和肠道菌群参与抑郁症的发生发展.同时,氯胺酮和抗菌素米诺环素等以MG为靶点的相关抗抑郁药物的研发也有了较大进展.深入探讨MG在抑郁症发病中的作用机制,对了解抑郁症的病理生理机制及研发以MG为治疗靶点的新型抗抑郁药物具有重要意义.
Tongue acupuncture therapy is an external treatment method of "micro acupuncture system" based on the theory of Zang Fu meridians and holographic theory. It acts on the tongue and can be used for the treatment of swallowing disorder after stroke. It has the advantages of improving tongue and pharyngeal function, high safety, simplicity and quickness. It is often used in combination with rehabilitation training, Traditional Chinese Medicine therapy or other acupuncture methods in clinic, and has achieved good curative effects.
目的 探讨化痰祛瘀方+咽三针治疗脑卒中后吞咽功能障碍的临床效果.方法 选取脑卒中后吞咽功能障碍患者64例,随机分成研究组和对照组,各32例.研究组接受化痰祛瘀方+咽三针治疗,对照组接受常规治疗,比较两组治疗效果.结果 干预后,研究组临床症状积分、NIHSS评分、洼田氏饮水试验评分均低于对照组(P<0.05),Barthel指数高于对照组(P<0.05);研究组治疗总有效率高于对照组,不良反应发生率低于对照组(P<0.05).结论 化痰祛瘀方+咽三针的效果更为显著,可促进相关症状快速消失,同时减低不良反应的发生率,改善患者吞咽能力.
Dysphagia is a common non-motor symptom of Parkinson's disease (PD). The pathogenesis of the disease includes central changes and peripheral changes. The mechanism of acupuncture intervention is mainly to protect DA neurons and central acetylcholinergic neurons, and the research on the peripheral mechanism of action is currently lacking. Acupuncture, electro-acupuncture, warm acupuncture, and combined acupuncture therapy have showed satisfactory clinical curative effects. Among them, Lianquan (CV 23),Fengchi (GB 20), Yamen (GV 15) and Yifeng (TE 17) are commonly used in routine acupuncture. The acupoints with high frequency of bloodletting are Jinjin (EX-HN 12) and Yuye (EX-HN 13). However, the existing studies have the problems such as small sample size, inconsistent operation plans, and effect evaluation standards difference, which need to be further improved.
帕金森病(Parkinson's disease,PD)作为一种好发于中老年人的运动障碍性疾病,以黑质致密区多巴胺(DA)神经元及其他含色素的神经元大量变性丢失为主要病理特征.目前认为帕金森病并非单一因素所致,而是多因素相互交织.该文检索近年来最新国内外相关文献,从氧化应激反应、炎症反应、蛋白质降解系统障碍、线粒体功能缺陷、细胞凋亡等众多方面详细阐述了帕金森病的发病机制,系统综述了针灸治疗帕金森病的作用机制.查阅文献显示,帕金森病的发病机制多与氧化应激、炎症反应、蛋白质降解系统障碍、线粒体功能缺陷、细胞凋亡相关,针灸疗法可通过抗氧化作用、调节机体免疫功能、抑制DA神经元凋亡、调节蛋白质降解系统障碍等对机体进行整体调节,在改善帕金森病方面疗效突出.
缺血性脑卒中是指脑部的供血不畅,导致相应组织出现缺血、缺氧,最终该部位损伤,并出现一些相应临床的表现,常见的有偏瘫、失语等[1].多发于老年人,因其致残率相对较高,如何预防和降低该疾病的发生,改善预后,是我们医者时刻关注的问题.然而该疾病的发病机制复杂,临床表现多样,单纯使用药物对临床症状改善作用有限.针灸对于偏瘫患者肢体活动不利的疗效确切,可促进自身的修复和恢复[2].长期针刺非常考验患者的耐受力,所以在治疗的同时也要兼顾患者的情绪,适时给予精神上的鼓励.与此同时,如何在减轻患者精神的压力和肉体痛苦的前提下来提高针灸的治疗效果,是值得深究的问题.眼针只在眼睛局部针刺,针刺浅,取的穴位少而精,疗效确切,在治疗偏瘫后肢体活动不利已有相关报道[3].本研究拟观察眼针傍刺法联合中药化痰祛瘀方治疗卒中后偏瘫的临床疗效.
