目的:观察支气管色素沉着纤维化(BAF)患者临床特征,为BAF的防治提供临床参考.方法:回顾性分析2018年4月—2019年10月遵义市红花岗区人民呼吸与危重症医学科行气管镜检查及治疗的113例BAF患者资料,所有患者均行胸部C T,55例患者行肺功能检查,分析其炎症指标(白细胞计数、红细胞沉降率、降钙素原、C反应蛋白)、氧合指标(氧分压及氧合指数)以及常规生化指标(如脑钠肽、肌钙蛋白、D-二聚体、血钠、血清蛋白).结果:113例BAF患者,平均年龄(66.2±9.3)岁,男性占46.9%;有结核病史者1
Objective:To investigate the expression of NLRP3 inflammatory corpuscles in patients with severe community acquired pneumonia (CAP), and to explore the effect of NLRP3 inflammatory corpuscles expression on the severity of CAP and the development of inflammation.Methods:Twenty patients with CAP who were admitted to the Department of Respiratory and Critical Care Medicine in People′s Hospital of Honghuagang District from April 2018 to April 2019 (including eight cases of severe CAP and 12 cases of common CAP) were randomly selected.The patients were routinely tested for inflammatory indicators such as procalcitonin, blood routine, and C-reactive protein, and the patients were assessed for criticality by biochemical and other laboratory tests.The expressions of NLRP3, interleukin-β (IL-1β) and tumor necrosis factor-α (TNF-α) in sputum cells and peripheral blood mononuclear cells were detected by Western blot.The expression of NLRP3 mRNA in peripheral blood mononuclear cells was detected by reverse transcription-polymerase chain reaction.Results:The expression of NLRP3 mRNA in peripheral blood mononuclear cells of common CAP group and severe CAP group was higher than that of healthy control group (all P<0.05).The expression of NLRP3 mRNA in peripheral blood mononuclear cells of severe CAP group was higher than that of common CAP group ( P<0.05).The expressions of IL-1β and TNF-α in sputum cells of common CAP group and severe CAP group was higher than those of healthy control group (all P<0.05).The expressions of IL-1β and TNF-α in sputum cells of severe CAP group was higher than those of common CAP group (all P<0.05).In addition, the expression of IL-1β in peripheral blood mononuclear cells of common CAP group was higher than that of healthy control group ( P<0.05).TNF-α expression in peripheral blood mononuclear cells of severe CAP group was significantly higher than that of common CAP group and healthy control group (all P<0.05). Conclusions:The expressions of NLRP3 inflammatory corpuscles are higher in CAP group, especially in severe CAP group, indicating that NLRP3 inflammatory corpuscles can represent the degree of inflammatory response or the severity of disease in CAP patients.
