Глобальное распространение новой коронавирусной инфекции диктует необходимость изучения клинических особенностей COVID-19 и его долгосрочных последствий у детей, особенно в постковидном периоде. У детей и подростков инфекция SARS-CоV-2 обычно вызывает менее тяжелое заболевание и реже приводит к смерти по сравнению со взрослыми. Данный обзор посвящен эпидемиологии, патогенезу, клинической картине, диагностике и профилактике COVID-19 и постковидного синдрома в педиатрической популяции. Иммунный ответ на инфекцию COVID-19 представляет собой сложную, многоэтапную реакцию организма на вирус SARS-CoV-2. Сбои в иммунной системе ребенка – важное направление изучения коронавирусной инфекции у детей. Клиническая картина COVID-19 и постковидного периода у детей характеризуется разнообразием проявлений. Дети, перенесшие COVID-19, могут столкнуться с различными последствиями заболевания, как физическими, так и психологическими. В этой связи следует подробно изучать не только симптомы болезни, но и последствия, которые могут возникнуть после перенесенной инфекции. Грамотный подход к диагностике и лечению COVID-19 у детей, ведению больных в постковидном периоде имеет важное значение, что послужило основанием для выполнения данной работы. The global spread of the new coronavirus infection dictates the need to study the clinical features of COVID-19 and its long-term consequences in children, especially in the post-COVID period. In children and adolescents, SARS-CoV-2 infection usually causes a less severe disease and is less likely to lead to death compared to adults. This review focuses on the epidemiology, pathogenesis, clinical picture, diagnosis and prevention of COVID-19 and post-COVID syndrome in the pediatric population. The immune response to COVID-19 infection is a complex, multi-stage reaction of the body to the SARS-CoV-2 virus. Failures in the child’s immune system are an important area of studying coronavirus infection in children. The clinical picture of COVID-19 and the post-COVID period in children is characterized by a variety of manifestations. Children who have had COVID-19 may face various consequences of the disease, both physical and psychological. In this regard, it is necessary to study in detail not only the symptoms of the disease, but also the consequences that may arise after the infection. A competent approach to the diagnosis and treatment of COVID-19 in children, and the management of patients in the post-COVID period is of great importance, which served as the basis for this work.
Буллезный эпидермолиз (БЭ) относится к группе редко встречающихся заболеваний кожи, которые характеризуются образованием пузырей, эрозий на кожных покровах и слизистых оболочках. Данная патология относится к орфанным заболеваниям, является генетически обусловленной, с разным типом наследования. Базальный слой эпидермиса, зона базальной мембраны и внеклеточный матрикс занимают центральное место в патофизиологии заболевания, а генетические нарушения изменяют структуру или функцию их белков. Мутации затрагивают 20 генов, что объясняет генетическую и клиническую гетерогенность БЭ с формированием четырех основных типов БЭ, включающих более 30 клинических подтипов. Важным диагностическим критерием при буллезном эпидермолизе является ДНК-диагностика. Количество новорожденных детей в Республике Северная Осетия – Алания с 2010 по 2022 г. составило 124 730 человек, при этом диагноз БЭ был подтвержден на молекулярногенетическом уровне в 2 случаях. Таким образом, за исследуемый период распространенность заболевания составила 1 на 62 365, или 1,6 на 100 000 новорожденных, или 16 случаев на 1 млн, что свидетельствует о достаточно высокой распространенности данной патологии в регионе. This pathology is an orphan disease and is genetically determined, with different types of inheritance. The basal layer of the epidermis, the basement membrane zone, and the extracellular matrix are central to the pathophysiology of the disease, and genetic changes determine the structure or function of their proteins. Mutations affect 20 genes, and this explains the genetic and clinical heterogeneity of EB with the formation of four main types of EB, including more than 30 clinical subtypes. An important diagnostic criterion for epidermolysis bullosa is DNA diagnostic. The number of newborn children in North Ossetia-Alania from 2010 to 2022 was 124,730 people, while the diagnosis of EB was confirmed at the molecular genetic level in 2 cases. Thus, during the study period, the prevalence of the disease was 1 per 62,365 or 1.6 per 100,000 newborns or 16 cases per 1 million, which indicates a fairly high prevalence of this pathology in the region.
One of the leading positions among the infant diseases belongs to the pathology of the respiratory tract. The causes of the obstructive bronchitis in children are very diverse including those, which can be associated with the disorder of the normal immune system functioning or immunodeficiency state. The purpose of the study is to determine the nature of the immune disorders in the children with the obstructive bronchitis, to assess the efficiency of the immunocorrecting therapy. For 2 groups of the children with the obstructive bronchitis, the comparative study with determination of the cytokine and immunoglobulin levels in the blood serum against the background of standard therapy (1st group) and against the background with immune stimulator ismigen included into the therapy was carried out; the ismigen is a multivalent antigenic complex of the bacterial lysates. The efficiency of the immune-enhancing therapy carried out towards the immune system condition of children during the comprehensive treatment of the obstructive bronchitis was shown. The practical importance of the work is to optimize diagnosis and immunocorrection in obstructive bronchitis in children, accompanied by the disorder of the body’s immunoreactivity.
