An article reflects modern concepts of the etiology of vitamin B12 deficiency including medication-induced conditions. Particular attention is paid to diagnostic algorithm in case of overt and latent forms of deficit. As the method of choice for the diagnosis of vitamin B12 deficiency it is recommended to determine the level of holotranscobalamin and use additional methods for detecting metabolic deficiency of vitamin B12 – to study the levels of homocysteine and methylmalonic acid. Diagnostic algorithms for patients with clinical suspicion of vitamin B12 deficiency and the presence of subclinical deficiency. We reviewed the possibility of vitamin B12 oral preparations to correct low levels of cobalamin in asymptomatic patients, as well as maintenance therapy in patients with clinical signs of vitamin B12 deficiency.
The acute interstitial pneumonia is a disease of group of idiopathic interstitial pneumonias. The article presents a case of clinical observation of female patient with acute interstitial pneumonia. The treatment applied included combination of therapy with average high doses of systemic glucocorticosteroids using protective modes of noninvasive ventilation of lungs that resulted in expressed positive effect during short period and permitted to avoid worser scenario of development of disease.
The asymptomatic increase of level of enzymes of pancreas often results in such incorrect diagnosis as chronic pancreatitis and in prescription of unnecessary treatment. Meanwhile, in many similar cases hyperenzymemia has a benevolent nature. The article discusses possible causes of pancreatic hyperenzymemia in healthy persons. The algorithm of diagnostic search in this clinical situation is proposed.
The article deals with the assessment of effectiveness of application of heated oxygen helium mixture (thermogeliox) as combined with the nebulizer therapy in complex treatment of patients with bronchial asthma. The examined sample consisted of 80 patients with bronchial asthma of moderate and severy degree at exacerbation phase of disease. The complex treatment with thermogeliox was applied to 40 patients of main group. The single session of combined application of thermogeliox and nebulizer therapy to patients with bronchial asthma reliably improves the bronchial patency and results in significantly increases saturation of blood with oxygen in patients with bronchial asthma of severe degree. The complex treatment with thermogeliox of patients with bronchial asthma of moderate and seveiy degree contributes to more evident improvement of clinical picture of disease as compared with standard treatment: reliable decrease of attacks of suffocation and stertor, relief of cough and dyspnea, improvement of indicators of pulmonary ventilation with reliable decrease of intensity of bronchial obstruction, significant increase of partial tension of blood oxygen and blood oxygenation. The highest effectiveness of complex treatment is noted in patients with severe form of bronchial asthma. Besides, a significant improvement of quality of life occurs against the background of tangible improvement of clinical functional indicators. The study results demonstrate the expedience of widespread application of thermogeliox combined with the nebulizer therapy for the purpose of increasing the effectiveness of rehabilitation treatment of patients with bronchial asthma.
