The article is determined to the analysis of different types of plastic reconstruction of the external nose with local and combine flaps after radical surgery in oncology. Reconstruction of the external nose is discussed under consideration of experience of our research center, which includes surgical treatment of 35 oncologic patients.
The new method of combined cryogenic and radiation therapy for patients with locally advanced buccal skin cancer was devised. The comprehensive analysis of results of cryoradiotherapy for 21 patients with basal cell and squamous cell buccal skin cancer (Т 2–4) and local recurrence was carried out. The treatment resulted in favorable long-term, cosmetic and functional results without local and general complications in these patients including elderly patients and patients with severe concomitant diseases
Cryosurgery was used for treatment of 71 patients with basal-cell skin cancer of the head II stages in selective group according to the indications. Follow-up was from 2 to 11 years (med. 6 years). Recurrence rate after cryosurgery treatment was 5,6 %. Recurrent tumors were treated by cryosurgery (2) and surgery (2). Good esthetic and functional results were noted.
The objective of the present work was to improve the efficacy of treatment of locally advanced auricular skin cancer with the use of the newly proposed combined method including cryogenic and radiotherapy. The choice of the optimal temporal and thermal parameters of combined cryoradiotherapy was based on the results of previous experimental and theoretical investigations. Each radiation treatment session was immediately preceded by local cooling of the tumour down to 0°C - minus 5°C with a nitrogen vapour. Radiotherapy was performed with the use of either photon or electron radiation at a single local dose of 2 Gy-2.25 Gy (the total cumulative dose 64 Gy-68 Gy). As many as 14 patients presenting with locally advanced auricular skin cancer received combined cryogenic and radiotherapy; 10 of them had basal cell carcinoma and 4 squamous cell carcinoma. In all these cases, the treatment resulted in the complete regression of the tumours. Further observation during a follow-up period from 2 to 14 years in duration demonstrated that only two patients developed tumour relapses. These secondary neoplasms were successfully treated by cryodestruction (n=1) and surgical ablation (n=1). The proposed cryoradiotherapy made it possible to preserve the anatomical relief of the auricle and the auditory passage with good aesthetic and functional outcomes.
Acomprehensive analysis of treatment outcomes for 476 patients with basal cell and squamous cell skin cancers of the head and neck was carried out. One hundred thirty four patients (28 %) underwent surgical incision of the tumor with the use of various methods of replacement of tissue defects. Of these patients, there were 100 (74,6 %) with basal cell cancer and 34 (25,4 %) with squamous cell skin cancer. The extent of the tumor was as follows: Tin 19 (14,2 %), Tin 32 (23,9 %), Tin 18 (13,4 %) and Tin 16 (11,9 %). Distant recurrence of skin 123 4 cancer was revealed in 49 (36,6 %) of patients. At a follow up of 2 to 14 years following surgical treatment, skin cancer recurrence occurred in 19 (14,2 %) patients. The frequency of recurrence increased by higher stage of the disease and it was the highest (28,6 %) in patients receiving treatment for recurrent tumor. Regional skin cancer metastases occurred in 12 (8,9 %) patients including 8 patients with squamous cell cancer and 4 patients with recurrent basal cell skin cancer. Strict adherence to indications for surgical treatment of head and neck skin cancer with simultaneous plastic surgery allows the optimal aesthetic, functional and long-term results to be obtained.
