The aim of the investigation was to provide a rationale for refusal from antibacterial therapy in the hypoplastic variant of chronic endometritis.Materials and Methods. The study involved 116 patients with burdened reproductive history (infertility, miscarriage, IVF and ET failures), who were diagnosed with the hypoplastic variant of chronic endometritis after complex examination (hysteroscopy, morphology and immunohistochemistry of the endometrium). The study was carried out in two stages. At the first stage, the patients were examined and the autoimmune genesis of the disease was proved. At the second stage, the patients were divided into two groups, the main group (84 women) and the control (32 women). The patients in the control group received conventional antibiotic therapy with simultaneous administration of trophotropic medications (placenta hydrolysate Laennec, phlebotonic diosmin 600). In the main group we used no antibiotic therapy having included only immunomodulating trophotropic agents in the treatment.Results. It was established that conventional therapy exerted no effect on the endometrial cell composition (CD3(+), CD4(+), CD8(+), CD56(+)) and humoral immunity indices. When using only immunomodulating trophotropic agents, positive tendencies were observed in normalization of the endometrial cell composition, autoimmunization indices decreased (IgG), the myometrium and endometrium blood supply improved, the endometrium proved to increase in thickness.Conclusion. The hypoplastic variant of chronic endometritis is a typical autoimmune process, in which administration of antibiotics is useless. At the same time, the positive tendencies revealed while using immunomodulating trophotropic agents have demonstrated expediency of their introduction.
The article presents the results of prospective, controllable, randomized research the purpose of which was examination of opportunities of optical coherent tomography using diagnostic monitoring of patients suffering from human papillomavirus associated changes of cervical mucous in low degree. There were 50 patients examined of basic and control groups with dynamic observation during 3 years. It was determined that inclusion of optical coherent tomography in diagnostic algorithm allowed to reduce the amount of ungrounded biopsies by 39 % and to decrease the frequency of instrumental and laboratory methods of monitoring.
The article deals with the study of characteristics of biocenosis of urogenital tract in women of reproductive age with using of "Femoflor" test. The scrapings of posterolateral wall of vagina were analyzed using the technique of real-time polymerase chain reaction using the reagents "Femoflor". The complex evaluation of urogenital biota identified three main types of biocenosis of vagina: type I--normocenosis (n = 50 or 11.5%): type II--mild dysbiosis (n = 88 or 21%); type III--marked dysbiosis (n = 133 or 30.6%). In the structure of alterations of biocenosis of urogenital tract the main role play the anaerobic bacteria with involvement of candida, ureoplasma and mcoplasma.
The article presents the assessment of the prevalence of viral loading with various genotypes at women with gynaecological pathology by means of using the method of polymerase chain reaction.
The work proved the urgency of the problem of HPV-associated cervical diseases in the reproductive age. In the process of observation 153 patients are offered approaches to the improvement of the methods of secondary prevention of cervical cancer through the optimization of non-invasive diagnosis of HPV-associated cervical infection as early precancerous indicators. Given the growth in the incidence of cervical cancer in the reproductive age, examined the options for management of patients with human papillomavirus infection (HPV) on the basis of the complementary use of traditional methods and new optical technologies – optical coherence tomography (OCT) on the diagnostic stage and photodynamic therapy (PDT) as a method of treatment. In the work of the expediency of a new modification of the OCT (speed of obtaining images of 8-10 fps), which allows you to define the size of the core and reservoir foci of infection, what is maximum excludes рандомные biopsy and allows to assess the dynamics of the surface lining of the ectocervix after treatment. The authors believe that the use of the proposed tactics with HPV – cervical unfection will allow to optimize the diagnostic process, eliminating unjustified trauma to the cervix of the uterus, as well as justify the gentle therapeutic effects in women of reproductive age.
Aim. To study the relationship between the condition of the vaginal microbiota and the severity of pathological changes of the cervical epithelium. Methods. Studied were 99 female patients aged from 18 to 56 years with secondary and precancerous cervical processes: the first group - 20 women with cervical ectopias; the second group - 20 females with cervical intraepithelial stage I neoplasia; the third group - 19 women with stage II neoplasia; the fourth group - 20 patients with cervical intraepithelial stage III neoplasia (cancer in situ); the fifth group - 20 women with invasive cancer. The investigation of the vaginal biocenosis was conducted by the real-time polymerase chain reaction technique. Results. Established was a correlation between the dysbiotic vaginal changes and cervical lesions. In the first group normocenosis was reported in 4 women (20%), in the groups with cervical intraepithelial stage III neoplasia (cancer in situ) and with cervical cancer - only in 1 woman, accounting for 5% of the total number of patients in the group. In the first group in 16 women out of 20 (80%) noted was the dominance of Lactobacillus, in the groups with cervical intraepithelial neoplasia the proportion of such patients decreased, reaching for the fourth and fifth groups - 45 and 50% respectively. Conclusion. Imbalance of the urogenital tract microbiota may contribute significantly to the development and progression of cervical lesions.
The article highlights studying local humoral immunity indices of cervical and vaginal biotope of cervix uteri diseases of infectious genesis.
The polymerase chain reaction technique was applied in real time format to evaluate the occurrence rate and infection ratio of various genotypes of human papilloma of high carcinogenic risk in virus-positive women and contact persons. The examination sampling consisted of 738 women aged of 17-50 years. The examination results permitted to establish high percentage of infection of 546 patients (74%) by carcinogenic papilloma viruses. The analysis of detection rate of various genotypes of human papilloma of high carcinogenic risk established that the 56th and 16th types of high carcinogenic risk are revealed more often than others--in 33% and 15.4% correspondingly. In males, first place in occurrence rate is for those types of virus of human papilloma: the 56th n = 10 (33.3%), 16th n = 3 (10%), 45th n = 3 (10%), 51th n = 3 (10%). The rest of genotypes are detected in 3-7% cases.
Study purpose: to assess the overall contamination with the human papillomatosis viruses and the viral load in virus-positive women using the polymerase chain reaction. Totally 4105 women referred to medical clinics of Nizhny Novgorod for the treatment of various gynecological diseases were examined. At assessment of overall viral contamination it was demonstrated that 962 women among 4105 (23.4%) were virus-positive. By incidence, the following viruses in virus-positive subjects were the leading: high cancerogenious risk types of viruses 16/18 in 33% of women, 31/33 types in 9.5%; 6/11 types in 12%. The amount of patients with diagnosed clinically significant concentrations of viruses was slightly higher than the amount of women with clinically insignificant concentrations of viruses as 119 and 99, respectively (54% against 45%).

Laboratory test "Femoflor" suggests radical new approach to the assessment of physiological equation and biota imbalance that enables to choose an adequate therapy and control it.
