АКТУАЛЬНОСТЬ. Необходимость точной стратификации риска является очевидной. Современные методы достаточно громоздки, что может вызывать трудности при применении в рутинной практике, в связи с чем большую популярность приобрели относительно простые, но точные методы прогнозирования, которые, однако, не были валидизированы в России: SORT, SRS, POSPOM, NZRISK, SMPM. ЦЕЛЬЮ настоящей работы являлось определение прогностической ценности шкал оценки риска в прогнозировании неблагоприятного послеоперационного исхода на основании анализа данных, полученных в исследовании STOPRISK у пациентов, подвергшихся открытым абдоминальным операциям. МАТЕРИАЛЫ И МЕТОДЫ. Представлен анализ данных о периоперационных показателях 1179 пациентов из 32 центров, расположенных в 8 федеральных округах Российской Федерации, подвергшихся открытым абдоминальным операциям. РЕЗУЛЬТАТЫ ИССЛЕДОВАНИЯ. Летальный исход был зафиксирован у 14 пациентов (1,18 %). Всего было зарегистрировано 135 осложнений у 92 пациентов (7,8 %). Все шкалы продемонстрировали удовлетворительную прогностическую ценность в оценке риска осложнений (площадь под кривой операционной характеристики (AUROC) для шкалы физического статуса Американского общества анестезиологов (ASA) составила 0,714 (0,687–0,739), для шкалы хирургического риска (SRS) — 0,727 (0,701–0,753), для шкалы риска исхода в хирургии (SORT) — 0,738 (0,712–0,763), для Новозеландской шкалы риска (NZRISK) — 0,763 (0,738–0,787), для шкалы вероятности хирургической летальности (SMPM) — 0,732 (0,706–0,757), для предоперационной шкалы прогнозирования послеоперационной летальности (POSPOM) — 0,764 (0,738–0,788)) и хорошую в оценке риска летального исхода (AUROC для шкалы ASA составила 0,82 (0,804–0,843), для шкалы SRS — 0,860 (0,838–0,879), для шкалы SORT — 0,860 (0,838–0,879), для шкалы NZRISK — 0,807 (0,783–0,829), для шкалы SMPM — 0,852 (0,831–0,872), для шкалы POSPOM — 0,811 (0,788–0,833)). ВЫВОДЫ. Все изученные шкалы обладали хорошей прогностической ценностью в оценке риска 30-дневной летальности после обширных абдоминальных операций. Хорошую прогностическую ценность в отношении сердечно-сосудистых осложнений демонстрировали шкалы NZRISK и POSPOM, а в отношении острого почечного повреждения — POSPOM и SRS. Отличную прогностическую ценность в отношении риска послеоперационного делирия показали шкалы POSPOM и NZRISK.
INTRODUCTION. The need for accurate risk stratification is obvious. Modern methods are quite cumbersome, which can cause difficulties when applied in routine practice, and therefore relatively simple but accurate forecasting methods have become very popular, which, however, have not been validated in Russia: SORT (Surgical Outcome Risk Tool), SRS (Surgical Risk Scale), POSPOM (Preoperative Score to Predict Postoperative Mortality), NZRISK (New Zealand RISK), SMPM (Surgical Mortality Probability Model). OBJECTIVES. The aim of this work is to determine the prognostic value of risk assessment scales in predicting an unfavorable postoperative outcome based on the analysis of data obtained in the STOPRISK study in patients undergoing open abdominal surgery. MATERIALS AND METHODS. The analysis of data on perioperative parameters of 1,179 patients who underwent open abdominal surgery is presented. RESULTS. The fatal outcome was recorded in 14 patients (1.18 %). A total of 135 complications were registered in 92 patients (7.8 %). All scales demonstrated satisfactory prognostic value in assessing the risk of complications (the area under the operating characteristic curve (AUROC) for the Physical Status Scale of the American Society of Anesthesiologists (ASA) was 0.714 (0.687–0.739), for the Surgical Risk Scale (SRS) — 0.727 (0.701–0.753), for the Surgical Outcome Risk Scale (SORT) — 0.738 (0.712–0.763), for the New Zealand Risk Scale (NZRISK) — 0.763 (0.738–0.787), for the Surgical Mortality Probability Scale (SMPM) — 0.732 (0.706–0.757), for the Preoperative Postoperative mortality Prediction Scale (POSPOM) — 0.764 (0.738–0.788)) and good in assessing the risk of death (AUROC for the ASA scale was 0.82 (0.804–0.843), for the SRS scale — 0.860 (0.838–0.879), for the SORT scale — 0.860 (0.838–0.879), for the NZRISK scale — 0.807 (0.783–0.829), for the SMPM scale — 0.852 (0.831–0.872), for the POSPOM scale — 0.811 (0.788–0.833)). CONCLUSIONS. All the studied scales have good prognostic value in assessing the risk of 30-day mortality after major abdominal surgery. The NZRISK and POSPOM scales demonstrate good prognostic value for cardiovascular complications, POSPOM and SRS scales — for acute renal injury. POSPOM and NZRISK scales showed an excellent prognostic value in relation to the risk of postoperative delirium.
Актуальность. Несмотря на успехи современной медицины, гарантировать полную безопасность пациента невозможно, и даже при плановых оперативных вмешательствах существует риск летального исхода. В настоящее время нет единства в подходах к оценке периоперационного риска и многие системы его определения не валидизированы в России. Также остается открытым вопрос о вкладе предоперационных факторов в вероятность неблагоприятного исхода, что требует проведения крупных многоцентровых национальных исследований. Цель исследования. Определить прогностическую ценность предоперационных факторов в определении риска летального исхода и осложнений на основании анализа данных, полученных за первый год исследования STOPRISK. Материалы и методы. Представлен анализ данных о периоперационных показателях 3002 пациентов, оперированных на органах брюшной полости и малого таза в 30 центрах 21 города России, участвующих в исследовании STOPRISK. Результаты. Летальность в исследуемой когорте составила 0,47 %, частота послеоперационных осложнений — 3,9 %. Чаще всего неблагоприятный исход развивался после обширных операций на верхнем этаже брюшной полости, колопроктологических и сосудистых операций. Несмотря на то что увеличение травматичности операции и класса ASA статистически значимо влияет на риск неблагоприятного исхода, применение этих показателей позволяет с ограниченной точностью прогнозировать послеоперационную летальность (AUROC = 0,85) и совместно с возрастом послеоперационные осложнения (AUROC = 0,77). Выводы. Таким образом, вероятность неблагоприятного исхода может быть оценена с помощью таких факторов, как травматичность операции и исходный физический статус, однако их прогностическая ценность для определения риска летального исхода недостаточна, а их возможности в оценке риска послеоперационных осложнений еще меньше. Как показывают данные литературы, включение в модель дополнительных факторов риска позволяет увеличить точность прогноза, однако с учетом выявленных особенностей структуры сопутствующих заболеваний и их влияния на исход в изучаемой популяции необходима дальнейшая оценка их вклада в периоперационный риск. Также, учитывая особенности встречаемости некоторых сопутствующих заболеваний, требуется продолжение исследования для выявления значимого влияния на летальность и послеоперационные осложнения.
Introduction. Despite the advances in modern anesthesiology, it is impossible to guarantee a safe course of anesthesia, and even with planned surgical interventions, there is a risk of death. At present, there is no unanimity in approaches to assessing perioperative risk, and many systems for determining this risk have not been validated in Russia. The question of the contribution of pre-operative factors to the likelihood of an unfavorable outcome also remains open, which requires large multicenter national studies. Objectives. To assessment the predictive value of preoperative factors in determining the risk of death and complications based on the analysis of data obtained during the first year of the STOPRISK study. Materials and methods. An analysis of data on perioperative indices of 3002 patients operated on the abdominal and pelvic organs from 30 centers in 21 cities of Russia participating in the STOPRISK study is presented. Results. The mortality rate in the study was 0.47 %, the rate of postoperative complications was 3.9 %. Most often, an unfavorable outcome developed after upper abdominal and colorectal surgery. Despite the fact that the severity of surgery and the ASA class are independent predictors of an unfavorable outcome, the use of these parameters allows to predict postoperative mortality (AUROC = 0.85) and (with age) postoperative complications (AUROC = 0.77) with limited accuracy. Conclusions. Thus, the probability of an unfavorable outcome can be estimated using factors such as the severity of surgery and the initial physical status, but their predictive value for determining the risk of mortality is clearly insufficient, and even less is their ability to assess the risk of postoperative complications. As shown by literature data, inclusion in model additional risk factors allows to increase the accuracy of the forecast, however, given the peculiarities of the structure of comorbidities and their impact on outcome in the studied population, we need further evaluation of their contribution to perioperative risk. Also, taking into account the peculiarities of the occurrence of some concomitant diseases, further research is required to identify a significant impact on mortality and postoperative complications.
Critical states, regardless of the cause, are characterized by the formation of a systemic inflammatory response with multi-directional translations of pro- and anti-inflammatory mediators. Under the conditions of uncontrolled inflammation and the development of persistent organ failure, there exists a possibility of developing uncontrolled abnormal myelopoesis that might result in a release of myeloid suppressor cells. The purpose of the overview is to determine the role of myeloid suppressor cells in the development of immune suppression in critical states from the viewpoint of diagnostic significance, with the focus on both sterile and infectious systemic inflammation. The paper describes a clinical case of early enteral feeding with an immune formula in a patient with significant heart failure after cardiosurgical intervention who is on venoarterial extracorporeal membrane oxygenation. The safety and efficiency of nutritional support were monitored, by estimating blood acid-base balance and oxygenation level and by controlling the residual gastric volume, which enabled the volume of the given enteral mixture to be increased up to 2600 kcal by day 3, by providing the body’s energy needs determined by calculation techniques. On postoperative day 4 when hemodynamic parameters were satisfactory, extracorporeal membrane oxygenation was disconnected; following 7 days the patient was weaned from artificial ventilation. The length of intensive care unit stay was 14 days. The patient was discharged from hospital in a satisfactory condition. Thus, early enteral feeding may be a safe and effective method of nutritional support in patients on extracorporeal circulation. Objective: to enhance the efficiency of intensive therapy and prevention of pyoseptic complications (PSC) of severe concomitant injury. Subjects and methods: A hundred patients who were divided into three groups were examined. A control group consisted of 30 apparently healthy individuals (donors). A study group comprised 38 patients to whom the standard of intensive care and prevention of PSC of severe concomitant injury was applied. A comparison group included 32 patients who received specific preventive therapy for pulmonary PSC, by incubating the cell mass with recombinant interleukin-2. Hemostatic parameters were followed up by biochemical tests and hemoviscosimetry. Immunological parameters, the data of semiquantitative procalcitonin test, and the clinical laboratory parameters of a systemic inflammatory response, as well as complications were analyzed. Results. In the intervention group, the signs of a systemic inflammatory response regressed beginning on day 2 of therapy, the results of the procalcitonin test and vascular thrombocytic hemostasis became normal and the time of respiratory support and the length of intensive care unit stay reduced. Conclusion. The early combination of cell mass with recombinant interleukin-2, beginning on day 2 of therapy for severe concomitant injury, enhances the efficiency of intensive therapy and prevention of pulmonary PSC.
