We studied clinical-electroencephalographic features in different forms of idiopathic generalized epilepsies (IGE) with variable phenotype in adulthood respective from type of course of the disease.The study included 44 patients with juvenile absence epilepsy (JAE), 97 patients with juvenile myoclonic epilepsy (JME) and 102 patients with IGE with isolated generalized tonic-clonic seizures (GTCS).Presence of absences and myoclonias, GTCS at awakening, interictal epileptiform activity (IEA) under photostimulation and in the period of slow sleep were shown as predictors of GTCS relapse in adults.Detection of clinicalelectroencephalographic markers of poor prognosis of IGE in adulthood was a factor excluding achieving the non-medication remission and withdrawal of antiepileptic drugs (AED) could heighten risk for GTCS relapse.
The purpose of this study was to examine the state of autonomic tone and autonomic reactivity in patients with neuroreflex syncope (NS) and patients with panic attacks during the passive orthostatic test (EPP). Study group were 97 patients with a history of episodes of loss of consciousness, the control group included 22 people. Heart rate variability (HRV) on the Neuron-unit range had been assessed. It was proved that patients with UA during orthostatic observed lack of activity of the sympathetic nervous system, in contrast to patients with PA and the control group. During the same clinostatic test in patients with UA observed recovery parameters to normal values, and in patients with PA activity observed increase suprasegmental parts of the sympathetic nervous system.
Objective. To study the efficacy and safety of phenotropil as add-on to treatment with standard antiepileptic drugs (AED) in patients with symptomatic locally-induced epilepsy (SLIE). Material and methods. Double-blind placebo controlled randomized trial was carried out. Ninety patients were treated by phenotropil dose (100 or 200 mg) or placebo. MMSE, HAM-D, MFI-20 and encephalography, including Р300 evoked potentials, were used. Results and conclusion. Phenotropil reduced the frequency of seizures and improved cognitive function in the absence of epileptiform EEG abnormalities. The drug did not decrease negative effects of standard AED therapy in 40% of patients.
Substance for investigation serve patient with locally-associated epilepsy (LAE), taking antiepileptic drugs — 151 men. From them — 73 patients with drug-resistant forms of the disease and 78 patients with control epileptic seizure. Spend monitoring indicators of EEG during sleep and wakefulness cycle. In the group of patients with drug-resistant forms LAE significantly detected more frequently epileptiform activity and increase index α-rhythm during sleep in comparison with period wakefulness.
106 patients with cerebral arachnoiditis diagnosed according to pneumoencephalographic and computer-tomographic findings were studied. 65 patients had clinical signs of progredient disease (lability and an increase of neurologic symptoms). In 41 patients clinical pattern was stable during several years. In the patients with a progredient course of the disease investigation of the immune status revealed that aggravation of clinical symptoms is accompanied by a decrease of a number of T-lymphocytes, an increase of the levels of immunoglobulins A and G as well as of P-proteins and of a general activity of serum interferons. Such alterations were not observed in patients with a stable course of the disease.
