Introduction L’entérocolite ulcéronécrosante (ECUN) survenant dans 90 % des cas chez l’enfant prématuré a pour facteurs de risques, une alimentation entérale retardée ou augmentée trop rapidement, une colonisation bactérienne, une lésion de la muqueuse intestinale (ischémie, hypoxie, KTVO). L’inflammation intestinale due à l’augmentation des médiateurs de l’inflammation tels que PAF, TNF ∝, IL1 et 6, stimulés par des endotoxines bactériennes, participerait à la physiopathologie en lésant la muqueuse. Cas cliniques Esteban A. premier jumeau (2 660 g) né à 32 SA (3e FIV) par césarienne d’une mère de 33 ans, traitée pendant sa grossesse par IMUREL® pour une hépatite auto immune. Adaptation initiale parfaite, nutrition entérale et parentérale (KTVO) pendant 36 heures, l’alimentation orale est augmentée progressivement jusqu’à 200 ml/j de lait maternel à J5 de vie, date à laquelle apparaissent vomissements, ballonnement abdominal, altération de l’état général. L’ASP montre une pneumatose associée à une augmentation de la CRP à 61 mg/l. Le diagnostic d’ECUN impose l’arrêt de l’alimentation, une antibiothérapie par CLAVENTIN®, AMIKLIN®, une ventilation artificielle. L’enfant décède de perforation digestive et de CIVD après 48 heures de réanimation. Le jumeau se porte bien.Amélie AC. née par césarienne à 31 SA pour souffrance fœtale aiguë sur RCIU d’une mère traitée par PLAQUENIL® pour une connectivite mixte. Naissance en état de mort apparente, hypotrophe. Le tableau clinique initial sévère avec CIVD, troubles métaboliques, insuffisance respiratoire, se complique à trois jours de vie d’un ballonnement et de résidus bilieux. L’ASP confirme un pneumopéritoine avec pneumatose. Décès à J8 avec perforation digestive, pneumopéritoine, insuffisance rénale par ischémie mésentérique. Conclusion La pathogénie de l’ECUN est encore incomplètement comprise, le dysmicrobisme intestinal, ainsi que l’augmentation du taux des médiateurs de l’inflammation pourraient jouer un rôle. Nous rapportons deux observations d’ECUN chez deux nouveau-nés dont les mères étaient traitées pendant la grossesse par immunosuppresseurs, afin d’évaluer leur influence d’un tel traitement dans la survenue de cette complication digestive.
Evaluer l’intérêt diagnostique de l’échographie en urgence dans les épanchements traumatiques du coude de l’enfant sans fracture visible. Etude prospective portant sur 10 enfants âgés de 4 à 15 ans présentant un traumatisme du coude avec épanchement sur les clichés standard sans fracture visible. Chaque patient a bénéficié en urgence (lt ; 48h) de 2 autres examens traités en double aveugle : une échographie visant surtout à rechercher un niveau graisseux dans l’épanchement (lipohémarthrose, signe indirect d’une fracture corticale), et une IRM considérée comme « gold standard », recherchant des signes directs et indirects de fracture. Sur les 10 patients, on trouve 5 lipohémarthroses en échographie associées à 5 fractures occultes intéressant la corticale en IRM. On trouve d’autre part 5 hémarthroses simples associées à des lésions restant sous-chondrales (contusions, fractures trabéculaires) en IRM. Une hémarthrose post-traumatique du coude chez l’enfant n’est pas spécifique d’une fracture. L’échographie est une technique facile, rapide, reproductible et non irradiante pour étudier les récessus articulaires à la recherche d’une lipohémarthrose. Elle permet donc d’orienter d’emblée vers la présence ou l’absence de fracture, modifiant aussitôt la prise en charge et le suivi des patients.
PURPOSE:Thoracic outlet syndrome includes arterial, venous or neurological symptoms. Frequently difficult to diagnose clinically, confirmatory imaging studies are usually required. The purpose of this study is to review the diagnostic work-up during management of patients with thoracic outlet syndrome and demonstrate the value of angiography in the sitting position.MATERIALS AND METHODS:Retrospective study of 81 surgical procedures for thoracic outlet syndrome, between 1997 and 2005, in 56 patients aged 17-57 years. Surgery was bilateral in 26 cases, with bilateral surgery in a single setting for 1 patient. All patients presented clinical symptoms confirmed on US, angiography, venogram or EMG. Angiography, from a transfemoral approach, was initially performed in the supine position, without and with dynamic maneuver, and in the sitting position with dynamic maneuver when needed.RESULTS:In 48 patients, supine rest angiography showed stenosis in 6% of cases and supine dynamic angiography showed stenosis in 81% of cases, with severe stenosis in only 35% of cases (stenosis>80% or arterial occlusion). Angiography in the sitting position was performed in 33 patients, showing worsening of stenosis in 91% of cases, with severe stenosis in 87%.CONCLUSION:Angiography in the sitting position with dynamic maneuver improves the sensitivity for detection of thoraci coutlet syndrome. This procedure may be considered in addition to other imaging modalities routinely used including Doppler US, CT and MRI.
Dans le cadre du programme Télésanté Aquitaine, une expérimentation rassemble une quarantaine de professionnels de santé autour d’échanges électroniques dans le domaine de l’imagerie médicale. Ceci implique des professionnels, cliniciens de ville ou d’établissement de santé (demandeurs) et radiologues d’établissement public ou de cabinet/établissement privé (effecteurs), dans 3 villes d’Aquitaine : Bayonne, Bordeaux, Pau. Elle est basée sur l’utilisation de la messagerie sécurisée du portail Télésanté Aquitaine (www.aquisante.fr), support des échanges électroniques entre médecins demandeurs et effecteurs. Elle associe l’envoi de la demande d’examen par le clinicien au radiologue sur un formulaire standardisé, et le retour du compte rendu accompagné de une ou plusieurs images significatives en pièces attachées. Les principaux objectifs attendus sont : – évaluation des impacts organisationnels liés à l’utilisation des outils (webmail), – évaluation du service rendu aux professionnels et aux patients, – identification des freins et des facteurs de réussite pour élaborer un plan d’accompagnement du changement auprès des professionnels de santé. Les premiers résultats observés sont liés à la charge de travail supplémentaire engendrée, admissible dans le cadre d’une expérimentation, mais préjudiciable dans une fonctionnement banalisé, la nécessité de disposer d’outils adaptés pour extraire et intégrer les données de façon simple et utiliser le serveur régional de façon rapide.
Nous rapportons le cas d’une luxation post-traumatique isolée de la tête radiale chez un enfant. Le diagnostic, souvent établi tardivement, repose sur la radiographie du coude de face et de profil. Le traitement chirurgical précoce et même tardif permet d’obtenir des résultats satisfaisants sur le plan fonctionnel.

We report one case of traumatic rupture of the inferior vena cava. CT was performed during the acute phase and showed retroperitoneal hematoma near the inferior vena cava with extravasation of contrast agent. After stabilization angiography showed pseudo-aneurysmal picture of inferior vena cava.

Intestinal duplications are rare and are usually diagnosed in the first two years of infancy. They can occur at any level of the alimentary tract from the oral cavity to the anus. Duodenal duplication accounts for approximately 7% of all intestinal duplications. A previously healthy 4-year 6-month old boy was admitted with progressive increasing vomiting. Physical examination was normal, the serum amylase level was moderately increased. Upper gastrointestinal series showed an extrinsic compression defect of the proximal duodenum. Ultrasonography showed a 3 cm diameter cyst in the head of the pancreas. A duplication was suggested. Helical CT cholangiography showed a communication between duplication and the biliopancreatic common channel. The biliary tree was normal. Surgical resection of the intrapancreatic lesion was performed. Microscopic examination of the specimen revealed a duodenal duplication.
PURPOSE:endoscopic retrograde cholangiography may be difficult or unfeasible in children. It also may be complicated by acute pancreatitis. The purpose of this study was to determine the diagnostic contribution of spiral CT cholangiography in pediatric patients.SUBJECTS AND METHODS:seven patients, age ranging from 10 months to 13 years were explored without general anesthesia. Eight spiral CT cholangiographies were performed. The patients were suspected to have biliary or pancreatic lesions. Spiral CT was performed 60 minutes after slow infusion of iodipamide (Transbilix-Guerbet). The dose was correlated to the body surface. 3D reconstructions were done using the surface rendering or the MIP technique.RESULTS:biliary tract opacification and 3D reconstructions were considered of good quality in 6 out of 8 studies. It was normal in 2 cases. A duplication of the duodenum communicating with the choledocus was found in 1 case. Biliary lithiasis with a stone in the cystic duct was found in 1 case. Two choledocal cysts with bile ducts dilatation were diagnosed. A post operative biliary leak with bile ducts dilatation was found in one case. The spiral CT was not diagnostic in 2 patients: a case of choledocal cyst, the study was of poor quality due to inadequate sedation and a case of Caroli's disease with a high serum bilirubin level. Four patients had an endoscopic or a percutaneous cholangiography: to explore the pancreatic ducts (2 cases), poor CT study due to the inadequate sedation in 1 child, and the case of Caroli's disease.CONCLUSION:spiral CT is feasible even in young children. It should reduce the indications for endoscopic or percutaneous cholangiography. It has to be performed as the first examination in cases of biliary or pancreatic diseases when surgery is contemplated.

Un patient de 73 ans a été admis en réanimation pour insuffisance rénale aiguë anurique de survenue brutale. Il n'avait pas d'antécédents néphrologiques ou urologiques. Le bilan initial avec radiographie simple de l'abdomen sans préparation et deux échographies a montré la vacuité de la vessie, deux reins de morphologie normale, l'absence de dilatation rénale ou urétérale et une lithiase urétérale droite. La tomodensitométrie abdominopelvienne permit d'expliquer l'anurie en montrant la présence d'une lithiase urétérale bilatérale. Après urétéroscopie, la pose de sondes dans les deux uretères a permis une évolution favorable avec restauration rapide d'une fonction rénale normale. Face á une anurie non expliquée par la radiographie de l'abdomen sans préparation et l'échographie, une lithiase bilatérale doit être recherchée par tomodensitométrie.A 73-year-old man was admitted to the ICU for anuria. He reported no history of urinary disease. The abdominal roentgenography and two échographies showed an empty urinary bladder, a right ureteral calculus without dilatation of the urinary tract. Computed tomography demonstrated the presence of a left ureteral stone. Bilateral retrograde ureteroscopy and drainage allowed a rapid recovery. When the abdominal roentgenography and echography cannot explain the occurrence of anuria, the computed tomography, or better the helical CT, can demonstrate the presence of otherwise unrecognized calculi.
