OBJECTIVE To assess trends in the susceptibility to beta-lactam agents and to fluoroquinolones of clinically relevant Enterobacteriaceae isolated over a 3-year period in 14 French hospital laboratories. METHODS During the second quarter of 1996, 1997 and 1998, 180 consecutive non-duplicate isolates of Enterobacteriaceae were collected in each center. Sixteen beta-lactams and four quinolones were tested by the disk diffusion method. In addition, the double-disk synergy test was used to screen for the production of extended-spectrum beta-lactamase (ESBL). RESULTS Totals of 2507, 2312 and 2506 clinical isolates were obtained in each period, respectively. The distribution of Enterobacteriaceae species according to clinical specimens and wards was similar in each study period. No significant variation in the susceptibility rates to beta-lactams and fluoroquinolones was observed, except in Klebsiella pneumoniae and Enterobacter aerogenes. The prevalence of ESBL-producing isolates decreased from 18% to 9% in the former, while it increased from 32% to 54% in the latter. At the same time, the susceptibility to ofloxacin and pefloxacin increased for K. pneumoniae (P < 0.003) and cephalosporinase-producing species (P < 0.05), except Enterobacter spp. CONCLUSION Over the 3-year study period beta-lactams and fluoroquinolones remained highly active against Enterobacteriaceae clinical isolates, with the exception of E. aerogenes, probably as a result of the dissemination of multiresistant clones in French hospitals.
Diphtheria is a disease with a long history that almost completely disappeared from developed countries. In addition, until 1987, systemic infections involving Corynebacterium diphtheriae were rare. However, in 1990, an epidemic occurred in Russia. These two circumstances have provided the stimulus to gain insight into the situation in France. In fact, between 1987 and 1993, a total of 59 C. diphtheriae strains were isolated. Epidemiological data were collected for patients from whom 40 strains were isolated from normally sterile sites, including 34 from blood cultures, and half of the bacteremic patients developed endocarditis. Osteoarticular involvement was noted in 11 of these 40 patients, including 5 bacteremic patients. The fatality rate following bacteremia was 36%, despite specific antibiotic treatment (beta-lactams and aminoglycosides). The mean age of the participants was 38 years, with half of the patients subsisting under low socioeconomic conditions and suffering from homelessness or alcoholism. Apparently, the skin turned out to be the major route of transmission in this reemerging disease. Eighty-eight percent of the isolates belonged to the C. diphtheriae biotype mitis. These were found predominantly in the Paris area, and most were of the same ribotype. Those isolates originating from the overseas territories (Guyana and New Caledonia) belonged to C. diphtheriae biotype gravis. No strains were positive for the tox gene by PCR. This study attests to the persistent circulation in France of C. diphtheriae in the form of systemic infections. The matter is especially significant since these strains are nontoxigenic and are of a unique ribotype. The strains are, however, sensitive to most antibiotics, although 20% are rifampin resistant.

Nous rapportons un cas de cryptococcose disseminee chez un patient presentant une lymphopenie CD 4 idiopathique. La presentation clinique initialement musculaire est tout-a-fait particuliere. Le diagnostic est evoque sur la mise en evidence de levures encapsulees a l'examen histologique de la peau et du muscle, retrouvees egalement dans le liquide cephalo-rachidien, les hemocultures, le lavage broncho-alveolaire. Le controle de la maladie est obtenu avec un traitement par amphotericine B pendant 2 mois 1/2 puis par itraconazole a vie en raison de la persistance d'une profonde lymphopenie CD 4 d'etiologie indeterminee.
We report a case of invasive cryptococcosis in a patient with idiopathic CD4 T lymphocytopenia. The presenting feature as a profound muscle weakness is particular. The diagnostic is evocated by the presence of encapsulated yeast on the histology of the skin and of the muscle and also in cerebrospinal fluid, blood-culture and bronchoalveolar lavage. The disease control is obtained with amphotericin B during two months a half and itraconazole all life long because of the persistance of profound CD4 T lymphocytopenia unexplained.

In the majority of cases, meningoradiculitis consecutive to tick bite most probably belongs to Lyme disease caused by Borrelia burgdorferi. The disease may be limited to the primary, though inconstant, stage of cutaneous migratory erythema, or it may develop into cardiac and, mostly, neurological disorders. The nervous system involvement, which we observed in 7 patients, mainly consists of peripheral motor neuropathy of moderate severity and sometimes limited to the cranial nerves, associated with very severe radicular pain regularly heralding radicular deficit. Lyme disease predominates in a rural environment, and most patients live near wooded areas where ticks parasitize numerous mammals. The diagnosis, suggested by the socio-environmental context, the clinical features and the results of CSF examination, is confirmed by serology at a 1/256 dilution. Tetracyclines are effective in the initial stage, but intravenous penicillin is necessary in the secondary stage and to reduce the risk of arthritis and also perhaps neuropsychiatric disorders at a late stage.
La majorité des méningo-radiculites survenant après morsure de tique s'intègre très vraisemblablement dans le cadre de la maladie de Lyme dont la nature infectieuse est due à Borrelia burgdorferi. Cette affection peut se limiter au stade primaire d'érythème cutané migrant, toutefois non obligatoire pour présenter des signes secondaires cardiaques et surtout neurologiques. L'atteinte neurologique que nous avons observée chez 7 patients est dominée par une neuropathie périphérique motrice d'intensité modérée, parfois limitée à un nerf crânien, associée à des douleurs radiculaires très pénibles qui systématiquement précèdent le déficit. L'affection sévit principalement en milieu rural et la majorité des patients vivent à proximité de zones boisées où les tiques parasitent de nombreux mammifères. Le diagnostic évoqué par le contexte, l'aspect clinique et les données du liquide céphalo-rachidien, est confirmé par la sérologie lorsque la dilution atteint 1/256. Si les cyclines sont indiquées au stade initial, la Pénicilline intraveineuse est nécessaire pour le traitement du stade secondaire et surtout pour limiter les risques du stade tardif à manifestations rhumatologiques mais peut-être également neuropsychiatriques.
Nous rapportons le quatrième cas français de babésiose humaine. Cette parasitosegrave, rare chez l'homme en Europe, est transmise chez le sujet splénectomisé par la morsure de tique. Elle est bien connue en médecine vétérinaire sous l'appellation de piroplasmose.
Les auteurs rapportent deux nouveaux cas de pasteurelloses humaines à Pasteurella pneumotropica. La première observation concerne une septicémie chez une cirrhotique décompensée sans notion d'inoculation préalable. La seconde décrit une forme focale liée à une morsure de chien. A cette occasion, une revue de la littérature permet de souligner la rareté des cas cliniques publiés notamment septicémiques et de préciser la pathogénie de l'espèce pneumotropica.
On rapporte l'observation et l'étude bactériologique d'un cas de botulisme gravissime survenu chez une femme de 21 ans et suivi de complications infectieuses ayant entraîné la mort. Dans le sérum de la malade, on a mis en évidence des toxines botuliques A et B, et on a isolé à partir de l'aliment toxique (pâté de préparation familiale) une souche de C. botulinum produisant simultanément deux toxines A et B.
Les auteurs rapportent deux nouveaux cas de septicémies à Erysipelothrix rhusiopathiae authentifiées par la positivité de 10 et 12 hémocultures prévelées respectivement sur une période de 8 jours et de 3 semaines. Ces deux observations se singularisent par les points suivants :-aucune porte d'entrée n'a été retrouvée pour l'une de nos malades et l'hypothèse d'un point de départ cutané n'a pu être prouvée pour l'autre ;-dans un cas, la septicémie s'est développée chez une malade immunodéprimée présentant un myélome ;-dans les deux cas, le traitement antibiotique (pénicilline seule et l'association pénicilline-streptomycine) a entraîné la guérison ;-bien qu'aucun argument clinique ni paraclinique convaincant de même que l'évolution sur plus de six mois ne militent en faveur d'une greffe oslérienne patente, le diagnostic d'endocardite infectieuse ne peut être a priori rejeté.
