Le logiciel Diabeo sur smartphone aide à déterminer les doses d'insuline et facilite un suivi télémédical. Nous avons évalué son apport pour renforcer une éducation à l'insulinothérapie fonctionnelle (IF) chez des patients sous schéma basal-bolus 59 adultes (37 ans [30 ; 47]), diabétiques de type 1 (durée 16 ans [9 ; 26]) sont inclus à l'issue immédiate d'un stage IF de 4 jours. Essai prospectif monocentrique randomisé en 2 groupes parallèles : suivi intensif (groupe PDA: smartphone+ téléconsultations hebdomadaires pendant 1 mois, puis retrait du smartphone), ou suivi conventionnel (groupe C: visite à M3). L'HbA1c diminue 3 mois après le stage (8.05 ± 1.16 vs 7.80 ± 0.98 %, p = 0.03). L'HbA1c tend à plus diminuer dans le groupe PDA (− 0.30 % [− 0.50 ; 0.30], p = 0.09) que dans le groupe C (− 0.05 % [− 0.55 ; 0.35], p = 0.37), mais la différence intergroupe n'est pas significative (p = 0.25). L'obtention d'une HbA1c < 7.5% à M3 concerne 45 % (PDA [26 ; 64]) et 29 % (C [13 ; 49]) (p = 0.20). Les règles de l'IF sont suivies pour 80 % ± 18.7 (PDA) et 71.2 % ± 18.4 (C) (p = 0.23). La fréquence de l'autosurveillance glycémique (PDA: 4.34±2.18 vs 5.59 ± 1.74, p < 0.001 ; C: 4±1.74 vs 5.07 ± 1.74 tests/j p < 0.001), et des hypoglycémies modérées (PDA : 10 ± 10.5 vs 5.6 ± 6.1, p < 0.01 ; C : 15 ± 12.5 vs 6.3 ± 4.3 hypos/14 J, p < 0.01) s'améliorent à M3, sans différence intergroupe. La durée cumulée des 4 téléconsultations en 1 mois est de 46 minutes [43 ; 60] par patient. La satisfaction pour le carnet glycémique électronique est bonne (médiane 9/10 [8 ; 9]). Malgré des circonstances connues pour améliorer les indicateurs métaboliques à 3 mois (stage IF), un avantage supplémentaire se dessine avec la télésurveillance par Diabeo pendant 1 mois qui tend à renforcer les résultats sur l'HbA1c de façon non significative. La place de la télémédecine chez les diabétiques de type 1 mérite d'être évaluée à plus large échelle.
Aim. - Basal insulin dose (BID) determination is the key to successful flexible insulin therapy (FIT). As our hypothesis was that BID changes over time, the primary objective of the present study was to determine the changes in BID 1 year after a therapeutic educational programme on FIT.Methods. - This single-centre retrospective study recruited the first 40 type 1 adult diabetic patients undergoing an educational FIT programme, which was conducted over a 4-day hospital stay and included a carbohydrate-fasting test.Results. - Patients' BIDs decreased between Day 0 and Day 4 after the programme (0.31 +/- 0.11 IU/kg/day vs 0.27 +/- 0.09 IU/kg/day; P < 0.0001), and was increased at 1 year (0.29 +/- 0.09 IU/kg/day; P = 0.004). There was no significant variation in prandial insulin requirements. A tendency toward a reduction in HbA(1c), was observed at I year (8.3 +/- 1.4% vs 8.1 +/- 1.6%; P = 0.075), with a decrease by more than 0.5% in 37.5% of patients. Body weight increased at 1 year (66.9 +/- 10.4 kg vs 68.1 +/- 10.7 kg; P = 0.003), and the gain was greater than 5% in 7.5% of patients. Frequency of mild hypoglycaemia either remained stable (40%) or decreased (30%). Only nine patients (baseline HbA(1c) 8.03 +/- 1.7%, baseline BID 0.27 +/- 0.09 IU/kg/day) had BID increases more than 20%, with no changes in prandial insulin requirements and no distinctive phenotype. Baseline HbA(1c) and BID have an impact on the BID at 1 year of approximately 0.3 IU/kg/day in most patients.Conclusion. - The stability of BID over I year, with values close to 0.3 IU/kg/day associated with a trend towards improvement in HbA(1c) reduction in the frequency of mild hypoglycaemic episodes and absence of major weight gain, supports the relevance of FIT educational training. La détermination des besoins en insuline basale (BIB) est un élément clé du succès d'une approche d'insulinothérapie fonctionnelle (IF). La dérive de cette dose dans le temps avec surestimation des BIB par rapport aux besoins prandiaux est une cause fréquente d'hyperglycémie post-prandiale précoce et d'hypoglycémie post-prandiale tardive. L'objectif principal est de déterminer l'évolution des BIB un an après un programme d'éducation thérapeutique à l'IF. Étude rétrospective monocentrique réalisée chez les 48 premiers patients diabétiques de type 1 adultes, sous pompe (n = 12) ou basal-bolus, ayant bénéficié d'un programme hospitalier d'éducation à l'IF, réalisé sur 4 jours d'hospitalisation avec épreuve de jeune glucidique. Les BIB diminuent entre J0 et J4 après le stage (20,7 ± 6.6 UI/j vs 18 ± 5.5 UI/j, p < 0,0001) puis augmentent à un an (19,7 ± 6,2 UI/j, p = 0,004). Parallèlement, il n'y a pas de variation significative des besoins en insuline prandiale. Le poids augmente (65.2 ± 10,5 vs 66.2 ± 10,9 kg, p = 0,01) avec hausse >5 % chez 18,7 %. Les BIB/kg passent de 0,32 ± 0,13 à J0 à 0,27 ± 0,01 à J4 (p = 0,0003) et 0,28 ± 0,1 UI/kg/j à J360, L'HbA1c diminue à un an du stage (8,21 ± 1,2 vs 7,95 ± 1,5 %, p = 0,053) et diminue de > 0,5 % chez 36,2 %. La fréquence d'hypoglycémies modérées reste stable (46,8 %) ou diminue (34 % des patients). L'assiduité de l'autosurveillance glycémique augmente chez 21 % des patients, à un an 74 % des patients font au moins 5 autocontrôles par jour contre 66 % avant le stage. Seuls 9 patients augmentent de plus de 20 % leurs BIB, sans variation des besoins en insuline prandiale. Le phénotype (poids, HbA1c, durée diabète) et le comportement de ces 9 patients (autosurveillance, suivi) ne dégagent aucune tendance. Le type de traitement (pompe externe vs glargine) n'influence pas l'évolution des BIB. La stabilité des BIB sur un an, avec amélioration durable de l'HbA1c, réduction de la fréquence des hypoglycémies modérées, absence de gain pondéral cliniquement significatif, tend à valider l'approche éducative mise en oeuvre, et souligne l'intérêt thérapeutique de l'IF.
Determining a threshold for a quantitative variable (arising in biological measurements for instance) is a common problem in medical decision making. We define seven commonly used strategies: each one leads to an optimal determination. To these strategies correspond relevant empirical curves: the ROC curve for strategies involving the sensitivity or the specificity, the predictive ROC curve (P-ROC curve) for strategies involving the positive and negative predicting values, and the well classified frequencies curve (WCF curve) for classification strategies where all misclassifications have the same importance. For one of the considered strategies, there also exists a theoretical formula for the optimal threshold, elicited within a classical probabilistic model, which gives a considerable advantage to this strategy. These strategies are applied to a simulated example containing 702 cases, where we see that they lead to different optimal threshold values. Finally, we briefly review a practical application in the determination of thresholds for glycemia measurements, leading to the choice of one of them as the optimal one to consider in the gestational diabetes mellitus prediction.
