Die CC-Chemokine CCL2, CCL3, CCL4 und CCL5 nehmen eine Schlüsselrolle in der postoperativen Immunmodulation nach orthotoper Lebertransplantation (OLT) ein. Durch ihre chemotaktische Potenz wird die Leukozytentransmigration in das Transplantat sowohl im Rahmen des Ischämie-Reperfusionssyndromes als auch bei akuter Rejektion reguliert. Dabei erfolgt die Signalttransduktion über den gemeinsamen CCR5 Rezeptor. Die hereditäre CCR5delta32-Mutation führt zu einer Funktionseinschränkung dieser Signalweiterleitung. Inwiefern diese Mutation das Ergebnis nach OLT beeinflusst wird in dieser Arbeit untersucht. 106 OLT von 09/97 bis 03/02 wurden in dieser retrospektiven Analyse untersucht. Mittels QIAamp DNA Blood-Kit wurde aus Vollblut DNA der Patienten isoliert. X mül DNA wurden durch CCR5 spezifische PCR (Primer sense: 5'-CAAAAAGAAGGTCTTCATTACACC-3', antisense: 5'-CCTGTGCCTCTTCTTCTCATTTCG-3') mit je 40 Zyklen amplifiziert. Die Wildtyp PCR erbrachte eine Größe von 189 bp, während die CCR5delta32-Mutation aus 157 bp bestand. 90 Patienten (84,9%) zeigten einen normalen CCR5-Rezeptor. In 15 Fällen lag eine heterozygote CCR5delta32-Mutation (14,2%) vor, in einem Fall trat eine homozygote CCR5delta32-Mutation (0,9%) auf. Von den 90 Patienten mit normalem CCR5-Rezeptor entwickelten 5 Patienten (5,6%) diffuse Galengangsschäden. Dagegen zeigten sich diffuse Gallengangsschäden bei 6 (37,5%) von 16 Patienten mit CCR5delta32-Mutation. Der Unterschied bezüglich der beiden Gruppen war hochsignifikant (p=0,001, Fisher). Eine univariate Analyse der Spender und Empfängerfaktoren (CCR5 gesund vs. CCR5delta32-Mutation) ergab keine Unterschiede für Spender gamma GT (48 vs. 39 U/l p=0,615), Spenderalter (45 vs. 50 Jahre p=0,210), kalte Ischämiezeit (529 vs. 566 Minuten p=0,382), warme Ischämiezeit (36 vs. 39 Minuten p=0,333), Intensivaufenthalt (5 vs. 5d p=0,77), Krankenhausaufenthalt (25 vs. 31d p=0,17), peak Transaminasen innerhalb 3 Tagen (GOT 965 vs. 954 U/l p=0,978, GPT 1046 vs. 929 U/l p=0,788, AP 291 vs. 297 U/l p=0,921, gamma GT 149 vs. 131 U/l p=0,701), Bilirubin Tag 14 (4,1 vs. 4,7mg/dl p=0,596), Kreatinin Tag 14 (1,2 vs. 1,1mg/dl p=0,876) und Quick Tag 14 (89 vs. 88% p=0,776). Das Empfängeralter war in der Gruppe CCR5delta32-Mutation (50 vs. 57 Jahre p=0,007) erhöht.
Einleitung: Die akute Transplantatrejektion (AR) ist eine der Hauptkomplikationen nach orthotoper Lebertransplantation (OLT). Bisher ist eine sichere Diagnose der AR nur durch die histologische Untersuchung der Leberbiopsie möglich. Hinzu kommt, dass eine AR in vielen Fällen differentialdiagnostisch schwierig von einer CMV-Infektion zu unterscheiden ist. Im Zuge der AR kommt es zur Ausprägung eines charakteristischen Leukozyteninfiltrats in der Spenderleber, welches mit einer IFNγ-Produktion einhergeht. Da IFNγ die Leukozyten-rekrutierenden CXC-Chemokine IP–10 (CXCL10), I-TAC (CXCL11) und Mig (CXCL9) induziert und wir vermuten, dass die hepatische Chemokinsekretion als Vermittler von Abstoßungsreaktionen eine Rolle spielen könnte, haben wir deren Spiegel im Serum von Patienten mit AR untersucht.
Einleitung: Der Wachstumsfaktor VEGF nimmt eine Schlüsselrolle in der Angiogenese maligner Tumoren, insbesondere des hepatozellulären Karzinoms (HCC) ein. Für den Promoter des VEGF Gens sind eine Reihe modulierender Polymorphismen beschrieben. Für den Single Nucleotid Polymorphism (SNP) –2578 ist bekannt, dass dieser eine VEGF-Sekretionssteigerung bewirkt. Wir haben für Patienten, die sich wegen eines HCCs einer transarteriellen Chemoembolistion (TACE) unterzogen haben und die danach lebertransplantiert wurden, untersucht, ob dieser Polymorphismus einen Einfluss auf den Verlauf nach Transplantation und auf die Rezidivhäufigkeit des HCCs hat.
Kupffer cells and liver sinusoidal endothelial cells (LSEC) clear portal venous blood from gut-derived bacterial degradation products such as lipopolysaccharide (LPS) without inducing a local inflammatory reaction. LPS tolerance was reported for Kupffer cells, but little is known whether sensitivity of LSEC toward LPS is dynamically regulated. Here, we demonstrate that LSEC react to LPS directly as a function of constitutive Toll-like receptor 4 (TLR4)/CD14 expression but gain a LPS-refractory state upon repetitive stimulation without loss of scavenger activity. LPS tolerance in LSEC is characterized by reduced nuclear localization of nuclear factor-kappaB upon LPS rechallenge. In contrast to monocytes, however, TLR4 surface expression of LSEC is not altered by LPS stimulation and thus does not account for LPS tolerance. Mechanistically, LPS tolerance in LSEC is linked to prostanoid production and may account for cross-tolerance of LPS-treated LSEC to interferon-gamma stimulation. Functionally, LPS tolerance in LSEC results in reduced leukocyte adhesion following LPS rechallenge as a consequence of decreased CD54 surface expression. Furthermore, LPS tolerance is operative in vivo, as we observed by intravital microscopy-reduced leukocyte adhesion to LSEC and improved sinusoidal microcirculation in the liver after repetitive LPS challenges. Our results support the notion that LPS tolerance in organ-resident scavenger LSEC contributes to local hepatic control of inflammation.

Einleitung: Ein Charakteristikum des hepatischen Ischämie/Reperfusionsschadens (I/R) ist die Infiltration der Leber mit inflammatorischen Leukozyten. Da Chemokine die Rekrutierung und Aktivierung von Leukozyten kontrollieren, ist es von Interesse die lokale Produktion von Chemokinen während der hepatischen I/R zu untersuchen. Material/ Methode: In Lewis Ratten (n=59) wurde eine lobuläre hepatische Ischämie (90 Minuten) durchgeführt. Zum Zeitpunkt des Reperfusionsbeginns sowie 2, 4, 6 und 24 Stunden nach Reperfusion wurden die Tiere hepatektomiert und Blut entnommen. Die Transaminasen ALT, AST sowie LDH wurden im Serum gemessen. RNA wurde aus den ischämischen Lappen extrahiert und die Expression der Chemokine MCP-1/JE, MIP-1alpha, MIP-1beta, RANTES und IP-10 mittels Real-time RT-PCR analysiert. Ergebnisse: Nach hepatischer I/R erfolgte eine rasche Zunahme von Chemokin mRNA in den Versuchstieren (MCP-1: 2.2-fold, MIP-1alpha: 1.44-fold; MIP-1beta: 6.2-fold; RANTES: 1.4-fold; IP-10: 4.4-fold). Eine signifikante Induktion von MIP-1alpha, MIP-1beta und IP-10 wurde bereits 2 Stunden nach Reperfusion gemessen. Eine simultane Erhöhung der ALT, AST und LDH im Serum der Versuchtiere mit einem signifikanten Maximum 2 Stunden nach Reperfusion (ALT: 1409±1200; AST: 1744±1589; LDH: 4090±828) unterstrich den hepatozellulären Schaden. Diskussion: Die hepatische I/R verursachte in unserem Modell eine deutliche Induktion der mRNA-Expression lokaler Chemokine, die von einer gleichzeitigen Erhöhung der Transaminasen und der LDH begleitet wurde. Unsere Ergebnisse unterstützen die Hypothese, dass die lokale hepatische Chemokinexpression ein wichtiger Vermittler des I/R-assoziierten Leberschadens sein könnte. Dies macht Chemokine zu potentiellen Zielen einer therapeutischen Intervention. Perspektivisch könnte ihre spezifische Blockierung einen bedeutenden Einfluss auf die Reduktion des hepatischen I/R-Schadens haben.
P976 Aims: CC-Chemokines play a key role in the recruitment of leukocytes during ischemia-reperfusion damage and acute rejection following orthotopic liver transplantation (OLT). Therefore the CC-chemokine-receptor 2 (CCR2), its functionless CCR2-V64I-polymorphism (CCR2-V64I) and the ligand CCL2 (MCP-1) might have an influence on the grafts pathology after OLT. Methods: In 180 patients after OLT between 09/97 and 03/04 the CCR2 was analyzed with regard to the CCR2-V64I by RT-PCR (Fluorescence resonance energy transfer DNA hybridization – light cycler). CCL2 was measured in patients serum with ELISA. Results: 150 patients (age 49.6 +/− 11.9 years, 16-69) showed a normal receptor (83,3%, wildtype) whereas 30 patients (age 52,3 +/− 10,5 years, 22-69) suffered from the CCR2-V64I polymorphism (16,7%, 29 heterozygote, 1 homozygote). 5 year graft survival was 92% in CCR2-V64I patients and 77% in CCR2 patients (p = 0,14 log rank). 5-year-patient survival was 96% and 88% in CCR2-V64I and CCR2, respectively (p = 0,34 log rank). Acute rejection occurred in 15,9% in CCR2-V64I patients and in 20% in CCR2 patients (p = 0,648, Chi-square). MCP-1 levels were significant lower (p<0,01, t-test) in patients with CCR2-V64I (171,9 ± 134,5 pg/ml) compared to CCR2 patients (260,4 ± 257,3 pg/ml). Conclusions: CCR2-V64I leads to excellent 5 year graft survival of 92% following OLT. A significant reduction of MCP-1 levels might lead to a reduced immunoactivation following transplantation and could be reasonable for these results. Larger series must be investigated to proof these results.
Ischemic-type biliary lesions are a major complication following orthotopic liver transplantation. They occur in up to 26% of liver transplant recipients. Among other factors, unknown immunologic factors have always been assumed to be partly responsible for these lesions. CC-chemokines and their receptors play a key role in postoperative immunomodulation after liver transplantation. The non-function CC-chemokine receptor 5delta32 polymorphism (CCR5delta32) has been shown to lead to a lower rate of acute rejection after kidney transplantation; in liver transplantation the role of CCR5delta32 is unclear. We investigated the influence of the CCR5delta32 after liver transplantation with special regard to ischemic-type biliary lesions. The CC-chemokine receptor-5 (CCR5) of 146 recipients was analyzed by polymerase chain reaction to detect CCR5delta32 in blood samples of patients after liver transplantation. One hundred twenty patients with wild-type CCR5 and 26 patients with CCR5delta32 (1 homozygote, 25 heterozygote) were identified. Ischemic-type biliary lesions occurred in 14 of 120 patients with wild-type CCR5 and in 8 of 26 patients with CCR5delta32 polymorphism (P = = 0.01). 5 year patient survival with CCR5delta32 and CCR5 was 70% and 85%, respectively (P =.0067). Our results show that the CCR5delta32 is a significant risk factor for the development of ischemic-type biliary lesions after liver transplantation and leads to a reduction in 5-year survival. In conclusion, the CCR5 status should be screened prospectively before liver transplantation.
Ischemic type biliary lesions (ITBL) are a major complication following orthotopic liver transplantation (OLT). In many cases re-OLT is indicated. Multiple factors have been claimed to be reasonable for ITBL; here we present a new immunological risk factor: CC-Chemokines play a key role in the recruitment of leukocytes during ischemia-reperfusion damage and acute rejection. Therefore the CC-chemokine-receptor 5 (CCR5) and its functionless CCR5-delta-32-polymorphism (CCR5Δ32) might have an influence on the grafts pathology after OLT. In 146 patients after OLT the CCR5 was analyzed with regard to the CCR5Δ32 by PCR. 120 patients (82,1%) showed a normal receptor (wildtype) whereas 26 patients suffered from the CCR5 Δ32 (17,9%) (25 hétérozygote, 1 homozygote). ITBL occurred in 14 patients out of 120 with normal CCR5 and in 8 cases out of 26 patients with CCR5Δ32 (p = 0,01). The univariate analysis of donor and recipient conditions (CCR5 wildtype vs. CCR5Δ32) showed no differences in donor gamma GT (48 vs. 39 U/1, p = 0,615), donor age (45 vs. 50 years, p = 0,210), cold ischemia time (529 vs. 566 min, p = 0,382), and warm ischemia time (36 vs. 39 min, p = 0,333). Acute rejection occurred in 34% in the wildtype group and in 31% in the CCR5Δ32 group (p = 1,000). The CMV-infection rate was 25% in the wildtype group and in 37,5% in the CCR5Δ32 group (p = 0,356). The multivariate analysis showed a significant influence on the development of ITBL by CCR5Δ32 (p = 0,003, OR 9,35). The hereditary CCR5Δ32 has a significant influence on the development of ITBL and should therefore be screened before OLT. This can be subclassified new as immunogen-etic-biliary-lesions. Ongoing investigations must show if an increased immunosuppression could be beneficial in patients suffering from CCR5Δ32-polymorphism.
Hepatic ischemia reperfusion injury as well as acute graft rejection (RE) after orthotopic liver transplantation (OLT) are associated with leukocyte invasion of the graft. Local synthesis of chemokines is a key reaction in the recruitment and activation of inflammatory leukocytes and consequent liver damage. In this paper we describe the role of monocyte chemoattractant protein (MCP)-1 (CCL2) in human OLT. We investigated the serum CC-chemokine levels for MCP-1 by specific ELISAs after OLT in 105 human liver allografts between September 1997 and January 2001. One hour after reperfusion we saw a significant (t test) increase of MCP-1 in peripheral blood (92.5 +/- 85.8 pg/mL to 774.2 +/- 319.6 pg/mL, 8.3-fold, P < .0001), hepatic venous blood (92.5 +/- 85.8 pg/mL to 866.7 +/- 376.1 pg/mL, 9.3-fold, P < .0001), and portal venous blood (92.5 +/- 85.8 pg/mL to 792.9 +/- 408.0 pg/mL, 8.5-fold, P < 0.0001) during hepatic ischemia reperfusion injury. An analysis of the correlation (Spearman's test, rs) between the expression of MCP-1 and the AST (rs 0.555, P < .025) and ALT (rs 0.852, P < .0001) showed a significant linear correlation. During RE a significant (t test) increase of MCP-1 (125.5 +/- 95.6 pg/mL to 188.5 +/- 124.6 pg/mL, 3.86-fold, P < .0001) was demonstrated. The successful treatment of the RE led again to a decline to lower base levels. Hepatic ischemia reperfusion syndrome as well as RE after OLT are characterized by typical patterns of CCL-2 overexpression. This finding proposes a new noninvasive, early diagnostic test after OLT.
Chemokines and hyperlipidemia are involved in the mechanism of chronic allograft nephropathy (CAN). In this study, the mRNA expression of RANTES and its receptor CCR5 on peripheral blood mononuclear cells were measured in renal transplant recipients with hyperlipidemia, and the effect of simvastatin treatment observed to investigate the mechanism and prevention of CAN. Sixty recipients selected from 167 renal transplant recipients were divided into two groups: group A without hyperlipidemia (n = 30) and group B with hyperlipidemia (n = 30). The control group consisted of 30 healthy volunteers. The recipients in group B were treated with simvastatin for 3 months. We estimated serum lipid levels and mRNA expressions of RANTES and CCR5. The mRNA expressions of RANTES and CCR5 were significantly higher in renal transplant recipients compared with controls. The expressions were much higher in group B than in group A patients. In group B patients, serum lipid levels decreased dramatically after simvastatin treatment. Meanwhile, the mRNA expressions of RANTES and CCR5 were reduced significantly after 1.5 months of simvastatin treatment to a level significantly lower than that in group A after 3 months of treatment. The increased expressions of RANTES and CCR5 mRNAs in renal transplant recipients with hyperlipidemia might be involved in CAN due to hyperlipidemia. Simvastatin seemed to reduce the chemokine transcripts in renal recipients with hyperlipidemia.
Einleitung: Chemokine sind niedermolekulare Zytokine, die für die Geweberekrutierung inflammatorischer Leukozyten verantwortlich sind. Eine Leukozyteninfiltration ist charakteristisch für die akute Transplantatabstoßung nach Lebertransplantation. Wir vermuten, dass die hepatische Chemokinsekretion als Vermittler von Abstoßungsreaktionen nach Lebertransplantation eine Rolle spielen könnte. Daten zum Chemokinverlauf nach Lebertransplantation im peripheren Blut liegen bislang nicht vor. Material und Methoden: In dieser Untersuchung wurden die Serumchemokinkonzentrationen für die CC-Chemokine CCL2 (Monocyte chemoattractant protein 1, MCP-1), CCL3 (Macrophage inflammatory protein I alpha, MIP-1γ), CCL4 (Macrophage inflammatory protein 1 beta, MIP-lß), und CCL5 (Regulated on activation of normal T cell expressed and secreted, RANTES) mittels spezifischen ELISAs im Verlauf nach humaner Lebertransplantation bei n = 100 Patienten bestimmt und mit dem klinischen Verlauf bezüglich Infektions- und Rejektionskrisen korreliert. Ergebnisse: Während der ersten 48 h nach Ischämie-Reperfusion trat nach einem initialen Anstieg (Ischämie-Reperfusionsschaden) ein Abfall von CCL2, CCL3, CCL4 und CCL5 auf. Bei n = 15 Patienten traten n = 20 CMV-Infektionen (IF) und bei n = 30 Patienten traten n = 43 akute Rejektionen (RE) auf. Während Infektion und Rejektion trat eine signifikante Überexpression von CCL5 (für IF 549,3 pg/ml auf 622,85 pg/ml, p = 0,355, für RE 413,95 pg/ml to 587,55 pg/ml, p = 0,0001) auf. CCL2, CCL3 und CCL4 zeigten dagegen eine Überexpression bei Rejektion (CCL2 125,48 pg/ml auf 188,44 pg/ml, p = 0,0001, CCL3 61,35 pg/ml auf 129,64 pg/ml, p = 0,0001, CCL4 38,44 pg/ml auf 78,23 pg/ml, p = 0,003) und einen Abfall bei Infektion (CCL2 259,9 pg/ml auf 206,75 pg/ml, p = 0,485, CCL3 146,75 pg/ml auf 103,4 pg/ml, p = 0,0001, CCL4 137,4 pg/ml auf 89,12 pg/ml, p = 0,092). Der Unterschied zwischen Rejektion und Inflammation in der Expression von CCL3 und CCL4 war hochsignifikant (Wilcoxon-Test, p = 0,0001 für CCL3, p = 0,02 für CCL4). Zusammenfassungg: Unsere Untersuchungen zeigen eine gleichsinnige Regulation von CCL5 im Rahmen von Entzündungen sowie Rejektionskrisen nach Lebertransplantation. Dagegen liegt eine differenzielle Expression von CCL2, CCL3 und CCL4 vor. Es tritt eine signifikante Überexpression von CCL2 und CCL3 bei akuten Abstoßungsreaktionen auf; dagegen führt eine Infektion zu einem Abfall. Möglicherweise bietet sich hier ein neuer Weg der nichtinvasiven Diagnostik von Abstoßungen nach Lebertransplantation.
