Paciente masculino de 75 anos. Es internado por insuficiencia cardiaca congestiva. Presenta antecedentes de reemplazo valvular aortico con protesis biologica realizado hace 6 anos y sindrome ascitico edematoso de 3 anos de evolucion, con enfermedad hepatica descartada. Un ecocardiograma realizado hace un ano mostraba derrame pericardico moderado sin signos de taponamiento. En la presente internacion se realizo un nuevo ecocardiograma, que revelo imagen pericardica ecodensa, en cara diafragmatica, de aproximadamente 3 centimetros de diametro. Mediante cateterismo cardiaco (Figura 1) se diagnostico constriccion pericardica. Se efectuo una resonancia magnetica nuclear para precisar la situacion anatomica y programar el abordaje quirurgico (Figura 2). El paciente rehuso ser intervenido quirurgicamente y es tratado con diureticos por via oral.
INTRODUCTION:Percutaneous treatment is currently the method of choice for ostium secundum atrial septal defects (OS-ASD) treatment. However, this procedure may be associated with an increased incidence of atrioventricular block. The incidence of these events could be related to the design of the device used. MATERIALS AND METHODS:This study featured a retrospective and analytical design. Consecutive patients with OS-ASD who underwent percutaneous treatment between July 2020 and June 2025 were included. The presence of a new atrioventricular conduction block (AVB) of any degree, was classified as an adverse effect. RESULTS:A total of 157 patients were included, with a median age of 8 years (5-15 years). Implantation was successful in 98.7% of patients. Three patients (1.9%) experienced AVB. Two cases were first-degree AVB, which resolved at 6 and 60 days respectively, and one was complete AVB, which resolved upon device removal. Multivariate logistic regression analysis determined that the size of the implanted device waist (OR 1.39 IC 95% 1.11- 1.97; p= 0.04) was independently associated with AVB. CONCLUSION:The prevalence of AVB after percutaneous treatment of OS-ASD with Nit-Occlud devices is rare. Devices with larger waist diameters were associated with a higher risk of AVB.

El síndrome de Rubinstein-Taybi es producido por una anomalía genética y se caracteriza por una facies típica, anomalías de manos y pies, microcefalia y retraso mental. Alrededor del 30% de los individuos afectados tienen defectos cardíacos congénitos asociados. En esta presentación se describen los casos de dos pacientes con diagnóstico de síndrome de Rubinstein-Taybi asociado con alteraciones cardíacas congénitas. Uno de ellos, de 25 años, presentaba coartación de la aorta nativa, localizada en la región yuxtaductal, y el otro, de 11 meses de edad, mostraba un conducto arterioso permeable de tamaño grande. Ambos pacientes recibieron tratamiento intervencionista percutáneo exitoso, con evolución alejada satisfactoria.
Objectives:The aim was to determine the relationship between the elevation of ultrasensitive troponin T (hs-cTnT) after percutaneous atrial septal defect (ASD) closure with deficient aortic rim and with standard and specific maneuvers of the intervention. Method:Baseline hs-cTnT was measured and repeated 6 hours after the procedure. To determine the influence of independent variables with the dependent variable (change in hs-cTnT), a generalized linear mixed model was used. Results:The total cohort consisted in 106 patients. The median age was 8 years, and 22 patients (21%) were older than 18 years. The hs-TnT before the procedure was 3.7 pg/ml and 6 hours after the intervention was finalized was 72.5 pg/ml. The hs-TnT at 6 hours was similar in patients with sufficient vs. deficient aortic rim. A generalized linear mixed model demonstrated a direct relationship between hs-cTnT change and ASD diameter (β: 2.8; CI: 0.8 to 4.9; p < 0.01) and fluoroscopy time (β: 2.7; CI: 0.6 to 4.7; p < 0.01) and an inverse relationship between hs-cTnT change and patient weight (β: -0.7; CI: -1.1 to -0.3; p < 0.01). Conclusions:The increase in hs-cTnT after percutaneous ASD treatment was directly related to ASD diameter and the fluoroscopy time and inversely to the patient weight. Aortic rim deficit was not associated with elevation of hs-cTnT.
Objetivo: El objetivo fue determinar la relación entre la elevación de troponina T cardiaca de alta sensibilidad (hs-cTnT) posterior al cierre percutáneo de una comunicación interauricular (CIA) con el déficit de borde aórtico y con maniobras propias y específicas de la intervención. Método: Se midió la hs-cTnT basal y se repitió a las 6 horas después del procedimiento. Para determinar la influencia de variables independientes con la variable dependiente (cambio en la hs-cTnT) se utilizó un modelo lineal generalizado mixto. Resultados: Se incluyeron 106 pacientes. La mediana de edad fue 8 años y 22 pacientes (21%) fueron mayores de 18 años. La hs-cTnT previa al procedimiento fue 3.7 pg/ml y 6 horas posterior a la finalización del procedimiento fue 72.5 pg/ml. El valor de la hs-cTnT a las 6 horas posteriores fue similar en pacientes con borde aórtico suficiente vs. deficiente. Un modelo lineal mixto generalizado demostró una relación directa entre el cambio hs-cTnT y el diámetro de la CIA (β: 2.8; IC: 0.8 a 4.9; p < 0.01) y el tiempo de fluoroscopia (β: 2.7; IC: 0.6 a 4.7; p < 0.01) y una relación inversa entre el cambio de hs-cTnT y el peso del paciente (β: –0.7; IC: –1.1 a –0.3; p < 0.01). Conclusiones: El incremento de hs-cTnT posterior al tratamiento percutáneo de la CIA se relacionó de forma directa con el diámetro del defecto y el tiempo de fluoroscopia y de forma inversa con el peso del paciente. El déficit de borde aórtico no se asoció con elevación de hs-cTnT.
Background: Stent implant is considered the treatment of choice for most cases of congenital or postoperative pulmonary artery branch stenosis in patients with congenital heart defects. However, there are still doubts on the clinical effectiveness and the incidence of complications of this technique that need to be elucidated.Methods: This multicenter study has a descriptive, observational and retrospective design. The procedures were performed between January 2005 and April 2015. Twenty-five patients with pulmonary artery branch stenosis underwent stent implant and were followed-up both clinically and by different imaging tests. Persistent clinical and/or anatomic improvement defined clinical effectiveness.Results: Average age was 9.48 years (2 months-34 years), mean weight was 27.54 kg (3-104 kg) and 44% were women. The most frequent congenital heart defects in the treated cohort were tetralogy of Fallot and its variants, followed by single ventricle heart physiology after bidirectional Glenn shunt and/or Fontan-Kreutzer procedures. Right ventricular systolic and pulmonary artery pressure decreased significantly after the intervention (from 68.35 mmHg to 45.8 mmHg and from 47.4 mm Hg to 32 mm Hg, respectively) (p < 0.0001). The initial minimum diameter of the treated pulmonary artery branch increased significantly after the procedure (from 3.98 mm to 9.82 mm, p < 0.0001). The incidence of complications was 8% (n = 2) and the clinical effectiveness of the procedure was good in 22 patients (88%).Conclusions: Stent implant in pulmonary artery branch stenosis is a safe and feasible though challenging technique, with significant clinical and anatomic improvement. The procedure represents an alternative to surgery and decreases the need for reinterventions in patients with congenital heart defects.
espanolIntroduccion: El manejo inicial de pacientes con tetralogia de Fallot (TOF) asociado a una anatomia desfavorable y flujo vascular pulmonar significativamente reducido es controversial y continua siendo un desafio clinico.Material y metodos: Estudio multicentrico, retrospectivo, observacional. Se incluyeron pacientes consecutivos (neonatos y lactantes menores) con diagnostico de TOF asistidos en cuatro centros diferentes de la ciudad de Cordoba, Argentina, que recibieron stent entracto de salida de ventriculo derecho (TSVD) como abordaje inicial de su cardiopatia congenita. La indicacion de intervencion paliativainicial fue consensuada en reunion del equipo cardiovascular en cada centro participante. Una anatomia compleja (ramas pulmonarescon Z score≤ -2,5), presentacion en shock cardiogenico o en crisis de cianosis, bajo peso o comorbilidades significativas (enterocolitis necrotizante, prematurez, insuficiencia renal, convulsiones) inclinaron la decision hacia un abordaje percutaneo inicial.Resultados: Entre agosto de 2017 hasta mayo de 2018, 6 pacientes con TOF sintomaticos recibieron stent en TSVD. Background The prevalence of patent foramen ovale (PFO) in the general population is about 25%. Most patients with a PFO as an isolated finding are asymptomatic and receive no special treatment; yet it is likely associated with cryptogenic stroke. Objective To assess the prevalence of PFO in patients with cryptogenic stroke or transient ischemic attacks (TIAs). Material and Methods We conducted a retrospective analysis of the transesophageal echocardiograms of patients with cryptogenic stroke or TIAs who were referred to our center to evaluate for an embolic source. The diagnosis of PFO was defined as the presence of a single microbubble in the first three beats after right atrium opacification. Atrial septum aneurysm (ASA) was diagnosed when the interatrial septum was abnormally re dundant with an excursion of more than 10 mm into the right or left atrium. Results A total of 43 patients were evaluated; mean age was 59.6 ± 16.9 years, and 41.9% were women. Patent foramen ovale was detected in 13 patients (30.2%; 95% CI, 15% to 44%), 7 of whom (16.3% of the total population) had large defects. ASA was present in 6 patients (14%) associated with PFO; 4 patients (66%) had large PFOs. Conclusions The prevalence of PFO in patients with cryptogenic stroke or TIAs is 30%, and in 33% of cases it is associated with atrial septum aneurysm.
Introducción: El foramen oval permeable se encuentra en alrededor del 50% de los pacientes que han sufrido un accidente cerebrovascular criptogénico. La recurrencia de un accidente cerebrovascular isquémico luego del cierre percutáneo del foramen oval permeable es de alrededor del 1% anual. Objetivo: Evaluar la prevalencia de recurrencia de accidente cerebrovascular isquémico en nuestra población tratada con cierre percutáneo del foramen oval permeable. Material y métodos: Se incluyeron en forma retrospectiva desde enero de 2007 hasta septiembre de 2015 todos los pacientes con diagnóstico de accidente cerebrovascular isquémico criptogénico a quienes mediante técnica percutánea se les ocluyó un foramen oval permeable. En el seguimiento se detectaron los casos en los que se diagnosticó recurrencia de evento en la forma de un nuevo accidente cerebrovascular isquémico y/o ataque isquémico transitorio posterior al cierre percutáneo. Resultados: Se incluyeron 28 pacientes, con edad promedio al momento del procedimiento de 47 años (20-71 años), el 50% eran mujeres, el 79% habían tenido un accidente cerebrovascular y el 21% un ataque isquémico transitorio. El puntaje de RoPE fue de 7,07 puntos (3-10 puntos). El cierre percutáneo fue exitoso en todos los casos. En un período de seguimiento (mediana 989 días, intervalo intercuartil 670-1.766 días) se identificaron dos pacientes (7%) que tuvieron un nuevo accidente cerebrovascular. En ambos pacientes se repitió un ecocardiograma transesofágico y se encontró foramen oval permeable cerrado sin fugas residuales. Conclusiones: La frecuencia de recurrencia de accidente cerebrovascular es baja luego del cierre percutáneo del foramen oval permeable y posiblemente un buen número de los casos recurrentes no sean prevenibles con su cierre.

(1) Existen evidencias de la asociacion entre la presencia de FOP y ACVC tanto en jovenes como en mayores de 55 anos. (2) Al evaluar a estos pacientes, la determinacion del tamano o el grado de gravedad del FOP, asi como la presencia de lesiones asociadas (p. ej., aneurisma del septum interauricular), tiene relevancia clinica. Si bien el ecocardiograma transesofagico (ETE) es el metodo de referencia para el diagnostico, el Doppler transcraneal simultaneo puede ser de utilidad para la cuantificacion del FOP. (3) Se presentan imagenes de un paciente masculino de 70 anos, hipertenso con historia de crisis isquemica transitoria (CIT) 5 anos atras y estenosis carotidea derecha del 40%, tratado con aspirina. Dos anos despues tuvo un nuevo episodio de CIT, del que se recupero integramente. En la actualidad se interna por paresia de hemicuerpo izquierdo; un nuevo eco-Doppler de vasos del cuello mostro ateromatosis carotidea sin progresion de la enfermedad. Se realizo un ETE combinado con ecoDoppler transcraneal con contraste, donde se observo un FOP grande (pasaje de derecha a izquierda con maniobra de Valsalva mayor de 20 burbujas) (Figura 1) con aneurisma del septum interauricular y efecto shower en el Doppler transcraneal de la arteria cerebral media (Figura 2). A pesar de la ausencia de evidencia clara, (1) debido a la presencia de cortocircuito de derecha a izquierda y a la recurrencia de eventos isquemicos, se aconsejo el cierre percutaneo del FOP. Troponin levels are used to rule in or rule out myocardial injury, with the T and I subunits being the biomarkers of choice. The use of high-sensitivity kits is recommended in clinical practice.1 There are no reference values after percutaneous occlusion of an ostium secundum–type atrial septal defect (ASD).The objective of this study was to describe the values of high-sensitivity cardiac troponin T (hs-cTnT) in a population that received percutaneous treatment for ASD without complications.This observational, prospective, and comparative study of repeated measurements was conducted between August 2020 and November 2022. Patients with ostium secundum–type ASD, dilated right heart chambers, and percutaneous closure of the defect with devices were consecutively included.High-sensitivity cardiac troponin T levels were measured before the intervention and at 6 hours after the end of the procedure. Measurements were performed on a Cobas autoanalyzer using an electrochemiluminescence assay (Roche). Ultrasensitive troponin T was used. The upper reference limit (URL) was 14 pg/mL. The protocol was approved by the Institutional Research Ethics Committee and Research Committee of the Hospital Privado Universitario de Córdoba. Patients and/or their guardians provided written informed consent.Categorical variables are expressed in percentages and 95% CIs and continuous variables in medians and IQRs or means and SDs, depending on their distribution. A paired t test was used to compare hs-cTnT levels before and after the intervention.Sixty-eight patients were included, 42 of whom were female (61.8%; 95% CI, 50.2%–73.3%), with a median age of 7 years (IQR, 5–14 years). Patient baseline characteristics are shown in Table I.During the procedure, the mean (SD) pulmonary artery pressure was 21.2 (5.2) mm Hg, and the mean (SD) ASD diameter measured on transesophageal echocardiography was 14.8 (5.2) mm. One device was implanted in 66 patients (97.1%; 95% CI, 93.0%–100%) and 2 simultaneous devices in 2 patients (2.9%; 95% CI, 0%–7.0%). The mean (SD) diameter of the devices was 19.2 (6.2) mm. Most devices were the Nit Occlud ASD-R Occluder (PFM Medical); the MemoPart ASD Occluder (Lepu Medical) and Cocoon Septal Occluder (Vascular Innovations) were also occasionally used.At baseline, the median hs-cTnT level was 3.85 pg/mL (IQR, 3.0–7.10 pg/mL), and at postimplantation (median, 362 minutes [IQR, 360–371 minutes]), it was 75.1 pg/mL (IQR, 49.4–119.3 pg/mL), which corresponds to 5.3 times (IQR, 3.5–8.5 times) the URL (P < .001). The 97.5 percentile for postimplant hs-cTnT was 219.3 pg/mL (15.6 times the URL).All patients were discharged 24 hours after the procedure. No significant arrhythmias, conduction disturbances, or ischemic changes were recorded on 12-lead electrocardiogram. After 7 to 10 days, the electrocardiogram and transthoracic echocardiogram were repeated, which showed no alterations (0%; 95% CI, 0%–7.3%).Other authors have explored the prevalence of myocardial injury after ASD closure. The estimated frequency varies from 16% to 100% of cases.2–5 One meta-analysis that studied troponin T or I levels in 347 patients, using blood samples obtained between 6 and 24 hours after percutaneous ASD closure, reported a prevalence of injury of 41.8% (95% CI, 36.6%–47.2%).6Different postoperative clinical scenarios have reported discrepancies between the value defined by the fourth definition as myocardial injury and its clinical relevance.The VISION study,7 which included 13,862 patients during the postoperative period of cardiac surgery (revascularization or aortic valve replacement), found that the ultrasensitive troponin I value measured at 24 hours, related to 30-day postoperative mortality, was 218 times higher than the URL and 499 times higher than the reference limit in other cardiac surgeries. The 97.5 percentile of hs-cTnT at 6 hours after the uncomplicated procedure in the present study reached 15 times the upper reference value.The main limitation of this research protocol is the small sample size of patients. The study was conducted in a single center, and there could be selection or reference biases. However, its prospective nature allowed for reliable and detailed data with internal validity following strict inclusion and exclusion criteria, which allowed for an understanding of causality owing to the prospective design of repeated samples. This study differs from those previously published in that the Nit Occlud ASD-R was the most common device and that, most importantly, ultrasensitive troponins were used.In conclusion, in a prospective cohort of patients with percutaneous occlusion of uncomplicated ostium secundum ASD, a consistent elevation of hs-cTnT levels was observed in all cases, increasing to approximately 15 times the URL at 6 hours after the intervention.
Background: The aim of this meta-analysis was to compare the efficacy and adverse events of percuta-neous occlusion among patients with sufficient and deficient rims.Methods: A systematic review of all articles published in the Pubmed, MEDLINE and Google Scholar databases was performed. Odds ratio (OR) and 95% CI were used as a measure of effect of the combi-nation of studies. I2 with 95% CI was estimated to assess study heterogeneity. For the meta-analysis, a random effects model was used.Results: The systematic search identified ten studies which included 4355 patients; 2661 of those had sufficient rim and the remaining 1694 patients showed some rim deficiency. Implant failure rate was 4.13% CI 95% 3.53-4.72%. Compared to frequency of failures in the group with a deficient rim (5.43% CI 95% 4.35-6.50%), implant failure in patients with a sufficient rim was significantly lower (3.30% CI 95% 2.62-3.97%), OR 2.27 CI 1.34-3.83 (p 0.002). The combined adverse events were 5.19% CI 95% 4.22-6.35% vs 2.7% CI 95% 2.08-3.31% in the deficient vs sufficient rim groups respectively (OR 2.21 CI 0.93-5.29; p 0.07). Implant failures and adverse events were more frequent in patients with posterior inferior rim deficiency.Conclusion: Patients presenting a posteroinferior rim deficiency are associated to both, an increased incidence of closure failure and a combined adverse events occurrence. More studies on posterior rim deficiency are necessary to ensure the feasibility and safety of the percutaneous approach.(c) 2023 Cardiological Society of India. Introducción: El reemplazo quirúrgico con colocación de bioprótesis es una estrategia utilizada frecuentemente para tratar valvulopatías en cardiopatías congénitas. Objetivo: Presentar la experiencia inicial en Argentina con fractura/remodelación del anillo valvular en bioprótesis y posterior reemplazo percutáneo “válvula en válvula” en pacientes con cardiopatías congénitas. Material y métodos: Estudio descriptivo y observacional. Se incluyeron todos los pacientes tratados con reemplazo percutáneo debido a disfunción de prótesis valvular biológica. Resultados: Desde agosto de 2021 hasta mayo de 2023, 5 pacientes (3 de sexo femenino) con disfunción de bioprótesis derechas recibieron tratamiento percutáneo como alternativa a la realización de un nuevo reemplazo quirúrgico. La edad media fue 21,2 ± 9,2 años, el peso medio fue 56,2 ± 22,2 kg. Fueron implantadas cinco válvulas balón expandibles: 3 en posición pulmonar y 2 en posición tricúspide y en 4 casos, con fractura previa del anillo valvular. En todos los pacientes se restauró la función valvular, sin complicaciones. En el seguimiento, en un sólo paciente se objetivó insuficiencia valvular moderada derecha Conclusión: El reemplazo percutáneo “válvula en válvula” en bioprótesis disfuncionantes derechas es una estrategia atractiva y segura en casos seleccionados, que restaura la competencia y elimina las obstrucciones. Esta estrategia es una opción razonable como alternativa a la realización de un nuevo recambio valvular quirúrgico.
