L’écoute pose un certain nombre de problèmes aux praticiens sur lesquels nous proposons de revenir à partir des effets d’abord embarrassants de la clinique. Que faisons-nous de ce que nous entendons ? Avec quelle oreille écoute-t-on ? Repartant des travaux de Theodor Reik, auteur du célèbre Écouter avec la troisième oreille , Lacan démontrera en quoi l’analyste aura déjà trop de deux oreilles. Occasion donc de réinterroger en quoi écouter ne va pas de soi. Qu’est-ce qui rend l’écoute si difficile ? Parcourir le trajet qui mena Lacan de la proposition de l’analyste-auditeur à l’analyste-interprète conduit à cette hypothèse déroutante : lire la parole , lire avec l’oreille . Apprendre à lire, voilà une surprenante proposition pour les bancs de nos Collèges… de clinique psychanalytique.
Cet article propose de réinterroger les valeurs du silence, à partir de l’articulation des silences aux manifestations de transfert. Côté analyste, comment ne pas faire du silence un silence dogmatique, voire un silence de refuge , selon l’expression de Lacan ? Côté analysant, comment entendre la pluralité des silences à partir du repérage de l’objet en jeu et de sa fonction ? En suivant le commentaire qu’il a fait du tableau de Munch Le Cri , je m’arrêterai plus précisément sur ce que Lacan nomme le « phénomène limite du champ du grand Autre », à savoir son absence de réponse – et ses conséquences. Qu’est-ce qu’une réponse ? Du reste, comme trait différentiel de la présence de l’analyste, le silence porte la parole de l’analysant, tout autant qu’il semble faire signe d’un désir d’entendre. L’ indécence du silence serait-elle alors de faire causer ?
La réalisation d'une biopsie d'artère temporale (BAT) reste l'examen de référence pour le diagnostic d'artérite à cellules géantes (ACG) mais est un geste invasif qui manque de sensibilité. Les techniques d'imagerie vasculaire, qui permettent de montrer des signes indirects de vascularite de l'artère temporale, de l'aorte ou d'autres gros vaisseaux, ont donc une place grandissante pour le diagnostic d'ACG. Les appareils de TEP-TDM numérique nouvelle génération permettent désormais l'étude des artères céphaliques avec une très bonne résolution, en plus de l'étude de l'aorte et de ses branches (1). L'objectif de cette étude était d'évaluer les performances diagnostiques de la TEP-FDG céphalique au cours de l'ACG. Entre novembre 2018 et juillet 2020, une TEP-FDG céphalique a été réalisée chez tous les patients suspects d'ACG du service de Médecine interne et immunologie clinique. En cas de traitement par corticoïdes débuté depuis plus de 72 h, de BAT réalisée avant la TEP-FDG, de glycémie capillaire supérieure à 7 mmol/L le jour de la TEP, d'âge inférieur à 50 ans ou de TEP réalisée dans le cadre du suivi, les patients ont été exclus de l'analyse. Une étude de 20 segments vasculaires, dont 8 céphaliques, a été réalisée en déterminant pour chaque segment le grade (0 à 3) de l'hypermétabolisme par rapport au bruit de fond hépatique (2). La TEP céphalique était considérée comme positive si au moins 2 segments céphaliques présentaient un hypermétabolisme de grade ≥ 2. La TEP des vaisseaux extra-céphaliques était considérée comme positive en cas d'hypermétabolisme de grade ≥ 2 d'au moins un segment artériel. Un diagnostic final d'ACG était retenu en cas de signe(s) clinique(s) compatible(s) et de prescription d'une corticothérapie pendant plus de 6 mois consécutifs. Une TEP céphalique a été réalisée chez 111 patients dont 51 remplissaient les critères d'inclusion. Parmi eux, 15/51 (29,4 %) présentaient un diagnostic final d'ACG. Parmi ces 15 patients, 7 avaient une ACG céphalique pure, 2 une ACG extra-céphalique pure et 6 une ACG céphalique et extra-céphalique. Tous les patients chez qui le diagnostic d'ACG a été retenu remplissaient au moins 3 des 5 critères de l'ACR.[3] La BAT a été réalisée chez 32/51 (62,7 %) patients et était positive dans 10/32 cas (31,3 %). Dans notre étude, la BAT avait une sensibilité de 71,4 % et une spécificité de 100 % pour le diagnostic d'ACG. La VPP et la VPN de la BAT étaient respectivement de 100 % et 81,8 %. Le coefficient kappa, évaluant la concordance entre les résultats de la TEP céphalique et ceux de la BAT, était de 0,56 [0,25–0,87]. La TEP des vaisseaux extra-céphalique avait une sensibilité de 53,3 % et une spécificité de 97,2 % avec une VPP et une VPN de respectivement 88,9 % et 83,3 %. La TEP céphalique avait une sensibilité de 73,3 % et une spécificité de 97,2 % avec une VPP et une VPN de respectivement 91,7 % et 89,7 %. L'association des résultats de la TEP céphalique et de la TEP extra-céphalique permettait d'obtenir une sensibilité de 80 % et une spécificité de 94,4 %. La VVP et la VPN de l'association TEP céphalique + TEP extra-céphalique étaient respectivement de 85,7 % et 91,9 %. La courbe ROC analysant la valeur diagnostique du ratio « SUV maximale des vaisseaux céphaliques/SUV hépatique » avec pour référence le diagnostic clinique d'ACG avait une aire sous la courbe de 0,95 avec une sensibilité de 100 % et une spécificité de 80,6 % pour un seuil à 0,7, de 86,7 % et 88,9 % pour un seuil à 0,77 et de 60 % et 97,2 % pour un seuil à 0,97. Même si le petit nombre de patients atteints d'ACG et la sur-représentation des formes céphaliques sont les deux principales limites de cette étude, nos résultats confirment l'intérêt diagnostique de la TEP céphalique au cours de l'ACG [1] et suggèrent qu'un ratio « SUV maximale de vaisseaux céphaliques/SUV hépatique » inférieur à 0,7 rendrait très improbable le diagnostic d'ACG. La TEP-TDM des artères céphaliques, surtout lorsqu'elle est couplée à la TEP-TDM corps entier, présente d'excellentes performances diagnostiques pour l'ACG. Des travaux sont nécessaires pour mieux déterminer le niveau d'hypermétabolisme de l'artère temporale cliniquement significatif.
The accumulation of fine sediments (<200 mu m) occurs in hydroelectric dams due to watershed erosion and solid transport. A rigorous and sustainable sediment management is required. Although most of these materials are transferred into the watercourse downstream, a significant quantity might have to be managed as on-land waste in the future. Considering that sediments contain silica, alumina, calcia and iron oxide as main chemical constituents, they could be useful candidate raw materials in the cement industry for the manufacture of clinker. This work concerns sediments from French dams. Whatever their origin, each sediment is suitable for a beneficial reuse as silico-aluminous raw material for clinker production. The characteristics of the raw sediments can lead to specific microstructures in the clinkers. However, clinker characteristics can still be controlled by adjusting raw mix proportions, maintaining a rather high sediment content in the mix (between 10 and 15 wt%). (C) 2019 Elsevier Ltd. Eco-materials are materials which can minimize environmental impacts, can be manufactured from recycled materials, or are beneficial for human wellbeing. This paper will illustrate these three aspects on mineral-based eco-materials through research that has been developed in our ceramic institute: preparation of dense tapes of cementitious materials at room temperature by tape casting, valorization of dam sediments as raw materials for synthesis of Portland clinker, monitoring the microstructure of plaster boards for improving the acoustical insulation in houses.

This chapter explores an engineering tool, topology optimizationTopology optimization, that is at the frontier of designing architectured materials and structures. Topology optimization offers a mathematical framework to determine the most efficient material layout for prescribed constraints and loading conditions. In engineering, topology optimization is identifying designs with interfacesInterface, materials, manufacturing methods, and functionalities unavailable to the natural world. The particular focus is on the variety of roles that interfaces may play in advancing architectured materials and structures with topology optimizationTopology optimization.
Understanding the formation of the solar system is a prerequisite to a comprehensive theory of our origins, while providing essential clues for the birth of planetary systems in general. In particular, the total amount of volatile carbon, oxygen, and nitrogen, and their spatial distributions, in the early protosolar nebula (PSN), and in PSN analogs, is a fascinating issue in our understanding of the primitive chemical composition of exoplanets. A key question from both astrophysical and planetary science perspectives is thus to know to what extent the reservoirs of volatiles (namely, gas and ice) in planetary systems are of interstellar nature or if chemistry was reset in the PSN at the epoch of planet formation. Isotopic ratios are powerful tools to evince the chemical heritage preserved during star and planet formation. Nevertheless, the isotopic ratios of nitrogen in the solar system is an unsolved problem, which is the central question we will adress in this talk.
Optimizing the shape and topology (S&T) of structures to improve their acoustic performance is quite challenging. The exact position of the structural boundary is usually of critical importance, which dictates the use of geometric methods for topology optimization instead of standard density approaches. The goal of the present work is to investigate different possibilities for handling topology optimization problems in acoustics and elasto-acoustics via a level-set method. From a theoretical point of view, we detail two equivalent ways to perform the derivation of surface-dependent terms and propose a smoothing technique for treating problems of boundary conditions optimization. In the numerical part, we examine the importance of the surface-dependent term in the shape derivative, neglected in previous studies found in the literature, on the optimal designs. Moreover, we test different mesh adaptation choices, as well as technical details related to the implicit surface definition in the level-set approach. We present results in two and three-space dimensions.
We study the gravitational collapse of pre-stellar sources and the associated evolution of their chemical composition. We use the University of Grenoble Alpes Astrochemical Network (UGAN), which includes reactions involving the different nuclear-spin states of H2, H + 3 , and of the hydrides of carbon, nitrogen, oxygen, and sulphur, for reactions involving up to seven protons. In addition, species-to-species rate coefficients are provided for the ortho/para interconversion of the H3 + H2 system and isotopic variants. The composition of the medium is followed from an initial steady state through the early phase of isothermal gravitational collapse. Both the freeze-out of the molecules on to grains and the coagulation of the grains were incorporated in the model. The predicted abundances and column densities of the spin isomers of ammonia and its deuterated forms are compared with those measured recently towards the pre-stellar cores H-MM1, L16293E, and Barnard B1. We find that gas-phase processes alone account satisfactorily for the observations, without recourse to grain-surface reactions. In particular, our model reproduces both the isotopologue abundance ratios and the ortho:para ratios of NH2D and NHD2 within observational uncertainties. More accurate observations are necessary to distinguish between full scrambling processes – as assumed in our gas-phase network – and direct nucleusor atom-exchange reactions.
This article addresses one of the major constraints imposed by additive manufacturing processes on shape optimization problems – that of overhangs, i.e. large regions hanging over void without sufficient support from the lower structure. After revisiting the ‘classical’ geometric criteria used in the literature, based on the angle between the structural boundary and the build direction, we propose a new mechanical constraint functional, which mimics the layer by layer construction process featured by additive manufacturing technologies, and thereby appeals to the physical origin of the difficulties caused by overhangs. This constraint, as well as some variants, is precisely defined; their shape derivatives are computed in the sense of Hadamard's method, and numerical strategies are extensively discussed, in two and three space dimensions, to efficiently deal with the appearance of overhang features in the course of shape optimization processes.
A level-set based shape and topology optimization framework is used to investigate the influence of graded interfaces in the optimization of micro-architectured multi-materials. In contrast to other studies found in the literature, interfaces are considered as smooth and graded transitions between constitutive phases instead of sharp delimitations. Case studies for extreme thermoelastic properties of 2D isotropic composites are analyzed and optimal designs are presented. It is shown that explicitly accounting for interfaces can influence the design of heterogeneous materials in composite microstructures.
Cette étude rétrospective évalue la faisabilité et la morbimortalité de l’assistance robotique pour réaliser un bypass gastrique avec une anse en Y montée en précolique comparée à la voie laparoscopique. Elle compare les 100 premiers by-pass avec assistance robotique aux 100 premiers by-pass par laparoscopie d’un même opérateur. Il n’y a pas de différence de durée opératoire ou de morbimortalité entre les deux groupes. L’assistance robotique pour réaliser un bypass gastrique est faisable et fiable, même lors de la période d’apprentissage.
Deux analyses de physique des particules a hautes energies sont presentees dans ce manuscrit de these, utilisant des evenements avec deux leptons de charges opposees et de l'energie transverse manquante. Ces evenements sont selectionnes parmi 9.7 fb-1 de donnees totales de collisions pp enregistrees par le detecteur DO aupres du collisionneur TeVatron du Fermilab a √s=1.96 TeV.La premiere analyse est la recherche du boson de Higgs se desintegrant en H→WW→μνμν. Aucun exces significatif au-dessus de l'attente pour le bruit de fond n'est observe.Des limites superieures sur la section efficace de production du boson de Higgs sont donc calculees dans le cadre du modele standard mais aussi dans l'hypothese de l'existence d'une quatrieme generation de fermions et dans le contexte de couplages fermiophobiques du boson de Higgs.Pour valider la methodologie de cette recherche, la section efficace de production de la paire de bosons W est mesuree.La seconde analyse est la mesure de l'asymetrie avant-arriere de production de la paire tt. Il s'agit de la premiere mesure dans le canal dileptonique de l'experience DO. Dans ce contexte, une methode inedite de reconstruction de la cinematique de la paire est utilisee (methode des elements de matrice) pour donner une mesure brute de l'asymetrie avant-arriere. A l'aide d'une methode de calibration dediee, nous donnons une mesure finale de AFB=18.0 ± 6.0 (stat) ± 3.3 (syst).

We report extensive, accurate fully quantum, time-independent calculations of cross sections at low collision energies, and rate coefficients at low temperatures for the H⁺ + H₂(v = 0, j) → H⁺ + H₂(v = 0, j') reaction. Different transitions are considered, especially the ortho-para conversion (j = 1 → j' = 0) which is of key importance in astrophysics. This conversion process appears to be very efficient and dominant at low temperature, with a rate coefficient of 4.15 × 10⁻¹⁰ cm³ molecule⁻¹ s⁻¹ at 10 K. The quantum mechanical results are also compared with statistical quantum predictions and the reaction is found to be statistical in the low temperature regime (T < 100 K).
The paper presents a new robotic system for beating heart surgery. The final goal of this project is to develop a tele‐operated system for the thoracoscopic treatment of patients with atrial fibrillation. The system consists of a robot that moves an innovative end‐effector used to perform lines as in the Cox‐Maze technique.
This article questions the emphasis that many historians have placed on community and solidarity in nineteenth-century working-class neighborhoods, and the associated idea that these values were under-mined by improved housing, social welfare systems, urban transit, and growing links with the outside world. Taking late nineteenth-century Paris as a case study, it shows that working-class neighborhoods always contained strong internal tensions and that forced proximity did not necessarily create solidarity. Far from being communitarian in outlook, people had a strong sense of private life and of boundaries. Poverty and unstable employment made residential and daily mobility so high, well before mass transit arrived, that attachment to a single neighborhood was impossible. Neighborhood solidarities did exist, but they were only one small aspect of urban life.
Chemicals applied to wheat at the flowering stage are used to reduce yield losses associated with Head Blight (HB) and trichothecene contamination of infected grains. Efficacy of fungicide treatments at anthesis were assessed against Fusarium spp. ( Fusarium graminearum, F. avenaceum, F. poae, F. culmorum and F. tricinctum were the most frequent Fusarium species) and Microdochium nivale and on Fusarium mycotoxin concentration over three years on naturally infected fields of soft wheat, durum wheat and barley. Deoxynivalenol (DON) and nivalenol (NIV) were the most frequently detected mycotoxins. Treatments including azoxystrobin significantly decreased M. nivale infection levels, while infection levels of F. graminearum and F. culmorum were unaffected. Treatments that included metconazole or tebuconazole significantly decreased F. graminearum or F. culmorum infection levels, but the effect was variable across the years and type of fungicide applied. In 2002, DON concentration in wheat treated with tebuconazole and metconazole was reduced by 46% and 48%, respectively, while NIV concentration was unaffected. Yet, in 2000 and 2001, significant reduction in F. graminearum or F. culmorum infection levels were not always associated with significant reductions in DON and NIV mycotoxins. The fungicides we applied in naturally infected fields did not always reduce mycotoxin producers or alternatively the trichothecenes produced. Different explanations for this phenomenon are discussed.
