Background: β-Lactams are the most widely used antibiotic family in the world. Nevertheless, they also stand out as the primary culprits for inducing drug hypersensitivity reactions (HSR). Methods: Between May 2018 and March 2023, patients with suspected HSRs to β-lactams, who underwent skin tests (ST), were retrospectively screened. The determinants of allergenic penicillin (DAP) tests, which include penicillin minor and major determinants, clavulanic acid, and amoxicillin, along with ampicillin, sulbactam, the identified culprit drugs, and alternative cephalosporins, which include cefuroxime, ceftriaxone prick and/or intradermal tests, were administered. The analysis focused on identifying positive ST results and determining the true HSRs rates in this patient cohort. Results: Of the 147 patients, 78.9% (n = 116) were women and the median (minimum-maximum) age was 41 years (18-71 years). Mild HSRs (grades 1-2) were observed in 72.78% (n = 107), whereas 24.4% (n = 36) had severe reactions (grades 3-4) and 2.7% (n = 4) had an unknown grade. Of the patients, 64% (n = 94) experienced HSRs within the first hour after the last dose of the identified culprit drug. The overall positivity rate for all STs was 26.5% (n = 39). ST positivity rates were notably higher in individuals who had experienced HSRs within the past 6 months (p = 0.02) and those with severe anaphylaxis (p < 0.001). Conclusion: β-Lactam ST positivity is higher, especially in those with grades 3-4 reactions and consulted a physician within the first 6 months after their HSRs.
Objective: Venom immunotherapy (VIT) is recommended as the first-line therapy in patients with Hymenoptera venom allergy. The frequency of adverse reactions and side effects during VIT has been reported at variable rates. Our study aims to investigate the factors affecting of primary bee sting allergic reactions, re-sting allergic reactions in the course of VIT, and allergic reactions due to VIT injections.Material and Method: The patients, who received VIT in the Ege University Medical Faculty, Department of Internal Medicine, Division of Allergy and Glinic Immunology, allergy outpatient clinic between January 2014 and September 2019, were reviewed retrospectively. Demographic data, basal serum tryptase (BST) levels, the severity of primary sting reactions according to Mueller classification, use of antihypertensive drugs, sbin pricb test and venom-specific IgE results, presence of allergic reactions after VIT injections and re-sting reactions in the field were investigated.Results: Sixty-seven (n=67, 23 F/44 M) patients with a median age (min-max) of 46 (19-66) were included. 43% (n=29) were receving VIT with Apis mellifera venom, 48% (n=32) Vespula vulgaris, and 9% (n=6) of them were receiving VIT with both venom types. In sbin tests, positivity was detected with a single venom in 69% of patients, positivity was detected with double venom in 30%, and positivity with all 3 venom allergen extracts (Apis, Vespula, and Polistes) in 1% of patients. The frequency of systemic reaction during VIT was higher in patients who were sensitive to two bees in sbin tests (Apis + Vespula); the frequency of large local reactions with VIT was found to be high in patients who are sensitive to three bees (Apis + Vespula + Polistes) (p=0.005). The frequency of local bee-sting reactions was higher in patients using antihypertensive drugs (n=6) and the frequency of systemic reactions was higher in patients who did not use the antihypertensive drugs (p=0.001). Primary hypersensitivity reactions (HSR) of the patients were graded according to Mueller classification, and groups were compared according to the mean BST levels. While no significant difference was found between grade 2 HSR (BST: 5.21 ng/ml) and grade 3 HSR (BST: 5.10 ng/ml); the mean BST value (10 ng/ml) in patients with grade 4 reaction were significantly higher than the other groups (p=0.023). The patients were classified according to the type of VIT and the groups were compared according to the mean BST levels. The BST level of the patients sensitive to both bee types (Apis+Vespula) (14.92 ng/ml) was compared to the BST level of the Vespula-only sensitive patients (6.57 ng/ml) and the BST level of the Apis-only sensitive patients only (6.09 ng/ml) ([p=0.020 vs 0.016)]. When the patients were evaluated according to the type of bee to which they were sensitive, the primary reaction, the re-sting reaction, and the reaction during VIT were compared and no significant results were found.Conclusion: The use of antihypertensive does not increase the frequency of life-threatening reactions with bee stings both during VIT and re-sting reactions in the field. During VIT; There is an increase in all reactions, especially in the dose escalation phase. Systemic reaction development during VIT is significantly higher in those with sensitivity to both venoms in sbin tests. The BST value of patients who are susceptible to both venoms and whose primary reaction is Mueller class 4 should be investigated. Bee species and venom specific IgE levels do not affect the frequency of allergic reactions.
Objective: The use of biological agents (BAs) has increased dramatically for inflammatory diseases and this increase has led to a rise in hypersensitivity reactions (HSRs). The symptoms and diagnostic tools for HSRs are not standardized. We aimed to analyze the prevalence of HSRs to anti-rheumatic BAs and to evaluate the usefulness of skin tests (STs) as a diagnostic tool. Materials and Methods: Our study was conducted at the Ege University Medical Faculty, Department of Internal Medicine, Division of Allergy and Clinical Immunology and Division of Rheumatology, Izmir, Turkey. Four hundred sixty patients who received BAs between Jan 1st, 2015, and Jan 1st, 2016, were reviewed in this retrospective cross-sectional study. The prevalence of HSRs was retrospectively evaluated. Ten patients with HSRs were evaluated with STs containing commercially available culprit drugs. The data was collected from hospital records. The age, sex, atopic diseases, primary rheumatic diseases, and anti-rheumatic therapies of the patients were recorded. Results: Two hundred fifty patients were treated with rituximab (RTX), 45 with infliximab, 40 with tocilizumab (TOC), 36 with golimumab, 35 with etanercept, 34 with abatacept, 15 with certolizumab, and 5 with adalimumab. Fifty reactions with RTX and two reactions with TOC were infusion-related (IRRs). Ten HSRs were observed. Eight patients had immediate (7 immediate systemic reactions and 1 local injection site reactions), and 2 had late-onset cutaneous reactions. We detected the ratio of IRRs as 11.3%, immediate HSRs as 1.73%, IgE-mediated reactions as 1.08%, and anaphylaxis as 0.86%. STs were positive in 5 of 8 patients with immediate HSRs. Four of them had anaphylaxis, and remarkably, 3 of these had positive STs. None of the ten patients had high levels of specific IgE and only four had atopic diseases. Total IgE levels were not high and specific IgE levels were not positive in the presence of HSR to BAs (p=0.039). Conclusion: Five of the 8 (62.5%) patients with immediate reactions had positive STs, which suggested IgE-mediated reactions. The prevalence of HSRs to BAs was less than the ones mentioned in the literature.
This study aimed to investigate the lifetime prevalence and risk factors for allergic rhinitis and asthma in geriatric outpatients in three big cities of our country. Elderly patients visiting geriatric outpatient clinics at university hospitals were included in this study. Two questionnaires were completed by trained investigators in face-to-face interviews of 236 people aged 65100 years. The median age was 73.71±7.09 year, and 63% participants were female. The prevalence of allergic rhinitis and asthma was found 15.3 % (n=36) and 10.2 % (n=24), respectively. The frequency of allergic rhinitis and asthma was high in females, in those with low education levels, in city, in persons having family history of atopy and in smokers, but it wasnt statistically significant. Allergic diseases affect the elderly as well as young people. Female sex, low educational level, smoking, and living in a city are among the important risk factors for allergic rhinitis and asthma in the elderly. [Med-Science 2020; 9(2.000): 334-7]
Xenotransplantation is viewed as the most promising way to overcome the problems of shortage of donor organ. It can be exploited from rapidly expanding capabilities in genetic engineering for xenotransplantation. The major immunological hurdle to tissue or organ transplantations between phylogenetically-distant individuals is humoral immunity. In particular, immonupathologic effecs mediated by the reaction of natural antibodies and complement of recipient with graft endothelium in the donor organ cause xenograft rejection. Nevertheless, the phenomenon called "accomodation" that will occur immunologically may allow long survival of xenografts. In this review, immunologic hurdles to xenotransplantation and studies for overcoming the consequences of these will be discussed.
Background: It has been suggested that latex-specific IgE analysis may lead to false-positive results, especially in patients with pollen allergy. In the present study, the reasons underlying clinically irrelevant latex-specific IgE positivity were investigated. Methods: Thirty patients with latex allergy (group 1), 89 patients sensitised to aeroallergens (group 2a), and 98 healthy individuals without allergy (group 2b) were enrolled. Participants from all 3 groups were subjected to skin prick tests with aeroallergens including latex, latex-specific IgE analysis (ImmunoCAP), and nasal provocation test with latex. All cases demonstrating positive latex-specific IgE also underwent specific IgE tests (ImmunoCAP) with latex profilin, birch pollen profilin, peach lipid transfer protein, and pineapple bromelain as cross-reactive carbohydrate determinants. Results: Comparison of the atopic and healthy control groups showed that the rate of positive latex-specific IgE was significantly higher in group 2a. Latex profilin-, birch pollen profilin-, and bromelain-specific IgE were remarkably higher in group 2a. Conclusion: False positivity to latex-specific IgE in ImmunoCAP analysis may be observed in approximately 19% of patients with pollen allergy. Profilins and bromelain are the main contributors to clinically irrelevant positive latex-specific IgE.
Allergen immunotherapy (AIT) has been used to treat allergic diseases for over a hundred years. AIT is the only treatment that can change the course of allergic disease by preventing the development of asthma and new allergen sensitizations and by inducing allergen-specific immune tolerance. A number of guidelines, consensus documents, or both are available on international area. There has been a need for a guideline abut the AIT at the national level. We aimed to overcome this need by preparing a guideline, named “Allergen Immunotherapy National Guideline 2016”. The guideline is prepared according to the evidence-based criteria and level of clinical recommendations. At the preparation period, the workgroup performed a comprehensive search of the medical literature and in addition, information from articles known to the authors was considered. At the end of the guideline, supplementary material have been added like practical aspects of schedules and dosages and samples of immunotherapy reports for Social Security Institution. Furthermore, both “Raising public awareness about allergen immunotherapy”, as a need for our patients, and strategies to increase awareness of allergen immunotherapy (AIT) among the medical community were aimed.

Objective: Hereditary angioedema (HAE) is a rare genetic disorder characterized by episodes of swelling in the skin, gastrointestinal tract and larynx. Laryngeal attacks can be fatal, especially in mis-/undiagnosed cases. In Turkey, the mean diagnostic delay of HAE is as long as 26 years. This study was conducted to assess Turkish doctors' awareness of HAE. Material and Methods: A 20-question questionnaire was completed by 155 internal medicine specialists from among the attendants of 14th National Congress of Internal Medicine in Turkey. The questionnaire included HAE-related questions as well as demographic items. Results: Most doctors (93.5%) reported that they had heard of HAE, and 41.9% had followed at least one patient with HAE, however, 22% of them understood the role of C1 inhibitor in HAE, but 38.7% had no idea about HAE pathogenesis. The only fatal symptom, laryngeal edema, was named by 18% of respondents. Five percent of the respondents knew C4 level was the screening test; 6% knew that C1-INH level/function analysis is necessary for diagnosis. Approximately 10.3% of respondents knew an effective treatment for acute attacks; 18.7% knew a long-term prophylactic therapy. Conclusion: We concluded that although most internists are aware of HAE, they are not knowledgeable enough to diagnose and manage the disease.
Background: It has previously been demonstrated that subcutaneous immunotherapy with allergoids positively affects clinical and immunological parameters even after 7 preseasonal injections. However, its effect on basophil activation remains unclear. We investigated the effect of preseasonal allergoid immunotherapy on basophils and concomitantly assessed its clinical and immunological efficacy in olive pollen-monosensitized patients. Methods: This study enrolled 437 consecutive patients with respiratory allergy and positive skin prick tests (SPTs); 212 (48.5%) patients were sensitized to olive pollen, and 33 (7.5%) patients were sensitized to olive pollen only. Of these patients, 23 received preseasonal immunotherapy with an olive pollen allergoid. The olive pollen-specific basophil activation, the titrated nasal provocation test, the nasal symptom score, and olive pollen-specific IgE, IgG1 and IgG4 levels were evaluated before immunotherapy and 8 months after the end of immunotherapy in the follow-up visit. Results: In comparison to baseline evaluation, 7 preseasonal injections of an allergoid resulted in a significant decrease in the percentage of basophils expressing CD63 (29 vs. 7%, respectively, p < 0.0001) and a significant increase in the titrated nasal provocative dose (1/10 vs. 1/1, respectively, p < 0.01). SPT induration diameters caused by an olive pollen extract decreased (12 mm at baseline vs. 5.5 mm at follow-up, p < 0.005), as did nasal symptom score (7 at baseline vs. 3 at follow-up, p < 0.01). Olive pollen-specific IgE (17.5 vs. 50 kU/l, p < 0.012), IgG1 (0.16 vs. 2.9 µg/ml, p < 0.0001) and IgG4 (0.07 vs. 1.92 µg/ml, p < 0.0001) levels significantly increased. Conclusions: Immunotherapy with 7 preseasonal injections of an olive pollen allergoid decreases olive pollen-specific basophil activation over 8 months, an effect observed in vitro and in vivo.
Objective: Exposure to indoor allergens during childhood has been associated with an increased risk of sensitization. There is no data about indoor allergen levels in day-care centers in Turkey. We hypothesized that day-care centers (DC) would be relevant sources of cat and mite allergens. Material and Methods: Fifty-seven dust samples were collected from 19 DCs in Izmir, their gardens, and classrooms of the primary schools where the DCs are located in. A questionnaire about characteristics of DCs was completed. Fel d 1, Der p 1 and Der f 1 allergen levels were quantitated by enzyme-linked immunoassay. Results: Fel d 1 was detected in all, and mite allergens in 94.7% of the samples. Levels exceeding sensitization threshold level for cat and mite allergens were present in 73.7%, and 21.1% of DCs, respectively. Fel d 1 levels exceeding threshold level that might cause asthma exacerbation was detected in 21% of DCs. Fel d 1 levels in DCs and their gardens were higher than the classrooms of the same school. Der f 1 levels were identical in DCs, gardens and classrooms. Der p 1 concentration was higher in DCs with air-conditioning, than DCs without a ventilation system. Although there was no difference for Fel d 1 levels in DCs with or without carpeted floor, Fel d 1 concentrations in DCs with carpet were significantly higher than in classrooms with no carpet. Conclusion: Day-care centers in Izmir are important sources of indoor allergens that could cause sensitization or even allergic symptoms in children and their staff.
Objective: The local allergen-specific IgE synthesis has been demonstrated via nasal provocation test in patients presented with allergic rhinitis symptoms in the absence of positive skin and specific IgE tests recently. In this study we searched the house dust mite allergy in patients who presented perennial allergic rhinitis symptoms with negative test results by means of nasal provocation test.Materials and Methods: Sixty-five patients presented perennial allergic rhinitis symptoms with negative skin prick test, intradermal test and serum-specific IgE analyses to house dust mites (Dermatophagoides pteronyssinus ve Dermatophagoides farinae) as well as 35 patients allergic to only house dust mites were performed nasal provocation test with D. pteronyssinus.Results: The nasal provocation test with D. pteronyssinus was found to be positive in 8 (12.3%) of the 65 patients with negative allergy tests. All the patients allergic to house dust mites were found to show positive nasal provocation test.Conclusion: The demonstration of the positive nasal provocation test to D. pteronyssinus in a group of patients presenting allergic rhinitis symptoms with negative skin tests and serum specific IgE to D. pteronyssinus supports the development of local allergic reaction in the absence of systemic allergic inflammation.
Background: The precision of the methods used to diagnose latex allergy is of great importance due to false-positive results. Neither the skin prick test (SPT) nor the latex-specific IgE assay has 100% diagnostic accuracy. We analysed the diagnostic value of latex-specific IgE by the first-ever concomitant use of the SPT and nasal provocation test (NPT). Methods: Twenty-seven latex-sensitive patients (group 1), 46 aeroallergen-sensitive patients (group 2a) and 33 healthy subjects (group 2b) participated in the study. All groups underwent an SPT with latex and aeroallergens and an NPT with latex. Latex-specific IgE and total IgE levels were measured by the ImmunoCAP assay. Results: Latex-specific IgE was positive in 92.6, 30.4 and 9.1% of groups 1, 2a and 2b, respectively. The 11 aeroallergen-sensitive patients in group 1 and all of the patients in group 2a were predominantly sensitised to pollens (grass, weed and tree) and reacted to a lesser degree to house dust mite, moulds and animal dander. Combined pollinosis was remarkably more prevalent in patients with positive latex-specific IgE in group 2a than in those with negative latex-specific IgE (p = 0.001). The NPT was positive in 84.6% of group 1 and negative in all control subjects. The sensitivity, specificity, negative predictive value and positive predictive value of the latex-specific IgE assay were 90.9, 72.2, 96.3 and 50%, respectively. Conclusion: The high rate of false-positive results for latex-specific IgE by ImmunoCAP should be taken into account when making a diagnosis of latex allergy in patients with pollinosis, especially in those sensitised to more than one pollen species.
BACKGROUND:Common variable immunodeficiency (CVID) is characterized by hypogammaglobulinemia and a defect in antibody production. Herein we describe 3 patients diagnosed with CVID in whom vitamin D deficiency was detected in the absence of enteropathy.METHODS:Biochemical and immunological analysis, serum osteocalcin, parathyroid hormone, 25-OH vitamin D, 1,25(OH)(2) vitamin D, vitamin A, vitamin E, urinary calcium, and deoxypyridinoline measurements were carried out. Vitamin D receptor (VDR) expression was examined in the peripheral blood mononuclear cells and hair follicles by reverse transcriptase polymerase chain reaction. VDR gene polymorphism was evaluated by high-performance liquid chromatography.RESULTS:None of the patients presented nutrient deficiencies other than vitamin D. Two of them were free of osteomalacia-related symptoms. VDR expression was found to be lower in the peripheral blood mononuclear cells and hair follicles when compared to the control group.CONCLUSIONS:Patients with CVID may present asymptomatic vitamin D deficiency. Vitamin D and VDRs play an important role in the innate immune system and modulate Toll-like receptor-related responses. Delay in diagnosis may predispose these patients not only to irreparable bone loss but also to infections, and autoimmune and malignant disorders, thus emphasizing the importance of prompt intervention.
