Except for a few specific lesions, the analysis of the margins of focal bone lesions with MRI has been largely overlooked in the literature. We observed that some lesions exhibited concave margins, suggesting a non-aggressive nature. This study aimed to determine whether concave borders are more frequently present in certain types of focal lesions of the long bones on MRI, particularly in benign lesions. MRI examinations of 586 focal intraosseous lesions of the long bones were retrospectively reviewed. Each lesion margin was independently classified by two musculoskeletal radiologists as concave, non-concave, or cortical. Magnetic resonance imaging of the sacroiliac joints (SIJ) is now frequently performed to detect subchondral inflammatory and structural changes in patients with early axial spondyloarthritis (SpA). However, similar changes can also occur in various other conditions, which may lead to the overdiagnosis of axial SpA. The aim of this article is to review the key imaging features of the most common disorders that may mimic inflammatory sacroiliitis, including mechanical changes and osteoarthritis, osteitis condensans ilii and pregnancy-related changes, other strain related changes, anatomical variants, pediatric SIJs, hyperostosis, infectious sacroiliitis, SAPHO syndrome, hyperparathyroidism, and sacral stress fractures.
The Society of Musculoskeletal Imaging (SIMS) is a French-speaking and multidisciplinary scientific society with more than 1,400 members. It provides a wide range of year-round training, such as an annual meeting of musculoskeletal (MSK) radiologists that draws an average of 850 participants from 15 countries, an annual MSK ultrasonography week, and twice yearly ultrasonography workshops in France or abroad. SIMS also runs the dedicated MSK imaging program at the annual meeting of the French Society of Radiology and the MSK imaging training for residents at the French College of Radiology Teachers.
