PURPOSE:The infrapatellar branch of the saphenous nerve (IPBSN) plays a critical role in cutaneous innervation of the anterior knee and is frequently implicated in both acute and chronic post-surgical pain. Due to its anatomical variability, precise localization is essential for effective nerve blocks and minimizing iatrogenic injury. This study evaluates the feasibility of two distal ultrasound-guided approaches for selective IPBSN blocks, with the aim of addressing the limitations of proximal techniques. METHODS:Anatomical layer-by-layer dissections of two paired lower limbs and transverse cross-sections of a third limb were used to map the IPBSN's course and its relationship to the sartorius muscle. Based on these findings, two distal ultrasound-guided approaches were tested: a proximal medial para-patellar approach at the level of the patella's superior aspect and a distal medial para-patellar approach near the medial femoral condyle. Ultrasound-guided injections (0.1-0.3 mL) targeting the IPBSN were performed on four lower limbs, followed by anatomical validation through dissection. RESULTS:The IPBSN was consistently identified as a honeycomb structure within a distinct fascial compartment. Both approaches effectively localized the nerve, with observed diffusion suggesting a potential compartment for selective nerve block. This dual-modality approach of ultrasound imaging and dissection enabled precise IPBSN localization, overcoming topographical variability relative to the sartorius muscle. CONCLUSION:Distal ultrasound-guided IPBSN blocks show promise for knee pain management by achieving targeted analgesia while preserving muscle function. Further studies are needed to validate these techniques and refine injection protocols for clinical use.
Introduction La branche infrapatellaire du nerf saphène (BIPNS) joue un rôle clé dans l’innervation cutanée de la face antérieure du genou et est fréquemment impliquée dans les douleurs postopératoires aiguës et chroniques. En raison de sa grande variabilité topographique, une localisation précise de ce nerf est indispensable pour optimiser les blocs nerveux et limiter les risques de lésions iatrogènes. Objectif Évaluer la faisabilité de deux techniques distales de bloc sélectif de la BIPNS sous guidage échographique, afin de tenir compte de la variabilité interindividuelle. Matériels et méthodes Des dissections anatomiques plan par plan de deux membres inférieurs appariés et l’analyse de coupes transversales d’un troisième membre ont permis de cartographier le trajet de la BIPNS et ses rapports anatomiques avec le muscle sartorius. À partir de ces données, deux approches distales sous guidage échographique ont été proposées : une approche parapatellaire médiale proximale au niveau du bord supérieur de la patella et une approche parapatellaire médiale distale près du condyle fémoral médial. Des injections échoguidées (de 0,1 à 0,3mL) ont été réalisées sur quatre membres inférieurs, suivies d’une validation anatomique par dissection. Résultats La BIPNS a pu être identifiée de manière constante sous la forme d’une structure en nid d’abeille, située dans un compartiment fascial distinct. Les deux approches proposées ont permis une localisation précise du nerf, avec une diffusion observée suggérant l’existence d’un compartiment spécifique adapté au bloc sélectif. La combinaison de l’imagerie échographique et des validations anatomiques a permis de contourner les variations topographiques liées aux rapports du nerf avec le muscle sartorius. Discussion et conclusion Les blocs distaux échoguidés de la BIPNS apparaissent prometteurs pour la prise en charge des douleurs du genou, en permettant une analgésie ciblée tout en préservant la fonction musculaire. Des études complémentaires sont nécessaires pour valider ces techniques et affiner les protocoles d’injection en pratique clinique.
Introduction Les lésions urétérales dans le cadre de la chirurgie colorectale sont fréquentes et associées à une forte morbi-mortalité [1], [2]. Bien que certaines techniques de repérage existent [3], [4], [5], il n’existe pas d’étude proposant des repères fixes et fiables. L’objectif principal de ce projet était d’effectuer des mesures des rapports des uretères sur cadavre permettant d’obtenir des repères anatomiques fiables pour leur localisation. Matériels et méthodes Notre population d’étude était constituée des cadavres des laboratoires d’anatomie des facultés de médecine d’Amiens et de Lille. Différentes techniques de conservation des cadavres étaient utilisées, l’utilisation d’une solution formolée, BIOMET ou des corps à l’état frais. Nous avions réalisé des injections d’uretères au latex néoprène sur cadavres et mesuré les distances entre les uretères, l’aorte abdominale et la veine cave inférieure. Nous avions également étudié le croisement des uretères avec les vaisseaux iliaques et les vaisseaux gonadiques. Résultats Quinze cadavres avaient été inclus dans l’étude, 9 corps formolés, 3 corps BIOMET et 3 corps frais. Il y avait 8 corps disséqués à Lille, et 10 corps de femmes d’étudiés. Le meilleur repère pour localiser l’uretère droit était le bord droit de la veine cave inférieure en regard de la bifurcation aortique où il se trouvait en moyenne à 16,5mm. L’uretère droit croisait le bord supérieur des vaisseaux iliaques statistiquement plus bas que l’uretère gauche (p=0,02). L’uretère gauche se situait à une distance moyenne de 31,75mm du bord gauche de l’aorte en regard de l’artère mésentérique inférieure. La loi de Luschka n’était respectée que dans un tiers des cas. Les vaisseaux gonadiques croisaient les uretères dans 100 % des cas, ce croisement pouvant être situé à tout niveau. Conclusion Notre étude devrait permettre aux chirurgiens colorectaux de repérer plus facilement les uretères, réduire leur temps opératoire et diminuer la morbi-mortalité liée aux lésions urétérales.
The temporo-mandibular joint (TMJ) links the mandibular condyle to the skull, with an interposed fibrocartilaginous articular disc. The TMJ disc has a nonuniform thickness that matches the topographic relation between the temporal bone and the mandibular condyle. The disc is a stress-distributing and load-absorbing structure that is capable of deforming and adapting its shape to that of the articular bone surfaces. The aim of this study was to analyze the morphology of human discs through thickness measurements. Forty-four temporomandibular joint discs were dissected bilaterally from 22 formalin-fixed human postmortem subjects. The thickness of each disc was manually measured at nine points distributed in a grid formed by three zones in the anteroposterior direction (anterior band, intermediate band and posterior band) and the three zones in the mediolateral direction (medial zone, middle zone and lateral zone) of the disc. Microtomographic imaging analysis was performed on four samples. The thickness of the postero-middle and the postero-medial points was significantly higher than all the other measured points. A significant difference was found between the thickness of the intermediate and the posterior bands and between the anterior and the posterior bands. A significant difference was also found between the thickness of the middle and the lateral zones (p < 0.01) and between the lateral and the medial zones (p < 0.01). Disc perforations were observed in 8 of the discs and were mostly (87.5%) located in the lateral area. Micro-CT imaging offered a new possibility to map thickness variations of the disc and showed very similar results to those taken with the calipers. The presented data contribute to increasing the knowledge of this specific joint and of the complex 3D relations between all its parts. They also open the door to pioneering studies that could lead to clinically applicable techniques in the future and enhance the diagnosis of disc perforations associated with osteoarthritis.
Introduction Les muscles oculomoteurs extrinsèques sont au nombre de 6 comprenant les muscles droit supérieur, inférieur, médial, latéral et obliques supérieur et inférieur. Ils permettent la mobilité de l’œil et peuvent être atteints en pathologie. L’augmentation en taille de ces muscles peut témoigner d’une atteinte de ceux-ci et il convient en pratique clinique d’évaluer leurs tailles. Peu de travaux rapportent la reproductibilité des mesures radio-anatomiques des muscles oculomoteurs extrinsèques. Matériels et méthodes Différents paramètres morphologiques des muscles oculomoteurs ont été étudiés en imagerie afin d’étudier leur reproductibilité. Résultats Les auteurs détails les résultats. Dans l’ensemble, et sur la base de ces résultats préliminaires, les reproductibilités des mesures ne sont pas homogènes entre les lecteurs et entre différentes modalités. Conclusion Nous discutons les résultats préliminaires concernant les reproductibilités des mesures des paramètres morphologiques des muscles oculomoteurs extrinsèques.
Aims Mechanical impingement of the iliopsoas (IP) tendon accounts for 2% to 6% of persistent postoperative pain after total hip arthroplasty (THA). The most common initiator is anterior acetabular component protrusion, where the anterior margin is not covered by anterior acetabular wall. A CT scan can be used to identify and measure this overhang; however, no threshold exists for determining symptomatic anterior IP impingement due to overhang. A case-control study was conducted in which CT scan measurements were used to define a threshold that differentiates patients with IP impingement from asymptomatic patients after THA. Methods We analyzed the CT scans of 622 patients (758 THAs) between May 2011 and May 2020. From this population, we identified 136 patients with symptoms suggestive of IP impingement. Among them, six were subsequently excluded: three because the diagnosis was refuted intraoperatively, and three because they had another obvious cause of impingement, leaving 130 hips (130 patients) in the study (impingement) group. They were matched to a control group of 138 asymptomatic hips (138 patients) after THA. The anterior acetabular component overhang was measured on an axial CT slice based on anatomical landmarks (orthogonal to the pelvic axis). Results The impingement group had a median overhang of 8 mm (interquartile range (IQR) 5 to 11) versus 0 mm (IQR 0 to 4) for the control group (p < 0.001). Using receiver operating characteristic curves, an overhang threshold of 4 mm was best correlated with a diagnosis of impingement (sensitivity 79%, specificity 85%; positive predictive value 75%, negative predictive value 85%). Conclusion Pain after THA related to IP impingement can be reasonably linked to acetabular overhang if it exceeds 4 mm on a CT scan. Below this threshold, it seems logical to look for another cause of IP irritation or another reason for the pain after THA before concluding that impingement is present.
INTRODUCTION:Several surgical options can be offered to manage iliopsoas impingement. Research published on cup replacements often concerns a small population size or multicentre studies, suggesting a variety of indications. We conducted a retrospective single centre study screening according to a specific protocol of a population of patients who had a cup replacement for iliopsoas impingement. The objectives were: 1) to specify the functional outcomes and the achievement of the Minimal Clinically Important Difference (MCID) and the Patient Acceptable Symptom State (PASS) according to the Oxford-12 score, and 2) to assess the complication rate. HYPOTHESIS:Our hypothesis was that acetabular replacements achieve a Minimal Clinically Important Difference (MCID) in more than 80% of cases. PATIENTS AND METHODS:Fifty-five hips underwent acetabular revision between 2011 and 2020. Forty-three were performed as first-line surgery, eight after failed tenotomy and four after failed anterior hip capsule thickening plasty. A CT scan of all the hips revealed a median overhang of 9 mm (7; 12) and a 7 ° cup anteversion (2; 19). Follow-up included assessment of the Oxford-12 score using MCID and PASS, the Merle d'Aubigné score, an assessment of hip flexion muscle strength using the Medical Research Council scale, and an assessment of satisfaction and complications. RESULTS:At a mean follow-up of 3 years (2-10), the difference in the Oxford score before and at follow-up was 18 points (15; 27) (p < 0.001), the median Medical Research Council score was 4.5 (4; 5) and patients were satisfied or very satisfied in 73% of cases (40/55). The MCID was achieved for 87% of the hips (48/55), and the PASS was achieved in 67% of cases (33/55). The rate of complications involving surgical revision was 10.9% (6/55) with respectively: two anterior dislocations, one early infection on day 10 resolved after wound irrigation and appropriate antibiotic therapy, one intraoperative fracture of the trochanter requiring osteosynthesis and one arthroscopic revision to remove a free cement fragment. CONCLUSION:Due to a good functional outcome but a high complication rate, a cup replacement can be offered for iliopsoas impingement associated with acetabular malposition or significant overhang. Introduction Plusieurs options chirurgicales peuvent être proposées pour la gestion des conflits ilio-psoas cupule. Les études publiées sur les changements de cupule ont des effectifs souvent limités ou sont issues de travaux multicentriques, laissant envisager des indications variables. Aussi nous avons mené une étude rétrospective monocentrique avec une sélection selon un protocole précis d’une population de patient ayant eu un changement de cupule pour conflit de l’ilio-psoas. Les objectifs étaient : (1) de préciser les résultats fonctionnels et l’atteinte du Minimal Clinically Important Difference (MCID) et le Patient Acceptable Symptom State (PASS) selon le score Oxford-12 ; (2) d’évaluer le taux de complications, Hypothèse Notre hypothèse était que les changements acétabulaires permettent d’atteindre du Minimal Clinically Important Difference (MCID) dans plus de 80 % des cas ? Matériel et méthode Cinquante-cinq hanches ont eu une révision acétabulaire de 2011 et 2020, 43 ont été réalisées en 1re intention, 8 après échec de ténotomie, et 4 après et échec de plastie d’épaississement capsulaire antérieur. Une analyse scanographique réalisée pour toutes les hanches permettant de retrouver un débord médian de 9mm (7 ; 12), une antéversion de la cupule de 7° (2 ; 19). Le suivi comprenait une évaluation du score Oxford-12 en utilisant le MCID et le PASS, le score de Merle d’Aubigné, de la force musculaire de la flexion de cuisse au moyen du Medical Research Council, la satisfaction et les complications. Résultats Au recul moyen de 3 ans (2–10), la différence du score Oxford pré- et au recul était de 18 points (15 ; 27) (p<0,001), le score médian du Medical Research Council était de 4,5 (4 ; 5) et au total 73 % (40/55) des hanches étaient satisfaits ou très satisfaits. Le MCID était atteint pour 87 % (48/55) des hanches, et le PASS dans 67 % (33/55) des cas. Le taux de complication impliquant une reprise chirurgicale était de 10,9 % (6/55) avec respectivement : 2 luxations antérieures, 1 infection précoce à J10 guérie après lavage chirurgical et antibiothérapie adaptée, 1 fracture peropératoire du massif trochantérien nécessitant la mise en place d’une ostéosynthèse, 1 reprise sous arthroscopie pour exérèse d’un fragment de ciment libre. Conclusion En raison d’un bon résultat fonctionnel mais d’un taux de complication élevé, les changements de cupule pour conflit ilio-psoas peuvent être proposés pour les hanches présentant un conflit ilio-psoas cupule associé à une malposition acétabulaire ou un débord important. INTRODUCTION:In 2015, we described a capsule-thickening technique via an anterior approach for iliopsoas cup impingement. To our knowledge, medium-term results have not been reported. We therefore retrospectively analyzed all cases in the initial series and those since 2015, to assess this original technique over a longer follow-up: (1) to analyze complications, and (2) to assess functional outcome. HYPOTHESIS:The study hypothesis was that this surgical solution has a success rate sufficient for it to be included in the therapeutic armamentarium for iliopsoas impingement. MATERIAL AND METHOD:Fourteen patients were included. Nine plasties were in first line, 3 after tenotomy, and 2 after cup exchange. The anterior Hueter approach was used, visualizing anterior cup overhang, sometimes associated with penetration of the anterior capsule, and enabling capsule-thickening by a folded Vicryl™ mesh. Functional results were analyzed. RESULTS:At a median 4years' follow-up (IQR: 2-5; range: 1-9), change over baseline in Oxford score was 7 points (p=0.004), median Medical Research Council thigh flexion strength score was 5 (IQR: 5-5), and 50% of patients (7/14) were satisfied or very satisfied. The major complications rate was 7% (1/14), for 1 irrigation of infected hematoma, cured without recurrence; there was also 1 minor case of injury to the lateral cutaneous nerve of the thigh. Forty-three percent of patients (6/14) exhibited a minimal clinically important difference (MCID) and 64% (9/14) a patient-acceptable symptom state (PASS). Median anatomic overhang on anatomic CT transverse slice was 7mm (IQR: 3-8; range: 0-13). Four patients underwent secondary acetabular component exchange; their median overhang was 7.5mm (IQR: 7-8) compared to 5mm (IQR: 2-8) for the other patients (p-value non-calculable). CONCLUSION:This surgical option seems interesting when acetabular overhang is not too great, especially as it does not affect flexion strength. LEVEL OF EVIDENCE:IV.
Des séances de dissection sont organisées chaque année pour tous les étudiants en 2e année de médecine à l'université de Lille. L'objectif de l'étude était, d'une part, d'identifier ce qui pouvait être amélioré, et d'autre part, d'évaluer les conditions du maintien des séances notamment pendant le confinement dû à la COVID-19. Un livret, remis préalablement, les informait du déroulement. Avant la 2e séance, un questionnaire de 26 questions aux modalités de réponse prédéfinies et 2 questions ouvertes a été distribué. Les analyses statistiques ont été réalisées avec le logiciel RStudio®. Au total, 504 (sur 516) étudiants ont répondu. Au total, 90,44 % ont estimé avoir les connaissances suffisantes et 86,77 % avoir atteint les objectifs requis. Les informations du livret, évaluées par une échelle numérique, leur ont paru suffisantes, notamment concernant les règles de sécurité (moyenne = 9,21/10). L'encadrement a satisfait les étudiants (moyenne = 8,71/10) et plus de 90 % ont convenu que l'aide pour la manipulation des instruments et les techniques de dissection était suffisante. Malgré un niveau moyen d'appréhension à 3,87/10 (± 2,48), 98,02 % des étudiants ont déclaré que leurs craintes étaient apaisées après la 1re séance. Les étudiants sont acquis à l'idée d'organiser et participer à une cérémonie civile en honneur des donateurs. Aucun étudiant n'a contesté l'utilité de la dissection pour comprendre l'anatomie ; 95,43 % d'entre eux ont exprimé la nécessité de maintenir les dissections obligatoires. Concernant le risque lié à la COVID-19, 98,81 % des étudiants ont souhaité le maintien des séances. Cette étude objective le fait que la dissection est plébiscitée par les étudiants. Elle est ressentie comme étant un outil important pour l'apprentissage de l'anatomie. Les résultats de l'étude montrent la volonté des étudiants de maintenir les séances de dissections comme partie intégrante de leur apprentissage.
Le nerf musculocutané, moteur pour les muscles de la loge antérieure du bras et sensitif pour la face latérale de l'avant-bras, a une origine apparente variable. La réalisation d'une anesthésie locorégionale comme le bloc axillaire pour la chirurgie du membre supérieur nécessite de connaître les variations anatomiques qui peuvent avoir un impact sur le déroulé du geste. Les objectifs de cette étude sont d'évaluer la possibilité d'un repérage échographique des variations de l'origine apparente du nerf musculocutané et de documenter ces variations. Dans un premier temps, afin d'évaluer la fréquence des variations d'origine apparente du nerf musculocutané, 22 membres supérieurs (11 donneurs) ont été disséqués. Dans un deuxième temps, les variations ont été recherchées par échographie (Canon Aplio i800, sonde linéaire 18 MHz) sur 12 membres (6 donneurs), disséqué secondairement afin d'établir la concordance entre l'échographie et l'anatomie. Le nerf musculocutané, disséqué chez 7 hommes et 10 femmes, pouvait soit naître « classiquement » du faisceau latéral (85,3 %), soit naître plus distalement au bras (8,8 %), accolé au nerf médian ; il n'était pas individualisable dans 5,9 % des cas et ses branches d'innervation étaient alors fournies par le nerf médian. En échographie, chez 2 hommes et 4 femmes, le nerf musculocutané apparaissait naître plus distalement au bras dans 25 % des cas. La dissection a permis de confirmer les données échographiques pour tous les sujets. Cette étude originale démontre la fiabilité de l'échographie comme moyen de visualisation des variations d'origine du nerf musculocutané. La plus fréquente des variations est la naissance du nerf musculocutané à partie moyenne du bras, après accolement au nerf médian. Les variations d'origine apparente du nerf musculocutané, fréquentes, peuvent être visualisées en échographie et doivent être prises en compte lors de la réalisation d'un bloc axillaire.
Algorithms for fracture detection are spreading in clinical practice, but the use of X-ray-only ground truth can induce bias in their evaluation. This study assessed radiologists’ performances to detect wrist and hand fractures on radiographs, using a commercially-available algorithm, compared to a computerized tomography (CT) ground truth. Le septum dorsal capsulo-scapholunaire (DCSS) est une structure capsulo-ligamentaire récemment décrite entre le faisceau dorsal du ligament scapholunaire (LSL) et la capsule articulaire du poignet. Cette structure serait un stabilisateur secondaire de l’articulation scapholunaire. Le but de cette étude était d’évaluer la visibilité et l’aspect normal du DCSS en échographie à haute fréquence. Le DCSS serait analysable en échographie à haute fréquence. Trois poignets de cadavre ont été disséqués pour étudier le DCSS, l’un sans marquage, les deux autres après marquage sous contrôle échographique. Sur deux autres poignets, une corrélation entre la structure considérée comme le DCSS en échographie et les coupes arthroscannographiques et anatomiques correspondantes a été réalisée. Enfin, des coupes échographiques sagittales de la région du DCSS sur 42 poignets sains ont été analysées rétrospectivement. En dissection, le DCSS correspondait à une structure fibreuse s’étendant jusqu’à la face dorsale du scaphoïde et du lunatum, certaines fibres convergeant vers le LSL. En échographie à haute fréquence, une structure fibrillaire hyperéchogène était visualisée à la position théorique du DCSS. Les dissections réalisées après transfixion échoguidée montraient que ses limites correspondaient aux limites du DCSS. Les coupes anatomiques, arthroscannographiques et échographiques dans la région du DCSS étaient concordantes. Le DCSS était visible rétrospectivement sur au moins une coupe échographique de référence sur 90,5 % des poignets sains, et était toujours hyperéchogène. Son épaisseur moyenne était de 1,38 ± 0,24 mm. Notre étude décrit les caractéristiques échographiques normales du DCSS, visible chez une majorité de patients sains en échographie à haute fréquence. L’analyse des caractéristiques échographiques du DCSS en contexte traumatique aigu du poignet devraient être évaluées et un arbre décisionnel diagnostique a été proposé. IV.
Gout, calcium pyrophosphate deposition disease, and apatite calcifications, the three main crystal disorders, may involve the spine. These disorders can be completely asymptomatic or associated with various clinical symptoms, such as acute flares and more chronic manifestations. This article presents the typical and more unusual imaging features encountered in these disorders.
We discuss several variants of the metaphyseal and diaphyseal bone surfaces that may be misleading in clinical practice. They include metaphyseal stripes, spiculated metaphyseal cortex, cortical desmoid, laminated lateral supracondylar ridge, cortical vascular canals, variations in shape or lucency of normal tuberosities, cortical thickening of normal ridges, and well-organized undulated hyperostosis at the proximal phalanges.
Rationale and Objectives: To develop a model using artificial intelligence (A.I.) able to detect post-traumatic injuries on pediatric elbow X-rays then to evaluate its performances in silico and its impact on radiologists’ interpretation in clinical practice. Material and Methods: A total of 1956 pediatric elbow radiographs performed following a trauma were retrospectively collected from 935 patients aged between 0 and 18 years. Deep convolutional neural networks were trained on these X-rays. The two best models were selected then evaluated on an external test set involving 120 patients, whose X-rays were performed on a different radiological equipment in another time period. Eight radiologists interpreted this external test set without then with the help of the A.I. models . Results: Two models stood out: model 1 had an accuracy of 95.8% and an AUROC of 0.983 and model 2 had an accuracy of 90.5% and an AUROC of 0.975. On the external test set, model 1 kept a good accuracy of 82.5% and AUROC of 0.916 while model 2 had a loss of accuracy down to 69.2% and of AUROC to 0.793. Model 1 significantly improved radiologist's sensitivity (0.82 to 0.88, P = 0.016) and accuracy (0.86 to 0.88, P = 0,047) while model 2 significantly decreased specificity of readers (0.86 to 0.83, P = 0.031). Conclusion: End-to-end development of a deep learning model to assess post-traumatic injuries on elbow X-ray in children was feasible and showed that models with close metrics in silico can unpredictably lead radiologists to either improve or lower their performances in clinical settings.
Summary Purpose Thoracic outlet syndrome (TOS) is a ductal syndrome that can have a significant functional impact. Various studies have highlighted positional factors and repetitive movements as risk factors for the development of TOS. However, there are few literature data on the socioprofessional consequences of TOS. Methods We performed a prospective, cross-sectional, descriptive, multicentre study of workers having received a Doppler ultrasound diagnosis of TOS between December 17th, 2018, and March 16th, 2021. Immediately after their diagnosis, patients completed a self-questionnaire on the impact of TOS on their work activities. We assessed the frequency of TOS-related difficulties at work and the associated socioprofessional consequences. Trial Registration Number (TRN) is NCT03780647 and date of registration December 18, 2018. Results Eighty-two participants (95.3%) reported difficulties at work. Seventy-seven of the participants with difficulties (94%) worked in the tertiary sector; these difficulties were due to prolonged maintenance of a posture, carrying loads, and repetitive movements. Although the majority of participants experienced organizational problems and lacked support at work, few of them had approached support organizations, expert and/or healthcare professionals. Conclusions TOS was almost always associated with difficulties at work (95.3%). However, poor awareness of sources of help or a perceived lack of need may discourage people with TOS from taking steps to resolve these difficulties. It is clear that the socioprofessional management of TOS requires significant improvements.
Anatomical variants are frequently encountered when assessing the sacroiliac joints (SIJ) using magnetic resonance imaging. When not located in the weight-bearing part of the SIJ, variants associated with structural and edematous changes can be misinterpreted as sacroiliitis. Their correct identification is necessary to avoid radiologic pitfalls. This article reviews five SIJ variants involved in the dorsal ligamentous space (accessory SIJ, iliosacral complex, semicircular defect, bipartite iliac bony plate, and crescent iliac bony plate) and three SIJ variants involved in the cartilaginous part of the SIJ (posterior dysmorphic SIJ, isolated synostosis, and unfused ossification centers).
In the fields of forensics, the identification of human remains is a recurrent problem. The estimated age at death is one of the copious criteria to be evaluated. In adult teeth, the height of the root dentin transparency is used to estimate age. However, in archaeological material, this phenomenon appears inconstant. The aim of this work was to observe the structural modifications of the sclerotic dentin in the teeth for different postmortem intervals. The study included two parts (retrospective and prospective study) with 21 human monoradicular teeth, from bodies donated to medical science with postmortem intervals (PMIs) of 0, 1, 2 and 5 years and archeological excavation. After inclusion based on resin, section and polishing, the samples were analyzed with a scanning electron microscope (SEM) JSM-7800F®, and the procedure was completed via a semiquantitative analysis of calcium and phosphorus using EDX microanalysis. The analysis showed the existence of tubular and chemical modifications of sclerotic dentin at different PMIs. Our SEM study allowed us to observe a difference in tubule aspects linked to an increased PMI: the loss of peritubular collar and the lumen obstruction of tubules with a hyperdense material. Microanalysis highlighted variations in phosphocalcic ratios among the different groups, especially in the pulp area and the canine. Our hypotheses that explain these differences are based on the postmortem modifications of the crystals of the mineral phase of sclerotic dentin under the influence of chemical and/or bacterial action.
