ObjectifÉvaluer l’impact de l’exposition professionnelle à la silice cristalline sur la fonction ventilatoire dans une population de sujets du Nord de la France.MéthodesNous avons inclus 1428 hommes âgés de 40 à 65 ans de l’étude transversale ELISABET, recrutés en population générale par tirage au sort sur liste électorale, ayant réalisé une spirométrie (normes Global Lung Function Initiative 2012 [GLI-2012]) et un questionnaire. L’indice d’exposition cumulée à la silice cristalline (IEC Silice) exprimé en mg.m−3. années a été calculé avec la matrice emploi-exposition (MEE) matgéné silice cristalline. Les co-expositions aux vapeurs, gaz, fumées et poussières (hors silice) (VGFP) ont été estimées avec une MEE créée pour cette étude. Les relations entre l’IEC silice et les indices spirométriques ont été étudiées à l’aide de modèles de régression logistique et linéaire ajustés sur l’âge, l’indice de masse corporelle, le statut tabagique, le centre, l’antécédent d’asthme, la saison et les co-expositions aux VGFP.RésultatsLa proportion de sujets exposés à la silice était de 20,5 % (293/1428). En régression linéaire multiple, les z-scores ajustés du VEMS/CVF (rapport du volume expiré maximal pendant la 1re seconde [VEMS] sur la capacité vitale forcée [CVF]) et du débit expiratoire moyen entre 25 et 75 % de la capacité vitale forcée (DEM25–75) diminuaient de manière significative à mesure que l’IEC silice augmentait. Après ajustement, les z-scores du VEMS/CVF (β : 0,426 (intervalle de confiance à 95 % [IC95 %] : 0,792–0,060) par augmentation de 1mg.m−3. année) et du DEM25–75 (β : 0,552 [IC95 % : 0,947–0,157]) étaient significativement plus faibles chez les sujets ayant un IEC silice ≥ 1mg.m−3.années que chez les sujets non exposés. En régression logistique multivariée, le risque de présenter un trouble ventilatoire obstructif (TVO) était significativement augmenté chez les sujets ayant un IEC silice≥1mg.m−3.années par rapport aux non-exposés (ORajusté=3,06 [IC95 %=1,10–7,63] ; p=0,022) ainsi que le risque de présenter une altération du DEM25–75 (ORajusté=4,31 [IC95 %=1,39–11,79] ; p=0,007).ConclusionCette étude renforce l’indication à réaliser une surveillance spirométrique pendant la carrière et lors du suivi post-professionnel chez les sujets atteignant ou dépassant un seuil d’exposition à la silice de 1mg.m−3.année, afin d’identifier une obstruction des petites voies aériennes ou un TVO.
SPECIFIC ASPECTS OF MEDICO-SOCIAL PROTECTION IN FRENCH PUBLIC SERVICES. Civil servants in the three French civil services (State, hospital and local governments) benefit from specific medical and social protection, unique to their status. This protection is very different from that of private sector workers under the French general social security regime, with specific participants and instances (licensed doctors and "conseils médicaux") and specific sick leave depending on the pathology of the civil servant, in particular long-term sick leave ("congés de longue maladie"), long-term leave ("congés de longue durée") and leave for temporary disability attributable to the service. Therapeutic part-time leave also has specific conditions for granting and renewal. Finally, civil servants benefit from specific measures for returning to and remaining in employment, particularly in the context of professional reclassification.
Objective To describe the proportions of subjects exposed to crystalline silica and the sectors of activity concerned between 1965 and 2010 in a sample of the general French population. Methods We included 2942 participants aged 40 to 65 years, recruited at random from electoral rolls, from the French general population in the cross-sectional ELISABET study between 2011 and 2013. The proportions of subjects exposed to crystalline silica and their sectors of activity were determined on the basis of their career history and the use of the Matgéné job-exposure matrix. Results In the total sample, occupational exposure to crystalline silica was found for 291 subjects (9.9%) between 1965 and 2010, with a predominance of men (20.2% of exposed subjects among men (282 out of 1394) versus 0.6% among women (9 out of 1548)). The highest proportion of participants exposed to crystalline silica was reached in 1980 with 6.1% and then decreases to 4.4% in 2010. Among men, the most frequently exposed sectors of activity were manufacture of basic metals (41.5% of exposed men (117 out of 282)), specialised construction activities (23.1% of exposed men (65 out of 282)) and construction of buildings (14.2% of exposed men (40 out of 282)). Conclusions Although the proportion of workers exposed to crystalline silica has been decreasing since the 1980s, it is still significant at least until 2010, particularly in the construction sector, and further research is needed to improve the monitoring of workers who are or have been exposed to crystalline silica.
INTRODUCTION:Although several studies have studied the relationship between occupational exposure to crystalline silica dust and respiratory mortality, few have examined the relationship with impairments in respiratory function and the exposure threshold triggering spirometric monitoring in exposed workers. The objective of the present study was to evaluate the impact of exposure to crystalline silica dust on respiratory function. METHODS:We included 1428 male participants (aged 40 to 65) recruited from the French general population, at random from electoral rolls, in the cross-sectional ELISABET study and for whom data on forced expiratory flow-volume curve indices z-scores (calculated using the Global Lung Function Initiative 2012 equations) and exposure (via a questionnaire) were available. A cumulative exposure index (CEI) for crystalline silica dust (CEIsilica, expressed in mg.m-3.year) was calculated using the Matgéné occupational exposure matrix. RESULTS:293 of the 1428 participants (20.52%) reported exposure to silica dust. We found that the adjusted z-scores for the forced expiratory volume in the first second/forced vital capacity (FEV1/FVC) ratio decreased significantly as CEIsilica increased. After adjustment, the adjusted z-scores for FEV1/FVC (β: -0.426 (95% confidence interval (CI): -0.792, -0.060) per 1 mg m-3.year increment) and the mean forced expiratory flow between 25 and 75% of the forced vital capacity (FEF25-75) (β: -0.552 (95% CI: -0.947, -0.157)) were significantly lower in the participants with CEIsilica ≥1 mg m-3.year than in non-exposed participants. The likelihoods of having airway obstruction (odds ratio (OR): 3.056 (95% CI: 1.107, 7.626)) or having an impaired FEF25-75 (OR: 4.305 (95% CI: 1.393, 11.79)) were also significantly higher in participants with CEIsilica ≥1 mg m-3.year. CONCLUSION:Our results emphasize the importance of spirometry-based monitoring in workers exposed to more than 1 mg m-3.year of crystalline silica dust, in order to identify small airway obstruction or airway obstruction as early as possible.
Objectives: We investigated serum neutralizing activity against BA.1 and BA.2 Omicron sublineages and T cell response before and 3 months after administration of the booster vaccine in healthcare workers (HCWs). Methods: HCWs aged 18-65 years who were vaccinated and received booster doses of the BNT162b2 vaccine were included. AntieSARS coronavirus 2 IgG levels and cellular response (through interferon gamma ELISpot assay) were evaluated in all participants, and neutralizing antibodies against Delta, BA.1, and BA.2 were evaluated in participants with at least one follow-up visit 1 or 3 months after the administration of the booster dose. Results: Among 118 HCWs who received the booster dose, 102 and 84 participants attended the 1-month and 3-month visits, respectively. Before the booster vaccine dose, a low serum neutralizing activity against Delta, BA.1, and BA.2 was detectable in only 39/102 (38.2%), 8/102 (7.8%), and 12/102 (11.8%) participants, respectively. At 3 months, neutralizing antibodies against Delta, BA.1, and BA.2 were detected in 84/84 (100%), 79/84 (94%), and 77/84 (92%) participants, respectively. Geometric mean titres of neutralizing antibodies against BA.1 and BA.2 were 2.2-fold and 2.8-fold reduced compared with those for Delta. From 1 to 3 months after the administration of the booster dose, participants with a recent history of SARS coronavirus 2 infection (n = 21/84) had persistent levels of S1 reactive specific T cells and neutralizing antibodies against Delta and BA.2 and 2.2-fold increase in neutralizing antibodies against BA.1 (p 0.014). Conversely, neutralizing antibody titres against Delta (2.5-fold decrease, p < 0.0001), BA.1 (1.5-fold, p 0.02), and BA.2 (2-fold, p < 0.0001) declined from 1 to 3 months after the administration of the booster dose in individuals without any recent infection. Discussion: The booster vaccine dose provided significant and similar response against BA.1 and BA.2 Omicron sublineages; however, the immune response declined in the absence of recent infection. The results were treated according to different themes including workload, perceived health, psycho-organizational constraints, balance private life/professional life or professional exhaustion.Results. - In total, 222 questionnaires were collected, representing a response rate of 53%. Forty-nine percent find the exercise of responsibility satisfactory, 93% consider it complex and 75% deplore a lack of support and would consider training useful. Seventy percent have already thought about giving up their role as managers for good in the last 12 months. The impact of work on their health and personal life is experienced negatively. Sixty-one percent believe they can play a role in the well-being at work of their colleagues, 86% believe that it is their duty to pay particular attention to the health of their colleagues. Regarding professional exhaustion, assessed using the MBI scale, 10% present a high level of emotional exhaustion, associated with a high depersonalization score.Conclusion. - The main areas for improvement concern: recognition and better consideration of medical management within the institution, training, additional tools to be provided to them in order to enable them to be more involved in risk prevention. psychosocial issues of hospital practitioners.
Dans un monde hospitalier en tension, les responsables de structure au sein d’un établissement hospitalo-universitaire sont investis d’une mission de manager en plus de celles de cliniciens, chercheurs et enseignants. Les objectifs sont de faire un point sur leur vécu au travail, leur état de santé global (dont le niveau d’épuisement professionnel) et leur sensibilisation à la question du bien-être au travail au sein de leur équipe. Notre étude a été réalisée au CHU de Lille par questionnaire diffusé aux responsables de structure en juin et juillet 2019. Les résultats ont été traités selon différentes thématiques dont la charge de travail, la santé perçue, les contraintes psycho-organisationnelles, l’équilibre vie privée/vie professionnelle ou encore l’épuisement professionnel. Au total, 222 questionnaires ont été recueillis, soit un taux de réponse de 53 %. Quarante-neuf pour cent trouvent l’exercice d’une responsabilité satisfaisant, 93 % le jugent complexe et 75 % déplorent un manque d’accompagnement et estimeraient une formation utile. Soixante-dix pour cent ont déjà songé à renoncer définitivement à leur mission de manager au cours des 12 derniers mois. L’impact du travail sur leur santé et leur vie personnelle est vécu négativement. Soixante et un pour cent pensent pouvoir jouer un rôle dans le bien-être au travail de leurs confrères, 86 % estiment qu’il est de leur devoir d’avoir une attention particulière à la santé de leurs confrères. Concernant l’épuisement professionnel, évalué grâce à l’échelle MBI, 10 % présentent un niveau d’épuisement émotionnel élevé, associé à un score de dépersonnalisation élevé. Les principaux axes d’amélioration concernent : la reconnaissance et la meilleure prise en compte du management médical au sein de l’institution, la formation, les outils complémentaires à leur amener afin de leur permettre d’être davantage acteurs dans la prévention des risques psychosociaux des praticiens hospitaliers. Background The present study aimed to evaluate the persistent immunogenicity offered by a third dose of BNT162b2 against Delta and Omicron variants, in nursing home (NH) residents. Methods In this monocenter prospective observational study, anti-spike IgG levels, S1 domain reactive T cell counts, serum neutralizing antibody titers against Delta and Omicron variants were compared before and up to three months after the BNT162b2 booster dose, in NH residents without COVID-19 (COVID-19 naive) or with COVID-19 prior to initial vaccination (COVID-19 recovered). Findings 106 NH residents (median [interquartile range] age: 86.5 [81;91] years) were included. The booster dose induced a high increase of anti-spike antibody levels in all subjects (p < 0.0001) and a mild transient increase of specific T cells. Before the booster dose, Delta neutralization was detected in 19% (n = 8/43) and 88% (n = 37/42) of COVID-19 naive and COVID-19 recovered subjects, respectively. Three months after the booster dose, all NH residents developed and maintained a higher Delta neutralization (p < 0.0001). Before the booster dose, Omicron neutralization was detected in 5% (n = 2/43) and 55% (n = 23/42) of COVID-19 naive and COVID-19 recovered subjects, respectively, and three months after, in 84% and 95%, respectively. Neutralizing titers to Omicron were lower than to Delta in both groups with a 35-fold reduction compared to Delta. Interpretation The booster dose restores high neutralization titers against Delta in all NH residents, and at a lower level against Omicron in a large majority of participants. Future studies are warranted to assess if repeated BNT162b2 booster doses or new specific vaccines might be considered for protecting such fragile patients against Omicron and/or future SARS-CoV-2 variants. Herpes zoster (HZ) was suspected as a predictive cutaneous manifestation of COVID-19, with a debated prognostic significance. We report a series of 5 cases of HZ occurring after vaccination with a nucleoside-modified messenger RNA (mRNA) COVID-19 vaccine (Comirnaty, Pfizer-BioNTech). These new cases do not prove causality between COVID-19 vaccination and HZ. The pathophysiologic mechanism remains elusive, but local vaccine-induced immunomodulation may be involved. The occurrence of HZ does not justify avoiding the second injection of vaccine due to the benefit of vaccination.
L’épidémie COVID-19 a placé le personnel des établissements de santé en première ligne mais, à ce jour, il existe peu de données dans la littérature sur la prévalence de l’infection à SARS-CoV-2 combinant la réalisation d’une RT-PCR et d’une sérologie. L’objectif de cette étude est de déterminer la prévalence de l’infection à SARS-CoV-2 chez le personnel du centre hospitalo-universitaire (CHU) de Lille. L’objectif secondaire est de décrire les déterminants d’expositions professionnelles et environnementaux de cette infection. Dans cette étude monocentrique transversale répétée réalisée entre le 4 mai et le 16 juillet 2020, un prélèvement nasopharyngé pour analyse par RT-PCR, une sérologie SARS-CoV-2 et un questionnaire clinico-professionnel ont été proposés à chaque participant. Ont été inclus les personnels du CHU de Lille travaillant sur site, présentant ou non des symptômes évocateurs d’une infection à SARS-CoV-2. Les sujets ont été classés dans l’un des trois groupes suivants selon les données du questionnaire : haut risque (personnel travaillant dans une unité accueillant spécifiquement des patients infectés par le SARS-CoV-2 ou suspects de l’être) ; risque intermédiaire (personnel travaillant dans une unité pouvant accueillir des patients infectés ou suspects de l’être) ; risque faible (personnel hors des unités de soins). Au total, 3786 sujets ont été inclus (3415 (90,2 %) RT-PCR et 3550 (93,8 %) sérologies) : 66 (1,7 %) avaient une RT-PCR positive ; 191 (5 %) une sérologie positive en IgM/A et/ou IgG. On dénombre 57,7 %, 31 % et 11,3 % de séropositifs dans le groupe à haut risque, dans le groupe intermédiaire et dans le groupe à risque faible respectivement ( p = 0,054). Parmi les séropositifs du groupe à haut risque, les gestes les plus fréquemment réalisés sur patients suspects ou confirmés étaient les aérosols (58,5 %), les soins de bouche (48,8 %) et les prélèvements urinaires (46,3 %) ; les types de contacts les plus fréquents étaient la manipulation du patient (73,2 %), les échanges verbaux (70,7 %) et la manipulation de linge sale (68,3 %) ; les équipements de protection individuelle les moins fréquemment utilisés étaient les surchaussures (39 %), la visière (46,3 %) et le tablier en plastique (73,2 %). La prévalence de l’infection à SARS-CoV-2 est faible dans notre échantillon de personnel du CHU de Lille (RT-PCR positives : 1,7 % ; sérologie positive : 5 %), y compris chez les personnels les plus exposés. Cette faible proportion peut être expliquée par la bonne compliance du personnel à utiliser les moyens de préventions mis en place.
We quantified S1-specific IgG, neutralizing antibody titers, specific IFNγ secreting T cells and functionality of specific CD4+ and CD8+ T cells in 130 young adults (median age 44.0 years) and 106 older residents living in a long-term care facility (86.5 years) after 2 doses of BNT162b2. Three months after the first injection, humoral and cellular memory responses were dramatically impaired in the 54 COVID-19-naive older compared to the 121 COVID-19-naive younger adults. Notably, older participants’ neutralizing antibodies, detected in 76.5% (versus 100% in young adults, P < 0.0001), were ten times lower than the younger’s antibody titers (P < 0.0001). Antibody and T cell responses were greater among the 52 COVID-19-recovered than among the 54 COVID-19-naive older adults (P < 0.0001). Our study shows that 2 doses of BNT162b2 does not guarantee long-term protection against SARS-CoV-2 in the older. An additional dose should be considered to boost their specific memory response.
BACKGROUND:The single-breath diffusing capacity of the lung for carbon monoxide (DLCO) interpretation needs the comparison of measured values to reference values. In 2017, the Global Lung Function Initiative published new reference values (GLI-2017) for DLCO, alveolar volume (VA) and transfer coefficient of the lung for carbon monoxide (KCO). We aimed to assess the applicability of GLI-2017 reference values for DLCO on a large population by comparing them to the European Community of Steel and Coal equations of 1993 (ECSC-93) widely used. METHODS:In this retrospective study, spirometric indices, total lung capacity, DLCO, VA and KCO were measured in adults classified in 5 groups (controls, asthma, chronic bronchitis, cystic fibrosis, and interstitial lung diseases (ILD)). Statistical analysis comparing the 2 equations sets were stratified by sex. RESULTS:4180 tests were included. GLI-2017 z-scores of the 3 DLCO indices of the controls (n = 150) are nearer to 0 (expected value in a normal population) than ECSC-93 z-scores. All groups combined, in both genders, DLCO GLI-2017 z-scores and %predicted are significantly higher than ECSC z-scores and %predicted. In the ILD group, differences between the 2 equation sets depend on the DLCO impairment severity: GLI-2017 z-scores are higher than ECSC z-scores in patients with no or "mild" decrease in DLCO, but are lower in "moderate" or "severe" decrease. CONCLUSION:GLI-2017 reference values for DLCO are more suitable to our population and influence the diagnostic criteria and severity definition of several lung diseases.
Long-term care facility (LTCF) older residents display physiological alterations of cellular and humoral immunity that affect vaccine responses. Preliminary reports suggested a low early postvaccination antibody response against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The aim of this study was to focus on the specific T-cell response. We quantified S1-specific IgG, neutralizing antibody titers, total specific IFN gamma-secreting T cells by ELISpot, and functionality of CD4(+)- and CD8(+)-specific T cells by flow cytometry, after two doses of the BNT162b2 vaccine in younger and older people, with and without previous COVID-19 infection (hereafter referred to as COVID-19-recovered and COVID-19-naive subjects, respectively). Frailty, nutritional, and immunosenescence parameters were collected at baseline in COVID-19-naive older people. We analyzed the immune response in 129 young adults (median age 44.0 years) and 105 older residents living in a LCTF (median age 86.5 years), 3 months after the first injection. Humoral and cellular memory responses were dramatically impaired in the COVID-19-naive older (n = 54) compared with the COVID-19-naive younger adults (n = 121). Notably, older participants' neutralizing antibodies were 10 times lower than the younger's antibody titers (p < 0.0001) and LCTF residents also had an impaired functional T-cell response: the frequencies of IFN gamma(+) and IFN gamma+IL-2(+)TNF alpha(+) cells among specific CD4(+) T cells, and the frequency of specific CD8(+) T cells were lower in COVID-19-naive older participants than in COVID-19-naive young adults (p p = 0.0018, respectively). However, COVID-19-recovered older participants (n = 51) had greater antibody and T-cell responses, including IFN gamma(+) and IFN gamma+IL-2(+)TNF alpha(+)-specific CD4(+) T cells (p < 0.0001), as well as TNF alpha(+)-specific CD8(+) T cells (p < 0.001), than COVID-19-naive older adults. We also observed that "inflammageing" and particularly high plasma levels of TNF alpha was associated to poor antibody response in the older participants. In conclusion, our results show that the COVID-19-naive older people had low counts and impaired specific CD4(+) and CD8(+) T cells, in addition to impaired antibody response, and that specific studies are warranted to assess the efficiency of SARS-CoV-2 mRNA-based vaccines, as in other immunocompromised subjects. Our study also shows that, despite their physiological alterations of immunity, vaccination is highly efficient in boosting the prior natural memory response in COVID-19-recovered older people.

Dans un monde hospitalier en tension, comment ces médecins appréhendent-ils leurs missions de médecins-managers ? Quels regards portent-ils sur leur santé au travail et celle de leurs confrères ? Les objectifs sont : faire un point sur leurs conditions d’exercice, évaluer leur charge de travail, la conciliation vie professionnelle/vie privée, leur état de santé global (physique, psychique, niveau d’épuisement professionnel) et leur sensibilisation à la question du bien-être au travail. Notre étude a été réalisée au sein de la population des responsables fonctionnels du CHU de Lille du 03 juin au 31 juillet 2019, par l’intermédiaire d’un questionnaire diffusé par messagerie professionnelle. 222 questionnaires ont été recueillis sur les 421 responsables recensés, soit un taux de réponse de 53 %. Les résultats ont été traités selon différentes thématiques dont la charge de travail, la santé perçue, les contraintes psycho-organisationnelles, l’équilibre vie privée/vie professionnelle ou encore l’épuisement professionnel. 49 % trouvent l’exercice d’une responsabilité satisfaisant. Ils le jugent complexe (à 93 %), déplorent un manque d’accompagnement (76 % le pensent nécessaire lors de la prise de fonction) et estimeraient une formation utile (à 75 %). 70 % ont déjà songé à renoncer définitivement à leur mission de manager au cours des 12 derniers mois. L’impact du travail sur leur santé et leur vie personnelle est vécu négativement (respectivement 58 % et 68 % d’insatisfaits de leurs horaires et rythme de travail pour leur santé et vie privée). Ils sont 61 % à penser pouvoir jouer un rôle dans le bien-être au travail de leurs confrères, 86 % à estimer qu’il est de leur devoir d’avoir une attention particulière à la santé de leurs confrères. Concernant l’épuisement professionnel, évalué grâce à l’échelle MBI, 18,9 % présentent un niveau d’épuisement émotionnel élevé, 18,5 % un score de dépersonnalisation élevé et 35,7 % un score d’accomplissement personnel faible. Les principales pistes de réflexion et axes d’amélioration dégagés par l’analyse des données concernent : la reconnaissance et la meilleure prise en compte du management médical au sein de l’institution, la formation spécifique de ces médecins, les outils complémentaires à amener aux responsables médicaux, déjà convaincus de leur rôle de veilleur auprès de leurs confrères, afin de leur permettre d’être d’avantage acteurs dans la prévention des risques psycho-sociaux des praticiens hospitaliers.

La relation médecin–patient en médecine du travail est singulière, notamment parce qu’elle se développe dans une démarche préventive, où le patient n’est pas forcément un malade (au sens d’une dégradation de son état de santé). Cette relation est par ailleurs imposée dans le cadre du suivi de santé obligatoire des travailleurs et le salarié n’a pas le choix de son praticien. Dans cette relation médecin–patient particulière, l’objectif a été d’étudier les modalités de transmission de l’information médicale du salarié au médecin du travail et d’analyser les déterminants de cette transmission. Distribution d’un auto-questionnaire aux salariés venant en visite médicale de santé au travail, quel que soit le type de visite, au sein d’un service de santé au travail inter-entreprises de la métropole lilloise (enquête réalisée entre avril et juin 2016). Le recueil de 359 questionnaires auprès des salariés a permis de mettre en évidence que 55,7 % des salariés interrogés déclarent transmettre spontanément leurs informations médicales au médecin du travail, 24,5 % déclarent les transmettre si le médecin aborde le sujet, 2,5 % déclarent les transmettre s’ils se sentent en confiance et enfin 3,6 % déclarent qu’ils ne transmettront sous aucun prétexte leurs informations médicales au médecin du travail. Les salariés sont 46,2 % à penser qu’ils sont obligés de délivrer toutes les informations concernant leur santé au médecin du travail et 10 % pensent même pouvoir être sanctionnés par leur employeur s’ils ne délivrent pas toutes ces informations au médecin. Un lien statistiquement significatif a été mis en évidence entre la transmission de l’information médicale du salarié au médecin du travail et le fait que le salarié connaisse le médecin avant la visite. Il y a par ailleurs une influence négative statistiquement significative entre la transmission de cette information et le fait que les salariés craignent que le secret médical soit trahi ; en sachant que 10,3 % craignent que le secret médical soit trahi. Par contre nous n’avons pas montré de lien statistiquement significatif entre le type de visite médicale réalisée et la transmission de l’information médicale. Enfin, il n’y avait pas non plus de lien entre la confiance envers le médecin du travail et le fait de connaître le médecin. La transmission de l’information médicale par le salarié est influencée par le lien de confiance entre le salarié et le praticien, indépendamment de la connaissance préalable du médecin et du type de visite médicale.
Introduction En santé travail, les visites médicales sont souvent imposées aux salariés dans le cadre du suivi de santé des travailleurs. Ces visites médicales se déroulent dans un objectif de prévention et pour des salariés qui n’ont pas forcément une dégradation de leur état de santé. Objectif L’objectif de ce travail était donc de mettre en regard les attentes et les craintes des salariés lors de la visite périodique et la perception qu’en ont les médecins du travail. Méthodologie Distribution d’un auto-questionnaire aux salariés venant en visite médicale périodique au sein d’un service inter-entreprises de santé au travail de la métropole Lilloise et d’un questionnaire aux médecins de ce même service (enquête réalisée entre avril et juin 2016). Résultats Nous avons recueilli 359 questionnaires de salariés et 39 de médecins. Concernant les salariés, les trois principales attentes lors de la visite périodique sont : obtenir l’avis d’aptitude (81,3 %), être apte sans restriction médicale (69,2 %) et enfin obtenir des conseils de prévention (43,7 %). Les médecins eux, pensent que les attentes principales des salariés lors de cette visite concernent l’échange sur leurs difficultés au travail (94,9 %), l’obtention de conseils de prévention (87,7 %) et des restrictions médicales adaptées à leur état de santé (87,7 %), et enfin l’obtention de l’avis d’aptitude (84,6 %). Il est intéressant d’observer que les médecins pensent majoritairement que les salariés souhaiteraient parler de leurs difficultés au travail alors qu’ils ne sont que 24,2 % des salariés à répondre ainsi (p<0,01). Concernant les craintes, il peut être noté que les salariés déclarent globalement peu de craintes. Les salariés placent en tête le fait de craindre que le médecin leur découvre une pathologie (12,1 %) alors même que les médecins placent cette potentielle crainte en dernier lieu (28,2 %). Les salariés placent en deuxième position leur crainte d’être inapte ou d’avoir des restrictions médicales (7,7 %). À l’inverse les médecins placent en tête comme éventuelles craintes des salariés le fait qu’ils craignent d’être mis inapte (59,0 %), puis la crainte de ne pas avoir de restrictions médicales adaptées (53,8 %). Conclusion Le décalage entre les attentes et les craintes des salariés et la perception des médecins sur celles-ci doit faire réfléchir sur le contenu du colloque singulier. La transmission d’un message adapté est nécessaire pour que la sensibilisation des salariés soit optimale.
