Background. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a definitive therapy for patients with different oncological and hematological diseases. The study of red cell chimerism is a crucial process for diagnosing transplant engraftment and functioning during the post-transplantation period. Aim. To assess the effect of immunohematological (АВО and HLA donor–recipient matching) and medical (conditioning regimens) parameters on the onset time of post-transplantation donor chimerism which is determined by RBC antigens. Materials & Methods. The study enrolled 54 patients at the Kirov Research Institute of Hematology and Transfusiology in Russia (25 female and 29 male patients) aged 3–60 years (median 32 years). All of them received allo-HSCT in 2013–2021. Acute leukemias were identified in 39 patients, 8 patients were reported to have malignant lymphoproliferative diseases, 3 patients had myeloproliferative neoplasms, and 4 patients were diagnosed with aplastic anemia. RBC antigens of donors and recipients were analyzed by gel hemagglutination using Bio-Rad (USA) reagents and equipment. Results. The onset time of donor chimerism depends neither on the degree of HLA donor–recipient matching, nor on conditioning regimen. Donor chimerism in recipients with major ABO-incompatibility occurs significantly later than in patients with minor АВО-incompatibility and ABO-identity. Conclusion. Monitoring of post-transplantation donor chimerism is an important diagnostic and prognostic tool to assess donor hematopoietic cell engraftment, hematologic recovery, graft rejection, and relapse of the disease. After allo-HSCT, first donor red cells occur in pairs with major АВО-incompatibility later than in pairs with minor АВО-incompatibility or ABO antigen compatibility. Other immunohematological and medical parameters do not affect the development rate of donor chimerism determined by RBC antigens.
Актуальность. Прямой антиглобулиновый тест (ПАГТ) предназначен для выявления фиксированных на поверхности эритроцитов антител и компонентов комплемента. Гелевые методики дают возможность дифференцировать класс и подкласс иммуноглобулинов при положительном ПАГТ, что позволяет изучить характер анемии и оценить риск возникновения иммунного гемолиза. Цель. Изучить частоту положительного ПАГТ у онкогематологических больных, установить класс и подкласс иммуноглобулинов, фиксированных на эритроцитах, оценить их участие в развитии гемолитических осложнений. Материалы и методы. Изучен дифференцированный ПАГТ у 393 онкогематологических пациентов клиники ФГБУН «Кировский НИИ гематологии и переливания крови ФМБА» в гелевом тесте с использованием реактивов и оборудования Bio-Rad (США). Результаты. Частота выявления положительного ПАГТ у онкогематологических больных варьировала при разных заболеваниях от 6,2 до 25,2 %, в общей группе она была 15,5 %. При острых лейкозах она составила 6,2 %, при миелодиспластическом синдроме — 6,3 %, хроническом миелолейкозе — 10 %, лимфоме Ходжкина — 11,9 %, хроническом лимфолейкозе — 15,4 %, неходжкинских лимфомах — 21 %, множественной миеломе — 25,2 %. У пациентов со множественной миеломой, острыми лейкозами, лимфомой Ходжкина, хроническим миелолейкозом положительный результат был обусловлен наличием IgG подклассов 2 и 4. У больных хроническим лимфолейкозом и неходжкинскими лимфомами на поверхности эритроцитов выявлены IgG подкласса 1, IgM и компоненты комплемента C3c, C3d. Показано, что наличие IgG2/IgG4 не сопровождалось клиническими и лабораторными признаками иммунного гемолиза, IgG1 вызывал разрушение эритроцитов в 50 % случаев, тогда как обнаружение компонентов комплемента C3c, C3d в 100 % случаев было связано с проявлениями гемолиза. Заключение. Положительный результат ПАГТ следует интерпретировать с учетом лабораторных и клинических данных. Дифференцированное исследование помогает прогнозировать развитие гемолитических осложнений у пациентов с онкогематологическими заболеваниями.
Background. Direct antiglobulin test (DAT) is used to identify erythrocyte-fixed antibodies and complement components. Gel methods are applied to differentiate immunoglobulin class and subclass in positive DAT, which allows to study the nature of anemia and assess the risk of immune hemolysis. Aim. To assess the rate of positive DAT in oncohematological patients, to determine class and subclass of erythrocyte-fixed immunoglobulins, and to evaluate their contribution in hemolytic complications. Materials & Methods. In 393 oncohematological patients at the Kirov Research Institute of Hematology and Transfusiology differentiated DAT was studied using gel test with BioRad (USA) testing sets. Results. The rate of positive DAT in oncohematological patients varied for different diseases from 6.2 % to 25.2 %, in the total group it was 15.5 %. It accounted for 6.2 % in acute leukemias, 6.3 % in myelodysplastic syndrome, 10 % in chronic myeloid leukemia, 11.9 % in Hodgkin’s lymphoma, 15.4 % in chronic lymphocytic leukemia, 21 % in non-Hodgkin’s lymphoma, and 25.2 % in multiple myeloma. In multiple myeloma, acute leukemia, Hodgkin’s lymphoma, and chronic myeloid leukemia patients the positive test was associated with IgG subclasses 2 and 4. In chronic lymphocytic leukemia and non-Hodgkin’s lymphoma patients IgG1 subclass 1, IgM and C3c, C3d complement components were detected on erythrocyte surfaces. It was shown that IgG2/ IgG4 detection was not accompanied by any clinical or laboratory signs of immune hemolysis, IgG1 was responsible for destruction of erythrocytes in 50 % of cases, whereas the detection of C3c, C3d complement components was associated with hemolytic manifestations in 100 % of cases. Conclusion. Positive DAT should be interpreted in light of laboratory and clinical data. Differentiated test helps to predict hemolytic complications in oncohematological patients.
