AIM:Despite its limitation, bag collection is still widely used for a preliminary urine screening test in non-toilet-trained children suspected of febrile urinary tract infection. A previous study conducted by our group raised the hypothesis that the absence of direct contact between urine and the perinea during urine collection could limit urine contamination by perineal flora. The aim of this study was to evaluate the impact of the patient's position during urine collection (upright standing position versus free position) on the rate of contaminated urine samples in non-toilet-trained children with suspected febrile urinary tract infection. METHODS:This prospective, randomized, controlled study took place in seven pediatric emergency departments. Two groups were compared: the intervention group (infants held in an upright standing position during urination) and the control group (free position during urination). RESULTS:Among the 800 pediatric patients randomized to the study, 124 had a urine culture, 60 in the intervention group and 64 in the control group. Among the 124 urine cultures, 12 (9.7%) were contaminated: eight (13.3%) in the intervention group and four (6.3%) in the control group (p = 0.1824). CONCLUSION:The results show that the patient's position does not have a significant impact on the quality of urine samples collected by bag.
BACKGROUND AND OBJECTIVESTherapeutic drug monitoring of mycophenolic acid can improve clinical outcome in organ transplantation and lupus, but data are scarce in idiopathic nephrotic syndrome. The aim of our study was to investigate whether mycophenolic acid pharmacokinetics are associated with disease control in children receiving mycophenolate mofetil for the treatment of steroid-dependent nephrotic syndrome.DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTSThis was a retrospective multicenter study including 95 children with steroid-dependent nephrotic syndrome treated with mycophenolate mofetil with or without steroids. Area under the concentration-time curve of mycophenolic acid was determined in all children on the basis of sampling times at 20, 60, and 180 minutes postdose, using Bayesian estimation. The association between a threshold value of the area under the concentration-time curve of mycophenolic acid and the relapse rate was assessed using a negative binomial model.RESULTSIn total, 140 areas under the concentration-time curve of mycophenolic acid were analyzed. The findings indicate individual dose adaptation in 53 patients (38%) to achieve an area under the concentration-time curve target of 30-60 mg·h/L. In a multivariable negative binomial model including sex, age at disease onset, time to start of mycophenolate mofetil, previous immunomodulatory treatment, and concomitant prednisone dose, a level of area under the concentration-time curve of mycophenolic acid >45 mg·h/L was significantly associated with a lower relapse rate (rate ratio, 0.65; 95% confidence interval, 0.46 to 0.89; P=0.01).CONCLUSIONSTherapeutic drug monitoring leading to individualized dosing may improve the efficacy of mycophenolate mofetil in steroid-dependent nephrotic syndrome. Additional prospective studies are warranted to determine the optimal target for area under the concentration-time curve of mycophenolic acid in this population.
Context. When a urinary tract infection is suspected, it is recommended to capture a midstream sample of urine for analysis, but this strategy is difficult to apply before successful toilet training. A cutaneous stimulation technique has been reported to be effective in provoking micturition in newborns.Objective. To evaluate the feasibility and the efficacy of this technique in a population of non-toilet-trained infants.Design and methods. Fifty infants were enrolled in this prospective non-controlled study in three pediatric emergency departments. The bladder was stimulated through suprapubic and paravertebral cutaneous stimulation as previously reported in newborns. The evaluation criterion was the proportion of patients from whom urine had been collected within the first 5 min of the procedure.Results. Forty-eight patients (21 boys) were evaluated. The procedure was successful in 27% of the cases overall, but reached 46% for children aged less than 3 months. Elevated weight was associated with failure of the procedure.Conclusion. Despite promising results in newborns, the cutaneous stimulation technique to provoke micturition appears to encounter limitations in older children. Nevertheless, the technique is an attractive alternative to urethral catheterization or suprapubic aspiration for infants younger than 3 months. (C) 2016, Elsevier Masson SAS. All rights reserved.
La résistance aux antibiotiques est un enjeu croissant de santé publique. Les souches multirésistantes d’hier (Staphylococcus aureus résistant à la méticilline [Sarm]) laissent aujourd’hui la place à de nouvelles bactéries multirésistantes (BMR) (entérobactéries sécrétrices de ß-lactamase à spectre élargi [EBLSE]) mais également aux plus redoutées bactéries hautement résistantes émergentes (BHRe). Alors que le développement de nouveaux antibiotiques se poursuit, l’émergence de ces résistances fait craindre le risque d’impasse thérapeutique. L’identification des mécanismes de résistances à l’origine de ces épidémies (Sarm, EBLSE, entérobactéries productrices de carbapénèmases [EPC], Enterococcus faecium résistantes aux glycopeptides [ERG]) s’avère crucial pour endiguer au plus tôt les transmissions croisées et limiter le mésusage des antibiotiques.Antibiotic resistance is a hot public health issue. Former MRSA gives the way to ESBL, and recently brand new CRE and VRE. New antibiotics are discovered rarely, and therapeutic impasses become real. Screening and identification of the resistance mechanisms (MRSA, ESBL, CRE,VRE) is essential to contain cross contamination and decrease antibiotic misuse.
C3 nephritic factor (C3NeF) has been described in association with membranoproliferative glomerulonephritis and is involved in 80 % of cases of dense deposit disease. C3NeF is an immunoglobulin G (IgG) autoantibody which binds to the complement component 3 (C3) convertase C3bBb, thereby inhibiting its decay and leading to massive C3 cleavage. Commonly associated with C3NeF are low C3 levels, decreased total haemolytic complement (CH50) and normal C4 levels. C3NeF patients often present with proteinuria, haematuria and high blood pressure. Evolution to end-stage renal disease is common. Treatment consists of steroids and/or immunosuppressants, with variable efficiency. Renal transplantation is marked by histological recurrence, leading to higher rates of allograft loss.
Myositis in childhood can occur under different conditions and with various aetiologies, juvenile dermatomyositis (jDM) being by far the most frequent entity. The exact diagnostic workup and precise assessment of muscular as well as extramuscular involvement of organs in these systemic autoimmune diseases are relevant for specific and adjunct treatment of complications. Many new insights have become available with respect to the pathophysiological concepts as well as modern diagnostic measures and therapeutic approaches. Autoantibody detection in the serum of children with myositis is one of the major novelties that has become widely used and that is indeed helpful for diagnostic and prognostic measures. The pathophysiological relevance of type I interferons in jDM has been studied intensively in the past years. jDM is now seen as an acquired interferonopathy and first therapeutic consequences have been drawn from this pathogenic finding with the use of Janus-kinase inhibitors for severe and not otherwise treatable children.
We report the case of a 13-year-old boy who had been treated since the age of 6 for moderate asthma. Except asthma, his past medical history was uneventful. The patient was referred for the sudden onset of bilateral leg edemas with peripheral purpuric lesions. Blood tests showed increased blood eosinophilia (9000/mm(3)) with no fever. The antineutrophil cytoplasmic antibodies (ANCA) were negative. The skin biopsy showed extensive ischemic subcutaneous necrosis related to necrotizing vasculitis. The general secondary symptoms occurred with multiorgan involvement (pulmonary infiltrates, peripheral neuropathy, gastrointestinal tract symptoms, and arthralgia). Genital infiltration was also noted. The child's general health was preserved. Neither cardiac nor renal involvement were found. The patient showed favorable clinical progression after oral prednisone therapy.
Objectif Infection à CMV compliquée d’une tamponnade cardiaque chez un nourrisson. Sujet Nourrisson de 2 mois et demi hospitalisé pour fièvre et tableau biologique de micro-angiopathie thrombotique sévère ayant justifié une transfusion globulaire. Cliniquement il existait une hépatomégalie et une tendance hypotonique. L”échocardiographie initiale était normale. Une triple antibiothérapie a été mise en place et des gavages gastriques institués devant des difficultés alimentaires.L’apparition soudaine d’une polypnée, d’une pâleur et de bradycardies ont justifié la réalisation d’une radiographie thoracique qui a mis en évidence une cardiomégalie ainsi qu’un épanchement pleural. L’échocardiographie de contrôle a mis en évidence un volumineux épanchement péricardique avec une tamponnade cardiaque. Il a été réalisé en urgence une ponction péricardique d’un liquide séro-hématique de 60 ml. La PCR CMV est revenue fortement positive dans le sang, le liquide péricardique, le LCR et le liquide pleural. L’évolution a été favorable sous traitement antiviral sans récidive d’épanchement séreux. Conclusion Un épanchement péricardique, classiquement décrit dans l’infection à CMV, peut donc s’avérer massif et potentiellement se compliquer de tamponnade cardiaque.
Patiente de 2ans et demi lors d'un diagnostic de tubulopathie proximale sur une cytopathie mitochondriale de type Pearson-like avec anomalies biologiques majeures (Réserve alcaline 6 mmol/l, kaliémie 2mmol/l, phosphore 0,3 mmol/l, acide urique indosable). Atteinte digestive associée (vomissements, poussées de pancréatite), pas d'atteinte hématologique. Recours à une nutrition entérale sur gastrostomie du fait de vomissements incoercibles. Aucune amélioration biologique malgré l'administration de 12 mEq/kg/j de potassium et de 22 mEq/kg/j de bicarbonates initialement per os puis en parentérale continue du fait de vomissements persistants. Échec d'un traitement par AINS puis IEC. Persistance d'anomalies hydro-électrolytiques majeures, stagnation pondérale et développement progressif d'un rachitisme hypophosphatémique justifiant une néphrectomie unilatérale, sans effet sur le bilan biologique. La bi-néphrectomie et la dialyse ont corrigé le bilan hydro-électrolytique. Il existe un projet de greffe rénal. Il s'agit du premier cas décrit de tubulopathie proximale ayant justifié une bi-néphrectomie. Une tubulopathie proximale peut parfois être réfractaire et justifier d'un traitement radical.
Nous rapportons l'expérience pédiatrique Française de sept cas d'inflammation digestive survenus sous traitement par etanercept. Étude multicentrique rétrospective. Les patients inclus étaient traités par etanercept pour une arthrite juvénile idiopathique (AJI) et ont développé une inflammation digestive histologiquement prouvée sous ce traitement. Une relecture centralisée de toutes les biopsies digestives a été effectuée. Sept patients ont été inclus. Il a été retrouvé une inflammation digestive aspécifique (3 cas), ou une maladie de Crohn (4 cas). Une pancolite sévère a été notée chez 4 patients. Les signes digestifs comportaient : douleurs abdominales (n = 7), diarrhée (n =5), anorexie (n = 5), abcès anal (n = 2), ulcères buccaux (n = 1) et fébricule (n = 1). Une rémission clinique a été constatée chez tous les patients à l'arrêt de l'etanercept et mise en place d'un traitement spécifique de l'inflammation digestive. L'imputabilité de l'etanercept a été retenue sur (i) la survenue des symptômes après son introduction, (ii) l'absence d'antécédents digestifs (iii) l'amélioration clinique à l'arrêt du traitement. Une inflammation digestive doit être évoquée devant l'apparition de symptômes évocateurs survenant sous traitement par etanercept.
Nous rapportons l'observation d'un patient de 13 ans atteint d'un syndrome de Churg et Strauss (SCS) révélé par une atteinte cutanée prédominante. Patient de 13 ans traité pour un asthme modéré depuis la petite enfance, ayant brusquement présenté des œdèmes récidivants des membres inférieurs associés à une hyperéosinophilie sanguine. La constatation de nodules sous-cutanés violacés et de lésions purpuriques périphériques ont justifié la réalisation d'une biopsie cutanée qui a montré une nécrose hypodermique complète associée à une vascularite à éosinophiles. L'apparition secondaire de manifestations systémiques pulmonaires, neurologiques, articulaires, digestives et génitales a permis de poser le diagnostic de SCS. L'état général est resté conservé. L'évolution a été favorable sous corticothérapie. Le SCS est une entité pédiatrique rare. Il faut l'évoquer devant un asthme associé à une hyperéosinophilie et au moins un autre signe d'atteinte systémique. Le pronostic pédiatrique reste réservé. Le SCS, rare en Pédiatrie, doit être évoqué systématiquement en cas d'hyperéosinophilie. L'asthme et l'atteinte ORL ont été au second plan dans notre observation.
Patiente de 15 ans souffrant de douleurs lombaires droites invalidantes depuis un an sans cause organique retrouvé malgré de multiples consultations. Secondairement, une HTA sévère et une altération de l'état général ont mené au diagnostic d'une atrophie rénale droite ischémique (écho-doppler). L'angio-TDM abdominal a montré une thrombose artérielle rénale droite, surrénalienne droite, de multiples infarctus spléniques et une thrombose de la veine cave inférieure. La scintigraphie pulmonaire a révélé une embolie pulmonaire distale. Les anticorps anti-cardiolipides et anti-B2GP1 étaient significativement positifs. Le reste du bilan immunologique était normal. Une héparinothérapie intraveineuse a été initiée, relayée par AVK. Les douleurs lombaires ont régressé mais la patiente reste très hypertendue avec un recul de six mois. Le syndrome des antiphospholipides (SAPL) primitif est une entité rare en pédiatrie. Sa gravité est liée à la survenue de thromboses artérielles et veineuses. Dans le cas présent, il n'a pas été retenu de SAPL catastrophique. La présentation du SAPL peut mimer une pathologie chronique. L'imagerie et le bilan immunologique ont ici permis de redresser une errance diagnostique.
