Le but de cette étude est d’évaluer l’efficacité et la tolérance de la curiethérapie en haut débit de dose (HDR) par l’applicateur en anneau (Ring).
L’adénocarcinome du col utérin est de plus en plus fréquent.Objectif de l’étude – déterminer les caractéristiques de cette entité.Patientes et méthodes – Entre 1986 et 1992, 83 cas ont été colligés. C’est une étude rétrospective avec analyse univariée des facteurs pronostiques chez les malades suivies jusqu’au décès ou ayant un recul d’au moins cinq ans selon le test de χ2. La survie a été analysée selon le test de Logrank.Résultats – La fréquence est passée de 2,1 % en 1986 à 8,2 % en 1992 avec une médiane d’âge de 49 ans. Les stades localisés ont été retrouvés dans 51 cas. Le traitement a consisté en une association radiothérapie /chirurgie dans 66 cas et en une irradiation exclusive dans 17 cas, 73 ont été évaluables pour l’efficacité thérapeutique. Ainsi, le contrôle local a été obtenu dans 83 % des cas. L’analyse univariée des facteurs pronostiques du contrôle loco-régional a montré que le type histologique n’a pas été significatif. Par contre l’âge, la taille tumorale, le stade, le grade et l’envahissement ganglionnaire histologique ont été des facteurs significatifs. Ces mêmes facteurs ont influencé la survie à cinq ans en plus du degré de différenciation et l’envahissement ganglionnaire histologique ont été corrélés au taux de survie.Conclusion – Les facteurs de pronostic sont un diamètre de plus de quatre centimètres, un envahissement ganglionnaire pelvien, un envahissement microscopique des paramètres et des marges de résection envahies.The frequency of the adenocarcinoma of cervix uteri is increasing.Aim of the study – To define the characteristics of the entity.Patients and methods – Between 1986 and 1992, 83 adenocarcinoma of cervix uteri, were treated at the Ibn Rochd Oncology Centre in Casablanca. It’s a retrospective study with univariable analysis of the prognostic factors. This analysis was done for patients who were followed until the death or have 5 years or more follow up and the survival was calculated according the Kaplan & Meier method at 5 years. The local and regional recurrence factors were evaluated statically according the χ[sup2 ] and those for survival rate according the Logrank test.Results – The frequency increased during the period with 2,1 % in 1986 and 8,2 % in 1992. The median of aged was 49 years. Fifty one patients have localised stage. Combined radiotherapy and surgery was performed in 66 cases and 17 received radiation alone. The loco-regional control was obtained in 83 % among 73 available patients for results. The univariable analysis showed that the histological type was not a significant factor. However, age, tumor size, stage, histological grade and lymph node involvement were significant factors. These factors, the degree of differentiation and the lymph node involvement were significant for the 5-years survival.Conclusion – The prognosis factors of the cervix uteri adenocarcinoma are, size tumor more than 4 cm, lymphe node involvement, microscopic involvement to the parameters and positives exicion marges.
Le sarcome stromal périductal est une tumeur fibroépithéliale maligne du sein extrêmement rare, longtemps confondue avec les tumeurs phyllodes. Cette confusion se justifie par son caractère histologique biphasique comprenant une double composante: épithéliale ou canalaire bénigne entourée par un stroma péricanalaire sarcomateux fait de cellules fusiformes. Actuellement, elle constitue une entité anatomopathologique distincte. La chirurgie large avec des marges saines est suffisante et constitue le seul traitement validé actuellement, l’intérêt d’un traitement adjuvant fondé sur la radiothérapie et/ou la chimiothérapie reste à démontrer. Le pronostic est marqué par les récidives locales parfois sous forme de tumeurs phyllodes ou de sarcomes type parties molles plus agressifs, ce qui impose une surveillance étroite fondée sur l’examen clinique.
Diamond-like coatings have very good mechanical properties and present a real area of interest in studying their tribological behaviour. However, their friction coefficient is strongly affected by gaseous environments. Under a vacuum, when the coating contains hydrogen, the friction coefficient is lower than that in ambient air. To explain the lubricating mechanism of the gases on the surface, we have experimentally studied the effect of active gaseous species, such as atomic and molecular oxygen, hydrogen, nitrogen and water vapour, on the frictional behaviour in relation to the presence of hydrogen in the substrate. A tribochemical interpretation is proposed in connection with the temperature of the contact asperities.
Background. - Carotid paragangliomas are rare. Surgical resection is the primary treatment. However, when surgery is impossible, radiation therapy is an alternative potentially enabling local control with less morbidity. We report a case of good local control of an unresectable paraganglioma after external beam radiation.Case report. - A 72-year-old-woman, had a 4-year history of right facial palsy associated with pulsatile tinnitus, episodic ear bleeding and ipsilateral hearing loss. Physical examination revealed a retro-mandibular and sub-mastoid pulsatile mass. Magnetic resonance imaging showed a large carotid paraganglioma involving the temporal bone. Since surgical resection was impossible, our patient was given external beam radiation therapy at a dose of 60 Gy. At 12 months follow-up, local control was good without significant toxicity.Conclusion. - External beam radiation therapy seems to be a good alternative therapy for local control of carotid paragangliomas if surgical resection is impossible. Décrire les paramètres épidémiologiques, cliniques et l’association au virus du VIH-sida des carcinomes épidermoïdes conjonctivaux au service d’ophtalmologie du CHU de Treichville.Il s’agit d’une étude transversale prospective et descriptive réalisée au service d’ophtalmologie du CHU de Treichville de juillet 2007 à juin 2011. Vingt-six (26) patients inclus présentaient une tumeur du globe oculaire et ou des annexes. Une imagerie (scanner) a été pratiquée dans les formes évoluées. L’examen histopathologique de la pièce opératoire ou biopsique dans tous les cas a été réalisé. La sérologie VIH a été pratiquée après un consentement éclairé. Tous les patients ont été adressés au service d’oncologie du CHU de Treichville où existe une unité de prise en charge des sujets séropositifs atteints d’affections tumorales malignes. L’analyse statistique a été réalisée à l’aide du logiciel EPI info version 6.0.On note une légère prédominance du sexe féminin, 15 (57,69 %) contre 11 de sexe masculin (42,3 %). L’âge moyen était de 42,32 ans et 13 patients (50 %) avaient un âge compris entre 25 et 45 ans. Les tumeurs intraorbitaires étaient le motif de consultation le plus fréquent, soit 38,46 %. Dix-sept patients (65,38 %) n’avaient aucun antécédent particulier. Neuf patients (34,61 %) étaient déjà sous traitement antirétroviral à leurs admissions. Trois patients, soit 11,54 %, avaient déjà bénéficié de biopsie tumorale et ou d’une éviscération. La durée moyenne d’évolution avant consultation en ophtalmologie était de 14,28 mois. Les tumeurs conjonctivolimbiques localisées ont été retrouvées dans 11 cas (42,38 %.) Les tumeurs évoluées avec extension régionale et ou cérébrale représentaient 34,61 % des cas. Six patients (23,078 %) avaient un statut sérologique (VIH-sida) négatif contre 20 (76,92 %) patients séropositifs. Le typage de lymphocyte CD4 a été réalisé chez 17 patients, soit 85 % et 45 % avaient un taux de CD4 inférieur à 200. Cinq patients seulement ont pu bénéficier d’une chimiothérapie néo-adjuvante dont 3 décès. Le délai moyen de suivi était de 29 mois.Cette étude a permis d’établir un lien statistiquement significatif entre la séropositivité au virus VIH-sida des patients et la survenue de carcinome épidermoïde, d’une part (p = 10−4), l’effondrement du taux de CD4 et la progression de la maladie (p = 10−3), d’autre part.Le mauvais pronostic du carcinome conjonctival témoigne du faible niveau socioéconomique des patients, la sous-médicalisation de nos structures sanitaires, le coût prohibitif du produit anticancéreux ainsi que le lien entre cette affection et VIH-sida.To describe the epidemiological and clinical parameters of conjunctival squamous cell carcinomas and their association with HIV-AIDS in the ophthalmology department of the university hospital of Treichville.This is a prospective and descriptive cross sectional study carried out in the ophthalmology department of the university hospital of Treichville from July 2007 to June 2011. Twenty-six (26) patients who were selected presented with a tumor of the globe and/or adnexa. A CT scan was performed in advanced cases. Histopathological examination of the surgical specimen was performed in all cases. HIV serology was performed after informed consent. All patients were referred to the oncology department of the university hospital of Treichville where a unit of care for HIV-positive patients with malignant tumors is available. Statistical analysis was performed using the software program EPI info version 6.0.We note a slight predominance of female patients 15 (57.69%) vs. 11 (42.3%) male patients. The mean age was 42.32 years, and 13 patients (50%) were between 25 and 45 years of age. Intraorbital tumors were the most common reason for consultation, i.e. 38.46%. Seventeen patients (65.38%) had no particular history. Nine patients (34.61%) were already on antiretroviral treatment on admission. Three patients, i.e. 11.54%, had already had tumor biopsy and/or evisceration. The mean duration of the disease before consultation in ophthalmology was 14.28 months. Localized limbal conjunctival tumors were found in 11 cases (42.38%). Advanced tumors with regional or cerebral extension accounted for 34.61% of cases. Six patients (23.078%) were HIV negative vs. 20 (76.92%) HIV positive patients. CD4 lymphocyte typing was performed in 17 patients (85%), and 45% had a CD4 count below 200. Only five patients were able to receive neoadjuvant chemotherapy, but three of them died. The average length of follow-up was 29 months.This study allowed us to establish a statistically significant link between HIV positive status of patients and the occurrence of squamous cell carcinoma, (P = 10−4), as well as the decline in CD4 count and disease progression (p = 10−3).The poor prognosis of conjunctival carcinoma reflects the low socioeconomic status of patients, physician under-staffing in our health care facilities, the prohibitive cost of anticancer drugs and the link between this disease and HIV-AIDS.
Le cancer du rhinopharynx est une indication privilégiée de radiothérapie conformationnelle avec modulation d'intensité. En raison du fort gradient de dose obtenu, la délimitation précise des cibles et des structures critiques est l'un des principaux facteurs de réussite clinique avec cette technologie. Ce constat impose une bonne maîtrise de la radioanatomie de cette région et des techniques d'imagerie optimales.Rhinopharyngeal cancer is one of the best indications for conformal radiotherapy with modulated intensity. Due to the high dose gradient, accurate delineation of target volumes and organs at risk is a critical success factor with this technology. This requires a good knowledge of rhinopharyngeal radioanatomy and optimal imaging techniques.
5527 Background: Oropharyngeal candidiasis (OPC) is the most frequently occurring infection in head and neck cancer (HNC) patients undergoing radiation therapy (RT). Methods: Patients (pts) who had completed RT for HNC at least 6 weeks before entering the study, with OPC confirmed by direct exam or fungal culture were included in 27 French and 9 North African centers. Pts with disseminated candidiasis, having received systemic antifungal agents within less than 14 days were excluded. Group A received the bioadhesive buccal tablet containing 50mg of miconazole (MBBT) qd and group B a buccal gel (MBG) containing 125mg of miconazole q.i.d. for 14 days. A sample size of 103 pts per arm was required to obtain a 90% power to demonstrate non-inferiority of MBBT compared with MBG (α level of 5%, δ of 20%, 2 one sided). Primary endpoint was success at D14, defined as complete (CR) or partial (PR) clinical response. Response was based on the score of oral lesion (0 = none, 1 = single localised, 2 = multiple localised, 3 = extensive or confluent). Response was assessed by the investigator and a blind evaluator. Results: The 282 pts (A=141, B=141) in the modified intent-to-treat (MITT) group included all pts receiving at least one day of treatment and with one efficacy data point. The per-protocol (PP) population was composed of 213 pts with a validated, blinded evaluation of the primary endpoint (A=106, B=107). Null hypothesis that MBBT is inferior to MBG is rejected at the 2.5% level of significance. Conclusions:The efficacy of the Bioadhesive Buccal Tablet (miconazole Lauriad) is achieved with 10 times less (50 mg vs 500 mg) miconazole and on a more convenient schedule (once vs four times per day) than the gel, suggesting the potential for better patient compliance in treating OPC in pts with altered mucosa and xerostomia. The authors report a case of a 61-year-old woman presenting a primary cervical non Hodgkin's lymphoma, FIGO stage IIIA and Ann Arbor stage IE. The patient was successfully treated by combination of multiple drug therapy and pelvic external radiation therapy. Twenty-eight months later, the patient is in good general health and complete remission.
