In den vergangenen 10 Jahren hat sich die Bewertung der Rolle von Vitaminen und Spurenelementen in der Primärprävention urologischer Tumoren gravierend verändert. Es ist bislang nicht gelungen, den Nachweis für die Wirksamkeit einer Einzelsubstanz zu erbringen. Entsprechende Empfehlungen sollten daher heute nicht mehr gegeben werden. Es gibt jedoch gute Hinweise darauf, dass maßvolle Ernährung, eine Reduktion des Verzehrs von Milchprodukten, eine asiatische oder mediterrane Ernährung neben Effekten auf den allgemeinen Gesundheitszustand auch der Entstehung von urologischer Tumorerkrankungen und insbesondere dem Prostatakrebs (PCA) vorbeugt. Zwar lassen sich diese Befunde nur schwer quantifizieren, es ist aber offensichtlich, dass sich mit diesen Maßnahmen erhebliche synergistische Effekte im Bereich der kardiovaskulären Erkrankungen erzielen lassen. Nachdem insbesondere beim PCA ein großer Teil der Patienten nicht an der Tumorerkrankung verstirbt, sollte gerade die Primärprävention dieses Tumors daher stets in Zusammenhang mit der allgemeinen Gesundheit der Zielgruppe gesehen werden. In der Sekundärprävention weisen neuere Studien auf eine mögliche Wirksamkeit von Nahrungsergänzungsmitteln auf das biochemische Rezidiv eines PCA nach definitiver Therapie hin. Hier steht nun eine Validierung der Ergebnisse in kontrollierten klinischen Studien an.

Urol. 2012; 43 (Editorial Comment) ▼ Sehr geehrte Damen und Herren, in seinem Editorial zu unserer Übersichtsarbeit zur Chemoprävention des Prostatakarzinoms fasst Prof. Dr. Peter Hammerer die kontroverse Diskussion bezüglich einer Chemoprävention mit 5α-Reduktase-Hemmern zusammen und weist insbesondere auf die Argumente der Kritiker eines solchen Vorgehens hin [ 1 ] . Die Autoren der Arbeit bedanken sich an dieser Stelle dafür, dass der in der o.g. Arbeit geäußerte Wunsch nach einer off enen Diskussion zu diesem schwierigen Thema durch die Herausgeber der Aktuellen Urologie aufgegriff en worden ist. Die Arbeit hat damit eines ihrer wesentlichen Ziele erreicht. Wir möchten mit diesem Schreiben nochmals darauf hinweisen, dass es an der Zeit ist, die Debatte zur Frage eines erhöhten Risikos von Gleason-Score 7–10-Tumoren unter 5α-ReduktaseHemmern zu einem Abschluss zu bringen. Wir sollten akzeptieren, dass in einer Prostata mit einem um rund 25 % verminderten Volumen ein Tumor früher erkannt wird als in einer größeren Prostata [ 2 ] . Diese Folgerung ist so logisch, dass eine Bestätigung durch eine Phase-III-Studie nicht erforderlich ist. Man könnte höchstens der Studienleitung von PCPT oder REDUCE den Vorwurf machen, diesem Eff ekt im Studiendesign nicht Rechnung getragen zu haben. Die konsequente Fortführung dieses Gedankens führt aber unmittelbar zu der Frage, ob die Risikoreduktion bei gut diff erenzierten Tumoren durch 5α-Reduktase-Hemmer vielmehr nicht sogar unterschätzt wird. Gemeinsam mit Prof. Hammerer halten auch wir eine Risikoreduktion von low-grade-Tumoren für klinisch relevant: Etliches der teilweise berechtigten Kritik in Bezug auf eine Überbehandlung von niedrig-Risiko-Patienten würde sich für Patienten unter 5α-Reduktase-Hemmern erübrigen – und umgekehrt könnte ein früherer Nachweis von Risikotumoren die Prognose der betroff enen Patienten günstig beeinfl ussen. Prof. Hammerer weist auf die Ablehnung des Zulassungsantrages für Dutasterid zur Prävention des Prostatakarzinoms hin. Die Autoren der Arbeit vertreten den Standpunkt, dass eine solche Ablehnung nichts an der tatsächlichen Datenlage ändert. Sie möchten darauf hinweisen, dass auch die Empfehlungen hochkarätiger Kommissionen wie der Food and Drug Administration (FDA) oder auch des Deutschen Instituts für Qualitätssicherung und Wirtschaftlichkeit im Gesundheitswesen (IQWiG) keinen Anspruch auf die absolute Wahrheit haben und daher kritisch hinterfragt werden können und müssen. Es ist sicherlich richtig, dass bislang keine Untersuchungen zum Einfl uss von 5α-Reduktase-Hemmern auf Metastasierung, Morbidität und Prostatakarzinom-spezifi sche Mortalität vorliegen. Wir möchten hinzufügen, dass auch Längsschnittuntersuchungen zur Lebensqualität ausstehen. Die bereits vorhandenen Kohorten aus PCPT und REDUCE sollten aber zu diesen Fragen weiter untersucht werden. Prof. Hammerer beweist mit seinem abschließenden Statement viel Mut. Er zeigt einen Weg auf, der einerseits medizinische Evidenz aber ebenso vorhandene juristische Aspekte berücksichtigt. Wir wissen, wie wenig praktikabel das beschriebene Vorgehen ist – es wird sich nur für Einzelfälle eignen –, aber die Priorisierung wissenschaftlicher Erkenntnisse gegenüber formalen Aspekten stellt einen wichtigen Schritt auf dem noch langen Weg zur praktischen Chemoprävention des Prostatakarzinoms dar.
Eine hohe Prävalenz, das bevorzugte Auftreten in höherem Alter, eine häufig langsame Progression und hohe Behandlungskosten machen die Diagnostik und Therapie des Prostatakarzinoms zu einer besonderen Herausforderung. Eine wirksame Prävention könnte etliche der genannten Probleme bessern. Zwei randomisierte, kontrollierte Studien belegen, dass eine wirksame Chemoprävention des Prostatakarzinoms mit einem 5α-Reduktase Hemmer (Finasterid, Dutasterid) tatsächlich möglich ist. Daneben existieren auch Evidenzen für weitere Substanzen aus der Gruppe der selektiven Östrogenrezeptor Modulatoren (SERMs), NSAIDs und Statine. Die vorliegende Übersicht untersucht die Risiken und Vorteile einer Chemoprävention nicht zuletzt auch unter gesundheitsökonomischen Aspekten. Die Autoren kommen zu dem Schluss, dass die Optionen einer Chemoprävention ergebnisoffen geprüft werden sollten.

Eine der zurzeit attraktivsten Methoden der unkonventionellen, komplementären Medizin ist zweifelsohne die Magnetfeldtherapie. Urologischerseits konnte sie sich bisher nicht etablieren, wenngleich vereinzelt schon mit der heilsamen Wirkung des Magnetfelds geworben wird. Will man sich vorurteilsfrei mit dieser Materie befassen, so müssen die zugrunde liegenden physikalischen Gesetze verstanden und die vorliegenden Daten sachlich überprüft werden. Den Bereich der medizinischen Seriosität zu erkennen, zu beschreiben und gegen Mummenschanz wie Magnetarmbänder abzugrenzen, ist eine Zielsetzung dieses Artikels.
In locally advanced or recurrent tumors of the female genital tract anterior or total exenteration may be mandatory in case of tumor invasion into the lower urinary tract or if a second course of radiation therapy is not feasible. The management of resection and reconstruction of the affected lower urinary tract has to be well integrated into the gynecological therapeutic concept. In 11/32 patients the reconstruction of the partially resected lower urinary tract was feasible with preservation of a functionally intact urinary bladder. Urinary diversion following pelvic exenteration was achieved in 13/17 patients with a continent urinary reservoir and in 4/17 patients with an ileal conduit. Operative reinterventions were needed only in patients with continent urinary diversion in 5 cases. All these patients had a past history of primary radiation therapy of their gynecological tumor. In the remaining other 11 patients with a history of primary radiation therapy no complications occurred. 9 of 32 patients survived the operative procedure 40.8 (25-57) month with no evidence of recurrent tumor. Continent urinary diversion represents an excellent therapeutic option for replacement of function lost due to exenterative pelvic surgery. Stringent selection of patients is mandatory to consider the presented therapeutic concept a reasonable tool in the management of the described clinical situations.

Carcinogenesis results from irreversible alterations of the physiological balance between protooncogene and tumor suppressor genes. In this study simultaneous investigation of alterations of the c-myc oncogene and the p53 tumor suppressor gene during development and progression of bladder cancer was performed. 154 paraffin-embedded urothelial specimens were examined. Normal urothelium (n=16) yielded no alterations. Low-grade (G1) papillary tumors (n=10) showed no p53 accumulation, but c-myc overexpression in 6 cases. In 3/12 dysplasia specimens p53 accumulation was detected, while c-myc overexpression was observed in only one case. In carcinoma in situ, c-myc overexpression (7/39) was also less frequent than p53 accumulation (12/39). The incidence of p53 accumulation was slightly higher in muscle-invasive tumors (T2-3) (n=14) than in T1 (n=24) bladder cancer. In contrast, the number of tumors with c-myc overexpression increased up to 70% in muscle-invasive tumors. A significant correlation between p53 accumulation and tumor progression was observed (p<0.0001). The simultaneous analysis of both genes yielded specific patterns for the different tumor stages. Extension of this study could provide the base for a new classification of bladder cancer based upon the molecular biology of this tumor entity.
Angiocentric lymphoma, which is the malignant counterpart of angiocentric immunoproliferative lesions, comprises a rare group of nonHodgkin's lymphomas of T-cell origin. It is characterized by marked invasion and destruction of small vessels by lymphomatoid cells, predominantly in the lungs. The prognosis is poor and many patients die within several months. To our knowledge primary involvement of the genitourinary tract has not been previously reported. We report a case of a solitary primary angiocentric lymphoma of the kidney in a patient with the acquired immunodeficiency syndrome. Therapy consisted of nephrectomy without adjuvant treatment. Histological characteristics as well as diagnostic and therapeutic options are discussed.
Transurethral resection of the prostate (TURP) and open adenectomy are regarded the "golden standard" in the treatment of benign prostate hyperplasia (BPH). Various alternative treatment forms (microwaves, laser, radio frequency, focused ultrasound) have been introduced recently. They are all aimed at reducing the morbidity related to TURP while achieving comparable efficacy at the same time. Since December 1992, 35 patients with BPH have been treated with high-intensity focused ultrasound (HIFU-P) in our department. By 6 weeks after HIFU-P, mean Q(max) had improved from 6.5 ml/s to 12.1 ml/s. Residual volume dropped from 207 ml to 95 ml, IPSS from 20.1 to 9.7. Both subjective as well as objective improvements remained stable after 3 and 6 months. Urinary tract infections were observed in 3 patients, macrohemato-spermia in all, and macrohematuria (caused by the suprapubic catheter) requiring blood transfusion in 1 patient.

Since December 1992, 50 patients with BPH have been treated by high intensity focused ultrasound (HIFU-P) at our department. Six weeks following HIFU-P mean Qmax improved from 5.7 ml/s to 11.6 ml/s. Post voiding residual volume (RV) dropped from 215 ml to 100 ml, the International Prostate Symptom Score (IPSS) from 19.8 to 9.9. Both, IPSS and RV further improved during the following weeks. Follow-up data one year after treatment demonstrate that results remained stable in the majority of patients. Urinary tract infections were observed in 3 patients, macrohematospermia in all and macrohematuria (caused by the suprapubic catheter) requiring blood transfusion in 1 patient.
The effect of the tumor suppressor p53 on urothelial carcinoma cells was studied by transfecting six cell lines containing different mutations in the p53 gene with an expression construct for the wild-type protein. In all cell lines, the number of cell clones resistant to a neomycin analogue was strongly diminished when pCMVhup53 was cotransfected with the resistance plasmid pRSVneo as compared to cotransfection with either a plasmid vector, a p53 deletion and a mutant p53 expression vector. Cytochemical analysis showed that cells cotransfected with pCMVhup53 and an expression plasmid for beta-galactosidase disappeared during the second day after transfection. Thus, reexpression of wildtype p53 efficiently and rapidly kills urothelial carcinoma cells, independent of the different mutations in p53 they contain.
The human bladder carcinoma cell line RT112 and the mitomycin C-resistant cell line RT112MMC, derived from RT112 cells, were examined for their expression of NAD(P)H:quinone oxidoreductase (NQOR) and glutathione S-transferases (GSTs). RT112 cells were 40-fold more sensitive towards mitomycin C than RT112MMC cells. The NQOR mRNA level in RT112MMC cells was decreased to 15% as compared to RT112 cells. NQOR enzyme activity was 391 +/- 140 mU/mg protein in RT112 cells, whereas NQOR activity in RT112MMC cells was not measurable. As shown by a fast PCR-based assay and DNA-sequencing, the cell line RT112 is heterozygous, whereas RT112MMC is homozygous for a null allele of the NQOR gene without enzymatic activity. Accordingly, both wild-type and null allele mRNAs were present in RT112 cells, whereas only null allele mRNA was found in RT112MMC. The lack of NQOR enzyme activity in RT112MMC cells was thus associated with loss of heterozygosity at the NQOR locus. By a PCR-RFLP assay, three kidney carcinoma patients without measurable NQOR enzyme activity were shown to be homozygous for the null allele. The PCR assay described here is useful for examination of large numbers of samples. The relative amount of GST-Pi mRNA was decreased by 30% in RT112MMC as compared to RT112, contributing to a diminished level of GST enzyme activity, using CDNB as a substrate, from 95 +/- 62 mU/mg protein in RT112 to 26 +/- 6 mU/mg protein in RT112MMC.
