Acute lymphoblastic leukemia (ALL) is an aggressive hematologic malignancy of lymphoid progenitor cells. During treatment of ALL, the patients may be complicated by fatal infections such as sinonasal mucormycosis. In patients with immunocompromised conditions like haematologic malignancies, mucormycosis can be deadly. Once mucormycosis has been detected, it is critical to act quickly to begin treatment. Amphotericin B, posaconazole, or isavuconazole are the medications of choice. Effective treatment requires a long-term course of therapy. Switching to other effective medications should be considered when the preferred treatment is not working. The factors contributing to mucormycosis must also be treated, such as the underlying conditions (haematologic malignancies). Herein, we aimed to discuss a clinically stable ALL patient complicated with extensive sinonasal mucormycosis. She improved with amphotericin B (5 mg/kg/day) and oral posaconazol (300 mg/day) treatment.
İlk oral proteazom inhibitörü (PI) olan iksazomib, lenalidomid ve deksametazon (IRd) ile kombinasyon halinde relaps refrakter multipl miyelomun (RRMM) tedavisi için onaylanmıştır. Bununla birlikte, klinik çalışma sonuçları her zaman gerçek dünyadaki sonuçlarla örtüşmez. Bu çalışmanın amacı, gerçek yaşamda RRMM'li hastaların tedavisi için iksazomib bazlı kombinasyon tedavisinin sonuçlarını değerlendirmektir. Çalışmaya Bursa Uludağ Üniversitesi Tıp Fakültesi Hematoloji Bilim Dalı’ndan toplam 45 RRMM tanılı hasta retrospektif olarak dahil edildi. Medyan takip süresi 15.9 ay (0.3-53.8) ve medyan yaş 66 (40-84) idi. Hastaların %8.8’i 2. sırada, % 15.5’i 3. sırada, %35.5’i 4. sırada ve %40’ı da ≥5. sırada iksazomib aldı. Genel olarak, IRd’den önce hastaların %100’ü PI (bortezomib %100, karfilzomib %31.1), %88.8’i immünomodülatör ilaç (IMID) (lenalidomid %86.6, pomalidomid %33.3, talidomid %4.4) almış ve %55.5’ine de otolog kök hücre nakli yapılmıştır. Hastaların hepsi lenalidomid ve deksametazon ile kombinasyon halinde iksazomib almıştır. Tedavi, yan etki nedenli düşük bir kesilme oranıyla (%6.6) iyi tolere edilmiştir. En sık görülen yan etkiler sitopeni (%50) ve enfeksiyon (%25) olmuştur. Genel yanıt oranı %77.7, çok iyi kısmi yanıt oranı %20, tam yanıt oranı % 28,5 kısmi yanıt oranı %11.4, minimal yanıt oranı %5.7, stabil hastalık oranı %5.7, progresif hastalık oranı %28.5 saptandı. İksazomib tedavisinde medyan progresyonsuz sağkalım (PFS) 29.1 aydı (%95 GA 17.2–40.9). PFS 12 ve 24 ayda sırasıyla %75 ve %59 idi. Medyan genel sağkalım (OS) 22.6ay (%95 GA 18.8–40.9) idi; OS 12 ve 24 ayda sırasıyla %73 ve %49 idi. Gerçek yaşamda RRMM hastalarında IRd etkili, güvenli ve oral verilebilmesi nedeniyle kolay uygulanabilir bir rejimdir.
The impact of inflammatory markers such as systemic immune-inflammation (SII) index and systemic inflammation response index (SIRI) on myelofibrosis (MF) prognosis was evaluated for the first time in this study. Data from 60 patients diagnosed with MF between March 2011 and September 2022 were retrospectively analyzed. In addition to disease-related markers, the impact of SII and SIRI on prognosis was evaluated. In our study, the overall median survival (OS) was 64 months. OS was significantly shorter in patients older than 65 years, with high ferritin and lymphocyte levels, transfusion dependence at diagnosis, platelet count below 100 × 10 9 /L, Hb level below 8 g/dl, and high risk according to the dynamic international prognostic scoring system (DIPSS)-Plus score. When these variables were included in the multivariate Cox regression model, it was found that being older than 65 years, having a high ferritin value, being at high risk according to the DIPSS-plus score and Hb values below 8 increased the risk of death. Platelet-to-lymphocyte ratio (PLR) and SII index were lower in patients with a fatal outcome. No statistically significant relationship was found between SIRI and mortality. The findings of this study showed that low PLR and high ferritin were associated with poor prognosis in MF. Elevated SII and SIRI, evaluated for the first time in patients with myelofibrosis, did not predict prognosis. Since non-inflammatory variables play a role in the pathogenesis of MF, bone marrow indicators and systemic inflammation indicators derived from hematologic parameters may not be accurate.
Topic: 6. Chronic lymphocytic leukemia and related disorders - Clinical Background: Chronic lymphocytic leukemia (CLL) is the most common adult leukemia accounting for 25-30% of all the leukemias. It originates from a mature B cell with either mutated or unmutated immunoglobulin heavy chain variable region (IGHV) transcripts. A better understanding of the molecular basis and IGHV mutations status of CLL allows targeted therapies as a treatment options. Aims: We aim to present real-life data emphasizing the clinical features of our patients with immunoglobulin heavy chain variable region unmutated (IGHV -U) Methods: This multi-center, retrospective study included CLL patients with known IGHV mutational status. Ninety patients with available clinical data out of 114 patients who had IGHV mutation outcomes were included in our study. We evaluated IGHV mutation status by RNA-seq providing high resolution and high dynamic range transcription data. Results: The median age of the patients was 61 years(29-84), and 64.4% were male (Table 1). Forty-t wo patients (46.7%) had IGHV-U and 8 patients (8.9%) had both IGHV unmutated disease and 17p del. Fifty one patients 51 (56.7%) were treated during follow-up. Thirty-six percent of the patients received chemoimmunotherapy consisting of fludarabine, cyclophosphamide plus rituximab (FCR), or bendamustine plus rituximab (BR); 10% received chlorambucil in combination with anti CD20 antibodies or monotherapy; and 31% received Bruton’s tyrosine kinase inhibitor or B-cell lymphoma-2 (BCL-2) inhibitor with anti CD20 antibodies. In the treatment group 76.2 % IGHV unmutated disease and 62.5% of these patients were treated with novel agents. Median follow up time was 30.5 months (1-48). Median overall survival was 95.9% at 36 month in the cohort. Three-years OS for IGHV-M and IGHV-U was approximately 92.3 % and 100 %, respectively (p>0.5) (Figure1). No significant survival difference was detected in both groups during 36-months follow-up period.Summary/Conclusion: There were no difference in survival of patients with or without IGHV mutations. Our real life data suggested novel agents targeting B-cell receptor (BCR) pathway kinases or BCL-2 inhibitors can overcome the poor prognosis associated with unmutated IGHV status. IntroductionOur study investigated leukapheresis's effect on delayed induction therapy outcomes in patients with acute leukemia presenting with symptomatic hyperleukocytosis.MethodsThis retrospective cohort study included 30 adult patients diagnosed with acute leukemia who underwent leukapheresis for leukostasis. The patients were divided into the first 24 h and >24 h groups, according to the time from diagnosis to induction therapy (TDT).ResultsThere was no significant difference between the TDT groups regarding complete remission (CR), 4-week mortality, and overall survival (OS) at a median follow-up of 409 days. Tumor lysis syndrome, disseminated intravascular coagulation, and hemoglobin levels were significant in early mortality. In univariate analysis, age, hemoglobin levels, patients' eligibility for intensive chemotherapy, and achieving CR were critical factors for OS.ConclusionThe study findings suggest that waiting for the clinical and laboratory results may be a safe and reasonable approach before assigning patients the best treatment option with leukapheresis.
Background: Dapsone is a second-line therapy for immune thrombocytopenia (ITP). It is cost-effective, with a response rate comparable to other drugs used as second-line therapy, such as azathioprine, danazol, cyclophosphamide, cyclosporine, vincristine, rituximab, and eltrombopag. Material and Methods: This retrospective study analyzed ten adult patients who presented to our hematology division outpatient clinic between March 2013 and July 2021, was diagnosed with chronic/refractory ITP, did not respond to first-line therapy, and used dapsone. Results: Eight (80%) patients were female, and 2 (20%) were male. The median age was 50 (range, 24-64) years. The mean pre-treatment platelet value was 12.8x109/L (range: 4-22.1x109/L). The median duration of symptoms before dapsone treatment was 60 (6-360) months. The median number of treatments received before dapsone was 4 (range: 3-6). All patients were routinely treated with oral dapsone 50 mg for two weeks, followed by 100 mg. The median time to treatment response was 39 (range: 14-90) days. The response rate was 60% (complete response 40%, partial response 20%). Asymptomatic anemia was observed as a side effect in only one patient. Conclusions: Based on these results, it can be speculated that dapsone is an effective, inexpensive, and well tolerated treatment option. Considering the economic status of developing countries, it seems very attractive to use dapsone as the second-line therapy for chronic/refractory ITP. To the best of our knowledge, this is the first study in Turkey on the use of dapsone for chronic/refractory ITP.
INTRODUCTION:Thrombotic thrombocytopenic purpura (TTP) is a life-threatening occlusive disease of the microcirculation characterized by systemic platelet plugs, organ ischemia, deep thrombocytopenia, and fragmentation of erythrocytes. One of the widely used scoring system to determine the clinical probability of TTP is the PLASMIC scoring system. This study aimed to evaluate the contribution of PLASMIC score modifications to sensitivity and specificity in patients with microangiopathic hemolytic anemia (MAHA) undergoing plasma exchange with a prediagnosis of TTP at our center.MATERIALS AND METHODS:The data of patients who were hospitalized with a previous diagnosis of MAHA and TTP and underwent plasma exchange at Bursa Uludag University, Faculty of Medicine, Department of Hematology between January 2000 and January 2022 were retrospectively analyzed.RESULTS:Overall, 33 patients (including 15 and 18 with and without TTP, respectively) were included in this study. Receiver operating characteristic (ROC) analysis revealed that the area under the curve (AUC) for the original PLASMIC score was 0.985 (95% confidence interval [95% CI]: 0.955-1.000), and AUC for the PLASMIC score without mean corpuscular volume (MCV) was 0.967 (95% CI: 0.910-1.000), which is close to the original AUC. With the removal of MCV from the scoring system, the sensitivity decreased from 100% to 93%, whereas the specificity increased from 33% to 78%.CONCLUSIONS:Based on the results of this validation study, removing MCV from the PLASMIC score led to the categorization of eight non-TTP cases in the low-risk category, and this could avoid unnecessary plasma exchange. However, in our study increasing the specificity was at the expense of the sensitivity by missing one patient with this new scoring system without MCV. Further multicenter studies with large sample sizes are required owing to the fact that different parameters may be effective in TTP prediction among different populations.
Introduction The standard approach for allogeneic stem cell transplantation (allo-SCT) is to administer donor cells on the same day as a fresh product to a patient who has been given a preparative regimen. The difficulty in collecting and transporting donor cells, especially during the COVID-19 pandemic, has made it essential to collect and cryopreserve the grafts before the recipient begins the transplant preparation regimen. However, the short- and long-term impacts of cryopreservation on transplant outcomes remain controversial. Materials and methods This retrospective study included 93 patients who underwent allo-SCT between January 2012 and August 2022 at the Stem Cell Transplant Unit of Bursa Uludag University Faculty of Medicine using frozen and fresh products of peripheral blood stem cells from a fully matched sibling donor. The effect of cryopreservation of donor grafts on engraftment kinetics was investigated. Results Frozen and fresh products were used in 37 and 56 patients, respectively. The majority of patients had acute myeloid leukemia and acute lymphoblastic leukemia. The median age at transplantation was 41 years. Neutrophil engraftment time was similar between the two groups (median: 14 vs. 16 days, p = 0.393). Platelet engraftment time was longer in the frozen product group (median: 12 vs. 15 days, p < 0.001). There was no statistically significant difference between freezing time and viability. The acute graft-versus-host disease (GVHD) rate was 37.8 % in the frozen product group and 28.6 % in the fresh product group (p = 0.349). There was no significant difference between the two groups in terms of primary and secondary graft failure, chronic GVHD, 30-day chimerism, relapse, overall survival, progression-free survival, and nonrelapse mortality. Conclusion Having donor cells ready before transplantation significantly prevents donor-induced adverse events and provides confidence and practicality to both the clinician and the recipient. Allo-SCT with frozen products is a successful method that can be safely applied, especially when disruptions in donor-derived cell collection or transportation are foreseen.
While autoimmune hemolytic anemia (AIHA) is expected in non-Hodgkin’s lymphomas, it is less common in marginal zone lymphoma. Sixty-year-old male patient was followed up with hemolytic anemia in our clinic and while the etiology was investigated, scattered nodes in the mediastinum and abdomen compatible with lymphoproliferative disease were observed on CT, and positive detection of CD5, CD19, CD20, CD22, CD45, HLA-DR, FMC-7 on flow cytometry sent from peripheral blood and a diagnosis of low grade B cell lymphoma (MZL) was made with the result of bone marrow pathology. We found it appropriate to present our MZL case because of its response to steroid treatment and its rare presentation with AIHA.
Objectives: We aimed to evaluate the potential prognostic factors of patients with primary central nervous system lymphoma (PCNSL). Methods: Thirty-two patients with PCNSL were retrospectively analyzed. Results: All the patients received high doses of methotrexate-based chemotherapy as the first-line treatment. Overall survival was 30.0 ± 7.2 months. Those with partial response and without response had a higher risk of mortality. The increased leukocyte and neutrophil levels were associated with high mortality. Besides, the SIIL as a product of the systemic immune inflammation (SII) and lactate dehydrogenase (LDH); the SIRIL as a product of systemic immune response index (SIRI) and LDH; and the NLL as a product of neutrophil-lymphocyte ratio and LDH were taken into consideration for the first time for the purposes of the present study. Elevated NLL, SIIL, and SIRIL indexes were associated with mortality. Elevated SIIL level, radiotherapy, and partial and no response were the independent predictors of mortality on the basis of the multivariable regression model including the risk factors associated with mortality. Conclusions: SIIL, SIRIL and NLL are prognostic factors in PCNSL. Determining the prognostic factors and risk profile may predict the requirement for more intensive treatment, especially in young patients at high risk.

In cancer treatment, efficient therapeutic strategies could be impeded by cellular mechanisms such as the multidrug resistance. Recently, drug-loaded nanoparticles have been reported to be useful, since they allow entering the cancer cell and act as an intracellular anti-cancer drug reservoir. A new approach is proposed here by the use of lipid nanocapsules (LNC) which were hypothesized to reverse multidrug resistance additionally by their P-glycoprotein (P-gp) inhibiting surfactant. LNC (mean diameter 25 to 100 nm) were loaded with etoposide, tested for the drug release and their efficiency to reduce cell growth in cell culture for C6, F98, and 9L glioma cell lines. Sustained etoposide release can be provided over a period of 1 week (t10%: 1.4+/-0.1h; t50%: 15.9+/-2.8h). The P-gp inhibiting activity in-vitro was found to be independent from the LNC size. In cell culture, an internalization of LNC was observed in all glioma cell types. Etoposide LNC showed a generally higher efficiency than the drug solution while blank LNC were found to be less inhibitory than the pure drug at equivalent concentrations (IC50: C6: etoposide: 25.2 microM; LNC: 2.6-8.9 microM, F98: etoposide: 46.5 microM; LNC: 1.4-14.7 microM, 9L: etoposide: 58.2 microM; LNC: 4.4-12.7 microM). This effect was found to be particle size dependent within a range of an 8- (C6) to 33-fold (F98) increased cytotoxicity for smallest particles. When cells were incubated with etoposide solution in the presence of blank LNC, a slight growth inhibition was observed, however, distinctly lower than the drug-trapping particles. Moreover, cell toxicity on astrocytes was similar for etoposide LNC and etoposide solution. The mechanism of action of etoposide LNC was proposed to be a cell uptake followed by a sustained drug release from the LNC in combination with an intracellular P-gp inhibition ensuring a higher anticancer drug concentration inside the cancer cells.
Background and Objective Infections are the most common cause of anal and perianal pathologies in patients with hematological malignancies. Perianal infection diagnosis in this group of patients is difficult; thus, a careful anorectal examination is necessary with imaging modalities. In addition, the literature reveals a knowledge gap in the approach to anal pathologies in patients with neutropenia during diagnosis or chemotherapy. This study aimed to examine our institutional data on perianal complications and investigate the relationship between the white blood cell-neutrophil count, perianal lesion, and the type of treatment in patients with hematologic malignancies during the neutropenic period. Methods Patients with a hematologic malignancy, hospitalized for cytotoxic chemotherapy, complicated by perianal pathology, documented by at least one imaging method, were included in the study. Results A total of 42 patients were included in the study. Most of them had acute leukemia, 31 were affected by acute myeloid leukemia (AML), and 7 by Acute lymphoid leukemia (ALL). There was no statistically significant relationship between the anal abscess formation, the neutrophil count, and a previous perianal pathology. Anal abscess development was significantly more frequent in acute myeloid leukemia. An inverse relationship was found between the total white blood cell number at onset and having a surgical intervention for anal pathology. In conclusion, this article has shown that white blood cell count at the time of hospitalization can affect the surgical intervention in patients with hematological malignancy (in the majority with acute leukemia) affected by anal pathologies occurring in the neutropenic period.
Although anemia is common after kidney transplant, pure erythroid aplasia due to parvovirus B19 infection is rare. Therefore, there are delays in diagnosis in transplant patients. Here, we aimed to raise awareness that pure red blood cell aplasia due to parvovirus B19 should be considered in the differential diagnosis of posttransplant anemia. Our report analyzes 2 kidney transplant recipients under immunosuppressive therapy who were diagnosed with pure red blood cell aplasia due to parvovirus B19 infection. Both patients were examined for anemia as a cause for transfusion dependence. Normochromic, normocytic anemia, and reticulocyte levels were low. Leukocyte and platelet counts and biochemical parameters were within reference ranges. Therefore, pure red blood cell aplasia associated with parvovirus B19 was included in the differential diagnosis. Bone marrow showed erythroid hypoplasia and megaloblastic changes with giant erythroblasts containing dark-stained inclusion structures. Results from the other series (neutrophils, lymphocytes, platelets) were within reference ranges. Parvovirus B19 immunoglobulin M and G levels were negative in both patients, yet serum parvovirus B19 DNA polymerase chain reaction test results were positive. Therefore, diagnosis was parvovirus B19-associated pure red blood cell aplasia. Anemia resolved completely by 4 weeks after reduction of immunosuppression and intravenous immunoglobulin. Both patients relapsed in month 5 of treatment. One patient relapsed 3 times during follow-up, with complete response to intravenous immunoglobulin for all 3 events. The second patient showed partial response to intravenous immunoglobulin after relapse. We suggest that pure red blood cell aplasia associated with parvovirus B19 should be considered in transplant patients who present with anemia and reticulocytopenia. Negative serology does not exclude the diagnosis, and it is important to perform a parvovirus B19 DNA polymerase chain reaction test. Intravenous immunoglobulin therapy is effective to cure anemia within weeks. Follow-up of patients is important because relapse may occur after treatment.
Background: This study aimed to evaluate the effects of the appropriate use of empiric glycopeptide therapy in hematologic malignancy patients with febrile neutropenia (FN). Materials and Methods: Patients with FN who were hospitalized in our clinic and started empiric glycopeptide therapy were retrospectively analyzed. Empiric glycopeptide treatment initial indications were determined according to 7 specific criteria in the IDSA guidelines. In addition, the duration of glycopeptide use according to initial indications, causative pathogens in culture positivity, frequency of VRE infection, and the mortality rate was identified. Results: 87 patients were included. Of these, 102 episodes of FN were analyzed. Appropriate use of glycopeptides was observed in 98% of patients. The most common initial indication for glycopeptide was skin or soft-tissue infection, with 52% (n = 53). The mean duration of glycopeptide use was 11 (2-22) days. The time of glycopeptide use was longer in patients with catheter-related infections than in those with severe mucositis and hemodynamic instability (p = 0,041/p = 0,016). The duration of glycopeptide use was shorter in patients with consolidation therapy than in those without consolidation therapy. The mortality rate in culture-positive patients was significantly higher than in culture-negative patients (p = 0.041). At 72 h, glycopeptide therapy was discontinued in 8 of 79 FN episodes within culture-negative patients. Conclusion: This study showed that the mortality rate was higher in culture-positive patients. Additionally, glycopeptides should be discontinued early with no evidence of gram-positive infection.
Kronik Myeloid Lösemide (KML), etyolojisinde sorumlu tirozin kinaz aktivitesi gösteren bcr-abl füzyon geninin keşfinden sonra, bu aktiviteyi inhibe eden ilaçların keşfiyle, daha uzun sağkalım sürelerine ulaşılabilmiştir. Bu ilaçlarla tedaviye zamanla direnç gelişmesi, ikinci ve üçüncü kuşak ajanların geliştirilmesinin önünü açmıştır. Bu çalışmamızda, merkezimizde ikinci kuşak tirozin kinaz inhibitörü (TKİ) – bosutinib - tedavisi alan hastaların, klinik, laboratuvar, moleküler yanıt, yan etki profili ve mortalite üzerindeki etkilerini değerlendirmeyi amaçladık. KML nedeniyle bosutinib tedavisi başlanan 17 hasta çalışmaya dahil edildi. Hastaların tedaviye kaçıncı sırada başlandığı, klinik, laboratuvar ve moleküler yanıt durumları retrospektif olarak elektronik hasta kayıtlarından tarandı. Elde edilen veriler hastaların ilaç başlanma sırasına göre karşılaştırıldı. İkinci (n=2), üçüncü (n=7) ve dördüncü (n=8) sırada bosutinib başlanan hastaların yaş, cinsiyet, komorbidite sayısı, bosutinib tedavi süresi açısından anlamlı bir fark gözlenmezken KML tanı yaşları arasında anlamlı bir farklılık mevcuttu. Moleküler yanıt ve yan etki profili açısından değerlendirildiğinde, ilacın başlanma sırası ile anlamlı bir farklılık yoktu. Hastaların genel sağkalımı 43,38 ± 4,98 ay (%95 GA: 33,62 – 53,16 ay) olarak gözlemlendi. Bosutinib tedavisinin her yaş grubunda, ilacın her başlanma sırasında kullanımının, stabil moleküler yanıt sağlaması açısından güvenli olduğu gözlemlendi. Yan etki profili açısından kullanımını sınırlayacak bir profile sahip olmaması nedeniyle KML tedavisinde tercih edilebilir bir molekül olarak düşünülmelidir.
Langerhans hücreli histiyositoz (LHH) erişkinde nadir rastlanılan multi-sistemik (MS) ve heterojen bir hastalıktır. Erişkin hastaya yaklaşım ve tedaviyle ilgili veri oldukça azdır. Bu çalışmada Mart 2000 ile Mart 2022 tarihleri arasında merkezimize sevk edilen LHH’li 16 erişkin hastayı retrospektif olarak inceledik. Hastaların %68,7’si erkek ve ortanca yaş 30,5 yıl idi. Tanıda hastaların %50’sinde tek bölgede, %50’sinde MS tutulum vardı. Riskli organ tutulumu 3 hastada (%18,75) mevcuttu. En çok tutulan alanlar kemik ve akciğer (%43.75), sonrasında ise santral sinir sistemi (SSS) (%31,25) idi. Sistemik tedavi endikasyonu olan 8 hastanın tamamına birinci sırada vinorelbin ve steroid tedavisi verildi. Diğer hastalara cerrahi eksizyon veya radyoterapi uygulandı. Tedaviye tam yanıt oranı %37.5, kısmi yanıt oranı %37.5 idi. Hastaların 3’ü (%18,75) nüks etti. Tek bölge tutulumu olan hastaların hepsi hayatta ve nüks gelişmedi. On bir hasta ise halen hayattadır. Median genel sağkalıma (OS) ulaşılamadı, 3 yıllık OS %79.8 idi. Otuz yaş üzerindeki hastaların sağkalımı daha düşük bulundu. Ancak bu fark istatistiksel olarak anlamlı değildi (p=0.2). MS hastalığı olanların median OS verileri daha kötüydü (48 aya karşı ulaşılamadı, p=0.02). Tanı yaşının 30’un üzerinde olması ve SSS tutulumu bulunması daha düşük OS ile ilişkiliydi ancak bu istatistiksel olarak anlamlı değildi. Dalak ve karaciğer tutulumu olan hastalarda hızlı hastalık progresyonu ya da tedaviye refrakterlik izlendi. Özellikle kötü prognostik özellikleri olan hastalara birinci basamakta vinblastin/steroid bazlı tedaviler yetersiz kalabilir. Erişkin LHH’da klinik ve prognostik özelliklerinin daha iyi belirlenebilmesi, uygun tedavi stratejilerinin geliştirilmesi için çok merkezli, prospektif çalışmalara ihtiyaç vardır.
Thrombotic thrombocytopenic purpura (TTP) is thrombotic microangiopathy caused by decreased activity of ADAMTS13, a von Willebrand factor-degrading metalloprotease. Here, we present a male patient with neurological symptoms, diagnosed with TTP and successfully treated with plasmapheresis.
Nodüler lenfosit predominant Hodgkin lenfoma (NLPHL) az görülen ve prognozu oldukça iyi olan bir hastalıktır. NLPHL tüm Hodgkin lenfomalı (HL) hastaların %5’ini oluşturmaktadır. Hastalıkla ilgili en önemli sorunlar hastalık nüksü, Hodgkin dışı lenfomaya transformasyon ve tedavi ilişkili yan etkilerdir. Erken evre hastalıkta tedavisiz izlem, cerrahi, tutulu alan radyoterapisi ve tek başına ritüksimab tercih edilirken, yüksek tümör yükü olan olgularda ve ileri evre hastalıkta kemoimmünoterapi kullanılır, radyoterapi eklenebilir. Hastalık nüksü sonrasında dahi tedavi yanıtları oldukça iyidir. Tedaviye yönelik verilerin tamamı retrospektif çalışmalardan gelmektedir. Çalışmamızda kliniğimizde Aralık 2011-Aralık 2020 tarihleri arasında NLPHL tanısı alan 10 hasta değerlendirildi. Hastaların medyan yaşı 36 (28-60) yıldı. NLPHL tanılı hastalar HL tanılı hastaların %2.08’ini oluşturmakta idi. Hastaların %80’ni (n=8) erkekti. Tanı anında hastalarımızın %70’ni (n=7) erken evre idi ve tüm hastaların ECOG (Doğu Kooperatif Onkoloji Grubu) performans skoru 0’dı. Hastalarımızın hepsinde başvuru şikayeti ele gelen lenfadenopatiydi. Tanı anındaki hemogram ve biyokimyasal parametreler normal referans aralığındaydı. Tüm hastalarımızın bakılan immünhistokimyasal boyamalarında CD20 pozitifliği mevcuttu ve yalnız 1 (%10) hastanın CD30 pozitifliği mevcuttu. Dokuz (%90) hastamıza ilk sıra tedavi olarak ABVD (doksorubisin, bleomisin, vinblastin, dakarbazin) kemoterapisi uygulandı. Bir (%10) hastamıza ise kombine modalite tedavisi (CMT) olan ABVD ile birlikte RT uygulandı. Bir hastamıza hastalık progresyonu nedeni ile diğer hastamıza ise geç nüks sebebi ile kurtarma tedavisi sonrası otolog kök hücre nakli yapıldı. Kliniğimizde tanı konulan NLPHL hastalarının Amerika ve Almanya gibi yabancı ülkelerde yayınlanan literatürlerde belirtilen insidans oranları ve verilen ilk basamak tedavi seçimi ile uyumsuz bir tablo oluşturmaktadır. Bu farklılığın Türkiye’den bildirilecek diğer verilerle karşılaştırılması ve tartışılması uygun olacaktır.
