This document introduces new IP Flow Information Export (IPFIX) Information Elements (IEs) to identify a set of information related to Segment Routing over IPv6 (SRv6) such as data contained in a Segment Routing Header (SRH), the SRv6 control plane, and the SRv6 Endpoint behavior that traffic is being forwarded with.
Le nombre de patients porteurs d’un implant cochléaire augmente ce qui nécessite pour les radiologues de savoir quelle attitude avoir en cas de demande d’examen par IRM. À partir des données de la littérature, nous proposons des recommandations générales pour la réalisation d’une IRM chez les patients porteurs d’implant cochléaire. Nous ajoutons à ces recommandations des précisions spécifiques propres à chaque constructeur. L’implantation cochléaire est indiquée dans les surdités de perception neurosensorielles bilatérales sévères à profondes, chez des patients pour qui le bénéfice des prothèses auditives conventionnelles est insuffisant. Lors de la réalisation d’une IRM chez ces patients, des évènements indésirables surviennent dans 15 à 30 % selon les données de la littérature, suite aux interactions entre la composante ferromagnétique de l’implant, le champ magnétique statique, les gradients de champ magnétique et les impulsions de radiofréquences. Il peut s’agir de la mobilisation de l’implant, de douleurs et d’inconfort, de l’échauffement de l’implant ou encore de la démagnétisation de l’implant. La perte de fonction de l’implant est un risque théorique. Les recommandations pour limiter ces événements indésirables varient d’un fabricant à l’autre et selon le type d’implant. La réalisation d’une IRM chez un patient porteur d’un implant cochléaire nécessite des précautions avant et pendant l’examen, reposant essentiellement sur les recommandations propres à chaque modèle d’implant cochléaire. Malgré toutes les précautions prises, des complications ou des effets indésirables mineurs restent possibles, bien que limités par l’arrivée récente sur le marché d’implants cochléaires avec aimants à rotation libre. The number of patients with a cochlear implant is increasing, making it necessary for radiologists to know what attitude to take when an MRI examination is requesting. Based on data from the literature, we propose general recommendations for performing MRI in patients with cochlear implants. We add to these recommendations specific details for each manufacturer. Cochlear implantation is indicated for severe to profound bilateral sensorineural hearing loss in patients who do not benefit from conventional hearing aids. During MRI in these patients, adverse events occur in about one third of the cases with an overall rate estimated between 15 and 30% of the time according to data in the literature, due to interactions between the ferromagnetic component of the implant and the static magnetic field, the dynamic magnetic field gradient and the radiofrequency pulses. This may include mobilization of the implant, pain and discomfort, heating of the implant or demagnetization of the implant. Loss of function of the implant is a theoretical risk. Recommendations for limiting these adverse events vary from manufacturer to manufacturer and by implant type and depend on the type of implant. Performing an MRI in a patient with a cochlear implant requires precautions before and during the examination, based primarily on the recommendations specific to each model of cochlear implant. However, it appears essential to remember that despite all the precautions taken, complications or undesirable effects are possible, in particular pain or discomfort, however limited by the recent arrival on the market of cochlear implants with free-rotating magnets.
Recently, Intent-Based Networking (IBN) has known an increasing interest from both the industry and research communities. IBN comes with the advantage of easily expressing the desired state of a network. In parallel, service assurance, through observability, has been becoming more prevalent to maximize the business continuity. In that spirit, Service Assurance in Intent-based Networking (Sain), is under standardization at the IETF and proposes a general framework towards closed-loop automation for service assurance. This paper introduces the Diagnostic Agent (DxAgent), an open-source Sain implementation whose purpose is to determine symptoms and health levels of the different subservices of a network service. As such, the DxAgent appears as a first step towards closed-loop automation for service assurance. This paper describes the DxAgent implementation and demonstrates its efficiency through use cases.

PURPOSE:To compare the evaluation of collaterals on multiphase computed tomography (CT) angiography using the score proposed by the reference study by Menon et al. and the Alberta Stroke Program Early CT (ASPECT) score for the prediction of favorable clinical outcome in patients with anterior ischemic stroke (IS). MATERIALS AND METHODS:Retrospective single center study including 199 patients with anterior ischemic stroke and evaluated using multiphase CT angiography. Collaterals were assessed using the reference score and ASPECT score. The early clinical outcome [National Institute of Health Stroke Score (NIHSS) over day 1] and later clinical outcome [90-day modified Rankin Scale (mRS)] were collected. The primary analysis related to the association between collateral scores and clinical outcome. RESULTS:Collaterals are an independent predictive factor of favorable clinical outcome with the two scores, ranging from an odds ratio (OR) [95% confidence interval (CI)] = 1.84 [1.23; 2.76], P = 0.003 for the reference score to an OR [95% CI] = 2.63 [1.21; 5.73], p = 0.015 for the phase 3 ASPECT score. The phase 3 ASPECT score offers better sensitivity (Se) for the prediction of a favorable clinical outcome [Se = 95%, specificity (Sp) = 37% for a threshold of 7/7] than the reference score (Se = 83%, Sp = 47% for a threshold of 4/5). CONCLUSION:This study demonstrates the value of the ASPECT score in analyzing collaterals using multiphase CT angiography for the prediction of clinical outcome.
This document specifies a way to complement IP Flow Information Export (IPFIX) Data Records with Management Information Base (MIB) objects, avoiding the need to define new IPFIX Information Elements for existing MIB objects that are already fully specified. Two IPFIX Options Templates, as well as a method for creating IPFIX Options Templates that are used to export the extra data required to fully describe Simple Network Management Protocol (SNMP) MIB objects in IPFIX, are specified herein.
Objective. - Deep brain mapping has been proposed for direct targeting in stereotactic functional surgery, aiming to personalize electrode implantation according to individual MRI anatomy without atlas or statistical template. We report our clinical experience of direct targeting in a series of 156 patients operated on using a dedicated Inversion Recovery Turbo Spin Echo sequence at 1.5-tesla, called White Matter Attenuated Inversion Recovery (WAIR).Methods. - After manual contouring of all pertinent structures and 3D planning of trajectories, 312 DBS electrodes were implanted. Detailed anatomy of close neighbouring structures, whether gray nuclei or white matter regions, was identified during each planning procedure. We gathered the experience of these 312 deep brain mappings and elaborated consistent procedures of anatomical MRI mapping for pallidal, subthalamic and ventral thalamic regions. We studied the number of times the central track anatomically optimized was selected for implantation of definitive electrodes.Results. - WAIR sequence provided high-quality images of most common functional targets, successfully used for pure direct stereotactic targeting: the central track corresponding to the optimized primary anatomical trajectory was chosen for implantation of definitive electrodes in 90.38%.Conclusion. - WAIR sequence is anatomically reliable, enabling precise deep brain mapping and direct stereotactic targeting under routine clinical conditions. Chez l’humain les aires gusta-tives et le système central de la récompense participent au traitement des informations induites par les stimuli alimentaires. Ce travail a pour objectif de mieux comprendre les mécanismes d’attribution de l’intensité et de la valeur hédonique au niveau de ces voies centrales et leurs relations avec les comportements qui en découlent. Pour cela les réseaux cérébraux activés par différents stimuli alimentaires sont évalués par imagerie par RMN fonctionnelle (IRMf). 10 eaux aromatisées variant en concentration de sucre, acide citrique et arôme citron ont été utilisées pour l’étude, classées par intensité de goût croissante. 50 sujets sains, hommes non-fumeurs de poids normal (âge 24 +/− 2,4 ans, IMC 22,1 +/− 1,77 kg/m2) ayant passé un test gustatif ont été recrutés. L’étude comportait trois sessions expérimentales : une session d’habituation avec notation à l’aveugle des produits d’un point de vue hédonique et sensoriel ; une session d’apprentissage avec 4 associations image-produit ; une session IRMf lors de laquelle les sujets ont été réexposés aux associations image-produit (10 tests par produit), dans un ordre aléatoire. Pour chaque test gustatif, les sujets recevaient 1mL de produit sur la langue directement dans le scanner IRMf. Les analyses statistiques ont été effectuées sur les sujets ayant manifesté le même ordre de préférences pour les produits à la session IRMf. Une ANOVA à un facteur (« Produit ») et à mesures répétées a été réalisée aux trois instants du test gustatif : présentation de l’image (Image), dégustation du produit (« Produit »), notation du produit (« Note ») 28 volontaires ont manifesté un ordre de préférence allant du produit le moins concentré (le moins apprécié) jusqu’au produit le plus concentré (le plus apprécié) (ANOVA à un facteur sur les notes hédoniques : F(df =3)=766,5, p < 0,001 ; t-test multiple : p < 0,001 pour toutes les combinaisons). Des activations cérébrales significatives ont été observées pendant les phases « Produit » et « Note ». En particulier, le gyrus postcentral droit et gauche a manifesté une activation clairement corrélée avec l’intensité gustative des stimulis (ANOVA sur postcentral droit : pic 54 -4 26, F = 20,04, pFWE < 0,001 ; ANOVA sur postcentral gauche : pic -55 -10 25, F = 22,02, pFWE < 0,001). L’activation du gyrus postcentral par les stimuli alimentaires semble confirmer l’implication de cette région dans le traitement primaire des signaux gustatifs. La modulation de son activité par les différents stimuli utilisés suggérerait un rôle dans le codage de l’intensité. Cette activation toujours observable à l’instant « Note » pourrait correspondre à la persistance du goût depuis la réception du produit. Ainsi, l’intensité d’un stimulus gustatif serait analysée par les aires gustatives primaires, avant même le traitement du signal par le système de central de la récompense.
