Homicides in psychiatric clinics are rare, however recurring incidents; using administrative data, homicides for the years 2016 to 2024 in psychiatric clinics in Lower Saxony were analysed. 8 completed and 5 attempted homicides were reported. All incidents occurred in adult psychiatry, and all but one took place in closed acute care units. In the majority of cases, the offender entered the victim's patient room. Some victims were in a fragile somatic or mental state. The majority of offenders were voluntarily or under legal guardianship hospitalized. Conclusions: Homicides in psychiatric clinics are not typically committed by patients considered acutely dangerous to others. Therefore, acute care units need to place a stronger emphasis on victim protection: Patient room doors should be lockable from the outside to prevent unauthorized access. Patients with fragile somatic and mental health conditions needing higher care should be better protected.
Tötungsdelikte in Kliniken für Psychiatrie sind seltene, aber wiederkehrende Ereignisse. Anhand administrativer Meldungen werden Tötungsdelikte der Jahre 2016 bis 2024 in niedersächsischen Kliniken für Psychiatrie untersucht. 8 vollendete und 5 versuchte Tötungsdelikte wurden gemeldet. Alle erfolgten in der Erwachsenenpsychiatrie und alle bis auf eine Tat auf geschlossenen Akutstationen. In der Mehrzahl drang die Täterin/der Täter in ein fremdes Patientenzimmer ein. Unter den Opfern waren mehrere mit erhöhtem Pflegebedarf aufgrund körperlicher oder geistiger Beeinträchtigungen. Die Mehrzahl der Täter war freiwillig oder nach Betreuungsrecht untergebracht. Schlussfolgerung: Die meisten Tötungsdelikte erfolgten nicht durch Personen, die als akut fremdgefährdend eingeschätzt waren. Akutstationen sollten einen stärkeren Fokus auf den präventiven Opferschutz legen: durch die Installation von abschließbaren Patientenzimmertüren und verstärktem Schutz von körperlich oder geistig fragilen Personen mit erhöhtem Pflegebedarf.
Various systematic reviews underscore the relevance of social support for resilience among refugees. This meta-analysis aims to determine the quantitative assessment of social support and resilience among refugees and the extent of the associations between social support and resilience among refugees. After a systematic literature search, we included twenty-three studies, and performed random-effects meta-regressions. Studies on resilience and social support among refugees very heterogeneously operationalize both constructs. While increases in social support among refugees accompany higher resilience in numerous studies, these associations are not significant. In contrast to previous reviews' conclusions, the current data cannot confirm a relationship between resilience and social support among refugees. Substantiated conclusions about the relationship between resilience and social support among refugees might be reached by a population-specific clear conceptualization and operationalization of the constructs, the content differentiation of the constructs, representative samples, and longitudinal and intervention studies.
OBJECTIVE:Characterization of medical emergencies of patients with psychiatric symptoms or in psychosocial crisis. METHODS:Analysis of emergency medical services (EMS) mission logs from four German EMS stations of patients aged 12-65 from 2019 to 2020. RESULTS:23.4% of missions involved patients with psychiatric or psychosocial symptoms (1.6% psychotic, 3.0% suicidal, 7.1% psychosocial crisis, 15.7% intoxicated). On average, these patients were younger than those with other symptoms. 48.9% exhibited additional somatic complaints. The COVID-19 pandemic showed only little impact on the frequency of such emergencies. CONCLUSION:Psychiatric and psychosocial symptoms account for a significant proportion of rescue missions in our sample. Managing psychiatric and psychosocial emergencies should be an integral part of the training of medical and paramedical staff.
Zusammenfassung Ziel der Studie Charakterisierung von Notfallrettungseinsätzen (NFR) mit Menschen in psychiatrischen oder psychosozialen Krisen. Methodik Analyse von Notfallprotokollen aus vier Rettungswachen (2019–2020) von NFR mit Patient*innen zwischen 12 und 65 Jahren. Ergebnisse Bei 23,4% der NFR zeigten Patient*innen psychiatrische oder psychosoziale Symptome (1,6% psychotische Symptomatik, 3,0% Suizidalität, 7,1% psychosoziale Krise, 15,7% Intoxikation). Diese Patient*innen waren im Mittel jünger als Patient*innen mit anderen Einsatzanlässen. Bei 48,9% lagen zusätzlich somatische Symptome vor. Die COVID-19-Pandemie hatte nur einen begrenzten Einfluss auf das Auftreten solcher NFR. Schlussfolgerung Psychiatrische und psychosoziale Einsätze machen einen bedeutenden Anteil der NFR aus. Dieses Ergebnis legt nahe, dass der Umgang mit psychiatrischen und psychosozialen Notfällen ein integraler Bestandteil von Aus- und Weiterbildung von ärztlichem und nichtärztlichem Rettungsfachpersonal sein sollte.
Zusammenfassung Thema Der Artikel gibt einen Überblick über den gegenwärtigen Stand der Versorgung von Menschen mit psychischen Erkrankungen, finanziert durch das Krankenkassen-, das Rehabilitations- und Teilhabesystem und durch die Länder. In den letzten 20 Jahren sind die Versorgungskapazitäten stetig angewachsen. 3 Bereiche mit Entwicklungsbedarfen werden näher beleuchtet: die Koordination der Versorgung für Menschen mit psychischen Erkrankungen und komplexen Hilfebedarfen, die Langzeitunterbringung von Menschen mit schweren psychischen Erkrankungen und herausforderndem Verhalten und ein zunehmender Fachkräftemangel. Fazit Deutschland verfügt über ein in weiten Teilen gut bis sehr gut ausgebautes psychiatrisches Versorgungssystem. Bestimmte Gruppen profitieren trotz alledem nicht von den vorhandenen Hilfen, sie finden sich unter anderem als LangzeitpatientInnen in psychiatrischen Kliniken. Modelle für eine koordinierte und ambulant orientierte Versorgung für Menschen mit schweren psychischen Erkrankungen existieren nur vereinzelt. Es mangelt insbesondere an (intensiv) aufsuchender Komplexversorgung und an sozialgesetzbuchübergreifenden Konzepten. Der Fachkräftemangel, der das gesamte Versorgungssystem für Menschen mit psychischen Erkrankungen betrifft, verlangt einen Strukturwandel hin zu stärkerer Ambulantisierung. Erste Instrumente hierfür existieren im krankenkassenfinanzierten System. Sie sollten genutzt werden.
Subject. The article provides an overview on the current state of service provision for people with psychiatric illnesses, financed by health-insurance companies, the rehabilitation and participatory systems, and the German Federal States. Over the last 20 years, service capacities have continually improved. Three areas that are in need of further development are highlighted here: the coordination of services for mentally ill persons with complex needs; the long-term placement of persons with severe mental illness and challenging behavior; and a growing shortage of specialists.Conclusions. The mental health system in Germany is largely well to very well developed. Despite this, specific groups do not benefit from the available assistance, and these often end up as "long-term patients " in psychiatric clinics. Models for coordinated and outpatient-oriented service provision for persons with severe mental illness exist, but are only sporadic. Intensive and complex outreach services in particular are lacking, as are service concepts that are able to cross the boundaries of social security responsibilities. The shortage of specialists, which affects the whole mental health system, requires restructuring toward a more outpatient-care focus. The first tools for this exist within the health insurance-financed system. They should be used.
AIM:To determine the effect of treatment volumes on the frequency and duration of special safety measures (SSM) such as seclusion or restraint.METHOD:Register Data of the Lower Saxony Ministry of Social Affairs, Health and Equal Opportunities is analysed for the number of cases hospitalised under State Mental Health Act, the proportion of cases experiencing SSM, and the frequency and cumulative duration of SSM per case.RESULTS AND DISCUSSION:The larger the treatment volume of cases that are hospitalised under State Mental Health Act, the smaller the proportion of cases experiencing SSM. This result is robust, even if statistical outliers are exempted from analyses.CONCLUSIONS:In light of indications that also in mental health care treatment volumes may be related to the desired treatment outcome, discussion is need about the tension between hospital care that is provide.
Zusammenfassung Studienziel Die Studie beabsichtigt, die Perspektiven auf Resilienz nach der Flucht von Geflüchteten und von unterstützenden Praxisakteur*innen zu identifizieren (Ziel 1) und die Perspektiven beider Gruppen zueinander in Beziehung zu setzen (Ziel 2), um die inhaltliche Tiefe des Verständnisses von Postfluchtresilienz zu erweitern und Erkenntnisse über Beziehungen zwischen Praxisakteur*innen und Geflüchteten bereitzustellen. Methodik Drei semistrukturierte, leitfadengestützte Fokusgruppen mit Geflüchteten (N=9) und Praxisakteur*innen (N=13) zur Erfassung der Perspektiven beider Gruppen auf Resilienz nach der Flucht wurden auf Deutsch durchgeführt. Sie wurden iterativ nach Constructionist Grounded Theory ausgewertet. Ergebnisse Geflüchtete und Praxisakteur*innen berichten Belastungsfaktoren Unsicherheit und Begrenzung, Adaptionsprozesse Gemeinschaftlichkeit und Anstrengung sowie die Adaptionsziele Stabilität, Verbundenheit und positive Emotionalität (Ziel 1). Die Aussagen von Geflüchteten und unterstützenden Praxisakteur*innen zu Resilienz nach der Flucht stimmen inhaltlich stark überein und ergänzen sich gegenseitig. Geflüchtete berichten individualisierter und spezifischer als Praxisakteur*innen (Ziel 2). Diskussion Die Aspekte der Resilienz, die Geflüchtete und unterstützende Praxisakteur*innen nennen, replizieren Ergebnisse vorhergehender qualitativer Studien zu Postfluchtresilienz, welche mit Geflüchteten durchgeführt wurden. Sowohl die Belastung, adäquate Adaptionsmöglichkeiten vor Ort z.T. nicht zu kennen, als auch Versuche, Resilienz-förderliche Bedingungen zu schaffen, könnten spezifisch für die Resilienz von Geflüchteten sein. Die inhaltlichen Übereinstimmungen zwischen Geflüchteten und Praxisakteur*innen könnten durch Erfahrungen der Praxisakteur*innen im Rahmen von Unterstützungsprozessen oder durch ähnliche Lebenserfahrungen der Praxisakteur*innen bedingt sein. Die weniger individualisierte Herangehensweise an Postfluchtresilienz der Praxisakteur*innen könnte durch ein höheres Abstraktionsniveau, Rollenerwartungen oder Umgang mit emotionaler Belastung verursacht sein. Schlussfolgerung Die Ergebnisse der Studie deuten darauf hin, dass die Resilienz Geflüchteter durch universelle und populationsspezifische Aspekte charakterisiert ist. Dass Praxisakteur*innen, die Geflüchtete unterstützen und Geflüchtete inhaltlich bezüglich der Resilienz nach der Flucht übereinstimmen, validiert die Ergebnisse bisheriger qualitativer Studien zu Resilienz nach der Flucht. Die Studie ermittelte darüber hinaus unterschiedliche Herangehensweisen von Geflüchteten und Praxisakteur*innen an Postfluchtresilienz, deren Verursachung weiterer Forschung bedarf.
Zusammenfassung Ziel Bestimmen des Einflusses des Behandlungsvolumens auf die Häufigkeit und Dauer der Anwendung von Besonderen Sicherungsmaßnahmen (bSm) wie Fixierung und Isolierung. Methode Datenbasis sind krankenhausbezogene Registerdaten aus Niedersachsen der Halbjahre 2/2019 bis 1/2021 zu Fällen, die nach dem Niedersächsischen Gesetz für Psychisch Kranke (NPsychKG) untergebracht sind, zum Anteil der Fälle, die eine bSm erhielten sowie zur Anzahl und kumulativen Dauer von bSm/Fall. Es wurden einfache lineare Regressionsmodelle gerechnet. Ergebnis und Diskussion Mit Zunahme des Behandlungsvolumens von nach NPsychKG untergebrachten Fällen sinkt der Anteil der Fälle, die bSm erhalten. Dieses Ergebnis bleibt bestehen auch wenn statistische Ausreißer aus den Berechnungen herausgelassen werden. Schlussfolgerung Angesichts von Hinweisen, dass auch in der psychiatrischen Versorgung das Behandlungsvolumen mit dem gewünschten Behandlungsziel assoziiert ist, sollte das Spannungsfeld wohnortnahe vollstationäre Versorgung in kleinen Kliniken versus Konzentration von Expertise in Zentren diskutiert werden.
In contrast to official mental health policy, psychiatric clinics accomodate long-term patients. Aim To quantify this problem. Method Cross-sectional assessment of the number of long-term patients in psychiatric clinics in Lower Saxony. Results 13 clinics of adult psychiatry report 60 long-term patients and one clinic for child and adolescent psychiatry reports 2 patients. Median duration of stay 9.5, maximum 215 months. Reasons for long-term hospitalisation include patients' challenging behaviour and, consequently, difficulties to find an accommodating long-term facility. Discussion and conclusion Housing willing to accommodate mentally ill persons with challenging behaviour is lacking, as well as local responsibility for the placement of these persons. Systematic transfer management from hospital to residential homes does not exist.
In the course of the recognition of mental health as an essential component of population health, the Robert Koch Institute has begun developing a Mental Health Surveillance (MHS) system for Germany. MHS aims to continuously report data for relevant mental health indicators, thus creating a basis for evidence-based planning and evaluation of public health measures. In order to develop a set of indicators for the adult population, potential indicators were identified through a systematic literature review and selected in a consensus process by international and national experts and stakeholders. The final set comprises 60 indicators which, together, represent a multidimensional public health framework for mental health across four fields of action. For the fifth field of action 'Mental health promotion and prevention' indicators still need to be developed. The methodology piloted proved to be practicable. Strengths and limitations will be discussed regarding the search and definition of indicators, the scope of the indicator set as well as the participatory decision-making process. Next steps in setting up the MHS will be the operationalisation of the single indicators and their extension to also cover children and adolescents. Given assured data availability, the MHS will contribute to broadening our knowledge on population mental health, supporting a targeted promotion of mental health and reducing the disease burden in persons with mental disorders.
Zusammenfassung Hintergrund Eltern stehen während der COVID-19-Pandemie vor einer Vielzahl persönlicher Herausforderungen, während sie gleichzeitig mit schulbezogenen Maßnahmen zur Pandemieeindämmung konfrontiert werden. Zielsetzung Dieser Beitrag fokussiert auf die Belastung von Eltern mit Kindern im Schulalter über verschiedene Phasen der COVID-19-Pandemie in Deutschland und identifiziert besonders vulnerable Subgruppen. Methoden Die COSMO-Studie ist eine repetitive Querschnittsstudie zur Erfassung der psychosozialen Lage der Bevölkerung in Deutschland während der Pandemie, mit einer Stichprobengröße von ca. n = 1000 Befragten pro Erhebungswelle. COSMO-Daten zur allgemeinen und elternspezifischen Belastung wurden von März 2020 bis Januar 2021 quantitativ analysiert. Ergebnisse Während der ersten COVID-19-Welle waren Eltern mit Kindern im Schulalter – verglichen mit der allgemeinen Studienpopulation – signifikant stärker belastet. Die Belastung nahm jedoch von März/April bis Juni 2020 deutlich ab. Während der zweiten COVID-19-Welle im Januar 2021 war die Belastung über alle Gruppen hinweg homogen hoch. Folgende Faktoren waren mit einer höheren Belastung assoziiert: Alleinerziehendenstatus, niedriges Haushaltseinkommen, eine chronische Erkrankung, eine COVID-19-Infektion sowie ein Migrationshintergrund; wobei diese Faktoren nicht über alle Erhebungswellen hinweg signifikant waren. Mütter gaben an, stärker von elternspezifischen Belastungen betroffen zu sein als Väter. Schlussfolgerung Schulbasierte Maßnahmen zur Infektionskontrolle müssen sorgfältig gegen die Auswirkungen auf die elterliche Belastung mit nachfolgenden negativen Auswirkungen auf das Familiensystem abgewogen werden.
Background Parents face a variety of personal challenges during the COVID-19 pandemic, while simultaneously being confronted with additional, school-related pandemic containment measures. Objectives To investigate burden in parents of school-aged children across different phases of the COVID-19 pandemic in Germany and to identify particularly affected subgroups. Methods The COSMO project is a repetitive cross-sectional survey monitoring the psychosocial situation of the population in Germany during the pandemic with a sample size of approximately n = 1000 respondents per survey wave. A quantitative analysis of COSMO data was conducted using closed survey questions on the item "burden" as the main outcome, and, if applicable, on parenthood-associated burden from March 2020 until January 2021. Results During the first COVID-19 wave, parents of school-aged children were significantly more burdened compared to the general study population. However, burden decreased significantly from March/April to June 2020. During the second COVID-19 wave in January 2021, burden was homogeneously high across all groups. Single parenthood, a low household income, having a chronic health condition, a COVID-19 infection and a migration background were associated with higher burden, although none of these factors was consistently significant across the survey waves. Mothers reported to be more affected by parenthood-related burden than fathers. Conclusions School measures for infection control have to be weighed carefully against the psychological impact on parental burden with subsequent negative impact on the family system. Aim There are differences in the prevalence estimates of depressive disorders based on primary and secondary data. The reasons for this are, for instance, the use of divergent indicators and varying observation periods. This study examines the prevalence of depressive disorders using survey and routine data for a comparable survey period and age range. Effects of differences between data sources and indicators are estimated. Methods For 2010, 3 indicators are compared: in a population survey collected a) self-reported medical diagnosis of depression, b) diagnosis of depressive disorders identified by clinical interviews and c) administrative depression diagnoses collected from routine data of a statutory health insurance. In sensitivity analyses, privately insured participants of the population survey were excluded, and insured persons with care needs were excluded from routine data. The definition of administrative depression diagnosis was varied depending on the frequency of coded diagnoses and the specificity of the diagnoses. Results The highest prevalence (9.8%) was found for depression diagnoses from administrative data, the lowest prevalence (5.9%) in self-reported medical diagnoses of depression in the population survey. The prevalence of depression identified by clinical interviews was 8.4%. Differing age and gender-related courses of illness were found. The administrative prevalence dropped significantly if unspecific diagnoses (F3x.8, F3x.9) were excluded. Discussion Depending on the definition of depression diagnoses used in administrative data, there was a reduction in differences of prevalence compared to the self-reported medical diagnoses. Differences in prevalence based on a diagnosis of a depressive disorder identified in a clinical interview remained stable, which indicates different groups of persons.
Zusammenfassung Ziel der Studie Prävalenzschätzungen für Depression zeigen in Primär- vs. Sekundärdaten divergente Ergebnisse. Ursachen liegen u. a. in der Betrachtung verschiedener Indikatoren zu abweichenden Zeitpunkten. In der vorliegenden Arbeit werden Depressionshäufigkeiten in Survey- und Routinedaten für einen vergleichbaren Erhebungszeitraum und Altersbereich ermittelt. Unterschiede zwischen den Datenquellen und den Depressionsmaßen werden untersucht. Methodik Drei Indikatoren werden für das Jahr 2010 gegenübergestellt: im Bevölkerungssurvey erhobene a) selbstberichtete ärztliche Diagnosen, b) Diagnosen in standardisiertem klinischen Interview sowie c) in Routinedaten einer gesetzlichen Krankenkasse administrativ erfasste Diagnosen. In Sensitivitätsanalysen werden privat versicherte Surveyteilnehmende sowie Versicherte mit Pflegebedarf aus den Routinedaten ausgeschlossen. Die Definition der administrativen Depressionsdiagnose wird variiert je nach Kodierhäufigkeit und Diagnosespezifität. Ergebnisse Die höchste Prävalenzrate zeigt sich mit 9,8% für administrativ erfasste Depressionen, die niedrigste mit 5,9% für die selbstberichtete Depressionsdiagnose im Survey. Die Prävalenz der im klinischen Interview erhobenen Depression beträgt 8,4%. Es zeigen sich jeweils spezifische alters- und geschlechtsabhängige Verteilungen. Bleiben unspezifische Depressionsdiagnosen (F3x.8, F3x.9) unberücksichtigt, sinkt die administrative Prävalenzrate deutlich ab. Schlussfolgerung Je nach Definition der administrativen Depressionsdiagnose reduzieren sich Prävalenzunterschiede im Vergleich zur selbstberichteten Diagnose. Differenzen zu Prävalenzen basierend auf klinischem Interview bleiben hingegen bestehen und weisen auf die Erfassung unterschiedlicher Personengruppen hin.
Working requirements of community mental healthcare professionals in integrated care are complex. There is a lack of research concerning the relation of job satisfaction, working atmosphere and individual characteristics. For the current study, a survey evaluating job satisfaction and working atmosphere of mental healthcare professionals in integrated care was performed. About 321 community mental healthcare professionals were included in the survey; the response rate was 59.5%. The professional background of community mental healthcare professionals included nursing, social work and psychology. Community mental healthcare professionals reported the highest satisfaction with colleagues and the lowest satisfaction with income. Moreover, it could be shown that more responsibility, more recognition and more variety in job tasks lead to an increase of overall job satisfaction. Healthcare for mentally ill patients in the community setting is complex and requires well-structured care with appropriate responsibilities within the team. A co-operative relationship among colleagues as well as clearly defined responsibilities seem to be the key for the job satisfaction of community mental healthcare professionals in integrated care.
Objective Multiple models of Integrated Care (IC) have been implemented in German mental health services in the last decade in order to improve cross-sectoral, interdisciplinary cooperation. This study investigates an IC network model providing home treatment, case management and a 24/7 hotline. The aim of the study was to explore how health professionals working in this service model perceive both cooperation within their facilities and with external stakeholders.Methods 5 focus groups with 39 health professionals working in an IC mental health network were conducted and analyzed with qualitative content analysis.Results Focus groups participants reported on excellent cooperation within their facilities. The cooperation with external stakeholders, i.e. physicians, psychotherapists and psychiatric clinics, leaves room for improvement.Conclusions Until now little consideration has been given to the perspectives of health professionals. Cooperation within IC mental health networks seems to be effective. Cooperation with stakeholders outside the networks needs to be enhanced.
