
INTRODUCTION:Work intensity is considered a key work-related risk factor for employee health. It describes the ratio between the amount of work and the time available. Its increase since the 1990s is seen as a key feature of the changing nature of work. METHODS:By using the 2024 BIBB/BAuA Employment Survey, the current prevalence of work intensity among around 18,000 employees was examined and compared with data from 2012 and 2018 to investigate how it has developed. Three work intensity characteristics were used for this purpose. The data were stratified according to gender, working hours, professional requirements, and occupational activity. RESULTS:Women, full-time employees, and employees with higher professional requirements are more affected by work intensity than men, part-time employees, or employees with low job requirements. Overall, multitasking increased and deadline or performance pressure and working at the limit of one's capacity decreased. However, these changes do not apply to all employee groups equally. There are still groups with noticeably high and even increasing work intensity. DISCUSSION:Efforts to create well-designed work must be intensified, especially for these employees. Despite positive overall trends, work intensity must continue to be kept in mind as a key factor in occupational safety, prevention, and work design, because prolonged high work intensity contributes to the development of cardiovascular, musculoskeletal, and depressive disorders.
Im geplanten Primärarztsystem sollen bei schweren chronischen Erkrankungen auch Fachärzte primärärztliche Aufgaben übernehmen können. Ziel der Arbeit war es, ein datenbasiertes Verfahren zur Identifikation potenziell geeigneter Erkrankungen zu entwickeln und die Versorgungsrelevanz einzelner Fachrichtungen darzustellen. Auf Grundlage bundesweiter vertragsärztlicher Abrechnungsdaten für 2024 wurden für jeden ICD-3-Steller (erste 3 Stellen der International Classification of Diseases) die haus- und fachärztlichen Anteile an den Behandlungsfällen und Behandlungstagen je Patient bestimmt. Als Indikationen mit hoher fachärztlicher Versorgungsrelevanz galten Kategorien, bei denen bei mehr als der Hälfte der Patienten mindestens 60
Vor dem Hintergrund der prekären finanziellen Lage der Gesetzlichen Krankenversicherung (GKV) wird die Einführung eines Primärarztsystems, bei dem Patienten zunächst eine hausärztliche Praxis aufsuchen und von dort bei Bedarf an einen Facharzt überwiesen werden, in Deutschland derzeit intensiver diskutiert. Ziel ist es, die Versorgung stärker zu koordinieren und die Effizienz im Gesundheitswesen zu erhöhen. Dieser Diskussionsbeitrag zeigt auf, inwiefern ein Primärarztsystem ein wesentlicher Baustein einer grundlegenden Transformation des Gesundheitswesens sein kann und welche systemischen Voraussetzungen hierfür notwendig sind. Neben einem Blick in das europäische Ausland werden auch Erfahrungen aus Deutschland erörtert und Empfehlungen abgegeben. Dabei wird eine Neudefinition der Eigenverantwortung ebenso diskutiert wie die Möglichkeiten von Anreizsystemen für eine nachhaltige Etablierung eines solchen Systems. Bonusprogramme nach dem Vorbild erfolgreicher Loyalitätsmodelle könnten die Chance bieten, Patientensteuerung wirksam, akzeptiert und sozial ausgewogen zu gestalten. Patientensteuerung ist keine Frage von Disziplin, sondern von Systemdesign. Eine Transformation der GKV, die Patientensteuerung und eine zeitgemäße Definition von Eigenverantwortung integriert, stärkt nicht nur die Wirtschaftlichkeit, sondern auch die Solidarität. Patientensteuerung und Eigenverantwortung sind keine moralischen Appelle, sondern ökonomische Notwendigkeiten. Ein solidarisches System bleibt nur dann stabil, wenn es effizient organisiert ist.
Arbeitsintensität gilt als ein zentraler arbeitsbezogener Risikofaktor für die Gesundheit von Beschäftigten. Sie beschreibt das Verhältnis zwischen Arbeitsmenge und zur Verfügung stehender Zeit. Ihre seit den 1990er-Jahren zu verzeichnende Zunahme wird als ein zentrales Merkmal des Wandels der Arbeit angesehen. Mittels der BIBB-/BAuA-Erwerbstätigenbefragung 2024 (BIBB Bundesinstitut für Berufsbildung; BAuA Bundesanstalt für Arbeitsschutz und Arbeitsmedizin) wird bei rund 18.000 abhängig Beschäftigten der Frage nach der aktuellen Verbreitung von Arbeitsintensität nachgegangen und mittels Daten aus den Jahren 2012 und 2018 untersucht, wie sie sich entwickelt hat. Dazu werden 3 Arbeitsintensitätsmerkmale herangezogen. Die Daten werden stratifiziert nach Geschlecht, Arbeitszeit, Qualifikationsniveau und beruflicher Tätigkeit betrachtet. Frauen, Vollzeitbeschäftigte sowie Beschäftigte mit höherem beruflichen Anforderungsniveau sind in stärkerem Maße von Arbeitsintensität betroffen als Männer, Teilzeitbeschäftigte oder Beschäftigte mit geringem beruflichen Anforderungsniveau. Insgesamt haben das Multitasking zu- und Termin- oder Leistungsdruck sowie das Arbeiten an der Leistungsgrenze abgenommen. Allerdings gelten diese Veränderungen nicht für alle Beschäftigtengruppen gleichermaßen. Nach wie vor gibt es Gruppen mit auffällig hoher sowie auch mit eher zunehmender Arbeitsintensität. Insbesondere für diese Beschäftigten müssen Bemühungen um gut gestaltete Arbeit verstärkt werden. Trotz positiver Gesamttendenzen muss Arbeitsintensität weiterhin als zentraler Schlüsselfaktor im Arbeitsschutz, in der Prävention und in der Arbeitsgestaltung im Blick behalten werden, denn langanhaltende hohe Arbeitsintensität trägt zur Entwicklung von Herz-Kreislauf- und Muskel-Skelett-Erkrankungen sowie depressiven Störungen bei.
Beschäftigte in der Landwirtschaft weisen internationalen Studien zufolge ein erhöhtes Suizidrisiko auf. Für Deutschland liegen hierzu bislang kaum belastbare Erkenntnisse vor. Ziel der Studie war es, auf Basis von Sekundärdaten die Suizidraten von Personen in landwirtschaftsnahen Berufen (grüne Berufe) im Vergleich zur übrigen Bevölkerung zu schätzen. Bevölkerungsbasierte deskriptive Studie anhand alters- und geschlechtsspezifisch aggregierter Daten der Polizeilichen Kriminalstatistik (PKS) Bayern zu Suiziden 2017–2024. Die PKS ordnet Suizidfälle dem vorwiegend ausgeübten Beruf zu. Als Grundgesamtheiten dienten verschiedene Bevölkerungsstatistiken. Aufgrund methodischer Unschärfen sind die Suizidraten als Schätzungen zu interpretieren. Im Untersuchungszeitraum wurden bei Personen in grünen Berufen 463 Suizide erfasst (im Mittel 57,9 ± 14,7 Fälle/Jahr). Die geschätzten Suizidraten lagen insgesamt sowie nach Geschlecht und Alter deutlich über denen der übrigen Bevölkerung. Bei 18- bis 59-jährigen Männern, der Gruppe mit der verlässlichsten Datengrundlage, war die Suizidrate 3,4-fach erhöht (95
Cancer registration in North Rhine-Westphalia began in 1974 and celebrated its 50th anniversary in 2024. The aim of this paper is to describe the development phases of cancer registration in North Rhine-Westphalia between 1974 and 2025.Following an international symposium in Düsseldorf in 1973 and with the support of the Society for the Fight Against Cancer (later: Cancer Society of North Rhine-Westphalia), a cancer registry was established in 1974 under the leadership of Prof. Ekkehard Grundmann, initially focusing on oncological aftercare in Münster. Pathologists who had no contact with patients were not able to report to the registry. Federal political debates between 1978 and 1981 promoted the establishment of cancer centers and cancer registries. The Cancer Registry Act of North Rhine-Westphalia of 1985 created the legal basis for epidemiological cancer registration in the administrative district of Münster. Pathologists were still unable to report to the registry. It was not until the establishment of an electronic procedure for transmitting pathology reports using control numbers in the early 2000s that pathologists were able to report directly to the registry. Only then was it possible to achieve the required completeness of registration of 95%.In 2005, an amendment to the law led to the comprehensive expansion of cancer registration in North Rhine-Westphalia. Since then, reports could only be submitted electronically. The Cancer Early Detection and Registration Act (KFRG) of 2013 led to the amendment of the Cancer Registry Act in North Rhine-Westphalia in 2016. Since then, the cancer registry has been an epidemiological and clinical cancer registry. The cancer registry currently covers a population of 18 million people and registers 173,000 new cases of cancer per year (including non-melanotic skin cancer).
INTRODUCTION:According to international studies, people employed in agriculture are at increased risk of suicide. To date, there is little evidence on this subject for Germany. The aim of this study was to estimate, on the basis of secondary data, the suicide rates among people in agriculture-related occupations (green occupations) compared with the rest of the population. METHODS:A population-based descriptive study using age- and sex-specific aggregated data from the Bavarian Police Crime Statistics (PCS) on suicides between 2017 and 2024. The PCS classifies suicide cases according to the primary occupation. Various population statistics were used as the reference populations; due to methodological uncertainties in these populations, the calculated suicide rates should be interpreted as estimates. RESULTS:During the study period, 463 suicides were recorded among individuals primarily employed in green occupations (an average of 57.9 ± 14.7 cases per year). The estimated suicide rates were significantly higher than those of the rest of the population, both overall and by sex and age. Among men aged 18 to 59-the group with the most reliable data-the suicide rate was 3.4 times higher (95% CI (confidence interval): 2.9-4.0). Even when using expanded populations and allowing for underreporting in the population, a suicide rate at least 50% higher remained robust. DISCUSSION:The increased risk is consistent with international studies. The Bavarian PCS provides, for the first time in a major German federal state, an estimate of elevated suicide rates among those primarily employed in green occupations. Limitations include the limited depth of the PCS data and a possible underreporting of full-time employees outside the family in the reference data used. The findings should be confirmed with other data. Causal factors were not investigated. Suicide prevention measures appear warranted.
Die Krebsregistrierung in Nordrhein-Westfalen (NRW) begann 1974 und hatte 2024 ihr 50-jähriges Jubiläum. Das Ziel dieses Beitrags ist es, die Aufbauphasen der Krebsregistrierung in NRW der Jahre 1974 bis 2025 darzustellen. Nach einem internationalen Symposium in Düsseldorf 1973 und mit Unterstützung der Gesellschaft zur Bekämpfung der Krebskrankheiten (später: Krebsgesellschaft NRW) wurde 1974 in Münster unter Federführung von Prof. Ekkehard Grundmann ein Krebsregister zunächst mit dem Fokus auf die onkologische Nachsorge aufgebaut. Da Pathologen ohne direkten Patientenkontakt nicht direkt an das Register melden konnten, blieb die Erfassung zunächst eingeschränkt. Bundespolitische Debatten der Jahre 1978 bis 1981 förderten den Aufbau von Krebszentren und Krebsregistern. Mit dem Krebsregistergesetz NRW von 1985 wurde die rechtliche Grundlage der epidemiologischen Krebsregistrierung im Regierungsbezirk Münster geschaffen. Die Einbindung der Pathologie erfolgte erst Anfang der 2000er-Jahre durch die Etablierung eines elektronischen Verfahrens zur Übermittlung von Pathologieberichten unter Nutzung von Kontrollnummern. The proportion of very elderly people in the German population is steadily increasing. As a result, their specific needs-for example, with regard to regional healthcare and support structures-are becoming increasingly important. This article introduces the current issue with an overview of the living conditions and health status of people aged 80 and over.First, the population group is described based on demographic characteristics. Women clearly predominate among the very elderly; furthermore, gender-specific differences are evident in terms of income, remaining life expectancy, and marital status. Their share of the population varies regionally-in some cases considerably-and is linked to the respective region's development potential. The proportion of very old people in the population is particularly high in structurally weak and low-income regions.Based on the representative Gesundheit 65+ study (Robert Koch Institute, RKI) and D80+ study (ceres), key data on the health status of the very elderly are presented. The article addresses subjective health, health limitations, diseases and multimorbidity, need for care, and dementia. The health data, which also highlight gender-specific differences and health inequalities, point to fields of action that should be addressed in order to ensure equal living conditions. Structurally weak regions in particular face major challenges in providing the necessary care and health infrastructure for a dignified life in old age.
INTRODUCTION:In light of demographic changes, preventing falls among very old people is becoming increasingly important. This article examines the incidence of falls among people aged 80 years and over in Germany, focusing particularly on the impact of medication use. METHODS:The analysis is based on data from the Gesundheit 65+ longitudinal epidemiological study of the health of people aged 65 and over in Germany. The analysis examined the prevalence of falls over a 12-month period among people aged 80 and over, as well as risk factors such as previous falls, polypharmacy (the long-term use of five or more medications), the use of fall-risk-increasing drugs (FRIDs), physical and cognitive limitations, and sociodemographic factors. RESULTS:The study population comprised 1742 individuals aged 80 and over. Data from 742 participants were available for the medication analysis. At baseline, 38.1% of participants reported at least one fall prior to the start of the study. Within the 12-month period following the baseline survey, 46.7% of participants experienced a fall. The prevalence of polypharmacy was 48.7%, and 59.2% of participants were taking at least one FRID. In the adjusted regression model, urinary incontinence, visual impairment, and depressive symptoms were associated with an elevated probability of falling. Polypharmacy and FRIDs were also associated with falls, but this association was not statistically significant after adjustment. DISCUSSION:The high prevalence of falls among people aged 80 and over highlights the need for more effective fall prevention measures in Germany. Key areas for preventive measures include mobility limitations, previous falls, urinary incontinence, visual impairment, depressive symptoms, polypharmacy, and FRIDs.

Der Beitrag beschreibt das Sturzgeschehen bei Personen ab 80 Jahren in Deutschland und untersucht relevante Risikofaktoren mit besonderem Fokus auf den Arzneimittelgebrauch. Grundlage sind Daten der epidemiologischen Längsschnittstudie Gesundheit 65+ zur gesundheitlichen Lage älterer Menschen in Deutschland. Analysiert wurden die 12-Monats-Sturzprävalenz bei Personen ab 80 Jahren sowie Risikofaktoren wie vorangegangene Stürze, Polypharmazie (dauerhafte Einnahme von ≥ 5 Arzneimitteln), sturzbegünstigende Arzneimittel (Fall-Risk-Increasing Drugs, FRIDs), körperliche und kognitive Einschränkungen sowie soziodemografische Merkmale. This discussion paper examines preventive home visits (PHVs) as low-threshold, outreach-based counselling and support services aimed at promoting the health of older people. PHVs can help to identify risks such as the risk of falls, social isolation or nutritional deficiencies at an early stage, strengthen health and care literacy, and support independent living in the home environment. In Germany, there is a growing number of projects whose approaches differ significantly in terms of target groups, access routes, the qualifications of visiting staff, the range of services, evaluation, organisational structure and funding. The analysis shows that PHVs are situated at the interface between local authority support for older people, the health service and long-term care. A key challenge lies in the long-term funding and sustainability of services that have so far often been organised on a project basis. Sustainable implementation requires conceptual clarification, robust evaluations and cross-sectoral structural and funding models. The prevention of the need for long-term care should be given greater focus in health policy and be more firmly embedded within the statutory health and long-term care insurance systems.
In diesem Diskussionsbeitrag werden Präventive Hausbesuche (PHB) als niedrigschwellige, aufsuchende Beratungs- und Unterstützungsangebote zur Gesundheitsförderung älterer Menschen diskutiert. PHB können dazu beitragen, Risiken wie Sturzgefahr, soziale Isolation oder Ernährungsmängel frühzeitig zu erkennen, die Gesundheits- und Versorgungskompetenz zu stärken und selbstständige Lebensführung im häuslichen Umfeld zu unterstützen. In Deutschland existiert eine wachsende Zahl von Projekten, deren Konzepte sich hinsichtlich Zielgruppen, Zugangswege, Qualifikation der Besuchskräfte, Leistungsspektrum, Evaluation, Trägerschaft und Finanzierung deutlich unterscheiden. Die Analyse zeigt, dass PHB an der Schnittstelle von kommunaler Altenhilfe, Gesundheitswesen und Pflege angesiedelt sind. Eine zentrale Herausforderung besteht in der langfristigen Finanzierung und Verstetigung bislang vielfach projektförmig organisierter Angebote. Für eine nachhaltige Implementierung sind konzeptionelle Klärungen, belastbare Evaluationen sowie sektorübergreifende Struktur- und Finanzierungsmodelle erforderlich. Die Prävention von Pflegebedürftigkeit sollte gesundheitspolitisch gezielter fokussiert und strukturell stärker in der gesetzlichen Kranken- und Pflegeversicherung verankert werden.
INTRODUCTION:Physical functional capacity is central to autonomy and participation in very old age. The aim of this study is to describe functional limitations among the oldest old in Germany using a global indicator (Global Activity Limitation Indicator - GALI) and domain-specific indicators (activities of daily living, mobility, sensory function, urinary incontinence). In addition, frailty is examined as a risk factor. METHODS:The analyses are based on data from the 2023 German Ageing Survey (DEAS) (n = 938) and the 2021/22 Health 65+ study (n = 2028) for persons aged 80 and older living in private households. GALI is analysed across datasets, while further indicators are analysed separately for each dataset. Results are stratified by gender and income. RESULTS:About one quarter of the oldest old have severe functional limitations; this finding is consistent across both datasets. In daily living activities, men show significantly better scores than women. Around one third are severely limited in walking more than one kilometre, and just under one sixth are severely limited in climbing stairs. Severe sensory limitations affect 10% in vision and 27% in hearing. Urinary incontinence is reported by 40% of the oldest old, more often by women than by men. According to the selected criteria, almost half are robust, 43% are pre-frail and 8.6% are frail. Differences between income groups are only partly evident and are not consistent. DISCUSSION:Functional limitations are widespread among the oldest old living in private households, particularly among women and, in some cases, among those with low income. Sensory impairments, urinary incontinence and frailty are common and highly relevant for health and social care. Prevention and care should start early and take gender and socioeconomic position into account.
Climate change poses significant challenges to people and societies at regional, national, and global levels. The associated extreme weather events increasingly affect older and very old people, particularly in rapidly ageing societies such as Germany. However, the diversity of ageing is not systematically considered in the discourse on adaptation strategies and environmentally conscious behavior. In particular, the oldest old who depend on care are often overlooked in both gerontology and environmental research.This article aims to shed light on the research field emerging at this intersection. First, we present the current state of research in gerontology and in environmental and sustainability research, before we outline their relation. Then, we identify gaps in the literature concerning adaptation strategies for the oldest old and their opportunities for environmentally conscious behavior. We conclude with a look at future theoretical and empirical development opportunities and necessities in this field.
Der Klimawandel stellt das menschliche Zusammenleben auf regionaler, nationaler und globaler Ebene vor große Herausforderungen. Die damit einhergehenden Extremwetterereignisse betreffen gerade in schnell alternden Gesellschaften wie Deutschland zunehmend ältere und hochaltrige Menschen. Im Diskurs über Anpassungsstrategien und umweltbewusstes Handeln wird die Diversität des Alter(n)s jedoch bislang nicht systematisch untersucht. Insbesondere Hochaltrige, die auf Unterstützung oder Pflege angewiesen sind, geraten sowohl in der Gerontologie als auch in der Umwelt- und Nachhaltigkeitsforschung häufig aus dem Blick. Ziel des Beitrags ist es, das an der Schnittstelle dieser beiden Forschungsbereiche entstehende neue Forschungsfeld zu beleuchten. Hierzu stellen wir zunächst den aktuellen Forschungsstand in der Alter(n)sforschung sowie der Umwelt- und Nachhaltigkeitsforschung dar und gehen anschließend auf die bisherigen Erkenntnisse zu ihren Verbindungen ein. Dabei zeigen wir bestehende Forschungslücken hinsichtlich der Anpassungsstrategien Hochaltriger sowie ihrer Möglichkeiten zu umweltbewusstem Handeln auf und skizzieren Perspektiven für die zukünftige theoretische und empirische Weiterentwicklung dieses Forschungsfeldes.

The risk of health impairments and the need for care increases with age. Approximately 14 % of people in need of care in Germany are cared for in nursing homes; among nursing home residents, the proportion of very old people is around 70 %. Nursing home residents are particularly vulnerable due to a variety of issues, but the data available on the health status of this target group are insufficient. This narrative review summarizes the empirical findings from Germany over the past decade on the health status of very old people in nursing homes. Furthermore, it presents intervention approaches and current reports on the quality of care.A systematic search was conducted in MEDLINE via PubMed in November 2025 and supplemented by a free search for quality reports. The search yielded a total of 1597 hits, and 76 publications were ultimately included. The studies were very heterogeneous in terms of topics and reporting. The main topics were skin health, medication, and challenging behavior in people with dementia, with some marked differences in reported frequencies. Relevant aspects such as pain, nutrition, oral health, and quality of life were not addressed or only rarely. Only a few reports were available on the quality of care.Overall, the health situation of very old people in nursing homes has not been adequately described to date. There is a need for methodologically robust studies with appropriate data collection methods and outcome measures in order to comprehensively assess and report on the specific needs of this target group.
Alzheimer's disease and other dementias are among the most serious widespread diseases, with societal and healthcare-system relevance expected to increase further in the coming years. Projections of the future disease burden are correspondingly concerning. At the same time, considerable progress has been made in research, particularly in the field of risk reduction. A broad range of established protective and risk factors at the individual level has been identified, alongside a growing number of related intervention studies. Increasingly, however, attention is shifting toward population-based measures aimed at reducing dementia cases in the long term.This article provides an overview of the current state of research. First, drawing on recent data on disease burden, established risk factors are summarized. Building on this, more recent and previously less considered potential determinants are discussed. The review further outlines existing intervention approaches and examines the relevance of population-based prevention strategies for public health and health policy.
