PURPOSE:To compare corneal characteristics of eyes with high myopia with those of eyes with no spherical ametropia using Orbscan and ultrasonic pachymetry.METHODS:Orbscan and ultrasonic pachymetry values were prospectively recorded in a study group of 105 patients with high myopia (i.e., axial length greater than 26mm in both eyes) and in a control group of 105 patients with no spherical ametropia (absolute value of spherical equivalent less than 1.25D regardless of cylinder value). Astigmatism data were expressed by rectangular coordinates in a dioptric plane. Axis was decomposed in 2 components (WTR/ATR and oblique) which were analyzed by Cos2axis and Sin2axis trigonometric functions. Enantiomorphism (mirror-image symmetry) between fellow eyes was quantified by a Euclidean distance for the location of the thinnest point and by the difference (in absolute value) between 180° and the sum of both axes for astigmatism.RESULTS:In the study group, the mean axial length and subjective spherical equivalent were, respectively, 27.82±2.14mm (26.00 to 34.06) and -9.00±3.46D (-4.71 to -19.82). The mean corneal astigmatism was +0.92D×91.3° in the study group and +0.65D×89.3° in the control group. The mean corneal cylinder was higher in the study group (1.44D versus 0.91D; P<0.001) whereas axis showed no significant differences between both groups. The mean maximal keratometry was steeper in the study group (44.53D versus 44.13D; P=0.03) whereas the mean keratometry and minimal keratometry displayed no significant differences between both groups. No significant differences in central corneal thickness (540.2μm versus 546.9μm; P=0.10), peripheral corneal thickness, corneal diameter, corneal irregularity, asphericity, and irregular astigmatism were found between both groups. There were no significant differences in enantiomorphism parameters between both groups. In the study group, correlation with axial length was significant only for spherical equivalent (r=-0.86; P<0.001) and corneal cylinder (r=0.16; P=0.04).CONCLUSION:High myopia exhibits corneal characteristics similar to corneas of eyes with no spherical ametropia, except for toricity. While the posterior segment elongates, development of high myopia appears not to affect corneal characteristics. La pachymétrie ultrasonique et l’Orbscan ont été prospectivement réalisés sur 105 myopes forts (longueur axiale supérieure à 26 mm) et sur 105 patients sans amétropie sphérique (équivalent sphérique inférieur à 1,25D en valeur absolue, indépendamment du cylindre). Les données de l’astigmatisme ont été transformées en coordonnées rectangulaires dans un plan dioptrique. L’axe a été décomposé en 2 composantes (directe/inverse et oblique) analysées par les fonctions trigonométriques (Cos2axe et Sin2axe). L’énantiomorphisme (symétrie en miroir) des 2 yeux d’un patient a été quantifié par une distance euclidienne pour la localisation du point le plus fin et par la différence (en valeur absolue) entre la somme des 2 axes et 180° pour l’astigmatisme. Dans le groupe myope, la longueur axiale était de 27,82 mm (±2,14 ; 26,00 à 34,06) et l’équivalent sphérique subjectif de −9,00D (±3,46 ; −4,71 à −19,82). L’astigmatisme cornéen moyen était de +0,92D × 91,3° pour les myopes et de +0,65D × 89,3° pour l’autre groupe. Le cylindre cornéen était significativement plus élevé chez les myopes (1,44D contre 0,91D ; p < 0,001) sans différence significative pour l’axe. La kératométrie maximale était plus élevée dans le groupe myope (44,53D versus 44,13D ; p = 0,03) contrairement à la kératométrie moyenne et à la kératométrie minimale qui ne présentaient pas de différence. Aucune différence significative entre les 2 groupes n’a été retrouvée pour l’épaisseur centrale (myopes : 540,2 μm versus 546,9 μm ; p = 0,10), l’épaisseur périphérique, le diamètre cornéen, l’irrégularité cornéenne, l’asphéricité ou l’astigmatisme irrégulier. Le degré d’énantiomorphisme ne présentait aucune différence entre les 2 groupes. Dans le groupe myope, la corrélation avec la longueur axiale était uniquement significative pour l’équivalent sphérique (r = −0,86 ; p < 0,001) et le cylindre cornéen (r = 0,16 ; p = 0,04). À l’exception de la toricité, la cornée des myopes forts est similaire à la cornée des yeux sans amétropie sphérique. Contrairement à l’élongation du segment postérieur, le développement de la myopie forte ne semble pas concerner la cornée. En perturbant le mécanisme d’emmétropisation de l’œil, la toricité de la cornée pourrait être un facteur favorisant l’évolution vers la myopie forte. The spherical component had no significant influence on refractive astigmatism (r(s)≤ 0.07, P ≥ 0.07) except for high spherical ametropia. Eyes with spherical equivalent greater than 4D (in absolute value) demonstrated higher cylinder (1.15D vs 0.84D, P<0.001). Cylinder influenced the WTR/ATR ratio (r(s)=-0.25, P<0.001) and the enantiomorphism (r(s)=0.36, P<0.001). Age also influenced the WTR/ATR ratio (r(s)=0.27, P<0.001) and the enantiomorphism (r(s)=0.14, P<0.001). Axes were more likely WTR and enantiomorphic when the cylinder was high and the subject young. Oblique axes were less enantiomorphic (35.5° vs 20.6°, P<0.001) and were associated with lower cylinder (0.56D vs 0.98D, P<0.001). Correlation between fellow eyes was significant for cylinder (r(s)=0.66, P<0.001) and for spherical equivalent (r(s)=0.96, P<0.001). The growth and function of the epididymis are regulated by testosterone produced by Leydig cells in the testes. In the present study it was observed that neutralization of endogenous follicle stimulating hormone (FSH) in immature rats using a highly specific antiserum to ovine FSH resulted in changes in the histology of the epididymis along with a decrease (50–60%) in its weight compared with the normal serum-treated controls. These changes were observed in both rat and monkey epididymis without any decrease in serum testosterone, on which epididymis is known to be dependent. A detailed study was therefore carried out on the effects of deprivation of FSH or testosterone on the histology of epididymis. The changes in epididymal histology following FSH deprivation included a decrease in the size of the tubule lumen in the rat as well as in the adult male bonnet monkey in which the antiserum against ovine FSH was raised. Intensive vacuolization and uneven surface of the luminal epithelium was also observed. In contrast, the effect of deprivation of testosterone support by way of administration of LH antiserum or flutamide resulted in a decrease in the size of the lumen and degenerative changes. These results suggest that cauda epididymidis is a target for FSH action.
Analyser les relations de symétrie de la cornée par rapport à l’œil controlatéral dans une population « normale ». Les 2 cornées de 325 patients ont été prospectivement analysées avec la topographie d’élévation Orbscan et la pachymétrie ultrasonique. L’énantiomorphisme (symétrie en miroir) des données directionnelles (axe de l’astigmatisme, localisation du point le plus fin) a été quantifié par des indices spécifiques calculés à partir des coordonnées cartésiennes (x,y). Les cornées droites et gauches ne sont pas différentes (p > 0,05) et présentent une forte corrélation (r ≥ 0,90 p < 0,001) pour les principaux paramètres géométriques ou optiques de la face antérieure et pour la pachymétrie. La pachymétrie centrale ultrasonique présente la plus faible différence entre les 2 yeux (1,2 % soit6,6 μm) et la plus forte corrélation (r = 0,97 p < 0,001). L’épaisseur cornéenne présente un degré élevé d’énantiomorphisme : dans 85 % des cas, la pachymétrie minimale est localisée dans le même quadrant pour les 2 yeux (74 % dans le quadrant inféro-temporal). L’astigmatisme cornéen est plus énantiomorphe que l’astigmatisme oculaire ou réfractif (17° par rapport à la symétrie en miroir contre 27° p < 0,001). Le degré d’énantiomorphisme de l’astigmatisme cornéen augmente avec le cylindre (r = 0,33 p < 0,001). Le degré d’énantiomorphisme de la cornée diminue avec l’âge (pachymétrie : r = 0,44 p < 0,001 ; astigmatisme : r = 0,21 p = 0,001). Les données de la face postérieure de la cornée présentent une corrélation moins élevée par rapport à l’œil controlatéral (r = 0,72 p < 0,001). La moins forte symétrie par rapport à l’œil controlatéral des données de la face postérieure de la cornée est liée à une qualité inférieure des mesures. L’origine de l’énantiomorphisme présenté par la cornée (épaisseur, toricité) est encore mal connue : simple conséquence du plan sagittal de symétrie de l’embryogenèse ou réel avantage phylogénique ? La cornée présente par rapport à l’œil controlatéral un degré élevé de symétrie et d’énantiomorphisme pour les principales caractéristiques géométriques et optiques. Toutefois, la relation de symétrie entre les 2 yeux tend à diminuer avec l’âge.
Background: Diagnosis of bacterial endophthalmitis (BE) often fails due to: (1) insufficient volumes of vitreous fluid (VF) and aqueous humour (AH); (2) lack of sensitivity of culture; (3) antibiotic treatments; (4) polymerase chain reaction (PCR) cross-contamination; and (5) limitations on the interpretation of the real-time PCR melting curve. We developed a fast real-time (f-real-t) PCR to improve the performance of the laboratory diagnosis of BE.Methods: The following samples were processed after adding an internal control: phosphate buffered saline (PBS); VF, AH and cell suspensions spiked with Bacteria (Bac); VF and AH from patients with endophthalmitis; and VF and AH from non-infective patients. DNA was extracted (MagNA Pure (R)) and added to four tubes containing selected primers and probes for the identification and quantification of all Bac and eight genera by f-real-t PCR. Diagnostic performances based on direct microscopic examination, culture and f-real-t PCR were compared.Results: The f-real-t PCR detected at least 0.01 colony-forming units (CFU) of Bac/mu l with no cross-reactivity with fungi. Correlation with culture-positive results was 100%. Sixty per cent of BE samples tested culture-positive, but f-real-t PCR tested positive for 90%. Samples from noninfective cases were negative.Conclusion: The f-real-t PCR detected and quantified Bac, Staphylococci, Streptococci, Haemophilus, Pseudomonas, Enterobacteria, Acinetobacter, Propionibacteriacae and Corynebacteria in one run. Cultures required several hours to days (with a non-negligible number of false-negative results) and the f-real-t PCR was completed in 90 min. The f-real-t PCR is presented as a new tool for the diagnosis of BE: its usefulness requires validation with larger series of samples.
La fréquence des décollements de rétine chez les patients greffés est de l’ordre de 2 à 5,4 % selon la littérature. Il s’agit d’une complication grave dont le pronostic visuel est mauvais dans la plupart des études. Ces mauvais résultats sont imputés à la possibilité de retard diagnostique et à la difficulté de visualisation du fond d’œil. L’étude rétrospective a porté sur sept patients (huit yeux) greffés qui ont présenté un décollement de rétine opéré dans le service entre 2003 et 2008. Dans la plupart des cas, le décollement de rétine est survenu après un traumatisme avec lâchage de suture chez des patients jeunes greffés pour kératocône (3 cas) ou pour glaucome congénital (1 cas). Les autres cas de décollement de rétine sont survenus chez des patients greffés pour décompensation endothéliale sur chirurgie de la cataracte avec issue de vitré (4 cas). La geste chirurgical réalisé a été une vitrectomie avec tamponnement interne dans sept cas, et cryo-indentation seule dans un cas. La vitrectomie a été possible, même lors de mauvaise transparence du greffon, grâce à l’utilisation d’un mini quad avec inverseur dans tous les cas. Le succès anatomique a été de 100 % après une seule chirurgie dans 6 cas (75 %) et deux chirurgies (récidive par PVR) dans deux cas. Les patients ont récupéré une acuité visuelle supérieure à 2/10 dans 2 cas (25 %), supérieure ou égale à 1/20 dans 3 cas (37,5 %) et limitée à VLMB dans les autres cas (37.5 %). Les principaux facteurs de mauvais pronostic sont : l’existence d’une atteinte maculaire (macula soulevée, OMC ou trou maculaire associé au DR) et le retard diagnostic. Les traumatismes oculaires et les ruptures capsulaires avec issue de vitré sont responsables de la majorité des cas de décollement de rétine dans cette série de patients greffés. Il est essentiel de sensibiliser les jeunes patients greffés à la nécessité de se protéger les yeux. Le pronostic visuel dans ce type de chirurgie est d’autant meilleur que le diagnostic et la prise en charge sont rapides.
Nous présentons le cas d’une découverte fortuite d’un hydrops aigu unilatéral. Un homme de 23 ans adressé pour deuxième avis ophtalmologique lors d’une baisse d’acuité visuelle brutale unilatérale à compte les doigts. Le patient présente un léger retard psychomoteur. L’acuité visuelle de l’œil controlatéral est de 10/10 avec + 1.75 (− 1 à 66°). Nous décrirons l’aspect clinique, topographique homo et controlatéral, les images de microscopie confocale (Heidelberg Retina Tomograph II avec module cornéen Rostock), de l’OCT du segment antérieur (Visante) et de la pachymétrie ultrasonique cornéenne. L’examen de l’œil controlatéral est très peu évocateur de kératocône : hypermétropie, kératométrie inférieure à 39,7 dioptries, pachymétrie > 540 microns. Les premiers examens sont compatibles avec le diagnostic d’hydrops aigu unilatéral dans le cadre d’un kératocône ; cependant les différents diagnostics différentiels infectieux et traumatiques seront discutés. Malgré l’absence de cas décrit dans la littérature de kératocône unilatéral, ce patient semble présenter les signes cliniques et para cliniques d’un hydrops aigu dans le cadre d’un kératocône.
Analyser l’évolution de la population cellulaire endothéliale après kératoplastie. Étude de cohorte observationnelle rétrospective incluant 1 144 kératoplasties transfixiantes et kératoplasties lamellaires antérieures centrales consécutives réalisées entre 1992 et 2006. Une microscopie spéculaire grand champ a été réalisée pendant la 2° (A2), la 4° (A4), la 6° (A6) et la 10° (A10) année post-opératoire. Le suivi moyen des patients est de 60 mois. Dans le groupe des kératoplasties lamellaires antérieures centrales, la densité endothéliale moyenne (cellules/mm2) et la perte cellulaire correspondante sont de 2093 (−11 %) à A2 et 2037 (−14 %) à A4. Dans le groupe des kératoplasties transfixiantes faites chez des patients ayant un endothélium normal en pré-opératoire, la densité endothéliale moyenne est de 1 767 (−24 %) à A2, 1 398 (−40 %) à A4, 1 096 (−51 %) à A6, et 853 (−61 %) à A10. Dans le groupe des kératoplasties transfixiantes faites pour une pathologie endothéliale, la densité endothéliale moyenne est de 1 414 (−36 %) à A2, 1 142 (−49 %) à A4, 981 (−56 %) à A6, et 920 (−58 %) à A10. Parmi les patients opérés de kératoplasties transfixiantes, la survie du greffon est d’autant plus importante que la densité cellulaire endothéliale à 1 an est élevée. Elle n’est par contre pas influencée par la densité endothéliale pré-opératoire du greffon. Après kératoplastie transfixiante, la perte cellulaire endothéliale constitue le facteur limitant de la survie du greffon à long terme. Si les densités sont plus élevées à court terme chez les patients ayant un endothélium normal avant kératoplastie transfixiante, à moyen et long terme ce facteur n’influence plus l’évolution de la population cellulaire endothéliale qui se raréfie inexorablement. La perte cellulaire endothéliale post-opératoire après kératoplastie lamellaire antérieure est très faible et devrait permettre une survie du greffon à très long terme.
Analyser l’influence de l’âge sur l’astigmatisme subjectif (oculaire) et sur l’astigmatisme cornéen dans une population normale. La réfraction subjective et la topographie (Orbscan) de 500 yeux « normaux » (en dehors d’une éventuelle amétropie) de 250 patients âgés de 9 à 97 ans ont été prospectivement analysées. La composante directe/inverse (ortho-astigmatisme), la composante oblique, l’énantiomorphisme ont été analysées après transformation des coordonnées polaires (cylindre, axe) de l’astigmatisme régulier en coordonnées cartésiennes (x, y) dans un plan dioptrique. Les différentes caractéristiques de l’astigmatisme ont été corrélées avec l’âge. Alors que la magnitude de l’astigmatisme subjectif ne varie pas significativement avec l’âge (p = 0,42), l’axe est significativement influencé par l’âge : la composante directe/inverse est plus influencée par l’âge que la composante oblique (r = 0,36 p < 0,001 et r = 0,09 p = 0,02). La prévalence de l’astigmatisme inverse augmente avec l’âge (moins de 10 % avant 60 ans, 39 % après 60 ans). La diminution de l’énantiomorphisme avec l’âge (r = 0,17 p = 0,007) est davantage liée à une diminution de la symétrie en miroir des axes entre les 2 yeux (p = 0,005) qu’à une asymétrie cylindrique (p = 0,16). L’astigmatisme cornéen est plus énantiomorphe que l’astigmatisme subjectif (p = 0,01). L’irrégularité de la surface cornéenne et l’astigmatisme irrégulier augmentent avec l’âge (r = 0,32 p < 0,001 et r = 0,12 p < 0,001). Les modifications de l’axe (inversion et diminution de l’énantiomorphisme) avec l’âge sont principalement d’origine cornéenne et concernent surtout les astigmatismes « physiologiques » (minimes ou faibles). L’axe de l’astigmatisme semble plus stable avec le temps quand la magnitude de l’astigmatisme est importante. La plupart des propriétés de l’astigmatisme subjectif et de l’astigmatisme cornéen se modifient statistiquement avec l’âge.
To evaluate the variability of the measurements obtained with the Ocular Response Analyzer (ORA), particularly for the corneal hysteresis (CH), corneal resistance factor (CRF) and the corneal-compensated intraocular pressure (IOPcc). Both eyes of 30 healthy volunteers were prospectively measured with the ORA, including ametropia. The standard deviation (SD) and coefficient of variation (CoV) were calculated according to the mean values. The difference between the mean values of eight repeated measurements and the mean values of five or less repeated sessions were illustrated in a diagram. The difference with the fellow eye was expressed in percentage. The mean values and SD/CoV obtained after five measurements were 16.04 ± 1.81/ 0.11 for the IOPcc, 10.64 ± 1.04/ 0.10 for the CH, and 10.72 ± 0.87/ 0.08 for the CRF. The mean values of five repeated exams only differed by 2.05% CRF, 2.51% CH, and 3.19% IOPcc from the mean values of eight exams, where as two exams differed by 3.9% CRF and 7.1% IOPcc. The ORA is at present the only device capable of quantifying the CH and CRF. Our mean values conformed to the mean values of recent studies from other authors. Regarding to the clinical application of the ORA the variability of the results makes it difficult to analyze the complexity of the biomechanical, especially if modified by surgery or other pathologies. We obtained repeatable scores especially for the CRF followed by the CH and lower results for the IOPcc. The variability of the results should be compensated by increasing the number of measurements depending on the required parameter. Further studies and technical evolution will help to improve standard methods to quantify visco-elastic properties of the cornea.
Analyser les relations entre l’astigmatisme oculaire (réfractif) et l’astigmatisme cornéen (antérieur et postérieur) d’une population normale. L’astigmatisme oculaire (réfraction subjective, réfractométrie automatique) et l’astigmatisme cornéen (Orbscan) ont été prospectivement analysés sur 600 yeux « normaux » (en dehors d’une éventuelle amétropie) de 300 patients. Les expressions « non polaires » (coordonnées cartésiennes, vecteur) ont été utilisées pour analyser les différences propriétés de l’astigmatisme : composante directe/inverse, composante oblique, énantiomorphisme, astigmatisme cornéen total (antérieur + postérieur). Les cylindres cornéens antérieurs présentent une plus forte corrélation avec le cylindre subjectif que le cylindre cornéen postérieur (r ≥ 0,77 p < 0,001 contre r = –0,44 p < 0,001). L’utilisation de l’astigmatisme cornéen postérieur n’améliore pas la prédiction du cylindre subjectif ou de l’axe subjectif. Le poids de l’astigmatisme cornéen antérieur dans l’astigmatisme subjectif est corrélé au cylindre subjectif (r = 0,54 p < 0,001. 83 % pour les cylindres ≥ 3D, 52 % pour les cylindres ≤ 0,50D). L’auto-réfractomètre est la meilleure technique pour prédire le cylindre subjectif, en particulier quand celui-ci est faible (r = 0,90 p < 0,001). L’asymétrie cylindrique entre les 2 yeux n’est pas significativement différente pour l’astigmatisme subjectif et pour l’astigmatisme cornéen antérieur (p = 0,08). L’axe de l’astigmatisme subjectif n’est pas statistiquement lié à la puissance cylindrique qu’elle soit cornéenne ou subjective (p > 0,05). La différence d’axe (en valeur absolue) entre l’astigmatisme subjectif et l’astigmatisme cornéen antérieur diminue avec la magnitude de l’astigmatisme (r = 0,42 p < 0,001). L’énantiomorphisme de l’astigmatisme entre les 2 yeux augmente avec le cylindre (r = 0,43 p < 0,001) et est plus important pour l’astigmatisme cornéen antérieur que pour l’astigmatisme oculaire (16,5° par rapport à la symétrie en miroir contre 27,4° p < 0,001). D’un point de vue théorique, la chirurgie réfractive cornéenne est d’autant plus pertinente pour corriger un astigmatisme réfractif que le cylindre est élevé puisqu’elle constitue dans ce cas un traitement étiologique. L’astigmatisme cornéen antérieur joue un rôle prépondérant dans l’astigmatisme réfractif en particulier quand la magnitude de ce dernier est élevée.
