The study addresses the gap in rehabilitation care of people with cognitive and/or multiple Impairments. Conventional medical care structures are often insufficiently adapted to the needs of this patient group. In this project, the rehabilitative care gap is practically closed with a social space-oriented rehabilitation concept for people with cognitive and/or multiple Impairments and to create sustainable solutions. The challenges of needs-oriented rehabilitative care for IdmD and the solutions from the experimental pilot project are presented and discussed.This study was designed as a prospective, monocentric pilot study with an exploratory character. An interprofessional team systematically documented the challenges and inhibiting factors during the implementation and intervention phase in a laboratory book.A total of 15 people with different cognitive and/or multiple Impairments and contextual factors participated in the pilot study. Even during the preparation for the intervention phase, challenging structural aspects arose, particularly in resource and process planning. During the intervention phase, organizational challenges came to the fore, such as scheduling appointments with caregivers, coordinating transport services and managing room availability. Direct interactions with patients required spontaneity and improvisation to address individual behaviors and challenging situations effectively.The social environment-oriented rehabilitation concept shows that the heterogeneous needs of people with cognitive and/or multiple Impairments can be addressed individually and that participants can be successfully supported in achieving their participation goals. The social environment-oriented rehabilitation concept is practical to implement. It appears to be a promising development in the health care system in the context of demographic changes, increase in chronic illnesses, and increased use of outpatient medical treatment.
Die Studie befasst sich mit der rehabilitativen Versorgungslücke für Menschen mit kognitiven und/oder mehrfachen Beeinträchtigungen. Die herkömmlichen medizinischen Versorgungsstrukturen sind häufig unzureichend an die Bedürfnisse dieser Patientinnen- und Patientengruppe angepasst. In diesem Projekt wird die rehabilitative Versorgungslücke mit einem sozialraumorientierten Rehabilitationskonzept für Menschen mit kognitiven und/oder mehrfachen Beeinträchtigungen praktisch geschlossen und wissenschaftlich begleitet, um die bestehende Versorgungslücke in der Rehabilitation dieser Patientinnen- und Patientengruppe zu adressieren und nachhaltige Lösungsansätze zu schaffen. Es werden die Herausforderungen einer bedarfsorientierten rehabilitativen Versorgung und die Lösungsansätze aus dem experimentellen Pilotprojekt dargestellt und diskutiert. Diese Studie wurde als prospektive, monozentrische Pilotstudie mit explorativem Charakter konzipiert. Ein interprofessionelles Team dokumentierte systematisch die Herausforderungen und hemmenden Faktoren während der Implementierung und Interventionsphase in einem Laborbuch. Es nahmen 15 Menschen mit unterschiedlichen kognitiven und/oder mehrfachen Beeinträchtigungen und Kontextfaktoren an der Pilotstudie teil. Bereits in der Vorbereitung der Interventionsphase gab es herausfordernde strukturelle Aspekte, insbesondere bei der Ressourcen- und Ablaufplanung. Während der Interventionsphase traten organisatorische Herausforderungen in den Vordergrund, z. B. die Terminplanung mit Zugehörigen, Transportdiensten und die Raumplanung. Im direkten Kontakt mit den Patientinnen und Patienten waren Spontaneität und Improvisation erforderlich, um auf individuelles Verhalten und herausfordernde Situationen flexibel reagieren zu können. Das sozialraumorientierte Rehabilitationskonzept zeigt, dass die heterogenen Bedürfnisse von Menschen mit kognitiven und/oder mehrfachen Beeinträchtigungen individuell adressiert werden können und die Teilnehmenden beim Erreichen ihrer Teilhabeziele erfolgreich unterstützt werden. Das sozialraumorientierte Rehabilitationskonzept ist praktisch umsetzbar. Es erscheint im Kontext des demografischen Wandels, der Zunahme chronischer Erkrankungen sowie der verstärkten Ambulantisierung medizinischer Behandlungen als eine vielversprechende Entwicklung im Gesundheitssystem.
The entry bonus (ESP) is an innovative benefit designed to promote participation in working life, which has been trialled as part of the rehapro support programme. Its main objective is to facilitate the rapid and self-directed reintegration of people with long-term health impairments into the labour market. The ESP is offered as an alternative to the integration subsidy (EGZ) and provides employees with monthly support of up to €400 over a period of six months, as well as a one-time success bonus after twelve months of employment. The measure is intended to motivate people to take up employment, facilitate their transition into working life and improve their chances of participating in social life.In the cohort study, rehabilitation statistics data (RSD) from insured persons of the pension insurance institutions Mitteldeutschland and Braunschweig-Hannover who received the ESP or EGZ were evaluated. The main target parameter was the reintegration rate six, 13 and 15 months after the start of the measure and a cost comparison. Secondary parameters such as the sectors of employment were analysed for both groups using descriptive analyses. In addition, a binary logistic regression analysis was performed to investigate predictors of 15 months of employment.During the intervention period, 485 persons received ESP. The RSD analysis showed a higher employment rate for the group receiving ESP for the periods of 6 months, 13 months and 15 months after the start of the measure (91.3% for 6 months, 83.7% for 13 months and 81.2% for 15 months after the start of the measure). The employment rate for the EGZ group was 89.3% after 6 months, 68.7% after 13 months and 63.4% after 15 months after the start of the programme. ESP recipients had 2.4 times higher odds to be in employment after 15 months than EGZ recipients. The measure costs per case were 2.8 times higher in the EGZ group than in the ESP group.Reintegration rate and economic analysis of the ESP show a positive outcome of the pilot project. The benefit reaches a target group that has long-term health restrictions and, in some cases, has been unemployed for a long-term period. KEY MESSAGE: : The entry bonus is a non-purpose-tied benefit for employees. Compared to payments to employers, ESP is an alternative option and achieves a better integration rate at lower costs than the EGZ.
Introduction The United Nations underlines the participation in all domains of daily living for people with intellectual and/or multiple disabilities in the Disability Rights Convention, which also includes medical services. In line with this, the German Federal Participation Act has further developed the relevant disability policy at the national level. This also implies access to comprehensive medical care. In 2015, Germany created a legal basis for the establishment of medical centres for adults with intellectual and/or multiple disabilities in order to ensure basic medical care for these patients. However, the medical centres cannot provide complex rehabilitation. Mobile rehabilitation can be another tool to address the underuse of medical rehabilitation for people with intellectual and/or multiple disabilities. Mobile rehabilitation refers to rehabilitation services provided in a patient’s home or local community, rather than in a traditional inpatient or outpatient rehabilitation facility. The advantages of mobile rehabilitation are its accessibility for patients with mobility problems, the comfort of a familiar environment, which can reduce stress, and the fact that rehabilitation can be tailored to the patient’s living conditions and daily routine. In Germany, mobile rehabilitation is currently only available in the field of geriatrics.Within the framework of the feasibility study ‘Social space-oriented individualised medical rehabilitation for people with intellectual and/or multiple disabilities (SIMRE),’ a social space-oriented rehabilitation concept was developed to close the rehabilitation gap for people with intellectual and/or multiple disabilities. It is funded by the German Federal Ministry of Health. This study protocol describes the procedure of this feasibility study.Methods This study is a prospective mixed methods feasibility study. The rehabilitation concept combines outpatient and home-based rehabilitation, medical, and therapeutic care for people with intellectual and/or multiple disabilities.Analysis The primary target criteria are the feasibility and acceptance of the concept by participants, relatives, carers and the rehabilitation staff. Guided interviews with participants and their relatives and/or carers will be analysed using the content-structuring analysis according to Kuckartz. Quantitative analysis will include a cost-benefit analysis to provide information on the economic feasibility of the rehabilitation concept. Changes in individual participation, quality of life and rehabilitation goals will be assessed using a before-and-after comparison with questionnaires. The frequency and type of rehabilitation procedures used will be evaluated quantitatively.The trial was prospectively registered in the German Clinical Trials Register on 17 August 2023. (https://www.drks.de/DRKS00032493).Ethics and dissemination Ethical approval was granted by the Ethics Committee of Hannover Medical School (reference number: 10985_BO_S_2023).1. INTRODUCTION:Collaboration between general practitioners (GPs) and specialist providers is of particular importance in the care of patients with post-acute COVID-19 syndrome. In the ViCoReK project, GPs and their patients had the option of receiving telemedical advice on rehabilitative care, self-management, and psychosocial support from a medical team consisting of members of the departments of rehabilitation medicine and general medicine of a university hospital. In this study, the project was evaluated in terms of its utilization, participant characteristics, added value for GPs, and patient satisfaction. METHOD:In the quantitative evaluation, the number of ViCoReK consultations was counted, and questionnaire data from GPs and patients on their health and experiences with the consultation were analyzed descriptively. RESULTS:Between 09/2023 and 10/2024, 218 ViCoReK consultations took place. Data from 118 patient and 104 GP questionnaires were analyzed. GPs registered patients with a high bio-psycho-social burden and previously unsuccessful treatments to the project, while also appreciating the opportunity to reduce their own workload. The project provided GPs with new insights into treatment options. However, at the time of data collection, 56 % of patients had not yet or only partially implemented the recommended measures, which was, e.g., due to difficulties in scheduling appointments for recommended follow-up treatments. DISCUSSION:The project's goal was achieved in that GPs managed to receive support with the care for patients suffering from complex cases of post-COVID. This suggests that projects promoting telemedicine collaboration between GPs and universities on the topic of post-COVID should be supported. These projects should also provide practical solutions for patients to bridge existing gaps in care.

Method The study, conducted in February and April 2024, utilized two focus groups, namely general practitioners who had and those who had not utilized consultation opportunities within the ViCoReK project. The focus groups were subjected to qualitative content analysis. Results The results of the 93- and 102-minute focus groups with 13 general practitioners and one physician assistant showed that only a small number of severely affected patients with post-COVID were currently being treated in the respondents' practices. Caring for these patients was time-consuming and put pressure on general practitioners, as the currently available treatment options were experienced as only partially helpful. The personal dedication of general practitioners encouraged participation in the project: The general practitioners expressed a desire to exchange ideas with experts on the subject of post-COVID and were committed to incorporating digital technologies into their practices. The most significant barriers to their participation were challenges related to the digitalization of their practices, inadequate reimbursement options, and lack of time. Conclusion Despite a willingness to utilise telemedical post-COVID services for networking with specialist outpatient clinics, general practitioners encounter significant barriers to their implementation at the structural level, including technical, financial and temporal constraints. To address these challenges, the development of digital services to support post-COVID care must be guided by principles of accessibility and pragmatism, ensuring their feasibility in general practices, even in the context of limited time.

Ergotherapie hat neben der Physiotherapie eine hohe Relevanz bei der Behandlung von Patient*innen in Universitätskliniken – vor allem hinsichtlich der Entlassungsfähigkeit. Trotzdem spiegelt sich diese Relevanz im derzeitigen System der Diagnosis Related Groups (DRG) nicht adäquat wider. Wie im physiotherapeutischen Bereich stellt sich auch im ergotherapeutischen Bereich in Controlling-Gesprächen immer wieder die Frage nach einem angemessenen Stellenschlüssel. Eine objektivierbare Analyse dieser Fragestellung sollte ein inhaltlich ausdifferenziertes Benchmark bieten, das Fallschweren, Patient*innenzahl und Fachabteilungen mit in die Bewertung aufnimmt. Leider sind diese Daten nur sehr schwer zugänglich, so dass es zwar erste Versuche über den Verband der Universitätsklinika Deutschlands (VUD) gab, aber bislang keine Ergebnisse proklamiert werden konnten. Die Analyse der Qualitätsberichte der Krankenhäuser biete dagegen einen pragmatischen Ansatz zur Erstellung eines Benchmarks – mit entsprechenden Limitationen. Die öffentlich zugänglichen Qualitätsberichte der Krankenhäuser enthalten unter anderem Angaben zum stationären ergotherapeutischen Personal sowie zur Bettenzahl. Um die ergotherapeutische Kapazität innerhalb der verschiedenen Universitätskliniken miteinander vergleichen zu können, wurde die durchschnittliche Zahl der Vollzeitkräfte (VK) des Jahres 2022 mit Hilfe der entsprechenden Bettenanzahl auf 100 Betten normiert. Die Darstellung der durchschnittlichen Personalkapazität erfolgt rein deskriptiv durch Angabe des Mittelwerts, der Standardabweichung und des Medians. Im Durchschnitt haben die analysierten 35 Universitätskliniken 0,84 (±0,45) VK pro 100 Betten im ergotherapeutischen Einsatz. Der Median liegt hier bei 0,82 VK. Im nationalen Vergleich zeigt sich ein Maximalwert von 1,87 VK im Universitätsklinikum Jena und ein Minimalwert von 0 im Universitätsklinikum Düsseldorf. Das hier vollzogene Benchmark anhand der Qualitätsberichte kann eine erste objektivierbare Orientierung über die unterschiedlichen ergotherapeutischen Personalkapazitäten geben. Klinikintern sollte überprüft werden, ob die Vorgaben zur Übermittlung der Daten für die Qualitätsberichte eingehalten werden. Der G-BA sollte an einigen Stellen seine Vorgaben konkretisieren, damit die korrekten Daten geliefert werden und damit die Qualitätsberichte auch ihre Funktion hinsichtlich einer Benchmark-Grundlage erfüllen können.
Einleitung Eine wichtige Schnittstelle bei der Versorgung von Post-COVID-Patient*innen ist die Zusammenarbeit von hausärztlichen Praxen und Spezialambulanzen. Hier setzt das Projekt ViCoReK an, bei dem Hausärzt*innen und Patient*innen sich telemedizinisch von einem universitätsmedizinischen Team aus den Fachbereichen Rehabilitationsmedizin und Allgemeinmedizin zu rehabilitativer Versorgung, Selbstmanagement und psychosozialer Unterstützung beraten lassen konnten. In der vorliegenden Evaluation wurde das Projekt im Hinblick auf die Inanspruchnahme, die Charakteristika der Teilnehmenden, den Mehrwert für Hausärzt*innen und die Patientenzufriedenheit bewertet. Methode In der quantitativen Evaluation wurden die durchgeführten ViCoReK-Beratungen ausgezählt und Fragebogendaten von Hausärzt*innen und Patient*innen zur Gesundheit und zu ihren Erfahrungen mit der Beratung deskriptiv ausgewertet. Ergebnisse Von 09/2023 bis 10/2024 fanden 218 ViCoReK-Beratungen statt. Ausgewertet wurden Daten aus 118 von Patient*innen und 104 von Hausärzt*innen ausgefüllten Fragebögen. Hausärzt*innen meldeten Patient*innen mit hohen Belastungen auf biopsychosozialer Ebene und bisher wenig erfolgreicher Behandlung im Projekt an und sahen darin auch eine Möglichkeit der eigenen Entlastung. Im Projekt erlangten Hausärzt*innen neue Erkenntnisse zu Behandlungsoptionen. Die Zufriedenheit der teilnehmenden Patient*innen war hoch. Jedoch konnten 56 % der Patient*innen die ihnen empfohlenen Maßnahmen zum Erhebungszeitpunkt noch nicht oder nur teilweise umsetzen, beispielsweise aufgrund von Schwierigkeiten bei der Terminvereinbarung für empfohlene Weiterbehandlungsmaßnahmen. Diskussion Das festgelegte Ziel des ViCoReK-Projekts wurde insofern erreicht, als dass Hausärzt*innen bei der Versorgung von Patient*innen mit Post-COVID-Syndrom in komplexen Fällen fachlich unterstützt werden konnten. Dies spricht für die Förderung von Projekten zur telemedizinischen Zusammenarbeit zwischen dem hausärztlichen und dem universitären Bereich zum Thema Post-COVID. Entsprechende Projekte sollten auch bei bestehenden Versorgungslücken pragmatische Lösungen für die Patient*innen vorhalten.
Amputation of a limb is often a necessary, yet irreversible surgical procedure with complex functional, psychosocial, and medical consequences. Comorbidities and concomitant injuries frequently complicate both treatment and rehabilitation. At the same time, modern surgical and technological advances offer new possibilities, while also increasing the demands on comprehensive care. In response, we established an interdisciplinary consultation service to support the multiprofessional care of amputees at our university hospital.A specialized interdisciplinary clinic was established at a German tertiary university medical centre. Its objectives include the structured assessment of amputation indications, quality assurance of orthopaedic-plastic surgical procedures, and holistic post-amputation care addressing functional, psychosocial, and prosthetic aspects. Over a 12-month observation period, we systematically recorded and analysed consultation parameters such as the number of visits, reasons for presentation, and patient and amputation demographics.A total of 474 consultations involving 183 individual patients were documented. Most cases concerned the lower extremity (86.25%), with 13.75% involving the upper extremity. The patient cohort consisted of 63.9% male (n=117) and 36.1% female patients (n=66), with a median age of 55.2±20 years. The most frequent causes of amputation were acute trauma (n=74; 45.7%), vascular diseases (n=24; 14.8%), and infections/sepsis (n=22; 13.6%). The main reasons for presentation were standard follow-up assessments (n=49; 23.1%), postoperative wound checks (n=39; 18.4%), and prosthetic counselling (n=30; 14.2%).The irreversible and multifaceted consequences of amputation require a high level of medical, therapeutic, and socio-medical coordination. Interdisciplinary care models are well established in the management of complex chronic diseases. This approach to amputee care represents a necessary paradigm shift, enabling us to meet the multidimensional needs of this patient population through integrated, patient-centred treatment.
Die Einstiegsprämie (ESP) ist eine innovative Leistung zur Teilhabe am Arbeitsleben, die im Rahmen des rehapro-Förderprogramms erprobt wurde. Ihr Hauptziel ist, die rasche und selbstvermittelte Wiedereingliederung von Menschen mit langfristigen gesundheitlichen Beeinträchtigungen in den Arbeitsmarkt. Die ESP wird als Alternative zum Eingliederungszuschuss (EGZ) angeboten und bietet den Beschäftigten eine monatliche Unterstützung von bis zu 400 Euro über einen Zeitraum von sechs Monaten sowie eine einmalige Erfolgsprämie nach zwölf Monaten im Arbeitsverhältnis. Die Maßnahme soll zur Beschäftigungsaufnahme motivieren, den Übergang in das Erwerbsleben erleichtern und die Chancen auf gesellschaftliche Teilhabe verbessern. In der Kohortenstudie wurden Reha-Statistik-Daten (RSD) von Versicherten der Rentenversicherungsträger Mitteldeutschland und Braunschweig-Hannover, die die ESP bzw. den EGZ bezogen, ausgewertet. Der Hauptzielparameter ist die Wiedereingliederungsquote sechs, 13 und 15 Monate nach Maßnahmenbeginn und ein Kostenvergleich. Sekundäre Parameter wie u. a. Tätigkeitsbranchen wurden mittels deskriptiver Berechnungen für beide Gruppen analysiert. Des Weiteren erfolgte eine binäre logistische Regressionsanalyse zu Untersuchung von Prädiktoren einer 15-monatigen Erwerbstätigkeit. Im Interventionszeitraum bezogen 485 Personen die ESP. Die Analyse der RSD ergibt für die Gruppe der ESP-Beziehenden eine höhere Beschäftigungsquote für die Zeiträume von 6 Monate, 13 Monate und 15 Monaten nach Maßnahmenbeginn (91,3% für 6 Monate, 83,7% für 13 Monate und 81,2% für 15 Monate nach Maßnahmenbeginn). Die Beschäftigungsquote der EGZ-Gruppe lag bei 89,3% nach 6 Monaten, bei 68,7% nach 13 Monaten und bei 63,4% bei 15 Monaten nach Maßnahmenbeginn. ESP-Beziehende hatten eine 2,4-fach erhöhte Chance 15 Monate Erwerbstätigkeit zu erreichen, als EGZ-Beziehende. Die Maßnahmenkosten pro Fall sind in der EGZ-Gruppe 2,8-fach höher als in der ESP-Gruppe. Aus der mit der ESP erreichten Wiedereingliederungsquote und der ökonomischen Analyse lässt sich eine positive Bilanz für das Modellprojekt ziehen. Die Leistung erreicht eine Zielgruppe, die gesundheitlich langfristig eingeschränkt ist und zum Teil langzeitarbeitslos war. Kernbotschaft : Die Einstiegsprämie ist eine nicht zweckgebundene Leistung an Arbeitnehmende. Im Vergleich zu Zahlungen an Arbeitgebende erzielt sie als Wahlalternative eine bessere Eingliederungsquote bei niedrigeren Kosten als der EGZ.
Die Amputation einer Extremität ist ein häufig notwendiger, jedoch irreversibler chirurgischer Eingriff, der mit weitreichenden funktionellen, psychosozialen und medizinischen Folgen einhergeht. Komorbiditäten und Begleitverletzungen können die Behandlung zusätzlich erschweren. Gleichzeitig eröffnen moderne chirurgische und technologische Fortschritte neue Möglichkeiten, stellen jedoch auch erhöhte Anforderungen an die Nachsorge. Vor diesem Hintergrund wurde an unserer Klinik eine interdisziplinäre Sprechstunde zur multiprofessionellen Versorgung von Amputierten eingerichtet. An einem deutschen universitären Maximalversorger wurde eine spezialisierte interdisziplinäre Sprechstunde etabliert. Ziel ist die strukturierte Indikationsstellung zur Amputation, die qualitätsgesicherte Durchführung orthopädisch-plastisch-chirurgischer Eingriffe sowie eine ganzheitliche Nachsorge, die funktionelle, psychosoziale und prothetische Aspekte berücksichtigt. Innerhalb eines Beobachtungszeitraums von zwölf Monaten wurden alle Patientenkontakte hinsichtlich Anzahl, Vorstellungsgründen sowie Patienten- und Amputationsmerkmalen systematisch erfasst und analysiert. Im Erhebungszeitraum wurden 474 Konsultationen bei 183 individuellen Patient:innen dokumentiert. Der Großteil der Fälle betraf die untere Extremität (86,25%), während 13,75% die obere Extremität betrafen. Das Patientenkollektiv bestand zu 63,9% aus Männern (n=117) und zu 36,1% aus Frauen (n=66), mit einem Altersmedian von 55,2±20 Jahren. Die komplexen, oft lebenslang wirksamen Folgen einer Amputation erfordern eine koordinierte, interdisziplinäre Versorgung mit hoher medizinischer, therapeutischer und sozialer Kompetenz. Interdisziplinäre Sprechstunden sind in der Versorgung chronisch und multimorbid Erkrankter etabliert und bewährt. Auch in der Behandlung von Amputierten markiert dieses Modell einen notwendigen Paradigmenwechsel, um eine patientenzentrierte, sektorenübergreifende Versorgung zu ermöglichen.
Background Physiotherapy in acute hospitals is highly relevant both for the recovery process of patients and for reducing hospital stay times. Nevertheless, this relevance is not adequately reflected in the current system of Diagnosis Related Groups (DRG). In university hospitals, the question of an appropriate job ratio for physiotherapeutic staff always arises in controlling discussions. An objective analysis of this question should offer a benchmark that is differentiated in terms of content and that takes case severity, patient numbers and specialist departments into account in the evaluation. Unfortunately, this data is very difficult to access, so although there have been initial attempts through the Association of University Hospitals in Germany (VUD), no results have been declared so far. Material and methods The hospitals' publicly accessible quality reports contain, among other things, information on inpatient physiotherapy staff and the number of beds. In order to be able to compare the physiotherapeutic capacity within the various university hospitals, the average number of full-time employees (FT) in 2022 was standardized to 100 beds using the corresponding number of beds. The average personnel capacity is presented purely descriptively by specifying the mean, median and standard deviation. Results On average, the 35 university hospitals analyzed have 3.10 FT per 100 beds for physiotherapeutic use. The median is 3.03 FT and the standard deviation is 0.98. Conclusion The benchmark carried out here based on the publicly available quality reports can provide an initial objective orientation about the different physiotherapeutic personnel capacities. In the future, a more detailed benchmark that also takes case mix points, specialist areas and case numbers into account inthe evaluation could significantly improve the quality of the results of the survey data.
Objectives To examine the effects of acupuncture and therapeutic exercise alone and in combination on temporomandibular joint symptoms in tension-type headache and to evaluate the potential interaction of existing temporomandibular dysfunction on the success of headache treatment. Design Pre-planned secondary analysis of a randomized controlled, non-blinded trial. Setting Outpatient clinic of a German university hospital. Subjects Ninety-six Participants with frequent episodic or chronic tension-type headache were randomized to one of four treatment groups. Interventions Six weeks of acupuncture or therapeutic exercise either as monotherapies or in combination, or usual care. Follow-up at 3 and 6 months. Main measures Subjective temporomandibular dysfunction symptoms were measured using the Functional Questionnaire Masticatory Organ, and the influence of this sum score and objective initial dental examination on the efficacy of headache treatment interventions was analyzed. Results Temporomandibular dysfunction score improved in all intervention groups at 3-month follow-up (usual care: 0.05 [SD 1.435]; acupuncture: −5 [SD 1.436]; therapeutic exercise: −4 [SD 1.798]; combination: −3 [SD 1.504]; P = 0.03). After 6 months, only acupuncture (−6 [SD 1.736]) showed a significant improvement compared to the usual care group ( P < 0.01). Subjective temporomandibular dysfunction symptoms had no overall influence on headache treatment. Conclusions Only acupuncture had long-lasting positive effects on the symptoms of temporomandibular dysfunction. Significant dental findings seem to inhibit the efficacy of acupuncture for tension-type headache.
INTRODUCTION:The personal and environmental contextual factors of the ICF describe not only physical and mental health but also (occupational) participation. So far, the extent to which these contextual factors are taken into account in the socio-medical evaluation has not been clarified. Contextual factors can guide allocation to disability pension or rehabilitative interventions in an individualized and targeted manner. The aim of this study was to determine the frequency of ICF contextual factors in socio-medical evaluation. METHODOLOGY:A frequency analysis of contextual factors in socio-medical reports for musculoskeletal disorders was performed. Contextual factors mentioned several times were coded once in the report. In the frequency ranking, each environmental and personal factor was categorized as "frequent," "moderate," and "rare." RESULTS:215 socio-medical reports with musculoskeletal diagnoses starting from 2017 were retrospectively analyzed. All socio medical reports were analyzed for the occurence of personal contextual and environmental factors. In particular, personal factors were identified, which provide information about general personal characteristics or the general state of health. Almost half of the environmental factors were rarely identified. CONCLUSION:Many ICF contextual factors are constantly recorded in the analyzed reports. The important influence of these factors on return to work is well known. Thus, the absence or low presence of the ICF contextual factors from the chapters Attitudes (i4), Basic Skills (i4) and Support & Relationships (e3), which are also contextual to the assessed person's experience of the world of work, was surprising. The relevance of the frequently and rarely identified contextual factors for the socio-medical evaluation of musculoskeletal disability pension applicants must be questioned.
