Abstract:Pharmacological therapy remains a cornerstone in the management of glaucoma. The therapeutic spectrum has been broadened by the introduction of new active substances and drug classes, preservative-free topical formulations, fixed combinations, and innovative drug delivery systems. In addition, alternative routes of administration are being investigated, and interventional approaches are increasingly gaining importance as primary treatment options based on emerging clinical evidence. This article provides an overview of current pharmacological therapies, highlighting their clinical relevance for everyday practice.
PURPOSE:To assess the quantitative changes in surgical glaucoma care in German hospitals between 2019 and 2022 with special focus on the impact of the COVID-19 pandemic on overall volume and trends within glaucoma surgery. METHODS:The quality reports of The Federal Joint Committee (G-BA), containing information on the quantity of surgical glaucoma procedures of 296 German hospitals were obtained in machine-readable form for the years 2019, 2020, 2021 and 2022. We analyzed the annual numbers and proportions of different glaucoma surgery types as categorized by German OPS codes, with special focus on 2020, the first year of the COVID-19 pandemic in Germany. RESULTS:The total number of surgical glaucoma procedures in German hospitals in 2020 decreased by 8.5% compared to 2019 and recovered to pre-pandemic levels in 2021. Within filtration surgery, the number of classic trabeculectomy steadily declined while bleb-forming filtration devices were used more frequently. In all four years, cyclodestructive procedures were the most frequently performed interventions overall. CONCLUSION:The impact of the COVID-19 pandemic on overall surgical volume, especially in 2020, was substantial but overall moderate and transient. The trend towards minimally invasive procedures and bleb-forming filtration devices accelerated after 2020, resulting in a pronounced decline of classic filtration surgery, such as trabeculectomy.
Objective: This retrospective study evaluated the safety and efficacy of the new minimally invasive MINIject implant placed in the suprachoroidal space. The aim was to assess its impact on intraocular pressure (IOP) reduction and complication rate. Methods: 18 eyes from 18 patients with insufficiently controlled glaucoma received the implant using topical medications. Outcomes were changes in IOP, change in IOP medication, need for other glaucoma surgery, and rate of adverse events. Results: IOP reduced by 15% (p < 0.05) following MINIject implantation. IOP medication decreased from 3 to 1 agent (p < 0.05). Four patients (22%) required other glaucoma surgery while we did not observe any clinically relevant adverse event. Conclusions: This retrospective study indicates that MINIject implants may be a safe and effective means of reducing IOP together with a reduction in IOP medications in most patients. Larger prospective studies with longer follow-ups are necessary to confirm our results, though.
Purpose To evaluate the efficacy of XEN ® -45 gel stent ab interno implantation for medically uncontrolled uveitic glaucoma. Methods Retrospective analysis of 25 eyes receiving XEN ® gel stent for medically uncontrolled uveitic glaucoma from February 2019 to February 2023 with recording of intraocular pressure (IOP) values, ocular hypotensive medication, requirement for revision or secondary surgery and complications. Prerequisites for XEN ® implantation were a clear cornea, an open iridocorneal angle and an unscarred, mobile conjunctiva at the implantation site. Minimum follow-up required for inclusion was 3 months. The primary outcome measure was IOP compared to baseline. Complete and qualified success were defined as final IOP of ≤ 18 mmHg without or with topical antiglaucomatous treatment, respectively. Failure was defined as IOP > 18 mmHg on two consecutive visits, IOP reduction < 20%, persisting complications from hypotony and open conjunctival bleb revision. Further glaucoma surgical intervention was defined as complete failure. Results Mean preoperative IOP was 35.3 ± 10.9 mmHg on 2.9 ± 0.9 topical antiglaucomatous agents. 19 of 25 patients (76%) received additional oral acetazolamide. 19 eyes were pseudophakic, 5 eyes phakic and 1 aphakic. Early postoperatively, mean IOP reduced to 7.7 ± 3.0 mmHg (75.8% reduction). At final follow-up (mean 17.7 months) mean IOP was 12.0 ± 3.8 mmHg (62.5% reduction) on 0.2 ± 0.6 medications. Six eyes (24%) required bleb revision at mean 28 weeks and therefore were categorized as failure. One eye failed despite bleb revision and restart of topical ocular hypotensive medication. Three other eyes (12%) had IOP spikes with uveitis flare-ups. Transient hypotony complications occurred in 32%. At final follow-up, 18 eyes (72%) achieved complete success and one eye (4%) qualified success. Conclusion The XEN ® gel stent effectively reduced IOP in uncontrolled uveitic glaucoma, with 72% complete success. Bleb revision was required in 24%. IOP spikes occurred in 12% despite functioning blebs. Further follow-up is needed to determine long-term outcomes.

The XEN® gel stent reduces intraocular pressure (IOP) in glaucoma. XEN®-45 is widely used; the newer XEN®-63 has a larger lumen targeting potentially lower IOP outcomes. We retrospectively compared the first 15 XEN®-63 cases to 15 matched XEN®-45 controls. With a preoperative IOP of 18.1 ± 3.9 mmHg (mean ± SD) and a final IOP of 9.1 ± 2.0 mmHg, XEN®-63 implantation resulted in an IOP reduction of 44.6 ± 16.5%. Similarly, with a preoperative IOP of 18.3 ± 4.5 mmHg and a final IOP of 10.3 ± 2.1 mmHg, XEN®-45 implantation resulted in an IOP reduction of 40.1 ± 17.2%. The median follow-up period was 204 days (range 78–338 days) for the XEN®-63 group and 386 days (range 99–1688 days) for the XEN®-45 group. In total, 5/15 eyes of each group underwent open conjunctival bleb revision within the period of observation. Three eyes of the XEN®-63 group had secondary glaucoma surgery. One eye in the XEN®-63 group and three eyes in the XEN®-45 group required a restart of antiglaucomatous medication. In conclusion, both stents effectively lower IOP and medication. XEN®-63 achieved a slightly lower IOP over a short follow-up. Complication and revision rates were similar.
Zusammenfassung Hintergrund Chirurgische Verfahren in der Augenheilkunde unterliegen Schwankungen in ihrer Anwendung und sind abhängig von Neuentwicklungen, Verfügbarkeiten und Erfolgswerten. Vor allem im Bereich der Glaukomchirurgie wurden in den letzten Jahren zahlreiche neue Verfahren eingeführt. Bislang existieren kaum Auswertungen zur tatsächlichen Anwendung dieser neueren Verfahren. Wir stellen in dieser Arbeit dar, in welchem Maße glaukomchirurgische Verfahren in deutschen Krankenhäusern im Jahr 2019 angewandt wurden. Methoden Die Qualitätsberichte der deutschen Krankenhäuser aus dem Jahr 2019 wurden bez. aller glaukomspezifischen Prozedurverschlüsselungen ausgewertet. Verglichen wurden im Folgenden insbesondere Laserverfahren, „klassische“ Glaukomeingriffe und „moderne“ Verfahren wie die MIGS (minimally invasive glaucoma surgery). Ergebnisse Im Jahr 2019 wurden an deutschen Krankenhäusern 49 031 Glaukomprozeduren durchgeführt. Zahlenmäßig am häufigsten wurden zyklodestruktive Verfahren angewandt. Die MIGS machten in den vorliegenden Daten etwa 10% der Eingriffe aus. Bei den filtrierenden Eingriffen wurden etwa 40% implantatunterstützt durchgeführt. Diskussion und Schlussfolgerung Es zeigt sich in den ausgewerteten Daten aus deutschen Krankenhäusern einerseits eine gemischte Erweiterung des Spektrums mit neueren Verfahren wie den MIGS und den Implantaten, andererseits kommen die klassischen Verfahren wie die Zyklodestruktion weiterhin zum Einsatz. Es ist anzunehmen, dass sich in den kommenden Jahren weitere Verschiebungen der Operationsmethoden zeigen werden.
BACKGROUND/AIMS:Retinal haemangioblastomas (RH) remain a major cause of visual impairment in patients with von Hippel-Lindau (VHL) disease. Identification of genotype-phenotype correlation is an important prerequisite for better management, treatment and prognosis.METHODS:Retrospective, single-centre cohort study of 200 VHL patients. Genetic data and date of onset of RH, central nervous system haemangioblastomas (CNSH), pheochromocytoma/paraganglioma (PPGL), clear cell renal cell carcinoma (ccRCC) and pancreatic neuroendocrine neoplasm (PNEN) were collected. The number and locations of RH were recorded.RESULTS:The first clinical finding occurred at an age of 26 ± 14 years (y) [mean ± SD]. In 91 ± 3% (95% CI 88-94) of the patients, at least one RH occur until the age of 60y. A total of 42 different rare VHL gene variants in 166 patients were detected. A higher age-related incidence of RH, CNSH, ccRCC and PNEN was detected in patients with a truncating variant (TV) compared to patients with a single amino-acid substitution/deletion (AASD) (all p < 0.01), while it is reverse for PPGL (p < 0.01). Patients with a TV showed 0.10 ± 0.15 RH per y during their lifetime compared to 0.05 ± 0.07 in patients with AASD (p < 0.02). The median enucleation/phthisis-free survival time in patients with a TV was 56y (95% CI 50-62) compared to 78y (95% CI 75-81) in patients with AASD (p < 0.02).CONCLUSION:Compared to patients with AASD, patients with a TV develop RH, CNSH, ccRCC and PNEN earlier. They experience a higher number of RH and bear a higher risk of enucleation/phthisis. Thus, patients with a TV might be considered for a more intensive ophthalmological monitoring.
Background Precise optic disc size measurements based on anatomically exact disc margins are fundamental for a correct assessment of glaucoma suspects. Computerized imaging techniques, such as confocal-scanning-laser-tomography (CSLT), which applies operator defined boundaries and optical-coherence-tomography (OCT), which incorporates an alternative detectable landmark (Bruch’s-membrane-opening (BMO)), have simplified the planimetry of the optic disc and BMO-area, respectively. This study’s objectives are to compare both modalities for area and to define a threshold for macro-BMO using BMO-OCT. Methods Retrospectively, patients that simultaneously received CSLT and BMO-OCT scans were included. Their images were correlated and agreement was determined using Bland-Altman-analysis. The diagnostic power of a macro-BMO threshold using OCT was derived after creating a receiver-operating-characteristics-curve using the well-established analogous CSLT threshold (2.43 mm 2 ). Results Our study included 373 eyes with a median optic disc area by CSLT/ BMO-area by OCT of 2.56 mm 2 and 2.19 mm 2 respectively. The Bland-Altman-analysis revealed a systematic deviation with a diverging tendency with increasing area, which enabled the creation of the following mathematical relation: disc-area (CSLT)*0.73 + 0.3 = BMO-area (OCT). BMO-area of 2.19 mm 2 showed the best diagnostic power for identifying macro-BMOs using OCT (sensitivity: 75%, specificity: 86%). Conclusions Area measurements (CSLT optic disc area vs. BMO-area by OCT) showed a systematic deviation with a divergent tendency with increasing size. Our mathematical equation offers an estimated comparison of these anatomically diverse entities. Considering BMO-OCT´ anatomical accuracy, the 2.19 mm 2 threshold may improve discernment between glaucoma suspects and norm variants.
Purpose: To study the complication rate of Nd:YAG laser posterior capsulotomy in patients with uveitis.Method: Retrospective case note analysis of pseudophakic uveitis patients having undergone Nd:YAG laser posterior capsulotomy between January 2016 and December 2018. Complications documented included uveitis flare, raised intraocular pressure, intraocular lens damage/displacement, cystoid macular edema, and retinal detachment.Results: There were 39 eyes of 38 patients (20M, 18F; age 27-89 years). Mean interval between cataract surgery and laser was 55 months (range 8-286 months). Mean laser energy was 79 mJ (range 33-207 mJ). At 3 months 62% of eyes achieved a 2-5 Snellen line improvement that was maintained at 12 months. Vision was unchanged in 21% of eyes due to preexisting pathology, with no eyes having worse vision. No post-laser complications were documented.Conclusions: Nd:YAG laser posterior capsulotomy is a safe procedure in uveitis patients, resulting in a good improvement in vision.
Purpose: To compare spectral-domain (SD) and swept-source (SS) optical coherence tomography angiography (OCTA) for imaging retinal capillary hemangioblastomas (RCHs) in von Hippel-Lindau disease (VHLD). Methods: Prospective single-center cross-sectional study. Tumor size (TS) of perfused RCHs was assessed clinically in relation to the optic disc size. For both technologies, SD-OCTA and SS-OCTA, corresponding images with a scan size of 3 x 3 mm(2) and 6 x 6 mm(2), respectively, were overlaid according to the set of marker positions to determine the TS. Results: From 200 patients with VHLD, 48 patients showed 84 RCHs. SD-OCTA images of 39 RCHs (46.4%) and SS-OCTA images of 48 RCHs (57.2%) were suitable for analysis. The average in OCTA-measured TS of 1.60 +/- 2.58 mm(2) (range, 0.01-10.43) was congruent to the clinically assessed TS in 81.3% of cases (r = 0.86, P < 0.0001). TS measured in SD-OCTA compared to SS-OCTA showed similar values and a high correlation (all P < 0.0001). Nevertheless, despite the similarities, a slight trend in SS-OCTA was observed whereby with increasing TS, an elevated TS was detected compared to SD-OCTA (3 x 3-mm(2) scans: mean difference of 0.03 +/- 0.04 mm(2), 6 x 6-mm(2) scans: 0.08 +/- 0.19 mm(2)). However, within the same imaging technology method, TS values almost did not differ (SD-OCTA: mean difference of 0.01 +/- 0.02 mm(2), SS-OCTA: 0.001 +/- 0.01 mm(2)). Conclusions: OCTA may serve as an additional tool for diagnosis and monitoring of RCHs. Nevertheless, due to the differences between the technologies, the values cannot be used interchangeably. Translational Relevance: SD-OCTA and SS-OCTA are suitable to detect and monitor RCHs and provide a more detailed assessment about the TS than this is clinically possible.

Ein 50-jähriger Patient stellte sich im Mai 2016 mit einer umschriebenen Schwellung im Bereich der tarsalen Oberlidseite sowie mit seit etwa 2 Jahren rezidivierendem Tränenlaufen und Fremdkörpergefühl am rechten Auge in unserer Klinik vor. Bereits im Oktober 2013 war bei klinischem Verdacht auf ein Chalazion am rechten Oberlid eine komplikationslose Exzision ab interno durchgeführt worden. Als Allgemeinerkrankung war eine familiäre adenomatöse Polyposis coli (FAP) bekannt, aufgrund derer 2002 eine Proktokolektomie erfolgt war.
BACKGROUND AND OBJECTIVE:The introduction of optical coherence tomography angiography (OCTA) provides new insights into the retinal vasculature. The aim of this study was to explore the value of OCTA in imaging retinal hemangioblastomas and monitoring laser treatment in patients with von Hippel-Lindau (VHL) disease.PATIENTS AND METHODS:Ten eyes of 10 patients with VHL disease were included in this retrospective case series. All patients underwent complete ophthalmological work-up including OCTA for retinal and optic nerve head hemangioblastoma.RESULTS:Two patients showed retinal scars and no recurrence of hemangioblastoma in OCTA. Three patients revealed recurrent hemangioblastomas. Two patients demonstrated a new hemangioblastoma. Three patients showed hemangioblastomas of the optic nerve head. Successful laser photocoagulation could be monitored with OCTA.CONCLUSIONS:Screening for retinal hemangioblastomas with OCTA alone is not possible. OCTA may help to distinguish hemangioblastomas and other lesions in VHL disease, especially after treatment and allows the differentiation from harmless non-vascular lesions in questionable cases. Purpose: Intravitreal antivascular endothelial growth factor (anti-VEGF) application has revolutionized the treatment of choroidal neovascularization (CNV), a hallmark of wet age-related macular degeneration. However, additional treatment options are desirable as not all CNV lesions respond to anti-VEGF injections. Here, we assessed the feasibility of targeted delivery of cationic liposome-encapsulated paclitaxel (EndoTAG-1) in treating CNV. Furthermore, we investigated whether a new formulation of verteporfin encapsulated in cationic liposomes (CL-VTP) enhances the effect of photodynamic therapy (PDT).Methods: EndoTAG-1, LipoSPA, and CL-VTP were produced by encapsulating paclitaxel, succinyl-paclitaxel, or verteporfin in cationic liposomes (CL). Mice underwent argon laser coagulations at day 0 (D0) to induce CNV. EndoTAG-1 and LipoSPA were injected into the tail vein at D1, D3, D5, D7, and D9. Taxol, CL, or trehalose buffer alone was injected in control animals. At D10, all animals were perfused with fluorescein isothiocyanate (FITC)-dextran. Flatmounts comprising the retinal pigment epithelium, choroid, and sclera were prepared for quantifying the CNV by measuring the area of lesions perfused with FITC-dextran. For PDT, mice received an injection with CL-VTP or Visudyne at D10. One eye was treated with PDT while the other served as a control. Evaluation of RPE-choroid-scleral and retinal flatmounts was performed at D12, D14, or D17. Perfusion with FITC-dextran and tetramethylrhodamine-5-(and 6)-isothiocyanate-lectin staining was used to distinguish between perfused and non-perfused choroidal vessels.Results: EndoTAG-1 or LipoSPA significantly reduced CNV size to 15% compared to trehalose controls. The mean CNV area of mice treated with CL was reduced (though not significantly) to about one-half of the value of the trehalose control group. The same was observed for paclitaxel. Thus, the reduction in the CNV size between treatment with CL and treatment with EndoTAG-1 or LipoSPA was 40%, which was not significant. PDT using either CL-VTP or Visudyne reduced CNV size to 65% (D17) of trehalose control size. CNV size was further diminished to 56% with Visudyne and 53% with CL-VTP when PDT was repeated twice. Most importantly, PDT-associated retinal damage was less pronounced using CL-VTP compared to Visudyne.Conclusions: Systemic intravenous injection of paclitaxel (EndoTAG-1)- or succinyl-paclitaxel (LipoSPA)- loaded CL had a significant antiangiogenic effect in a CNV mouse model. PDT with CL-VTP was as effective as Visudyne in neovascular obliteration but induced less tissue damage. Our data suggest that systemic application of cationic liposome formulations may serve to treat ocular neovascular diseases. This approach may reduce the need for intraocular injections and may benefit patients with neovascular lesions irresponsive to anti-VEGF treatment.
