Ultrasound examination is a vital imaging tool in medicine, yet opportunities for comprehensive theoretical and practical training during medical education are limited. Effective ultrasound education requires both theoretical knowledge and hands-on practice. To address this, we developed SonoGame, a gamified platform designed to teach ultrasound skills to medical students. SonoGame provides a step-by-step introduction to ultrasound imaging and aims to improve skills such as image orientation, spatial visualization, and hand-eye-coordination through mini-games using geometric objects. This study evaluates the impact of SonoGame training on ultrasound skills.In this prospective, single-center study, 56 medical students, the majority of whom had no prior ultrasound experience, were randomly assigned to either a control group or a SonoGame group. The SonoGame group trained with the platform in four 30-minute sessions over 4 weeks, while the control group received no training. Ultrasound skills were assessed in both groups before and after the training period.The SonoGame group showed significant improvement in ultrasound skills, particularly with complex anatomical structures, compared to the control group. Participants in the SonoGame group also reported increased familiarity and confidence with sonography, viewing SonoGame as a valuable tool in their training.SonoGame is an effective supplement to conventional ultrasound education, enhancing the skills necessary for ultrasound examinations in medical students.
Ultrasound examination is an important imaging method used across the medical field (e.g. [1]). However, during medical education, practical training opportunities for ultrasound imaging are not yet widely available, while good potential exists for approaches that employ gamification (e.g. [3, 5]). SonoGame is a hands-on platform for medical students developed to train ultrasound skills using a game-based approach [6, 7]. It provides a step-by-step introduction to ultrasound imaging and aims at improving ultrasound skills. It consists of several mini games that challenge the player to examine, identify, and recreate 2D cross-sectional images of a 3D virtual volume underneath the table top that contains geometric objects (see Fig. 1). SonoGame focuses on learning how to orient in an ultrasound image and on developing the spatial visualization as well as hand-eye coordination skills that are needed for ultrasound imaging. In this contribution, we present and discuss a prospective, single-center study of the effects of SonoGame-based training on ultrasound skills. We recruited 56 medical students without previous practical ultrasound experience and randomly assigned them to either a control or to a SonoGame group. While the control group received no training, the SonoGame group repeatedly got to play the SonoGame over a period of four weeks (four sessions at 30 minutes each). For both groups, individual ultrasound skills as well as spatial cross-sectioning skills [2] were assessed before and after the four weeks. Results: Compared to the control group, the SonoGame group significantly improved in their ultrasound skills, particularly with complex anatomical structures (see Fig. 2), and, post-training, they reported feeling more familiar with and confident in their use of sonography. What is more, they considered SonoGame a valuable supportive tool for ultrasound skills training. For individual spatial cross-sectioning skills, both groups improved from first to second testing, though the extent of improvement did not differ between groups (see Fig. 3). We conclude that SonoGame-based training can usefully supplement conventional ultrasound education of medical students, as it promotes and supports the skill set required for ultrasound examinations.
Fallvorstellung Die Vorstellung der 48-jährigen Patientin erfolgte bei nachgewiesener PALB2-Mutation zur Planung einer prophylaktischer Mastektomie und Rekonstruktion. Die Vorsorgeuntersuchungen mittles Ultraschall, Mammographie und Mamma-MRT zeigten keine abklärungsbedürftigen Befunde. Daraufhin wurde 3 Monaten nach Vorstellung eine Nipple Sparing Mastektomie beidseits mit Implantatrekonstruktion durchgeführt. Die histologische Untersuchung des rechten Abladats zeigte ein 8 mm messendes DCIS, G1 mit positivem Östrogenrezeptor (100 %). Das linke Abladat zeigte keine auffälligen Befunde. In der postoperativen senologischen Konferenz wurde der Patientin eine prophylaktische Adnexektomie beidseits in Postmenopause und die üblichen onkologische Nachsorgemaßnahmen empfohlen.
Zusammenfassung Ziel Die ultraschallgestützte Stanzbiopsie (CNB) gilt als Standardbewertung für die Diagnose sonografisch nachweisbarer verdächtiger Brustläsionen. Basierend auf nicht randomisierten Studien, empfehlen die aktuellen deutschen Leitlinien die Biopsie von mindestens 3 Zylindern mit ≤ 14-Gauge(G)-Nadel. Es wird jedoch keine Empfehlung abgegeben, wie viele Proben bei kleinerer Nadelstärke von z. B. 16-G benötigt werden oder ob die koaxiale Führung die diagnostische Genauigkeit (Treffsicherheit) und Qualität verbessert. Daher wurde in einer prospektiven, monozentrischen, nicht verblindeten, randomisierten, kontrollierten klinischen Nichtunterlegenheitsstudie die diagnostische Genauigkeit der Stanzbiopsie mit 16-G- versus 14-G-Nadel mit und ohne Koaxialführung bewertet. Material und Methoden 1065 Brustbiopsien wurden aufgenommen, um die Zahl der erforderlichen Stanzproben zu analysieren, um eine entsprechende diagnostische Qualität zu erhalten, die für die histologische Beurteilung ausreicht, und um eine hohe diagnostische Genauigkeit und Ausbeute zu erreichen. Die histologischen Ergebnisse wurden durch eine Operation oder eine Langzeitbeobachtung von mindestens 2 bis zu 5 Jahren bestätigt. Ergebnisse Um eine additive diagnostische Genauigkeit von > 99 % zu erreichen, waren mindestens 2 Zylinder mit 14-G-Biopsie erforderlich. Die diagnostische Genauigkeit und die Qualität der 14-G-Biopsie wurden durch die koaxiale Technik nicht beeinträchtigt. Bei der Durchführung einer 16-G-Biopsie waren 5 Zylinder erforderlich, um eine additive diagnostische Genauigkeit von > 99 % zu erreichen. Ohne koaxiale Führung benötigte die CNB mit 16-G mindestens 3 Proben, während 5 Nadelpassagen mit koaxialer 16-G-Biopsie erforderlich waren. Schlussfolgerung Die diagnostische Genauigkeit und die Qualität der ultraschallgeführten Stanzbiopsie waren unabhängig vom Einsatz einer koaxialen Technik mit der 16-G-Nadel geringer als mit der 14-G-Nadel.
PURPOSE:Ultrasound-guided core needle biopsy (CNB) is considered the standard assessment to diagnose sonographically visible suspicious breast mass lesions. Based on nonrandomized trials, the current German guidelines recommend at least three cylinders with ≤ 14-gauge needle biopsy. However, no recommendation is made as to how many specimens are needed with a smaller needle size, such as 16-gauge, or if biopsy with coaxial guidance improves diagnostic accuracy and quality. Therefore, in a prospective monocentric unblinded randomized controlled clinical noninferiority trial, the diagnostic accuracy of 16-gauge versus 14-gauge core needle biopsy, with and without coaxial guidance, was evaluated. MATERIALS AND METHODS:1065 breast biopsies were included in order to analyze the number of core samples necessary to obtain an appropriate rate of diagnostic quality adequate for histological evaluation, and to achieve high diagnostic accuracy and diagnostic yield. Histological results were verified by surgery or long-term follow-up of at least two years up to five years. RESULTS:In order to obtain an additive diagnostic accuracy of > 99 %, a minimum of two cylinders with 14-gauge biopsy were required. The diagnostic accuracy and the diagnostic quality of 14-gauge biopsy were not affected by the coaxial technique. When performing a 16-gauge biopsy, five cylinders were required to achieve an additive diagnostic accuracy of > 99 %. Without coaxial guidance, 16-gauge CNB required at least three samples, whereas five needle passes with coaxial-guided 16-gauge biopsy were needed. CONCLUSION:The diagnostic accuracy and quality of ultrasound-guided 16-gauge core needle biopsy were inferior to the 14-gauge needle size, regardless of the use of a coaxial technique.
Das juvenile Fibroadenom kann als Riesenfibroadenom (> 5 cm oder > 500 g) auftreten und Deformierungen der betroffenen Brust verursachen. Die wichtigsten Differentialdiagnosen sind der phylloides Tumor und die juvenile Gigantomastie.
Ziel dieser Kohortenstudie war die Analyse therapie- und prognoserelevanter klinischer und pathohistologischer Parameter des invasiv lobulären Mammakarzinoms in der klinischen Routine.
Aktuell werden zur Abklärung mittels Stanzbiopsie in der S3 Leitlinie min. 3 Stanzzylinder ≤14 G gefordert. In der Literatur gibt es keine prospektiv randomisierte Studie, welche die Treffsicherheit der 14 vs. 16 G Stanzbiopsienadel mit/ohne Coaxialkanüle verglichen hat. Diesen Vergleich zu untersuchen hatte diese Studie zum Ziel.
Evaluation des Mindestabstands zwischen Haut und Tumor bei ultraschall-gesteuerten HIFU Behandlungen von Fibroadenomen.
Ziel der Studie war es zu überprüfen, wieviele Mammakarzinompatientinnen aufgrund ihrer Familiengeschichte und/oder ihres besonders jungen Erkrankungsalters die Kriterien für eine humangenetische Beratung zum einen erfüllen und zum anderen diese auch wahrnehmen.
In dieser Studie sollte die Prävalenz des Mammakarzinoms im Seitenvergleich untersucht und der Stellenwert der Lateralität als möglicher Co-faktor für Patienten-, Tumor- und Outcome-Merkmale analysiert werden.
Einige seltenere histologische Subtypen des Mammakarzinoms nehmen laut Literatur einen günstigeren Verlauf; dazu zählen tubuläre, invasive kribriforme, muzinöse, adenoid-zystische, tubulo-lobuläre und papilläre Karzinome. Ziel dieser Studie war die Analyse dieser selteneren Sonderformen hinsichtlich Prävalenz und Prognose.
Hintergrund: Nach den aktuellen S3-Leitlinien sollen zur histologischen Abklärung sonographisch auffälliger Mammabefunde bei Benutzung einer 14 G Biopsienadel mindestens 3 Stanzbiopsien gewonnen werden. Empfehlungen zu anderen Nadelstärken und zu Coaxialkanülen liegen nicht vor.
Hintergrund: Die unter sonographischer Sicht durchgeführte Stanzbiopsie dient zur histologischen Abklärung auffälliger Herdbefunde der Brust. Nach derzeitigem Stand der S3-Leitlinien sollten mindestens 3 repräsentative Stanzbiopsieproben bei Benutzung einer 14 G Biopsienadel gewonnen werden. Zu Benutzung der 16 G Stanznadel und einer Coaxialkanüle gibt es derzeit keine Empfehlungen.
Die anstehenden Einführung des NKLM (Nationalen Kompetenz-basierten Lernzielkataloges Medizin) wird in den nächsten Jahren nicht nur die Lehre und Ausbildung und im Fach Humanmedizin strukturell verändern, sondern auch die Facharztausbildung im Fach Gynäkologie mit neuen Inhalten und Ausbildungs-anforderungen (Kernkompetenzen) bereichern. Die Ausbildung in der Senologie wird hier auch neue Impulse, Zielvorgaben und Ausbildungsinhalte erhalten.
PURPOSE:The purpose of the study was to analyze sonographic criteria for the differentiation of benign and malignant breast lesions using real-time spatial compound imaging (CT) in combination with adaptive image processing (XRES).MATERIALS AND METHODS:In a retrospective analysis of 460 patients, the sonographic criteria: shape, orientation, margin, echo pattern and posterior acoustic features were determined using CT and XRES. All investigations were performed using a 12 MHz linear transducer. The findings were classified according to the DEGUM criteria analogous to BIRADS and were histologically confirmed by core needle or vacuum biopsy. Statistical analysis was performed using a Chi-square test, logistic uni- and multivariate regression analysis and an ROC-curve analysis to detect the false-positive rate.RESULTS:All investigated diagnostic criteria were significant in the descriptive analysis (Chi-squared). The multivariate analysis showed that the criteria of irregular shape versus round, and not circumscribed margin versus circumscribed margin as well as the posterior acoustic features of enhanced versus reduced have a significant influence on the prediction of a malignant finding. The univariate analysis also showed a statistical significance using the indifferent and not parallel orientation versus parallel. To attain a detection rate of > 95 %, a false-positive rate of 60 % must be expected.CONCLUSION:This study shows that when using CT in combination with XRES, the analyzed sonographic criteria for the differentiation of benign and malignant breast lesions are still of diagnostic value. In particular, the shape, margin and posterior sonographic features are important.
Hintergrund: Trotz der hohen Inzidenz von Buskrebserkrankungen im fortgeschrittenen Alter, gibt es keine Studien, welche die Sicherheit und Wirksamkeit von Trastuzumab speziell für dieses Kollektiv untersuchen. Daten liegen lediglich in Form von Subgruppen-Analysen vor. Trastuzumab stellt jedoch insbesondere bei der älteren Patientin (>70) mit lokal fortgeschrittener oder metastasierter Brustkrebserkrankung eine wichtige therapeutische Option dar. Die Wirksamkeit und Sicherheit von Trastuzumab zur Behandlung älterer Patientinnen wird derzeit an der Universitäts-Frauenklinik in Tübingen untersucht.
Das Fibroadenom (FA) ist einer der häufigsten benignen Befunde der Brust. Die Entfernung von FA ist, sofern diese histologisch gesichert sind, nicht zwingend erforderlich. Darüber hinaus gibt es aber Indikationen FA zu therapieren. Gerade Frauen mit symptomatischen FA, die schmerzhaft oder größenprogredient sind oder auch Frauen, bei denen der Befund einen Leidensdruck durch Angst aufbaut, profitieren von der Entfernung.
Trotz der hohen Mamma Karzinomrate im Alter existieren keine spezifischen Studien die Wirksamkeit und Sicherheit von Trastuzumab bei älteren Patientinnen (>70 Jahre) überprüft haben. Daten hierzu können maximal aus Subgruppenanalysen eruiert werden.
