Lifestyle, particularly regular physical activity, is known to be an important component in the prevention and treatment of type 2 diabetes mellitus and should be an integral component of the care of people with diabetes. There is broad consensus that an efficient prevention and treatment of diabetes in most cases must also be accompanied by a modification of the lifestyle. The aims of promoting physical activity are initially the training of the cardiovascular system, strengthening the musculature, increasing the energy turnover and the reduction of inactivity. To gain substantial health benefits a minimum of 150 min of moderate or 75 min of vigorous intense aerobic physical activity and additionally muscle strengthening activities per week are necessary. The extent of the positive effect of activity is directly proportional to the degree of cardiorespiratory fitness achieved and can only be maintained by appropriate training. Physical training is effective in all age groups and for both sexes. By reducing the insulin resistance and functional improvement of insulin secretion, physical training has a positive influence on glycemia and the cardiovascular risk is also reduced. Additionally, inactivity per se should be recognized as a risk factor, independent of the construct of the physical activity. In particular, prolonged episodes of sitting should be avoided.
Lebensstil, insbesondere regelmäßige körperliche Aktivität, ist ein wichtiger Bestandteil in der Prävention und Therapie des Typ-2-Diabetes mellitus und sollte fester Bestandteil jeglicher Betreuung von Menschen mit Diabetes sein. Es besteht breiter Konsens, dass eine effiziente Diabetes-Prävention und Therapie in den meisten Fällen auch von einer Modifikation des Lebensstils begleitet sein muss. Ziele der Förderung der körperlichen Aktivität sind zunächst das Training des Herz-Kreislauf-Systems, Kräftigung der Muskulatur, Steigerung des Energieumsatzes und die Reduktion von Inaktivität. Für einen substanziellen gesundheitlichen Nutzen sind wöchentlich mindestens 150 min aerobe körperliche Aktivität mit mittlerer oder 75 min höherer Intensität und zusätzlich muskelkräftigende Bewegungen erforderlich. Das Ausmaß des positiven Effektes von Bewegung steht in direktem Verhältnis zum Grad der erreichten kardiorespiratorischen Fitness und kann nur durch entsprechendes Training aufrechterhalten werden. Körperliches Training ist in jedem Alter für beide Geschlechter wirksam und effektiv. Durch die Reduktion der Insulinresistenz und funktionelle Verbesserung der Insulinsekretion hat körperliches Training positiven Einfluss auf die Glykämie, und zusätzlich wird das kardiovaskuläre Risiko gesenkt. Additionally, based on the large evidence of exercise referral and prescription, the Austrian Diabetes Associations aims to implement the position of a "physical activity adviser" in multi-professional diabetes care. Hintergrund Das KWP beschäftig 5.000 Mitarbeiter*innen aus unterschiedlichen Berufsgruppen vorrangig in den 29 "Häusern zum Leben". Seit der Reaktivierung der Gesundheitszirkel im Jahr 2022 wird die Ressourcenförderung und die partizipative Einbindung der Mitarbeiter*innen gezielt unterstützt. Gegenwärtig werden in 24 Häusern Gesundheitszirkel mit mehr als 200 geschulten Mitarbeiter*innen umgesetzt.

Zusätzlich fordert die Österreichische Diabetes Gesellschaft die Position der Bewegungsberater:in als fixen Bestandteil eines multidisziplinären Behandlungsansatzes. Leider gab es in den letzten Jahren weder im Aufbau standardisierter Bewegungsangebote noch in der Bewegungsberatung erfolgsversprechende Entwicklungen.
Abstract Background In 2015 a co-operation between the social insurance and the organized sport has been established in order to install Jackpot.fit, a standardized sports club based program, initially in 8 regions in Styria, a federal state of Austria. Inactive adults receive information about the long-term exercise program during a 3-week residential stay in a health resort. We aim to investigate who attends Jackpot.fit, the adherence to the program in 2019 as well as the progress of dissemination of Jackpot.fit since 2016. Methods Participantś personal data were collected by telephone at the registration. Instructors documented the participants' attendance each single session with a standardized form. The number of participating regions as well as courses within the regions were monitored by the evaluator. Results Between 2016 and 2019 2624 people (70.8% female; age: M = 57.1, SD = 9.1 years; BMI = 25.8, SD = 4.3 kg/m2) registered for Jackpot.fit and 84.3% (n = 2212) attended as participants in at least one session. Between September and December 2019, 824 people registered for the courses with an adherence rate as follows: 291 (35.3%) visited 75% or more, 200 (24.3%) between 50% and 74.9% and 232 (28.2%) less than 49.9% of all 638 provided sessions. 101 (12.3%) registered but never attended once. The number of regions rose from 8 in 2016 to 28 in 2019 and the number of courses from 25 to 55 over the same period. Conclusions According to the total number of participants, regions, and courses the dissemination moves forward. Further strategies to improve adherence must be developed. Abstract Background Joining forces through collaboration between the health- and sports-sector led to the project Jackpot.fit, which aimed at increasing physical activity (PA) among inactive adults in Styria (a state of Austria). The main objective was to assess the short- and long-term effectiveness of a mixed PA intervention, consisting of counselling in a health care setting combined with a standardised sports club programme, on PA in an adult population. Methods In a quasi-experimental study with two follow-up time points (4 and 12 months after baseline), participants from eight regions were allocated to the intervention group (IG) and participants from three other regions to the comparison group (CG). Half of the study participants were women and the mean participants' age was 53 (SD ± 6) years. During the health resort stay, the IG received PA counselling and coupons for 12 complimentary, standardised training sessions in a sports club near the participants' home. Participants of the CG received identical PA counselling, but written material only. PA was measured with an accelerometer (GENEActive). Linear mixed-effects models were applied to examine changes of PA within and between groups over time. Results From 217 participants (IG = 167; CG = 50) at least one follow-up measurement was available. IG data showed a significant increase in moderate-intensity PA from baseline (101 min/week) to 4 months (+58 min; 95% CI = 36 to 80) and 12 months (+24 min; 95% CI = 2 to 46) whereas there was no significant change in CG. Between-group comparisons revealed a significant difference after 4 but not after 12 months. Conclusions The study confirms the feasibility of coordinated actions between health care institutions and sport clubs to recruit people into a standardized PA programme. In addition, those who participated in the sports club programme increased their PA in a long term.
Background: Little is known about the implementation of lifestyle interventions in frail, community-dwelling people. This study highlights different domains of adherence to explain an effectively delivered home-based intervention. Methods: Eighty prefrail and frail persons (≥65 years) participated in a physical training, nutritional, and social support intervention over 24 weeks. A detailed log book was kept for comprehensive documentation in order to assess adherence and further organizational, exercise, and nutritional parameters. Results: Participants reached an adherence rate (performed home visits/number of planned visits) of 84.0/80.5% from week 1–12/13–24. Out of those, 59% carried out ≥75% of the offered visits. Older age was associated with a higher adherence rate. A mean of 1.5 (0.6) visits/week (2 were planned) were realized lasting for a mean of 1.5 (0.9) hours (154% of the planned duration). Per visit, 1.2 (0.6) circuits of strength training were performed (60.5% of the planned value) and 0.5 (0.3) nutritional interventions (47%). After twelve months, 4.2% still carried out the home visits regularly and 25.0% occasionally. Conclusion: Adherence is much more than “being there”. Adherence rate and category are limited parameters to describe the implementation of a complex lifestyle intervention, therefore a comprehensive documentation is needed.
Background: The performance of physical activity (PA) in different domains varies between different occupational groups and they contribute differently to the prevention and management of chronic diseases. This study aimed to give a fuller picture of the potential influence occupational categories have on the different domains of PA among the Austrian population of working age. Methods: A total of 8251 gainfully employed persons in 9 major and 39 sub-major occupational groups from the Austrian Health Interview Survey 2014 were analyzed. PA was measured with the Physical Activity Questionnaire of the European Health Interview Survey (EHIS-PAQ) and the prevalence of 17 chronic diseases was obtained. Results: A total of 48.2% were mostly active when working, 18.4% reported transport-related PA in the upper quintile, 50.4% performed at least 150 min per week of moderate PA or cycling, 32.7% performed muscle-strengthening PA at least twice a week, and 76.3% were either mostly physically active when working or complied with the aerobic PA guidelines. As a general rule, people in physically active occupational groups tended to perform less PA in their leisure time and vice versa. Occupational groups with especially low amount of PA were Information Technology workers, directors, and secretarial staff. People with a chronic disease tended to perform less PA, but there was an interaction between occupation and chronic disease on PA. Conclusions: Domain-specific programs to promote PA should be developed for various occupational categories.
Background: Not smoking, performing >150 min of aerobic physical activity (PA) and muscle strengthening exercises/week, and consuming >5 portions of fruit and vegetables/day are lifestyle recommendations for both the general population and people with diabetes mellitus (DM). Methods: A total of 15,771 and 15,461 persons from the Austrian Health Interview Surveys 2014 and 2019, respectively, including 4.9% and 6.0% of people with DM, were analysed in terms of their smoking, PA, and nutritional behaviours. Logistic regression models were performed for the lifestyle factors, adjusted for socio-demographic and health-related factors. Adjusted interactions between the survey year and DM on the lifestyle factors were computed. Results: The proportions of smokers were 23.9% and 20.2%, of people complying with the PA recommendations were 24.9% and 21.4%, and with fruit and vegetables recommendations were 7.1% and 5.5%, respectively, with significantly lower proportions of smokers and persons complying with the PA recommendations among people with DM. The fully adjusted odds ratios (95% confidence interval) for people with DM were 1.09 (0.94–1.26), 1.44 (1.23–1.69), and 0.90 (0.71–1.13) for smoking, not complying with PA recommendations, and not complying with fruit and vegetables recommendations, respectively. The proportion of people complying with PA recommendations decreased to a greater extent (p < 0.001) in people with DM (16.5% to 8.3%) compared to people without DM (25.3% to 22.3%). Conclusion: Diabetogenic lifestyle behaviours increased in the general Austrian population in recent years, which was especially true for people with DM regarding PA.
Hintergrund Im Jahr 2015 wurde eine Kooperation zwischen der Sozialversicherung und dem organisierten Sport gegründet, um Jackpot.fit, ein standardisiertes Bewegungsprogramm, ins Leben zu rufen. Inaktive Erwachsene werden während eines 3-wöchigen Kuraufenthaltes über das wohnortnahe Bewegungsprogramm informiert. In dieser Studie wird untersucht, wer an Jackpot.fit teilnahm, wie die Adhärenz im Jahr 2019 war und wie sich die Anzahl an Kursen im Rahmen der Dissemination entwickelte.
There is sound scientific evidence that regular physical activity enhances physical, psychological, and mental health. Specific physical activity guidelines for target groups make an essential contribution to the promotion of physical activity behavior at a population level. In this article, we introduce the updated Austrian physical activity guidelines for adults and older adults with and without physical, sensory, or mental disabilities, as well as for adults with chronic diseases. We have also added comments to key elements of the guidelines. The scientific basis of the physical activity guidelines is the scientific report by the US advisory committee, as well as the 2(nd)edition of the physical activity guidelines for Americans. Guidelines for a new target group - adults with chronic health conditions - have been included. Furthermore, people with disabilities are now explicitly part of the (older) adult target groups. Instead of providing one cut-off point to separate people into meeting the guidelines/not meeting the guidelines, a range of 150 to 300 minutes per week is now recommended. Placing the guidelines for strength training above those for aerobic training emphasizes the importance of this type of training. In addition, it is now recommended that prolonged sitting is avoided by regularl interruptions. We believe that the publication of the physical activity guidelines for Austria will make an important contribution to the promotion of health through regular physical activity. However, the promotion of regular physical activity will only be successful if all target groups have a fair chance to reach physical activity competence, and attractive environments are created for regular physical activity.
Zusammenfassung Ziel der Studie Ziel dieser Analyse war es, Zusammenhänge zu untersuchen zwischen Personen, die die ausdauerorientierten Bewegungsempfehlungen erfüllen und dem Wissen um die Bewegungsempfehlungen, die wahrgenommene bewegungsförderliche Kultur und Umgebung sowie den Gesundheitszustand getrennt nach Bundesländern. Methodik Für die Analyse wurden Daten aus dem österreichischen Bewegungsmonitoring 2017, der österreichischen Gesundheitsbefragung 2014, der Mortalitätsstatistik 2015, der Fit-Sport-Austria Datenbank, der Wirtschaftskammer, dem Verkehrsministerium und einer Online-Lauf-Plattform herangezogen. Korrelationskoeffizienten (Pearson) verschiedener Parameter mit den Bevölkerungsanteilen, die die ausdauerorientierten Bewegungsempfehlungen erfüllten, wurden errechnet. Ergebnisse Ausdauerorientierte Bewegung korrelierte signifikant mit dem Wissen zum empfohlenen Bewegungsumfang (0,91), Angeboten in (0,87) und subjektiver Erreichbarkeit von Sportvereinen (0,85), Erreichbarkeit von Grünräumen (0,84), Anzahl an Sportvereinen mit zertifizierten gesundheitsförderlichen Angeboten (0,76), und den Möglichkeiten Laufen zu gehen (0,72). Außerdem korrelierte ausdauerorientierte Bewegung negativ mit der altersstandardisierten kardiovaskulären Mortalität (−0,96), der Gesamtmortalität (−0,95), der Prävalenz von Adipositas (−0,83), der Mortalität an ischämischen Herzerkrankungen (−0,76), der Prävalenz chronischer Krankheiten (−0,68), sowie positiv mit der subjektiv wahrgenommenen Gesundheit (0,79). Schlussfolgerung Zum einen zeigten sich positive signifikante Zusammenhänge zwischen Wissen um die Bewegungsempfehlungen, der Infrastruktur und der bewegungsförderlichen Kultur mit dem Bewegungsverhalten. Zum anderen korrelierte das Bewegungsverhalten mit dem Gesundheitszustand in den jeweiligen Bundesländern. Zusätzlich lassen sich spezifische Empfehlungen, zugeschnitten auf die einzelnen Bundesländer, ableiten.
Hintergrund Der Übergang vom Erwerbsleben in den Ruhestand stellt aufgrund des Wegfallens bestehender und der Chance auf die Erschließung neuer Gesundheitsressourcen ein einschneidendes Lebensereignis und eine Herausforderung für die Gesundheit dar. Im Burgenland wurde von der Sozialversicherung gemeinsam mit der Medizinischen Universität Wien im Rahmen der HEPA-Strategie ein Modell eines Pensionsantrittskurses entwickelt und pilotiert. Die Prozessevaluation untersuchte folgende Fragestellungen: Passt das Modell für die Zielgruppe? Wie bewerten die Organisatoren die Umsetzung? Was kann für zukünftige Kurse gelernt werden?
