Die Schockraumversorgung von schwer verletzen und polytraumatisierten Patienten hat sich als Modell auch für die Behandlung nichttraumatologischer akut-vital bedrohter Patienten etabliert. Für die Behandlung von Traumapatienten wird deshalb spezifisch der Begriff Traumateam verwendet. In diesem Beitrag werden die evidenzbasierten Anforderungen an den Traumaschockraum dargestellt. Im Weiteren werden die Grundlagen und Gründe erörtert, weshalb ein Traumateam das Outcome verbessert. Die Zusammensetzung eines Traumateams hängt partiell von den organisatorischen Regelungen der einzelnen Krankenhäuser, aber auch von deren Versorgungsstufe ab. Dies ist für die Zertifizierung als Traumazentrum von Relevanz. Die Kriterien für die Traumateamaktivierung sind ein entscheidendes Kriterium für die Qualität der Versorgung und für einen Ressourcen-schonenden Einsatz. Zur Qualitätskontrolle und Reduzierung von Über- und Untertriage wird dargestellt, anhand welcher Parameter beurteilt werden kann, wann eine Traumateamaktivierung angemessen war.
Notaufnahmen sind zentraler Bestandteil der Notfallversorgung in Deutschland. Der vom Gemeinsamen Bundesausschuss (G-BA) beschlossene Aufbau von Zentralen Notaufnahmen und die geforderten Qualifikationen des ärztlichen Personals verändern die deutschlandweite Notfallversorgung. Über die verschiedenen in der Notaufnahme versorgten Patientengruppen ist bisher allerdings nicht viel bekannt. Für die vorliegende Analyse erfolgte eine retrospektive Auswertung der Behandlungsdaten volljähriger Patienten der Jahre 2019–2022 von 25 Notaufnahmen aus dem AKTIN-Notaufnahmeregister. BackgroundWith the revision of the German S3 guideline on polytrauma/severe injury management, mechanism-based criteria for trauma team activation (TTA) were largely abandoned. At the same time, the use of third-party service (TPS)-eCall systems, which transmit objective crash data, is gaining importance.ObjectiveWhat challenges and potentials arise in the use of TPS-eCall data for prehospital and clinical interpretation of crash mechanisms? Materials and methods Prospective observational study with trend analysis of activation frequency and case-based evaluation of all severely injured persons using emergency medical services (EMS), hospital, and registry data. All TPS-eCall incidents in the city of Essen between 01/2022 and 05/2025 were analyzed and linked to EMS data, treatment reports from a supraregional trauma center, and the TraumaRegister of the German Society for Trauma Surgery. Results A total of 209 TPS-eCall activations were recorded, showing a significant monthly increase (beta = 0.16; 95% CI 0.06-0.26; p < 0.01; R-2 = 0.21). In all, 25 persons were transported to the emergency department of the participating hospital, of whom 11 (44%) were treated by a trauma team. Four persons (16%) met the inclusion criteria of the TraumaRegister DGU (R). Interpretation problems of TPS-eCall data were present in all cases. Conclusion TPS-eCall systems can complement prehospital assessment. However, heterogeneous data quality, lack of standardization, and missing technical integration currently complicate interpretation. Nevertheless, the increasing number of cases and the expected improvement in data quality suggest that TPS-eCalls may play a relevant role in future prehospital decision-making.
The shock room treatment of severely injured and polytraumatized patients has become established as a model for the treatment of non-traumatological critically ill patients. For the treatment of trauma patients the term trauma team is therefore used. This article presents the evidence-based requirements for the trauma shock room. In addition, the reasons why a trauma team improves the outcome are discussed. The composition of a trauma team depends partly on the organizational regulations of the individual hospitals but also on their level of care. This is relevant for certification as a trauma center. The criteria for trauma team activation is a decisive criterion for the quality of care and resource-saving use. For quality control and to reduce overtriage and undertriage, it is shown which parameters can be used to assess when trauma team activation has been appropriate.
INTRODUCTION:Emergency departments are a central component of emergency care in Germany. The decision by the Federal Joint Committee (G-BA) to establish central emergency departments and the required qualifications for medical personnel are changing emergency care throughout Germany; however, so far not much is known about the various patient groups treated in emergency departments. MATERIAL AND METHODS:For the present analysis a retrospective evaluation of treatment data for adult patients for the years 2019-2022 from 25 emergency departments in the AKTIN emergency department register was carried out. The reasons for presentation were classified in accordance with the Canadian Emergency Department Information System Presenting Complaint List 3.0 (CEDIS-PCL) and the emergency department diagnoses and inpatient treatment diagnoses based on the 10th edition of the International Statistical Classification of Diseases and Related Health Problems, German modification (ICD-10-GM) codes. RESULTS:Of the total of 2,102,029 treatment cases 795,904 (38%) patients can were assigned to the disciplines of orthopedics and trauma surgery. The majority of patients from the field of orthopedics and trauma surgery attended independently (51.5%) and remained outpatients (68.8%). CONCLUSION:Even though many patients remained outpatients, expertise in the field of orthopedics and trauma surgery is needed to recognize potentially dangerous developments and indications for inpatient admission ("red flags"). At the same time there is the high potential to treat patients who remain outpatients at other levels of care.
Mit der Einführung der S3-Leitlinienaktualisierung Polytrauma/Schwerverletztenbehandlung wurden unfallmechanismusbasierte Kriterien für die Trauma-Team-Aktivierung (TTA) weitgehend verworfen. Gleichzeitig gewinnt die Nutzung von TPS-eCall-Systemen, die objektive Unfalldaten übermitteln, an Bedeutung. Welche Herausforderungen und Potenziale entstehen in der Anwendung von TPS-eCall-Daten in der präklinischen und klinischen Interpretation von Unfallmechanismen? Abstract Background Clamping of the endotracheal tube during disconnection from the ventilatory circuit appears to be commonly used to prevent abrupt loss of positive end-expiratory pressure (PEEP) in invasively ventilated patients. Despite its physiological rationale, this maneuver is not addressed in major guidelines, and data on its prevalence of use and safety are lacking. Objective: To assess the prevalence of endotracheal tube clamping during ventilator disconnection, describe clinical situations in which it is applied and explore reported adverse events associated with its use. Methods We conducted a cross-sectional, anonymized electronic survey among members of the German Interdisciplinary Association of Critical Care and Emergency Medicine (DIVI). Results A total of 1,344 participants completed the survey. Overall, 87% reported using the clamping maneuver at least occasionally. In only 7.6% of cases, its use was based on local guidelines or standard operating procedures. Clamping was applied across different ventilation modes and with various types of clamps. A total of 11.4% of respondents reported having observed adverse events temporally associated with the maneuver, including airway device damage. Conclusions Endotracheal tube clamping during ventilator disconnection appears to be a widespread but largely non-standardized clinical practice. Reported safety concerns underscore the need for systematic evaluation of its clinical benefits and risks, as well as for the development of evidence-based recommendations and standardized training concepts. Eine ausreichende personelle Ausstattung, insbesondere bei Ärzt*innen und Notfallpflegekräften, ist für eine hochwertige Notfallversorgung unerlässlich. Ziel der Studie war es, die aktuelle Personalsituation deutscher Notaufnahmen mit den empfohlenen Vorgaben abzugleichen. Dazu wurde ein Fragebogen entwickelt, der sich an den Mindeststandards der Deutschen Interdisziplinären Vereinigung für Intensiv- und Notfallmedizin (DIVI) sowie der Deutschen Gesellschaft für Interdisziplinäre Notfall- und Akutmedizin (DGINA) orientierte. Erfasst wurden sowohl die tatsächliche Ausstattung als auch die Einschätzung der Angemessenheit. Die Bereitstellung spezialisierter Schockraumteams zur Schwerverletztenversorgung ist nach den Vorgaben der S3-Leitlinie Polytrauma/Schwerverletztenversorgung der AMWF obligat und die Zusammensetzung durch das Weißbuch Schwerverletztenversorgung festgelegt. In jeder Versorgungsstufe wird das Basisteam aus den 4 Disziplinen Orthopädie und Unfallchirurgie, Anästhesie, Radiologie und der Notfallpflege der Notaufnahme zusammengesetzt, mit weiteren Anpassungen je nach Versorgungsstufe des Krankenhauses. Ziel der vorliegenden Studie ist die Untersuchung der gelebten Realität bei der Zusammensetzung der Schockraumteams. BACKGROUND:With the 2023 revision of the German S3 guideline "Polytrauma/severely injured patient care" the criteria for trauma team activation (TTA) were revised. Accident mechanism-based criteria were largely eliminated. The impact of these changes on emergency department (ED) management, over- and undertriage following traffic accidents, and hospital length of stay has not yet been systematically evaluated. OBJECTIVE:The aim of this study was to assess the effects of the guideline update on patient treatment in the emergency department of a level I trauma center. METHODS:In a prospective, single-center observational study, all patients presenting after motor vehicle accidents were recorded over a 2-year period (pre- vs. postguideline update). Primary endpoints were the frequency and appropriateness of TTA as well as the distribution of treatment locations. Secondary endpoint was hospital length of stay. RESULTS:A total of 1438 cases were analyzed. The number of red-triaged patients decreased significantly (257 vs. 157; p < 0.001). The overtriage rate among TTA patients declined (27.6% vs. 21%, p < 0.01) without a significant increase in undertriage. Mortality and hospital length of stay did not differ significantly between groups. CONCLUSION:The revision of TTA criteria led to a marked reduction in trauma team activations while maintaining quality of care. No significant increase in undertriage was observed. The updated TTA criteria may support resource-efficient patient care without compromising safety.
Validation and Reliability of the Tracheostoma Well-Being Score (TWBS) in critically ill patients. Prospective observational study involving tracheotomized patients of a tertiary university referring center. Sixty-five tracheotomized patients completed the TWBS on at least two or more days. External assessments of probable problems with the inserted cannula for each patient were provided by the nurse treating that patient on the respective day. The test-retest reliability demonstrated a stable response pattern over repeated measurements. Criterion validity revealed a limited agreement between external assessment by healthcare providers and patients’ self-reported experience, highlighting discrepancies in symptom perception. The 12-Item-TWBS appears to be reliable. The results further underline the necessity of using a validated self-reporting tool to assess patient comfort, as external assessment appears to have shortcomings. The TWBS might be a valuable tool to more accurately appraise patient comfort with the indwelling tracheostomy cannula.
BACKGROUND AND RESEARCH QUESTION:Adequate staffing, especially with physicians and emergency nursing staff, is essential for high-quality emergency care. The aim of this study was to compare the current staffing situation in German emergency departments with recommended standards. METHODS:A questionnaire was developed based on the minimum standards of the German Interdisciplinary Association for Intensive and Emergency Medicine and the German Society for Interdisciplinary Emergency and Acute Medicine. Both the actual staffing and the perceived adequacy were assessed. The anonymous online survey was sent via the joint emergency department registry to the management of 1008 emergency departments. RESULTS:Between 1 June and 31 July 2023, 176 emergency departments (18% response rate) participated. Annual patient numbers ranged from 17,610 to 37,251 depending on the level of care. Qualified nursing and medical leadership was mostly present (about 90%, medical leadership in level 1: 68%). Continuous physician presence was 76%, specialist presence 50%. Specialists with additional emergency medicine training were available in level 1 (one physician) and in levels 2 and 3 (two physicians each). Only 50% of hospitals offered the full 24-month training period. A nurse-to-patient ratio of 1:1200 was met in 40-63% of departments; triage nurses met requirements in 54% of cases. CONCLUSION:The survey shows that significant staffing deficits persist in German emergency departments at all levels of care-especially regarding the presence and qualifications of physicians, nursing staff, social services, and case management. At the same time, a positive trend in equipment, infrastructure, and staff qualifications is evident.
Mit der Aktualisierung der S3-Leitlinie „Polytrauma/Schwerverletzten-Behandlung“ im Jahr 2023 wurden die Kriterien zur Traumateamaktivierung (TTA) überarbeitet. Dabei wurden unfallmechanische Kriterien weitgehend entfernt. Die Auswirkungen der Leitlinienänderungen auf die Versorgung in der zentralen Notaufnahme (ZNA), auf die Über- und Untertriage nach Verkehrsunfällen sowie auf die Krankenhausverweildauer wurden bisher nicht nachuntersucht. Ziel der Arbeit war, die Auswirkung der Leitlinienaktualisierung auf die Behandlung in einer Zentralen Notaufnahme eines überregionalen Traumazentrums (ÜTZ) zu bewerten. In einer monozentrischen, prospektiven Beobachtungsstudie wurden über 2 Jahre (Prä- vs. Post-Leitlinienaktualisierung) alle Patient:innen nach motorisierten Verkehrsunfällen in einem ÜTZ erfasst. Primäre Endpunkte waren Häufigkeit und Adäquatheit der TTA sowie die Verteilung der Behandlungsorte. Sekundärer Endpunkt war die Verweildauer. Insgesamt wurden 1438 Fälle ausgewertet. Over the past 50 years, the concept of the golden hour of shock was established as one of the central tenets of emergency trauma medicine. A shorter duration of prehospital care correlates with a positive change in outcome in numerous studies. Dispatching by the public safety answering points has hardly been discussed to date. Thanks to improved vehicle safety, additional accident data is now available to the emergency call centers. We investigated the effects of third-party system emergency calls (TPS-eCalls), which have become mandatory in new passenger cars in the EU in 2018, on dispatching in the emergency medical services (EMS). For this purpose, we linked the data of a public-safety answering point (PSAP) and an EMS. All emergency service deployments from 01/01/2023 to 31/12/2023 were evaluated. N = 1546 rescue missions were dispatched after motor vehicle accidents (MVA), 111 after TPS-eCall-alerts, 1435 after conventional alerts. Dispatching in the PSAP currently took longer after TPS-eCall alerts than conventional alerts (01:39 ± 01:40 min vs. 02:41 ± 02:01 min, p ≤ 0.001). The differences were only significant in the case of accidents involving ≤ 2 passengers. TPS-eCall data will be available increasingly. The future expansion data availability offers the opportunity to include objective accident data (airbag deployment, number of occupants, change of velocity) in the dispatching process. Adequate technical connection can improve dispatching and shorten preclinical treatment, especially for complex events with more than 2 passengers.
Acute gastrointestinal injury (AGI) after cardiopulmonary bypass (CPB) is associated with poor prognosis. This study aimed to evaluate the effect of preoperative probiotic supplementation on the incidence of AGI in patients undergoing CPB procedure. This was a double-blind, randomized controlled trial conducted in a single center. The patients undergoing HVR with CPB between September 2022 and February 2023 were randomly assigned to receive either probiotic (Lac group) or placebo (Placebo group). The probiotic was administered daily for seven days prior to surgery.Univariate and multivariate logistic regression analysis was performed to identify independent risk factors for AGI. A P-value < 0.05 was considered statistically significant. Gut microbiota composition was assessed using 16 s rRNA analysis. A total of 52 patients were randomly assigned to two groups (26 in the Lac group, 26 in the Placebo group). Patients were followed for at least 30 days after surgery. In Deutschland ist seit dem letzten Deutschen Ärztetag im Mai 2024 eine Diskussion über die Verkürzung der Facharztweiterbildung und eine Verlagerung von Inhalten einer Zusatzweiterbildung in die bisherige Facharztweiterbildung entstanden. Dies betrifft auch die Intensivmedizin, mit der Perspektive, einen Facharzt für fachgebietsspezifische Intensivmedizin (z. B. Facharzt für chirurgische Intensivmedizin) zu schaffen. Die damit einhergehende Reduktion allgemeiner, fachgebietsspezifischer Inhalte halten wir aus mehreren Gründen für nicht sachgerecht: Die Kenntnis der fachgebietsspezifischen Auslösefaktoren („Foci“) einer kritischen Erkrankung (Organdysfunktion) sowie die Kenntnis der jeweiligen, auslösefaktorenspezifischen Symptomatik, Diagnostik und Abläufe zur Einleitung einer kausalen Therapie sind entscheidend für die Prognose. Neueste Erkenntnisse weisen darauf hin, dass bei septischen Foci eine Zeitspanne zwischen Diagnosestellung und Fokustherapie von ca. 6 h nicht überschritten werden sollte, um eine Verschlechterung der Prognose zu vermeiden. Um die Zeit zwischen Symptombeginn und effektiver Therapie der Auslösefaktoren nicht zu lang werden zu lassen, ist eine vertiefte fachspezifische Kompetenz im gesamten Prozess erforderlich. Diese Kompetenz ist unabhängig von der intensivmedizinischen Qualifikation und kann nur im Rahmen einer ausreichenden, fachgebietsspezifischen Weiterbildung (mit darauffolgender, intensivmedizinischer Zusatzweiterbildung) erworben werden. Fachgebietsspezifische Kenntnisse sind Voraussetzung für eine effektive Therapie kritisch kranker Patient*innen. Die Beibehaltung der bisherigen fachgebietsspezifischen Weiterbildung und der damit verbundene Erwerb spezifischer Kenntnisse im jeweiligen Fachgebiet ermöglichen es ferner, Fachärzt:innen im Klinikbetrieb breiter einsetzen zu können sowie diagnostische und therapeutische Ressourcen schonender zu verwenden. Die intensivmedizinische Zusatzweiterbildung sollte nicht zulasten fachgebietsspezifischer Inhalte gehen und muss in der nächsten Änderung der Weiterbildungsordnung weiter von allen Facharztqualifikationen des Gebietes Chirurgie heraus erreichbar bleiben. Aufgrund des unumgänglichen Umfangs kann die intensivmedizinische Zusatzweiterbildung selbst nur hauptberuflich und die gesamte Arbeitszeit einnehmend erfolgen.
