AIM:Intrathyroid metastases are uncommon in cytology practice. We report a case of metastatic lesion in the thyroid from breast carcinoma which was recognized in a fine-needle aspiration (FNA) biopsy and confirmed by immunohistopathology. In addition, we provide an overview of the literature describing similar cases.STUDY DESIGN:The patient was a 54-year old woman with a large, multinodular goiter and bilaterally enlarged lymph nodes in the supraclavicular areas. Fourteen years earlier she had undergone radical mastectomy followed by chemio- and radiotherapy due to a breast carcinoma.RESULTS:FNA of the thyroid nodules showed a metastatic breast carcinoma and was followed by total strumectomy and lymphadenectomy. Histological reassessment of the surgical thyroid specimens as well as the neck lymph nodes revealed multiple breast metastases. This was strongly confirmed by immunohistochemical examinations, which revealed a positive staining for: CKMNF 116, CK7, CEA as well as for ER, PgR and HER2, and a negative staining for: CK20, thyroglobulin, TTF1, calcitonin, and chromogranin.CONCLUSION:Every new aggregate in the thyroid in patients with even a long-term history of cancer should be considered as potentially metastatic until proved otherwise. FNA could be helpful in the diagnosis of thyroid metastatic lesion, but it should be confirmed by immunohistopathology.
21115 Background: Breast cancer occurring during pregnancy or within the first year after delivery is considered to be a pregnancy-associated breast cancer. Some studies have shown a decreased estrogen receptor-positive and progesterone receptor-positive status in pregnant patients with breast cancer, others, however, have not. Moreover, little information is available about the expression of HER2/neu and about the other human estrogen receptor, estrogen receptor beta (ERbeta) in pregnancy-associated breast cancers. The aim of this work was to determine the extent of the expression of ER alpha and beta, progesterone receptor (PgR) and HER2/neu in pregnancy- associated breast cancer. Material and Methods: Formalin-fixed, paraffin embedded tissues from 16 patients with pregnancy- associated breast cancer were used in this study. Immunostaining for ERalpha, ERbeta and PgR was performed using monoclonal antibodies against ERalpha, PgR (DakoCytomation) and against ERbeta (CHEMICON). The EnVision detection system was applied. Tumors were considered to be expressing receptors if a positive reaction (regardless of intensity) could be identified in at least 10% of cells. The HER-2 status was analyzed using HercepTest TM (IHC), and IHC 2+ results were confirmed with FISH test. Results: 44% of the tumors (7/16) were ERalpha and PgR positive. The expression of ERbeta protein was observed in 94% of pregnancy-associated breast cancers. As many as 50% of ERbeta positive tumors showed no expression of ERalpha. Further, 31% (5 of 16) of breast cancers were HER2/neu positive. Conclusions: This frequent expression of ERbeta in pregnancy-associated breast cancer may result from their prodifferentiative functions which increase during pregnancy and lactation. In Poland, where soil is deficient in iodine, supplementation of iodine was introduced in 1935, discontinued in 1980, and then re‐introduced in 1997. One of the effects of inadequate iodine intake, prior to 1997, was an increase in the prevalence of thyroid nodular disease (TND) in children. Chernobyl, located in the neighbouring country of Ukraine, suffered a catastrophic nuclear explosion in April 1986.
Porównanie zmian w błonie śluzowej jelita grubego w sąsiedztwie guza, po krótkoterminowej przedoperacyjnej radioterapii u pacjentów z rozpoznanym rakiem odbytnicy, ze zmianami w błonie śluzowej pacjentów nie naświetlanych. Badaniami objęto 39 pacjentów hospitalizowanych w Wielkopolskim Centrum Onkologii im. Marii Skłodowskiej-Curie w Poznaniu w latach 2000–2001. Grupę badaną stanowiło 20 pacjentów po przedoperacyjnej radioterapii. 19 pacjentów nie naświetlanych stanowiło grupę kontrolną. W wycinkach błony śluzowej jelita grubego oznaczono 6 markerów nowotworowych: CEA, TP53, Ki-67, CK-7, CK20 i CK-MNF 116. Zastosowano dwustopniową metodę immunoperoksydazową z kompleksem EnVision+TM/HRP, Rabbit lub z kompleksem EnVision+TM/HRP, Mouse. Wykonano barwienia histochemiczne: barwienie hematoksyliną i eozyną, barwienie substancji śluzowych mucykarminem, AB/paS, AB pH=1 i AB pH=2,5. Radioterapia zwiększyła w sposób statystycznie wysoce istotny ekspresję białka TP53 i statystycznie istotny ekspresję markera proliferacji komórkowej Ki-67 w kryptach jelitowych. W żadriej z badanych grup nie wykazano korelacji pomiędzy ekspresją TP53 i Ki-67. Radioterapia spowodowała zmniejszenie ekspresji CK-20, zwłaszcza na powierzchni błony śluzowej i wzrost ekspresji cytokeratyn wykrywanych przeciwciałem CK-MNF116. Napromieniowanie nieznacznie nasiliło ekspresję antygenu CEA w gruczołach i w małym stopniu zmniejszyło ją na powierzchni błony śluzowej. Radioterapia spowodowała szereg zmian morfologicznych w błonie śluzowej, z których część można było by uznać za zmiany dysplastyczne. Obserwowano pojawienie się zmian atypowych, jak np. olbrzymich jąder komórkowych z wyraźnie widocznymi jąderkami w kryptach, a także nasilenie stopnia zapalenia błony śluzowej u pacjentów po radioterapii. Zauważono zmiany zanikowe na powierzchni błony śluzowej, a także występowanie obok siebie dwu rodzaju krypt: zanikowych i rozrostowych.
Nowotwory piersi rozwijające się u nosicieli mutacji genów BRCA1 i BRCA2 cechuje podwyższona ilość abberacji chromosomowych w porównaniu z nowotworami "sporadycznymi", zaobserwowano też różnice fenotypowe w analizie klinicznej i histopatologicznej. Celem pracy było (i) określenie związku pomiędzy występowaniem mutacji genu BRCA2, a cechami klinicznymi, histopatologicznymi, (ii) analiza czynników prognostycznych dla mężczyzn chorych na raka piersi, ze szczególnym uwzględnieniem roli mutacji genu BRCA2. Grupę badawczą stanowiło 43 pacjentów Wielkopolskiego Centrum Onkologii, u których rozpoznano raka piersi, w tym 11 ze zidentyfikowanymi mutacjami genu BRCA2 (5 mutacji zmiany ramki odczytu, 3 mutacje zmiany sensu). Ekspresje ER, PR i AR w tkankach nowotworowych badano przy pomocy metod immunohistochemicznych. Analizę statystyczna wykonano w oparciu o następujące metody: test chi kwadrat, metoda Kaplana i Megiera, model proporcjonalnego hazardu Coxa. Nowotwory piersi związane z mutacjami BRCA2 pojawiały się u mężczyzn w młodszym wieku w porównaniu z grupą kontrolną (48,1/62,3; p=0,002). Nie stwierdzono różnic w wielkości guza, obecności przerzutów do węzłów chłonnych, stopniu złośliwości, czy ekspresji receptorów hormonów płciowych. Zaobserwowano różnice w pięcioletnich przeżyciach całkowitych (overall survival, OS) i przeżyciach bez nawrotu choroby (disease-free survival, DFS). Odsetek przeżywających był znacznie obniżony w grupie nosicieli mutacji BRCA2 (68%/28%, p=0,017 dla DFS; 86%/25%, p=0,006 dla OS). Krótsze przeżycia związane były również z ekspresja receptora androgenowego w tkance nowotworowej (74%/33%, p=0,029 dla DFS; 71%/57%, p=0,05 dla OS). Mutacje genu BRCA2 oraz ekspresja receptora androgenowego są niezależnymi, niekorzystnymi czynnikami rokowniczymi raka piersi u mężczyzn. Nowotwory piersi u mężczyzn związane z mutacjami BRCA2 pojawiają się w młodszym wieku, ale nie różnią się pod względem cech kliniczno-patologicz-nych od nowotworów sporadycznych.
W ostatnich dziesięciu latach w Wielkiej Brytanii i w Szwecji kontrola jakości i akredytacja zakładów histopatologicznych zostały wzajemnie powiązane. W tych krajach jest obecnie niemożliwe, aby laboratoria uzyskały akredytację, jeśli nie mogą udowodnić, że aktywnie uczestniczą i wykazują się dobrymi wynikami w zewnętrznym systemie kontroli jakości (national external quality assurance system – NEQAS). Do chwili obecnej w przypadku raka piersi usunięcie układu chłonnego pachy jest obowiązkowym elementem postępowania chirurgicznego. Zajęcie węzłów chłonnych w przypadku raka piersi jest jednym z czynników rokowniczych jak i wpływa na podjęcie dalszego leczenia uzupełniającego i dlatego usunięcie pachowych węzłów chłonnych jest bardzo ważnym elementem leczenia operacyjnego raka piersi. Technika biopsji węzła wartowniczego (WW) rozwinęła się w przypadkach czerniaka złośliwego skóry i ma na celu precyzyjną ocenę stanu całego dorzecza węzłów chłonnych przy użyciu barwnika Paten Blau V, radioizotopu Tc99 ręcznej sóndy gamma kamery oraz małoinwazyjnej techniki chirurgicznej.
Dotychczas nie przeprowadzono badań umożliwiających obiektywne, oparte o analizę statystyczną częstości występowania cech rodowodowo – klinicznych określenie kryteriów diagnostycznych typu "Nuclear pedigree" dla wszystkich, zależnych nie tylko od mutacji w BRCA1/2 rodzajów dziedzicznego raka piersi (HBC). Celem pracy był określenie kryteriów diagnostycznych typu "Nuclear pedigree" (NP) dla HBC na podstawie porównania częstości występowania u pacjentek z rakiem piersi oraz wśród krewnych pierwszego stopnia, cech charakterystycznych dla HBC w dwóch grupach: (i) nieselekcjonowanej grupie pacjentek z rakiem piersi (n=220), (ii) pacjentek z HBC (n=14). Kryteria diagnostyczne zdefiniowano poprzez analizę statyczną związku poszczególnych cech z HBC. W pierwszym etapie porównano częstość występowania ponad 20 różnych cech rodowodowych i klinicznych w badanych grupach. W kolejnym etapie wytypowano cechy najbardziej charakterystyczne dla HBC – na podstawie uzyskanych wartości OR (odds ratio). Ostatecznie określono przydatność poszczególnych cech jako kryteriów diagnostycznych poprzez porównanie niezależnego i uzupełniającego działania wytypowanych cech. W wyniku przeprowadzonej analizy do kryteriów rodowodowo – klinicznych umożliwiających rozpoznanie HBC z wysokim prawdopodobieństwem należą niewątpliwie: (i) stwierdzenie u matki pacjentki z rakiem piersi zachorowania na raka piersi lub raka jajników, niezależnie od wieku rozpoznania tych nowotworów, a najprawdopodobniej również (ii) zachorowanie na raka piersi przed 40 rokiem życia oraz (iii) stwierdzenie obustronnego raka piersi, jeśli co najmniej jedno zachorowanie wystąpiło przed 50 rokiem życia.
Purpose: Germline mutations of the BRCA2 gene are involved in the development of a considerable number of male breast cancer cases. Although phenotypic differences have been observed between sporadic and BRCA-related breast carcinomas, conflicting data exist on the differences in prognosis of women with hereditary and sporadic breast cancer. The purpose of the study was to investigate the prognostic value of BRCA2 status in male breast carcinoma (MBC).Experimental Design: We studied 43 male breast cancer patients, including 12 with BRCA2 mutations. Tumor samples were characterized immunohistochemically using antibodies to estrogen receptor, progesterone receptor, and androgen receptor (AR).Results: BRCA2-related tumors presented at the earlier age compared with sporadic tumors (P = 0.005). Patients positive and negative for BRCA2 mutations did not differ with respect to tumor size, lymph node involvement, histological grade, and sex hormone receptor status. Five-year disease-free survival (DFS) and overall survival (OS) were significantly decreased in BRCA2-positive patients (67% versus 28% for BRCA2-negative versus positive patients, respectively, P = 0.017 for DFS; 86% versus 25%, P = 0.006 for OS). Shorter survival was also correlated with expression of AR in tumor tissue (74% versus 33% for patients with tumors staining negatively and positively for AR, P 0.029 for DFS; 71% versus 57%, P = 0.05 for OS).Conclusions: The BRCA2 mutations and AR expression in tumor tissue are independent adverse factors for MBC prognosis. BRCA2-related MBC presents at the earlier age compared with non-BRCA2-related cancer, but do not differ with respect to other clinicopathological features.
In an iodine deficient area, a high incidence of hot thyroid nodules was observed in children after the introduction of iodine supplementation in 1997. Thirty-one children (28 girls, 3 boys) were identified with hot nodules between the years 1996-2000 (3 patients in 1996, 4 in 1997, 10 in 1998, 7 in 1999, and 7 in 2000). The incidence ratio of hot nodules in this study population increased significantly from 0.23 in 1996 to 0.80 in 1998. In 17 children, radionuclide uptake was confined exclusively to areas corresponding to the nodules. Cancer was detected in one child in post-operative histological examination. In the other 14 children, the predominant uptake was in nodules, but it was also registered within extranodular tissue. In the latter group, eight tumors were eventually diagnosed as cancer and six as benign tumors. The majority of tumors in the entire group was located in the right lobe (19/31) and was accompanied by reduced TSH levels (23/31), but only 14 of the 31 patients had signs of hyperthyroidism. The years 1996-2000, in which the increase in the incidence of "hot" nodules in children with nodular goiter was observed, correspond to a period of enforced salt iodinization. The existence of cancer within hot nodules is rare, ranging from 2-5% of all nodules. By contrast, the risk of cancer in hot nodules in the cohort of this study was significantly higher (9/31; 29.0%), especially in cases of hot nodules with a rudimentary radionuclide uptake in the extranodular area. We conclude that, in geographical regions affected with iodine deficiency, the therapeutic protocol for children with hot nodules should be based primarily on surgery rather than on radioiodine.
Breast cancer occurs rarely in men and risk factors for the disease include germline mutations of the BRCA2 gene. High frequency of allelic loss at the BRCA2 locus has been reported in sporadic breast tumors, but somatic mutations of BRCA2 are very rare. Here we report the first case of somatic BRCA2 mutation in male breast cancer with demonstrated loss of heterozygosity. We analyzed a series of 27 archival samples from male breast cancer patients for BRCA2 mutations and loss of heterozygosity at BRCA2 locus. The mutation analysis of BRCA2 gene was performed using SSCA-HA and sequencing methods. PCR was used to detect LOH at 3 highly polymorphic microsatellite markers spanning BRCA2 region on 13q by comparing the allelic pattern in matched tumor and blood DNA samples. In this study LOH at the BRCA2 locus was observed in 82.6% of informative cases, confirming previous observations on high frequency of LOH affecting the BRCA2 region in male breast cancer. We identified 5 somatic BRCA2 mutations in a set of 23 sporadic male breast cancers (21%). Two silent and 1 missense alterations were novel BRCA2 variants. Here we also report first somatic frameshift BRCA2 mutation in male breast cancer 8138del5. In 3 tumors with somatic BRCA2 alterations, 1 missense, 1 silent and frameshift LOH at chromosome 13q12-13 were detected and losses involved a wild-type allele of BRCA2 gene.
The authors report the feasibility an accuracy of intraoperative sentinel lymphnode biopsy (SLN) in patients with operable breast cancer to test the hypothesis that the histologic characteristic of SLN predicts the histologic status of remaining lymph nodes in the axilla. In between August 1999 and May 2000 to SLN biopsy there were 124 patients enrolled with median age of 55,2 years of age. All patients had operable breast cancer and all axillary lymphnodes were clinically negative. A day before surgery 4 cc of Nannocol marked with Tc99 was injected subcutuanesly nearby the tumor bed and an hour later the lymphoscyntygraphy was performed to achieve axillary lymphnodes mapping. Then immediately prior to surgery Blue Dye Patent Blau V was injected in the same manner as the radiological marker. Then with the use of hand – held gamma probe the hotspot was defined and marked. Five to ten minutes later first cut was performed over marked hotspot. After visualization of stained lymph vassals the SLN was traced and confirmed with high dose output (using the Neoprobe) and then was harvested for histologic examination. After this all patients were submitted for regular lymphadenectomy regardless main surgery type (mastectomy or BCT). Employing above method we were able to define SLN in 93,3%, only in 6,7% of our patients identification failed. In the group with defined SLN we found cancer cells deposits only in 20%. And in that group remaining lymphnodes had cancer metastasis in almost 80%. We had two cases of false negative results but still our specify rate was over 97%. In conclusion we think that SLN biopsy is quite safe and efficient method to evaluate axillary lymphnodes status in patients with operable early stage breast cancer. Our findings are also confirmed by other authors world wide.
BACKGROUND. Interleukin-6 (IL-6) is secreted by normal epithelial breast cells but not by oncogene-transformed cells. Interleukin-6 is able to inhibit growth of breast carcinoma cells in culture. Interleukin-6 exerts its activity via two receptor subunits, IL-GR and glycoprotein 130 (gp130). The expression of these receptor subunits in breast tumors has been studied, but there are no previous reports of their prognostic significance, to the authors' knowledge.METHODS. mRNA of IL-6, IL-GR, and gp130 was studied in 75 tumor samples obtained from breast carcinoma patients. Patients were followed for a maximum of 71 months (median follow-up, 61 months; 60 patients were followed for a minimum of 5 years or died during the observation period). Prognostic factors were analyzed in univariate and multivariate analysis.RESULTS. mRNA specific to IL-6, IL-GR, and gp130 was detected in 57%, 53%, and 71% of breast carcinoma tissues, respectively. Expression was strongly correlated with earlier stages of the disease. In univariate analysis, expression of IL-6 and its receptor subunits proved to be a positive prognostic factor for overall survival (OS) and disease free survival (DFS). IL-GR and gp130 expression were good independent prognostic factors for OS. The 5-year OS of all patients was 66%. The 5-year OS in IL-6, IL-GR, and gp130 positive groups was 95%, 94%, and 90%, respectively, whereas in negative groups it was 26%, 31%, and 9%, respectively.CONCLUSIONS, Expression of IL-6, IL-6R, and gp130 in breast carcinoma tissue is associated with earlier stages of the disease. In advanced stages, expression of IL-6 and its receptor subunits predicts better prognosis. (C) 2000 American Cancer Society.
17-year-old girl with thyroid tumor of the right lobe is described in this paper. Based on clinical picture and diagnostic criteria the follicular adenoma was suspected pre-operatively as the most likely final diagnosis. Post-operative histopathological evaluation proved the presence of the oxyphilic type of follicular thyroid cancer within the follicular adenoma. The authors analyze the possibilities of the pre-operative discovery of the discussed cancer.
Diagnostyka histopatologiczna jest jedną z najważniejszych metod diagnostyki nowotworów od co najmniej początku obecnego stulecia. Pozornie wydaje się, że metoda ta nie uległa większym zmianom gdyż standardem pozostaje ocena preparatów barwionych hematoksyliną i eozyną mikroskopie świetlnym. Mikroskop świetlny, podstawowe narzędzie patologa, uległ w tym czasie wielu modyfikacjom i ulepszeniom, jednak jego zdolność rozdzielcza nie zmieniła się. Również preparaty histopatologiczne barwione hematoksyliną i eozyną, które oceniamy dziś, nie różnią się w zasadniczy sposób od tych z początku stulecia. Należy jednak, że patomorfologiczna nowotworów w chwili obecnej różni się zasadniczo od tej z przed stu lat, chociaż nie uległa tak rewolucyjnym przemianom jak na przykład diagnostyka laboratoryjna. Podstawowe zmiany w patomorfologii nastąpiły w wyniku wprowadzenia wielu nowych metod badania oraz nagromadzenia ogromnego materiału dokumentacyjnego. Nowe metody to przede wszystkim cytologia, histochemia, mikroskopia elektronowa, immunohistochemia, metody biologii molekularnej i coraz częściej komputerowa analiza obrazu. Ścisła współpraca patologów i klinicystów zaowocowała opisaniem bardzo wielu nowych onkologicznych jednostek chorobowych co ma zasadnicze znaczenie dla optymalizacji metod terapeutycznych. Łączne wykorzystanie nowych metod patomorfologicznych i doświadczeń uzyskanych dzięki korelacjom kliniczno-patomorfologicznym pozwoliło na ustalenie wielu nowych histopatologicznych klasyfikacji nowotworów. Zadania patologa onkologa uległy znacznemu rozbudowaniu. W chwili obecnej jest on nie tylko zobowiązany do określenia typu nowotworu, lecz także określa jego stopień złośliwości i zaawansowanie (ang. Typing, Grading, Staging). Wydaje się, że na początku przyszłego stulecia należy oczekiwać rewolucyjnych zmian w patologii onkologicznej. W szczególności można spodziewać się następujących zmian: Zastąpienia "rzemieślniczego warsztatu" techników histopatologicznych przez nowoczesne, automatyczne techniki wykonywania preparatów. Upowszechnienie technik analizy obrazu w badaniach cytologicznych i w ocenie ilościowej preparatów histochemicznych. Upowszechnienie konsultacji "telepatologicznych" pomiędzy ośrodkami zlokalizowanymi w różnych miejscach naszego globu. Zapewne będzie możliwa automatyczna, komputerowa ocena preparatów histopatologicznych. Te nowe możliwości nie zastąpią pracy patologów, lecz wymuszą dalsza specjalizację i ściślejsza współpracę z kliniką.
Mechanizmy immunologiczne ustroju mogące eliminować komórki nowotworowe nie są do końca poznane. Dużą rolę w modulacji odpowiedzi immunologicznej przypisuje się cytokinom. Są one wytwarzane głównie przez komórki biorące udział w odpowiedzi immunologicznej, jednak ostatnie doniesienia wskazują, że komórki nowotworowe mogą je również syntetyzować. W centrum naszego zainteresowania znalazły się cytokiny typu IL-6: IL-6, LIF, OSM, IL-11, CNTF. Wspólnym elementem łączącym wymienione czynniki jest podjednostka gp130 błonowego kompleksu receptorowego. Celem pracy była ocena ekspresji genów cytokin typu IL-6 w tkankach 75 usuniętych chirurgicznie raków piersi i w 20 hodowlach pierwotnych założonych z komórek usuniętych guzów. Z tkanek nowotworowych i hodowli wyizolowano RNA. Po przeprowadzeniu reakcji odwrotnej transkrypcji (RT), za pomocą polimerazowej reakcji łańcuchowej (PCR) zużyciem skonstruowanych uprzednio swoistych primerów dokonano analizy ekspresji mRNA cytokin typu II-6 i ich receptorów. Stwierdzono, że w komórkach raka piersi mogą ulegać ekspresji wszystkie badane cytokiny i ich receptory z wyjątkiem CNTF R. Wykazano duże zróżnicowanie ekspresji swoistego mRNA dla badanych cytokin w poszczególnych przypadkach. Ponadto dokonano analizy klinicznej przebadanych raków piersi, która wykazała częstszą ekspresję mRNA IL-6 i jej receptorów w przypadkach niższego zaawansowania klinicznego (ekspresja mRNA II-6 na poziomie 93% w stopniu I i NA, 30% w stopniu MB oraz 20% w stopniach INA i IIIB). W przypadkach szybkiej progresji choroby brak było ekspresji mRNA IL-6 w tkance nowotworu. Powyższe wyniki sugerują, iż obecność IL-6 w tkance raka piersi stanowi dodatni czynnik prognostyczny w przypadku tego nowotworu.
Sixty seven patients underwent aorto-femoral double velour grafting for aortoiliac occlusive disease and 38 patients were treated for recurrent lower limb ischemia following aortofemoral grafting. Apart from various clinical examinations the patients underwent angiography as well as the morphological investigation of specimens obtained from abdominal aortae, common femoral arteries and from grafts implanted into the aorto-femoral section. The examinations showed that diffuse intimal thickening may, in the course of atherosclerosis, convert itself into uncomplicated fibrous and further into complicated plaques impairing mainly graft outflow. The drawbacks of grafts are: 1) persistens of fibrin deposits on the luminal surface of the prosthesis which may contribute to thrombosis development, 2) appearance of foci or bands of hyalinized connective tissue causing the stiffening of the graft, 3) development of lesions resembling atheromatous changes at the anastomotic areas and, 4) anastomotic hyperplasia which may contribute to stenosis at the femoral implantation site.
