Il y aurait actuellement entre 5500 et 7000 mineurs isolés étrangers (MIE) en France placés sous la tutelle des présidents des conseils départementaux. Cette population peu connue bénéficie d'une prise en charge disparate en fonction des départements. L'Unité conseil technique et santé (UCTS), propose un protocole de soin unique en France. Il s'agit d'une étude épidémiologique, rétrospective, observationnelle, monocentrique. Un recueil de données a été réalisé sur 170 dossiers de MIE. Les critères d'inclusion : tout MIE pris en charge par l'UED du 15/09/14 au 31/12/15. Le critère de jugement principal était la présence d'une pathologie infectieuse. Les critères de jugement secondaires évaluaient la présence d'autres pathologies, la mise en place d'un ou plusieurs traitement(s) anti-infectieux et le nombre de consultation spécialisée réalisée en dehors du bilan de santé initial. Cent dix-sept dossiers ont été inclus. Soixante et onze cas (62 %) présentaient une ou plusieurs pathologie(s) infectieuse(s), 67,5 % (87 cas) d'autres pathologies hors infectiologie, 50,8 % (58 cas) ont cumulé pathologie infectieuse et autre pathologie. Les principales pathologies infectieuses étaient : bilharziose 22 % (26 cas), parasitose digestive 21 % (24 cas), infection tuberculeuse latente 14,9 % (17 cas), virus de l'hépatite B 8,77 % (10 cas), Helicobacter pylori 2,63 % (3 cas), syphilis 1,75 % (2 cas), tuberculose pulmonaire 0,87 % (1 cas). Sur le plan thérapeutique : 72,8 % (83 cas) ont reçu un traitement, 58,77 % (67 cas) ont reçu au moins un traitement anti-infectieux. Les principaux étaient : praziquantel 22 % (26 cas), traitement anti-tuberculeux 15,7 % (18 cas), ivermectine 10,5 % (12 cas), metronidazole 8 % (9 cas), albendazole 7 % (8 cas). Par ailleurs 74,5 % des MIE (85 cas) ont bénéficié d'une consultation spécialisée (185 consultations au total) dont consultations de soins dentaires (23 consultations), consultations d'hépato-gastro-entérologie (15 consultations), de prise en charge psychologique (15 consultations). Les autres pathologies rencontrées étaient principalement des pathologies dentaires (42 cas), des troubles orthopédiques (24 cas), des troubles psychiatriques (19 cas), des anomalies de l'audition (17 cas), des anomalies ophtalmologiques (15 cas) et des hémoglobinopathies (11 cas). Ces résultats montrent la nécessité de proposer un bilan de santé global et coordonné, ainsi qu'une inclusion précoce dans le système de santé à ces enfants. L'éducation à la santé et la prévention doivent être abordées auprès de cette population vulnérable. L'évaluation de leur état psychologique semble fondamentale.

Ce travail vise à étudier les caractéristiques cliniques et mycologiques des onychomycoses à Candida à l’unité de mycologie l’Institut de Pasteur de Côte d’Ivoire.C’est une étude rétrospective réalisée au laboratoire de mycologie de l’Institut Pasteur de Côte d’Ivoire. Elle a porté sur les dossiers de patients adressés au laboratoire entre 1990 et 2016 pour diagnostic mycologique d’onychomycoses pour lesquels les données sociodémographiques, les résultats de l’examen direct et de la culture ont été enregistrés.Au total, 1898 dossiers de patients ont été retenus. L’âge moyen des patients était de 31,69 ans (écart-type = 15,11) avec un sex-ratio de 0,87. La prévalence des onychomycoses à Candida chez les patients reçus était de 61,7 %. Les ongles des doigts (67,7 %) étaient significativement plus atteints par cette affection, suivi de ceux des orteils (25,3 %). Quatre-vingt-dix cas d’atteintes doubles des ongles des mains et orteils ont été retrouvés. Candida albicans était l’espèce la plus fréquente représentant 79,1 % des levures isolées. Parmi les espèces non albicans, C. parapsilosis et C. tropicalis ont été isolés au niveau des ongles des orteils avec des fréquences de 11,2 et 9,6 %.Les onychomycoses à Candida sont relativement fréquentes à Abidjan et dominées sur le plan étiologique par C. albicans. La confirmation mycologique de l’étiologie fongique d’une onychopathie et une bonne hygiène des ongles permettront une meilleure prise en charge.This study aims to study the clinical and mycological characteristics of onychomycosis due to Candida in mycology unit of Institut Pasteur of Côte d’Ivoire.This is a retrospective study which was carried out on patients from 1990 to 2016 for mycological diagnosis of onychomycosis and which socio-demographic characteristics, direct examination and culture results were recorded.In this study, 1898 patient files were selected. The average age of the patients was 31.69 years (standard deviation = 15.11) with a sex ratio of 0.87. The frequency of Candida onychomycosis from patients received was 61.7%. Finger nails (67.7%) were more affected by this condition, followed by those of the toes (25.3%). Ninety cases of double localization of the nails of the hands and toes have been found. Candida albicans was the most frequent species accounting for 79.1% of isolated yeasts. Among the non-albicans, C. parapsilosis and C. tropicalis were isolated at the level of the toenails with frequencies rate of 11.2 and 9.6%.Onychomycosis due to Candida are relatively common in Abidjan and are dominated by C. albicans. The mycological confirmation of the fungal etiology in onychopathy and a good hygiene of the nails will allow a better management.
This session shares experience of obtaining and implementing a highly successful University Linkages Program between the University of Cincinnati, USA and Salahaddin University, Kurdistan, Iraq. The authors will reflect on practices and make recommendations for setting and implementing achievable goals; negotiating and maintaining personal and institutional relationships; and strategies for addressing potential challenges during and beyond the linkage. This session should be of interest to administrators, faculty, and staff who are currently involved in and/or planning on working with international partners.
Mycoses à champignons noirs : chromoblastomycoses et phaeohyphomycoses D. ChabasseLes mycoses à champignons noirs, appelées aussi mycoses à dématiés recouvrent, en dehors des mycétomes à grains noirs, deux entités cliniques : les phaeohyphomycoses et la chromoblastomycose, caractérisées par la présence dans les tissus de filaments isolés pigmentés associés à des éléments fongiques aussi foncés, vésiculeux avec ou sans cellules fumagoïdes.Les espèces incriminées sont toutes issues du sol et des végétaux en décomposition.La chromoblastomycose est une dermatose verruqueuse, d'évolution chronique, habituellement sans tendance à la dissémination, sévissant essentiellement dans les régions tropicales et subtropicales.Madagascar est le foyer francophone le plus touché.On n'observe en France que des cas importés.La maladie fait suite à une blessure avec souillure tellurique ou par l'introduction accidentelle d 'un végétal (écharde).Fonsecaea pedrosoi et Cladophialophora carrionii sont les espèces les plus rencontrées. Le diagnostic immédiat repose sur la mise en évidence des cellules fumagoïdes (formes parasitaires du champignon inoculé) objectivées dans les prélèvements cutanés superficiels ou à partir d'un examen anatomopathologique. Le traitement est long et difficile, souvent peu accessible pour les populations touchées et les rechutes sont fréquentes. La prise en charge associe le traitement physique (excise) au traitement médicamenteux essentiellement par des azolés (itraconazole, posaconazole) seuls ou associés à la terbinafine. Les phaeohyphomycoses, selon une terminologie « parapluie », coiffent un ensemble disparate de mycoses cosmopolites, cutanées et profondes dues aussi à des champignons telluriques, à parois mélanisées, appelés aussi dématiés. Mais contrairement à la chromoblastomycose, les phaeohyphomycoses ne produisent pas dans les tissus de cellules fumagoïdes et surviennent habituellement, (mais pas exclusivement), dans un contexte d'immunosuppression. Les espèces incriminées, dont certaines sont également responsables de chromomycose, sont très nombreuses, et répandues aussi bien en zone tropicale que tempérée. Le spectre clinique des phaeohyphomycoses est bienplus varié que celui observé dans la chromomycose.On décrit aussi bien des atteintes superficielles, cutanées, unguéales, oculaires que profondes -sinusiennes, respiratoires, péritonéales, cérébrales, cardiaques -et disséminées au pronostic très sombre.Leur aspect dans les tissus à l'état parasitaire est différent de celui de la chromoblastomycose, des filaments septés sur un seul plan, plus ou moins dilatés ou avec des éléments globuleux, à paroi épaisse et foncée, observés avec certaines colorations et sans les cellules fumagoïdes pathognomoniques de la chromomycose.Les espèces appartenant aux genres Alternaria et Exophiala sont les plus isolées en France métropolitaine.Le diagnostic moléculaire est actuellement plus contributif au diagnostic car plus rapide et plus précis (permettant le diagnostic d'espèce) suppléant aux difficultés d'identification des cultures.Le traitement est chirurgical -exérèse des lésions extirpables, retrait des cathéters ou des corps étrangers souillés -, et aussi médical avec essentiellement les azolés (itraconazole, voriconazole, posaconazole) associés ou non avec la terbinafine.La guérison dépend aussi largement de la maîtrise de l'affection sous-jacente et du renforcement des défenses immunitaires du patient.
Cryptococcal meningitis is the most common fatal central nervous system infection in AIDS patients in Sub-Saharan Africa. The purpose of this prospective study conducted from March 2003 to February 2004 in the internal medicine and infectious diseases departments of the Point G University Hospital Center was to investigate the clinical, prognostic and epidemiological profile of Cryptococcus neoformans infection in patients hospitalized for brain and meningeale infection (BMI). Diagnosis of neuromeningeal cryptococcosis (NMC) was based on positive identification of Cryptococcus by direct exam of the cebrospinal fluid (CSF) after India ink staining and/or culture on Sabouraud medium without actidione. During the study period, a total of 569 patients were hospitalized including 235 (41.3%) with HIV infection. Overall C. neoformans was identified in 14 patients. Median patient age was 39 +/- 8 years. There was a male preponderance with a sex ratio of 1.8 (9 men/5 women). Patients with BMI were HIV-positive in 85.7% of cases (n=12) and HIV-negative in 14.3% (n=2). The overall and HIV-specific prevalence of BMI was 2.5% and 5.1% respectively. The CD4 lymphocyte count was between I and 49 cells/mm3 in 64.3% of cases. The main clinical symptoms were cephalea in 85.7% of cases, altered consciousness in 50% and nausea/vomiting in 35.7%. Neurological manifestations (hemiparesis and cranial nerve deficit) were noted in 14.3%. HIV infection is the main purveyor of NMC in Mali. The actual incidence of cryptococcosis is unclear due to the poor sensitivity of diagnostic techniques. This study highlights diagnostic difficulties related to clinical polymorphism and poor technical facilities. Agglutination testing of blood and CSF is recommended, but mortality remains.
The authors conducted a survey in an international vaccination centre. The aim of the study was to assess the travelers' knowledge of vaccines and to check their health record to determine the vaccination rate. We used a self-administered questionnaire and added vaccination certificates to the document.Results. - Most of the travelers knew about complications due to vaccinations, but ignored their contraindications. Knowledge decreased over the age of thirty. The travelers agreed with antihepatitis B vaccination and only 10% mentioned the risk of multiple sclerosis. The list of compulsory vaccinations and their schedule were well-known but some of the travelers ignored their vaccine status and said they trusted their general practitioner. Only 40% of people over 20 years of age were able to present their national or international vaccination certificate, a reason for underestimating the vaccination rate.Conclusion. - It would be necessary to have a "World Health Organization" type vaccination certificate available for all French people over 20 years of age, while improving the smart health card and training practitioners to better inform their patients about vaccinations. We report 3 clinical observations of neuromeningeal cryptococcosis which occurred without any immunodepression related to HIV infection. Our patients were male. They did not present any particular medical history indicating a diagnosis of cryptococcosis. Nevertheless we found a professional exposure to the risk of inhalation of C. neoformans capsules. The diagnosis was based on presence of encapsulated yeast of Cryptococcus in the direct exam by China ink and culture on Sabouraud medium. CD4 lymphocytic count was done in two patients (case 1:899 cells/mm3; case 2:347 cells/mm3). Idiopathic lymphocytopenia was noted in one case. Co-morbidity of 5. pneumoniae meningitis was reported in one patient (case 3). Treatment was based on injectable amphotericin B in monotherapy (case 1), followed by perfusion of fluconazole cure (case 2). Case 3 was treated by perfusion of amphotericin B associated with ceftriaxone (case 2). No secondary prophylactic treatment was administered. Evolution was favorable after follow-up of 5 years (case 1) and of 4 years (case 2). The third patient died during hospitalization.
Background: In order to identify intraspecific variations in Trichophyton rubrum and to correlate them to the immunological status of the host, sixty strains isolated from AIDS, HIV-positive and HIV-negative patients were compared for the production of extracellular enzymes and for their susceptibility to several antifungal drugs. Methods: The isolates were tested for their ability to secrete keratinases, proteinases, phospholipases, lipases and DNases. Likewise, we investigated their susceptibility to amphotericin B, ketoconazole, ciclopiroxolamine, griseofulvin, miconazole and tolnaftate. Results: Variations in the Minimal Inhibitory Concentration (MIC80) values were observed for all antifungals tested, but they were similarly distributed among the three clinical groups. Griseofulvin showed the most prominent differences among the three groups of isolates. Regarding enzyme secretion, all samples secreted keratinases and DNases, while none secreted phospholipases. Proteinases and lipases were secreted by some of them. Conclusions: The differences among isolates of the three groups were not statistically significant and therefore could not be ascribed to a given clinical status. Intraspecific variations similarly occurred in each group, irrespective of the immunological status of the patients.
Although the association between tropical bats and pathogenic fungi has been reported long ago, no study has been performed in Europe to our knowledge. The fungal biota associated with European Chiroptera in the region of Pays de la Loire (Western France) was therefore analyzed to determine if these mammals harbor fungi pathogenic for humans or household animals.One hundred and seven samples of guano or hair/skin coming from six bat species were collected from caverns of Western France during the national annual counting of Chiroptera performed in the course of the hibernation period. They consisted of 82 samples of guano and 25 samples of hair/skin swabs. Mycological analysis of these samples revealed the great diversity of the fungal biota, more pronounced for Rhinolophidae Chiroptera than for Vespertilionidae. Among the pathogenic fungi, yeasts of Candida genus (C. glabrata and C. guilliermondii) and eight species of keratinophilic fungi (Trichophyton terrestre, Scopulariopsis brevicaulis, Aphanoascus fulvescens, Myceliophthora thermophila, Chrysosporium lobatum, Chrysosporium merdarium, Chrysosporium pseudomerdarium and Chrysosporium pruinosum) were identified. In addition, an Onygenaceae highly resembling Histoplasma capsulatum was isolated.Our results demonstrate that several human or animal pathogenic fungi may be associated with bats. Moreover, their habit to gather, sometimes in vast colonies in certain caverns, creates an environmental medium suitable for the proliferation of these fungi by enrichment of the soil with hair, skin, feces and bats corpses. However, due to the particular habitat of these animals, one may consider from this study that bats do not constitute a real danger for human or animal health.
Aspergillus fumigatus est le principal agent causal des aspergilloses humaines dont la forme la plus grave est l'aspergillose pulmonaire invasive tres frequente chez des sujets fortement imunodeprimes, Nous avons donc tente de mettre au point un modele experimental d'aspergillose pulmonaire invasive chez la souris se rapprochant le plus des conditions naturelles d'infestation, ce qui nous permettra d'etudier le developpement in vivo de ce champignon. Le modele experimental que nous avons mis au point est realise par depot d'une suspension de spores sur les narines de l'animal immunodeprime par administration quotidienne d'acetate de cortisone.
Purpose. Although the shell of tortoises is covered by keratin, keratinophilic fungi have not been described as yet in these animals. The purpose of this study was therefore to identify the fungal biota colonising the lesions of the shell, or responsible for these lesions, in mediterranean soil tortoises living in Western France. Animals and Methods. One hundred and thirteen tortoises belonging to the genus Testudo were examined during the annual meeting of the Society for Protection of Tortoises in Western France. The tortoises which presented with lesions of the carapace were sampled by scraping and swabbing after washing with sterile distilled water. For each sample, a direct examination was performed, afterward it was inoculated on two Sabouraud agar plates supplemented, for one of them, with cycloheximide. Results. Among the tortoises examined, fifty-three presented with lesions of the shell. Direct examination of the samples revealed the presence of fungal hyphae for 73% of them. Thirty four fungal genera were identified, some of which had not been reported as yet in soil tortoises. Some keratinophilic fungi (Chrysosporium sp., Anixiopsis stercoraria and Scopulariopsis brevicaulis) were recovered from the shell of 11 tortoises, responsible for S. brevicaulis for a confirmed mycosis in one of them. Conclusion. The lesions of the shell of tortoises are colonised by a huge diversity of fungal biota, principally of telluric origin. No dermatophyte was isolated, and the frequency of the other keratinophilic fungi encountered was similar to that observed in the environment. However, some of these fungi may have a pathogenic role in these animals.
Objectif. Bien que la carapace des tortues soit recouverte de keratine, les champignons keratinophiles n'ont pas ete decrits chez ces animaux jusqu'alors. Le but de ce travail etait donc de preciser la flore fongique colonisant les lesions de la carapace, ou a l'origine de ces lesions, chez des tortues terrestres de compagnie de l'Ouest de la France. Animaux et methodes. Cent treize tortues du genre Testudo ont ete examinees a l'occasion de la reunion annuelle de la Societe Protectrice des Tortues de l'Ouest. A la suite de l'examen clinique, les tortues presentant des lesions de la carapace ont ete prelevees par grattage, puis ecouvillonnage, apres nettoyage des lesions a l'eau distillee sterile. Chaque prelevement a fait l'objet d'un examen direct et d'un ensemen cement sur deux geloses de Sabouraud additionnees, pour l'une d'elles, de cycloheximide. Resultats. L'examen direct des prelevements effectues sur les tortues presentant des lesions de la carapace (53 tortues) s'est revele positif dans 73 % des cas. Trente quatre genres fongiques ont ete mis en evidence dont de nombreux n'avaient pas ete rapportes auparavant chez les tortues terrestres. Des champignons keratinophiles (Chrysosporium sp., Anixiopsis stercoraria et Scopulariopsis brevicaulis) ont ete retrouves sur la carapace de 11 tortues, a l'origine pour S. brevicaulis d'une mycose confirmee chez une de ces tortues. Conclusion Les lesions de la carapace des tortues terrestres sont colonisees par une flore fongique abondante, principalement d'origine tellurique. Aucun dermatophyte n'a ete isole, et la frequence des autres champignons keratinophiles identifies etait comparable a celle observee dans l'environnement. Certains de ces champignons pourraient cependant avoir une action pathogene chez ces animaux.
Les onychomycoses representent les etiologies les plus frequentes des onychopathies. Mal supportees par les patients, elles sont un des motifs les plus frequents de consultation en dermatologie mycologique. Pour autant, la frequence dans la population est difficilement appreciable par manque d'uniformite et d'absence de groupe temoin. Les etudes de prevalence portent sur la population generale (2 a 13 p. 100 selon les series) ou sur des populations ciblees consultant un cabinet medical ou de specialite (dermatologie). Les recentes enquetes realisees a grande echelle en Europe chez l'adulte avancent des taux de prevalence eleves, 20 a 30 p. 100 en moyenne selon que le medecin examinateur est un generaliste ou un dermatologue. Toutes les etudes s'accordent pour confirmer que les onychomycoses sont en progression constante depuis ces vingt dernieres annees. Rares chez l'enfant, frequentes chez l'adulte, elles atteignent avec predilection les sujets âges. En Europe de l'Ouest et en Amerique du Nord, ce sont les atteintes des pieds, touchant surtout le sexe masculin, qui predominent. A l'oppose, les etudes realisees dans le sud de l'Europe, au Moyen et Extreme-Orient montrent une prevalence plus elevee des onyxis (avec perionyxis) des mains surtout chez la femme. Parmi les facteurs favorisants, il convient de distinguer les facteurs individuels locaux (troubles trophiques et circulatoires, malposition des doigts...), generaux (deficit immunitaire, diabete, psoriasis...), des facteurs comportementaux ou culturels (profession, mode de vie, pratique sportive...) qui favorisent la rencontre avec le champignon responsable.
Purpose. - Candida endocarditis are rare, with a poor prognosis. Actually, the principal problem concerns, the growing incidence of nosocomial fungal infections. The objective of the present investigation is to assess a disease which risks becoming more pronounced in the future.Methods. - We have collected observations of Candida sp. endocarditis between 1985 to 1997 from three French university hospitals.Results. - Twelve of the observations fit the Duke criteria of acute endocarditis. Patients were eight men and four women, with a mean age of 46 years. An immunodepression was found in seven cases, and four patients were active drug addicts. Six had an underlying heart disease at risk to acute endocarditis. Candidemia risk factors were found in nine cases, with an average of 2.7 risk factors per patient. The fungal agents detected were Candida albicans (eight cases), C. tropicalis (one case), C. parapsilosis (two cases), and C. glabrata (one case). These vegetations were on aortic (seven cases), mitral (three cases), tricuspid valves (two cases) or in other areas (three cases), with multiple localizations (two cases). In three observations, vegetations were associated with myocardium abscesses. Eight patients had embolic complications, two had a cardiac insufficiency leading to death. The treatment was medical in all of the cases and combined with a surgical treatment in ten cases. The surgery was performed, on an average, 17 days after diagnosis, allowing seven surviving patients. Among them, five received a secondary prophylaxis and no recurrence was recorded.Conclusions. - Prognosis remains severe because of the voluminous, friable and necrotic vegetations, which favor embolic migrations and which are not easily accessible to antifungals, which penetrate poorly into these vegetations. Therapy is based on a medical treatment combined with a valve replacement which needs to be done early on, and is followed by a relapse prevention which can occur several years after the initial episode.
